Abstract

Background: The manifestation of dengue is a decrease in hematocrit and platelet levels, leading to a decrease in the patient’s quality of life and having an impact on the patient’s medical costs.Objective: This study objective was to ascertain the quality of life and medical costs associated with dengue patients.Method: This study employed a cross-sectional design at PKU Muhammadiyah Hospitals in Bantul and Yogyakarta from January to August in 2023. Pediatric patients’ quality of life was assessed using the EQ-5D-Y questionnaire, while adult patients’ using the EQ-5D-5L questionnaire. Data on patients’ medical costs was obtained from the hospital's financial database. The data were then analyzed descriptively and presented as a percentage of quality of life and average medical costs.Results: The results showed that pediatric DF patients in both hospitals reported anxiety and depression, with percentages of 100% (Yogyakarta) and 75% (Bantul), respectively. Meanwhile, most adult DF patients experienced problems in carrying out routines, 71.4% (Yogyakarta) and 50% (Bantul), respectively. Adult DHF patients in both hospitals reported pain and discomfort of 66.7% (Yogyakarta) and 100% (Bantul), respectively. The highest average medical costs paid by DF and DHF patients at PKU Jogja Hospital were IDR 4,919,450 and IDR 6,981,500. DF and DHF patients at PKU Hospital with BPJS insurance, with the highest average medical costs of IDR 2,726,245.5 (for DF patients) and IDR 4,797,700 (for DHF patients) to cover for laboratory costs.Conclusion: Dengue fever infections are impacting the patient's quality of life and medical costs.

Abstract

Most of the patients with end-stage renal disease who undergo hemodialysis receive erythropoietin (EPO) therapy for anemia treatment. Anemia with EPO deficiency is a common complication of chronic kidney disease (CKD). EPO therapy is expected to improve anemia and quality of life of patients. This analytic observational study was conducted in a cross-sectional design. It aimed to identify the difference in red blood cell profiles, namely hemoglobin (Hb), Mean Corpuscular Volume (MCV), Mean Corpuscular Hemoglobin (MCH), and Mean Corpuscular Hemoglobin Concentration (MCHC), and quality of life of CKD patients on EPO and non-EPO therapy at PKU Muhammadiyah Hospital in Yogyakarta. The inclusion criteria were PGK ICD N18.9 outpatients who received hemodialysis therapy at this hospital and were literate, able to understand the questionnaire, and willing to be a respondent. The results showed that there was no significant difference (p>0,05) between the red blood cells (Hb, MCV, MCH, MCHC) of the EPO and non-EPO therapy groups. The correlation between the EPO and the quality of life of patients in all of the KDQOL domains was p> 0.05. Aside from the insignificant difference between the EPO and non-EPO therapy groups, the research concluded that there was no correlation between either the EPO or the non-EPO therapy with the quality of life of hemodialysis patients.

Abstract

High Alert Medications (HAM) are drugs that pose a significant risk of causing harm to patients if used incorrectly. This study aimed to evaluate the direct medical costs and quality of life of elderly patients aged 60 years and above using HAM at Wongsonegoro Hospital. An observational prospective cohort study design was employed, utilizing both secondary and primary data collection methods. Secondary data included patient demographics, medication usage, and direct medical costs obtained from hospital records and the finance department. Primary data on quality of life were gathered using the European Quality of Life 5 Dimensions (EQ-5D-5L) questionnaire. A total of 25 elderly patients were identified as users of HAM, with a gender distribution of 48% male and 52% female; 64% of these patients were aged between 60 and 70 years. The most prevalent medical condition among participants was diabetes mellitus, affecting 44% of one group and 32% of another. Sodium Chloride (NaCl) 500 mL infusion at a concentration of 3% was the most frequently administered medication (27.5%). The average direct medical cost for elderly patients using HAM was IDR 4,255,050. Additionally, the quality of life assessment revealed that 20% of patients experienced significant challenges in self-care activities. These findings highlight the impact of HAM on healthcare costs and quality of life among elderly patients, emphasizing the need for careful monitoring and management strategies to mitigate risks associated with high-alert medications.

