Abstract

Latar belakang: Antibiotika merupakan golongan obat yang paling banyak digunakan pada pencegahan infeksi bakteri karena perlakuan bedah. Penelitian ini bertujuan untuk mengetahui kerasionalan penggunaan antibiotika profilaksis dari parameter tepat indikasi, tepat obat, tepat pasiendan tepat dosis dan mengetahui hubungan kerasionalan penggunaan antibiotika dengan lama rawat inap pasien.Metode: Rancangan penelitian ini adalah observasional dengan pengambilan data restropektif. Subyek penelitian ini adalah semua rekam medik pasien di instalasi bedah RSUD Tugurejo Semarang dengan kriteria inklusi rentang usia 20-60 tahun, mendapatkan terapi antibiotika profilaksis, data rekam medik jelas. Data penggunaan antibiotika dianalisis kerasionalannya berdasarkan Pedoman Penggunaan Antibiotika 2011, Clinical Practice Guidelines for Antimicrobial Prophylaxis in Surgery2008, Antibiotic Guidelines 2014-2015 dan Drug Information Handbook 2010.Hasil: penelitian menunjukkan penggunaan antibiotika cefuroxime 10%, cefazoline 44%, ceftriaxone18%, cefotaxime 15% dan ampisiln 13%. Kerasionalan penggunaan antibiotika profilaksis meliputi tepat indikasi 86%, tepat obat 54%, tepat pasien 54% dan tepat dosis 44%. Tidak terdapat hubungan kerasionalan penggunaan antibiotika dengan lama rawat inap pasien (p>0,05).Kesimpulan: Penggunaan antibiotika rasional sebesar 44%, tidak ada hubungan antara kerasionalan penggunaan antibiotika profilaksis dengan lama rawat inap pasien operasi di RSUD Tugurejo Semarang periode April 2014.

Abstract

Diabetic neuropathy is gathering symptom clinic which influence system nerves, well single or combination and could happen in a manner suddenly, no detected, with symptom clinic and no specific signs but dangerous. Incidence diabetic neuropathy happen between 60% to 70% on type I and type II DM patients. Methods of data collection carried out prospectively. Data were obtained from outpatient data with a diagnosis of diabetic neuropathy who met the inclusion criteria and received analgetics from September to December 2020. Then a cost-effectiveness analysis was performed by calculating the ACER (Average Cost-Effectiveness Ratio) and ICER (Incremental Cost-Effectiveness Ratio) values. The results showed that the most commonly used analgetics was gabapentin at 65.96%, either alone or in combination with an effectiveness percentage of 83.87%. The use of analgetics with the lowest cost is a single Paracetamol of IDR 73,770. The lowest ACER values were shown in the use of the single analgetics Paracetamol and the combination of Amitriptyline-Paracetamol, namely 737.70 and 805.30.

Abstract

Health-related quality of life (HRQoL) is an important aspect in the management of chronic diseases such as hepatitis B, considering the long disease course and the risk of complications including cirrhosis and hepatocellular carcinoma. The European Quality of Life instrument (EQ-5D-5L) is widely used globally to assess quality of life; however, psychometric evidence in patients with hepatitis B in Indonesia remains limited. This study aimed to evaluate the preliminary validity and reliability of the Indonesian version of the EQ-5D-5L among patients with hepatitis B in primary healthcare settings. This study employed an observational design with a cross-sectional approach involving 42 female patients with confirmed HBsAg positivity who participated in a hepatitis B early detection program at primary health centers in Banjarmasin City between April and October 2025. Data were collected using the EQ-5D-5L questionnaire, the EQ visual analogue scale (EQ-VAS), and a demographic characteristics form. Reliability was assessed using Cronbach’s alpha, while construct validity was evaluated using item–total correlation with a significance threshold of r-table = 0.304. Statistical analyses were performed using SPSS version 26. The results indicated that the EQ-5D-5L questionnaire demonstrated good internal consistency with a Cronbach’s alpha value of 0.799. All items showed item–total correlations greater than the r-table value (0.304), indicating that all domains were valid. Correlation analysis revealed a significant relationship between EQ-VAS scores and the EQ-5D-5L utility index (r = 0.669; p < 0.001), suggesting adequate construct validity. Within the limitations of this study, the Indonesian version of the EQ-5D-5L demonstrates potential as a tool for measuring quality of life among female patients with hepatitis B in primary healthcare settings.

Abstract

Patients with type 2 diabetes mellitus and hypertension comorbidity generally receive more than one medication, increasing the risk of drug interactions that may affect therapeutic outcomes. This study aimed to determine the potential drug interactions in outpatients with type 2 diabetes mellitus and hypertension at PKU Muhammadiyah Gamping Hospital during January–December 2024. This research employed a descriptive-retrospective observational method using 44 medical records selected through purposive sampling based on inclusion and exclusion criteria. Drug interactions were assessed using the MedFacts Pocket Guide of Drug Interactions, Stockley’s Drug Interactions (2009), and drug interaction checkers from Medscape and Drugs.com based on interaction mechanisms and severity levels. The results showed 129 cases of drug interactions, with the most common combination being metformin–amlodipine (10 cases; 7.7%). The most frequent interaction mechanism was pharmacodynamic interaction (68 cases; 52.71%), while the severity level was predominantly moderate according to Medscape (65 cases; 50.38%) and Drugs.com (79 cases; 61.24%). Major interactions were identified in the bisoprolol fumarate–clonidine combination (4 cases; 5.3%). This study indicates the potential for drug interactions in antidiabetic and antihypertensive therapy, highlighting the need for therapeutic monitoring and dose adjustment to improve treatment safety and effectiveness.

Abstract

High cholesterol and gout are prevalent health problems in the community and may lead to serious complications if not properly managed. This community engagement program aimed to enhance public knowledge and understanding of the use of red ginger (Zingiber officinale var. rubrum) as a supportive approach for the prevention and management of cholesterol and gout. The program was conducted in Kauman Hamlet, Gilangharjo Village, Pandak Subdistrict, Bantul Regency, employing health education sessions, dissemination of educational materials, and practical demonstrations of red ginger processing, with evaluation based on a pre-test and post-test design involving 38 participants. The results demonstrated a substantial improvement in participants’ knowledge, as indicated by an increase in the mean percentage of correct responses from 66.4% prior to the intervention to 90.5% following the intervention. These findings suggest that community-based education integrating local resources is effective in improving public understanding of cholesterol and gout prevention and management and in promoting healthy lifestyle practices.
