Abstract

Vaccination has proven effective in preventing HBV transmission and reducing related health burdens. This study aimed to systematically review the long-term effectiveness of the hepatitis B vaccine following primary vaccination. A literature search was conducted using PubMed, ProQuest, and the Cochrane Library following PRISMA guidelines. Keywords included "effectiveness," "hepatitis B," "HBV," "HepB," "vaccine," "vaccination," "immunization," "immune memory," "seroprotection," and "birth dose." Of 555 articles initially identified, nine met the inclusion criteria. Results showed that the duration of protection after primary vaccination ranges from 5 to 25 years, with an average seroprotection rate of 83.5%. Vaccination is vital for newborns, regardless of maternal HBV status, and healthcare professionals play a key role in promoting early immunization. Monitoring anti-HBs antibody levels is also recommended to assess the need for booster doses, which may help prevent HBV transmission and further reduce morbidity and mortality.

Abstract

Background: Tuberculosis (TBC) is a major health problem in Indonesia, especially in Gorontalo, with high spread due to poor ventilation, overcrowding, and unhealthy lifestyles. Purpose: To analyze the characteristics of TB patients in one of Gorontalo City's hospitals using K-Means Clustering. Methods: Data including age, gender, TBC history, HIV status, diabetes history, hypertension, drug resistance, drug side effects, and treatment results were analyzed for the number of clusters using the K-Means method because it is effective in grouping data based on similarity, easy to implement, and works well on large datasets. Results: The analysis resulted in three clusters. Cluster 0 (219 individuals): majority female (63.50%), mean age 45.37 years, low address score (0.49), low resistance and therapy (6.40%), no comorbidities, all experienced side effects (100%), and survival rate 4.10%. Cluster 1 (150 individuals): mean age 52.21 years, higher address score (0.77), resistance 7.30%, therapy 5.30%, comorbidities 100%, all experienced adverse events, and survival rate 4.70%. Cluster 2 (98 individuals): mean age 48.58 years, address score 0.65, very low resistance and therapy (2%), no side effects, 42.90% had comorbidities, and the highest survival rate (12.20%). Conclusion: Three clusters were obtained from the analysis using K-Means. Clustering supports specific interventions such as comorbidity management or intensive surveillance, improving TB control programs in Gorontalo.

Abstract

Patient education is vital in diabetes management, empowering patients with necessary knowledge and skills to manage condition effectively. However, traditional educational methods often face challenges such as limited access to healthcare professionals and variability in information quality. This study aimed to assess the reliability and readability of artificial intelligence (AI)-driven chatbot responses in disseminating diabetes knowledge. Technically, the diabetes knowledge questionnaire (DKQ-24) was administered to evaluate the effectiveness of AI-driven chatbot in disseminating diabetes-related information. Responses were evaluated for reliability and quality applying the modified DISCERN (mDISCERN) scale and global quality scale (GQS), and readability was assessed using the Flesch reading ease (FRE) score, Flesch-Kincaid grade level (FKGL), gunning fog index (GFI), Coleman-Liau index (CLI), and simple measure of gobbledygook (SMOG). The mean mDISCERN score was 31.50±2.89, indicating generally reliable responses. The median GQS score was 4, reflecting the high overall quality. The readability assessment revealed a mean FRE score of 66.30, indicating that the text was fairly easy to read. FKGL mean score was 6.54±3.19, suggesting the text was suitable for readers at a sixth-grade level. In conclusion, AI-driven chatbot provides reliable and high-quality information on the diabetes self-management, but it requires improvements to enhance accessibility.