Abstract

Kanker merupakan masalah kesehatan global. Di Indonesia, kanker payudara merupakan kasus terbanyak, yaitu 58.256 kasus dari total 348.809 kasus. Pada tahun 2017, Daerah Istimewa Yogyakarta (DIY) memiliki jumlah kasus kanker payudara terbanyak, yaitu sebesar 1.564 pasien rawat jalan dan 823 pasien rawat inap. Terapi kanker payudara dapat dilakukan dengan kemoterapi, radiasi, pembedahan dan terapi hormonal. Kemoterapi menimbulkan efek samping neutropenia, anemia, mual muntah, nyeri neuropati. Efek samping akibat penggunaan kemoterapi memengaruhi besarnya biaya perawatan. Penelitian ini merupakan studi observasional dengan metode pengambilan data secara retrospektif. Subjek penelitian adalah semua pasien kanker payudara yang sesuai dengan kriteria inklusi. Pengambilan data dilakukan secara retrospektif, yaitu dari Januari 2018 hingga Desember 2019. Data yang dikumpulkan dari rekam medis pasien meliputi biaya medis dari billing tagihan, profil kemoterapi dan efek samping kemoterapi. Efek samping diidentifikasi berdasarkan referensi buku dan artikel terkait. Data diolah menggunakan uji deskriptif untuk melihat gambaran biaya dan efek samping. Jumlah subjek penelitian adalah 53 subjek dengan 9 variasi regimen kemoterapi. Biaya medik langsung tertinggi adalah regimen docetaxel dan carboplatin (DC) 5 siklus sebesar Rp2.967.330, sedangkan terendah adalah regimen 5-fluorouracil (5-FU), adriamycin, cyclophosphamide (FAC) 8 siklus Rp2.024.165. Efek samping kemoterapi yang banyak terjadi adalah mual muntah (100%), neutropenia (86%), neuropati perifer (47%), pusing (42%), mukositis/stomatitis (25%), anemia (19%), diare (13%), infeksi kulit (11,3%), konstipasi (9%), serta penurunan nafsu makan (2%). Regimen 5-FU, adriamycin, cyclophosphamide, taxane (FAC-T) adalah regimen yang paling banyak digunakan (39,6%). Biaya medis langsung obat kemoterapi tertinggi adalah regimen DC. Efek samping yang paling umum adalah mual, muntah dan neutropenia.

Abstract

High-Alert Medications (HAM) are drugs that have a high risk of causing significant harm to patients if misused. It is estimated that one in ten geriatric patients experiences undesirable events during hospitalization related to medication use. Detect undesirable events due to the use of high-alert insulin and high-alert heparin drugs in geriatric patients at Rumah Sakit Umum Pusat Nasional (RSUPN) Dr Cipto Mangunkusumo using the trigger tool method based on positive predictive value (PPV) and determine the effect of using high-alert insulin and high alert heparin drugs on undesirable events. A retrospective cohort study will review secondary data from patient medical records (MR). The sample used was from 300 geriatric patients, 82 patients used insulin and 79 patients used heparin which met the inclusion criteria. Data were analyzed in the form of frequencies and percentages (%), calculation of PPV values, and the effect of using high-alert insulin and high-alert heparin on undesirable events using the Chi-Square test. There were 8 trigger-positive patients with undesirable events in the form of hypoglycemia from 82 insulin patients and 4 trigger-positive patients with undesirable events in the form of extended activated partial thromboplastin time (aPTT) values from 79 heparin patients. Based on the PPV value, both insulin (9.75%) and heparin (5.33%), the trigger tool has not been able to detect undesirable events when using high-alert insulin and high-alert heparin. Based on statistical tests, the p-value= 0.033 (p<0.05) for insulin and heparin p-value= 0.043 (p<0.05) meaning that there is an influence of the use of high-alert insulin and high-alert heparin on undesirable events when using both drugs. The trigger tool method cannot detect undesirable events when using high-alert insulin and high-alert heparin drugs based on laboratory triggers.

Abstract

Lansia merupakan kelompok usia yang rentan terhadap penyakit degeneratif, seperti hipertensi, diabetes melitus, dan hiperkolesterolemia, yang memerlukan pengelolaan jangka panjang. Di Kelurahan Wirobrajan, Yogyakarta, prevalensi penyakit ini cukup tinggi, sementara pemahaman dan keterampilan lansia dalam pengelolaan kesehatan secara mandiri masih rendah. Program pengabdian ini bertujuan meningkatkan pengetahuan dan keterampilan lansia dalam pemanfaatan Tanaman Obat Keluarga (TOGA) secara tepat guna sebagai langkah preventif dan pengendalian penyakit degeneratif. Kegiatan dilaksanakan melalui program Sekolah Lansia Segar (Sehat dan Bugar) dalam tiga sesi edukatif yang bersifat partisipatif, diikuti oleh 63 lansia anggota Majelis Tabligh Aisyiyah Ketanggungan. Hasil evaluasi menunjukkan pengetahuan peserta mengenai penyakit degeneratif, penggunaan obat dan pemanfaatan TOGA. Seluruh peserta memahami bahwa TOGA memiliki khasiat sebagai obat, namun konsistensi dalam penggunaannya masih perlu ditingkatkan. Kegiatan ini terbukti efektif dalam memberdayakan lansia untuk mengelola kesehatannya secara mandiri.

Abstract

Kesehatan mental remaja menjadi isu penting di Indonesia, terutama di Yogyakarta, seiring dengan meningkatnya tekanan akademik, sosial, dan emosional yang dihadapi oleh mereka. Pengabdian ini bertujuan untuk melihat pengaruh edukasi berbasis leaflet terhadap peningkatan pengetahuan siswa mengenai kesehatan mental. Kegiatan pengabdian dilakukan di SMK Muhammadiyah Mlati, Yogyakarta. Edukasi dilakukan dengan menggunakan media leaflet sebagai media edukasi yang berisi informasi tentang pengertian, penyebab, gejala, dampak, serta cara pencegahan dan penanganan gangguan mental. Pengukuran keberhasilan program dilakukan dengan meberikan pretest dan posttest untuk mengevaluasi perubahan pengetahuan siswa. Hasil pengabdian menunjukan peningkatan pemahaman siswa mengetai Kesehatan mental. Temuan ini memberikan kontribusi dalam upaya meningkatkan kesadaran kesehatan mental di kalangan remaja, serta dapat menjadi dasar pengembangan program edukasi yang lebih inovatif dan berkelanjutan untuk mendukung kesejahteraan mental remaja di Indonesia.

Abstract

Non-Hodgkin lymphoma (LNH) is a heterogeneous malignancy of the lymphatic system with CHOP and RCHOP (Rituximab, Cyclophosphamide, Doxorubicin, Vincristine, Prednisolone) chemotherapy regimens as main options. Local data on patient characteristics undergoing these treatments are still limited, especially at Dr. Sardjito General Hospital. This study evaluates the characteristics of LNH patients receiving both regimens at Dr. Sardjito (2020–2023). It is a descriptive observational study with a cross-sectional design. Data from patient medical records were analyzed descriptively. Of 176 patients, 80 received CHOP and 96 R-CHOP. Most were male (CHOP: 70%; R-CHOP: 60.4%) and aged >50 years, with peak age of 51–60 in CHOP (40%) and >60 in R-CHOP (44.8%). Comorbidities were found in one-third of patients. Advanced stage predominated (CHOP: 31.3% stage 4; R-CHOP: 38.5% stage 2), and most completed 6 chemotherapy cycles. Rituximab ≥550 mg was widely used in R-CHOP; cyclophosphamide >1100 mg, doxorubicin 70–79 mg, and vincristine ≥2 mg were standard in both regimens. Prednisone 100 mg was common. Diffuse Large B-Cell Lymphoma (DLBCL) dominated R-CHOP (98.96%), while CHOP patients included DLBCL (38.75%), FL, PTCL-NOS, and others. Findings highlight the importance of individualized treatment based on histopathology and disease stage.

Abstract

Vaccination has proven effective in preventing HBV transmission and reducing related health burdens. This study aimed to systematically review the long-term effectiveness of the hepatitis B vaccine following primary vaccination. A literature search was conducted using PubMed, ProQuest, and the Cochrane Library following PRISMA guidelines. Keywords included "effectiveness," "hepatitis B," "HBV," "HepB," "vaccine," "vaccination," "immunization," "immune memory," "seroprotection," and "birth dose." Of 555 articles initially identified, nine met the inclusion criteria. Results showed that the duration of protection after primary vaccination ranges from 5 to 25 years, with an average seroprotection rate of 83.5%. Vaccination is vital for newborns, regardless of maternal HBV status, and healthcare professionals play a key role in promoting early immunization. Monitoring anti-HBs antibody levels is also recommended to assess the need for booster doses, which may help prevent HBV transmission and further reduce morbidity and mortality.

Abstract

Background: Tuberculosis (TBC) is a major health problem in Indonesia, especially in Gorontalo, with high spread due to poor ventilation, overcrowding, and unhealthy lifestyles. Purpose: To analyze the characteristics of TB patients in one of Gorontalo City's hospitals using K-Means Clustering. Methods: Data including age, gender, TBC history, HIV status, diabetes history, hypertension, drug resistance, drug side effects, and treatment results were analyzed for the number of clusters using the K-Means method because it is effective in grouping data based on similarity, easy to implement, and works well on large datasets. Results: The analysis resulted in three clusters. Cluster 0 (219 individuals): majority female (63.50%), mean age 45.37 years, low address score (0.49), low resistance and therapy (6.40%), no comorbidities, all experienced side effects (100%), and survival rate 4.10%. Cluster 1 (150 individuals): mean age 52.21 years, higher address score (0.77), resistance 7.30%, therapy 5.30%, comorbidities 100%, all experienced adverse events, and survival rate 4.70%. Cluster 2 (98 individuals): mean age 48.58 years, address score 0.65, very low resistance and therapy (2%), no side effects, 42.90% had comorbidities, and the highest survival rate (12.20%). Conclusion: Three clusters were obtained from the analysis using K-Means. Clustering supports specific interventions such as comorbidity management or intensive surveillance, improving TB control programs in Gorontalo.