Abstract

Pada tahun 2017, sebanyak 1,5 juta kematian anak disebabkan oleh penyakit yang dapat dicegah dengan vaksinasi. World Health Organization (WHO) melalui Global Vaccine Action Plan (GVAP) merekomendasikan setiap negara mencapai ≥90% cakupan imunisasi. Data Riset Kesehatan Dasar tahun 2018 proporsi cakupan imunisasi dasar lengkap pada anak sekitar 57,9% dari target 93%. Upaya peningkatan pengetahuan, sikap dan praktik orang tua terhadap vaksinasi anak diperlukan sebagai bentuk promosi kesehatan. Penelitian ini bertujuan untuk mengetahui hubungan antara pengetahuan dan sikap terhadap praktik vaksinasi anak, serta pengaruh edukasi media video vaksinasi terhadap tingkat pengetahuan dan sikap orang tua di Kota Jayapura, Provinsi Papua. Desain penelitian ini pre-eksperimental dengan one-group pre-test post-test study. Pengambilan data dilakukan pada bulan September–Oktober 2021 dengan teknik purposive sampling. Jumlah sampel ditentukan dengan rumus Lameshow diperoleh 290 orang tua dengan memberikan intervensi edukasi media video. Kuesioner tentang pengetahuan, sikap dan praktik terhadap vaksinasi yang telah divalidasi diberikan sebelum dan sesudah intervensi. Analisis statistik dilakukan dengan uji Spearman dan Wilcoxon signed rank. Hasil penelitian dari data sebelum intervensi menunjukkan tidak ada hubungan antara pengetahuan dengan praktik vaksinasi anak (p>0,05), namun terdapat hubungan signifikan (p<0,05) antara sikap dengan praktik vaksinasi anak. Terdapat pengaruh edukasi media video terhadap tingkat pengetahuan dan sikap orang tua terkait vaksinasi anak  (p<0,05). Kesimpulan, edukasi video vaksinasi meningkatkan pengetahuan dan sikap orang tua di Kota Jayapura Provinsi Papua. Penelitian ini bermanfaat sebagai bukti ilmiah bahwa media edukasi video dapat mempromosikan pemahaman orang tua tentang vaksinasi anak di Kota Jayapura, Provinsi Papua.

Abstract

Diabetes melitus (DM) membutuhkan waktu terapi yang panjang sehingga dapat menyebabkan beban pengobatan pada pasien. Penelitian ini bertujuan untuk mengetahui beban pengobatan serta hubungan antara karakteristik pasien dengan beban pengobatan pada pasien DM rawat jalan. Desain penelitian ini adalah cross-sectional dengan pengambilan data menggunakan kuesioner. Jumlah sampel ditentukan dengan rumus Lemeshow diperoleh 100 pasien. Kriteria inklusi sampel penelitian adalah pasien DM tanpa komorbid yang menjalani rawat jalan selama bulan Juni–Juli 2022. Beban pengobatan dikumpulkan menggunakan Living with Medicines Questionnaire (LMQ) yang terdiri dari 8 domain. Analisis data secara univariat dan bivariat menggunakan uji spearman. Hasil penelitian menunjukkan bahwa beban pengobatan yang dialami adalah 56 pasien rendah dan 44 pasien sedang.  Pada domain 1 komunikasi dengan tenaga kesehatan sudah baik, domain 2 pasien masih mengalami kesulitan teknis penggunaan obat. Domain 3 pasien tidak merasakan beban biaya, sedangkan domain 4 pasien merasakan efek samping obat. Pada domain 5, pasien belum merasakan efektivitas obat. Domain 6 dan 7 pasien khawatir terhadap penggunaan obat serta dampak obat terhadap aktivitas sehari-hari. Domain 8, pasien mengikuti instruksi dokter tanpa melakukan perubahan regimen secara mandiri. Usia pasien, lama menderita dan jumlah pemberian obat tidak berhubungan dengan beban pengobatan (p≥0,05). Koefisien korelasi pada variabel usia dan lama menderita  negatif lemah artinya semakin bertambah usia dan lama menderita, semakin rendah beban pengobatan pasien. Koefisien korelasi pada variabel jumlah obat yang dikonsumsi postif lemah artinya semakin banyak jumlah pemberian obat, semakin tinggi beban pengobatan. Beban pengobatan pada pasien DM adalah rendah dan sedang, tidak ada hubungan antara usia, lama menderita dan jumlah obat dengan beban pengobatan.

Abstract

Background: Geriatric patients are patients who are often associated with multiple diseases and decreased organ function so they are at greater risk of developing unwanted drug reactions due to drug use when compared to other adults. High Alert Medication (HAM) are drugs that have a high risk of causing significant harm to patients if used incorrectly.Objectives: This study aims to determine the pattern of HAM use among inpatient geriatric patients at Rumah Sakit Umum Pusat Nasional Dr. Cipto Mangunkusumo Jakarta.Methods: Retrospective cohort study by reviewing primary data from medical records (MR) of inpatient geriatric patients who used HAM for the period January 1 to December 31, 2023. The sample consisted of 300 geriatric patients who met the inclusion criteria using a purposive sampling technique. Descriptive data analysis includes data in the form of frequencies and percentages (%).Results: Based on gender, women with a total of 162 (54%) patients dominated. Based on age, the age range of 60 to 70 years with a total of 104 (68.87%) patients with a diagnosis of hypertension dominated compared to other ages. Based on the type of HAM used namely Insulin group HAM with 53 (20.38%) times of administration with the type of insulin in the form of novorapid flexpen 3 ml (13,532%) and anticoagulant group HAM with 49 (18,84%) times of administration with the type of anticoagulant in the form of heparin sodium 5 ml (13,181%) with the diagnosis of hypertension.Conclusion: The types of HAM that dominate the use in the therapy of hospitalized geriatric patients are novorapid flexpen 3 ml and heparin sodium 5 ml given parenterally with a diagnosis of hypertension.

Abstract

Diabetes melitus merupakan sekumpulan penyakit metabolik dengan karakteristik hiperglikemia yang terjadi karena kelainan sekresi insulin, kerja insulin atau kedua-duanya. Penelitian ini dilakukan untuk mengetahui kepatuhan pasien DM tipe 2 di RSU PKU Muhammadiyah Bantul.Penelitian ini dilakukan dengan metode observasional crossectional dengan mengambil data pasien secara prospektif selama periode Oktober-Desember 2013. Subyek penelitian adalah pasien diabetes melitus tipe 2 rawat jalan di RSU PKU Muhammadiyah Bantul yang telah menerima antidiabetik oral minimal 6 bulan terapi sebelum pengukuran kepatuhan. Subyek yang memenuhi kriteria inklusi sejumlah 56 pasien diabetes melitus tipe 2 dibagi menjadi dua kelompok yaitu kelompok monoterapi sejumlah 24 pasien dan kelompok kombinasi terapi sejumlah 32 pasien. Pengumpulan data dilakukan dengan melakukan wawancara dan pengisian kuesioner Medication Adherence Report Scale (MARS) untuk mengukur kepatuhan.  Hasil penelitian menunjukkan bahwa tingkat kepatuhan antara monoterapi dan kombinasi terapi berbeda signifikan (p>0,05). Pada faktor karakteristik hanya jenis kelamin yang berpengaruh terhadap kepatuhan  [RR=0,463; interval kepercayaan 95%: 0,202-1,062]

Abstract

Pharmacists should give more attention in giving pharmaceutical care services to the elderly patients, because the decrease of physiological function could result more drug related problems. Currently, elderly patients are interesting to do self- medication before visiting the physician. Thus, our study was aimed to understand  the proportion of elderly patients who are interesting to do the self-medication and to explore the factors predicted self-medication behavior. This study was carried out by cross-sectional design. Data was collected by doing interview and giving questionnaire to the 544 elderly patients in the pharmacies of YogyakartaProvince which were chosen by proportional area sampling method. Data was analyzed descriptively and using the chi square test to understand factors which could be influenced self medication behavior. Most of the patients choose to do the self medication (63.6%) rather than directly went to the physician (36.4%).  There were 3 predictors of self medication behavior in our study, which were occupation, income and type of disease with the RR were; 0.823 (95% CI:0.7010.965), 1.433 (95% CI:1.173-1.751) and 1.027 (95% CI: 1.059-1.376), respectively.The occupation, income and chronic disease could predict of self medication behavior in elderly patients.

Abstract

Diabetes mellitus (DM) requires long-term therapy. Selection of the right therapy was very important to provide optimal results at an affordable cost. One of the main standards of oral antidiabetic therapy prescribed for type 2 DM (T2DM) is metformin, either alone or in combination therapy. The purpose of this study was to describe the direct medical costs in type 2 DM patients between metformin and metformin-glimepiride therapy. This research was an observational and descriptive. Demographic and clinical data were taken prospectively from the complete medical records of data patients with the main diagnosis of DM who fulfilled the inclusion criteria in the period January 2019–November 2020 at RSU Nene Mallomo Sidenreng Rappang, South Sulawesi and the two Public Health Centers namely Puskesmas Galur. II and Panjatan II Kulon Progo Yogyakarta. Cost data includes total medical costs from the hospital/ healthcare perspective (cost of drugs, doctor's services and checkups, fees for checking Fasting Blood Glucose/ Gula darah Puasa (GDP) and Blood Glucose Level While/ Gula Darah Sewaktu (GDS)) and total costs from the Badan Penyelenggara Jaminan Sosial (BPJS)/ payer perspective (INA-CBG's rates) were covered by insurance. The results showed that the average direct medical costs at the hospital and the two Public Health Centers respectively were metformin therapy in IDR (Indonesian Rupiah) namely IDR 120.736 and IDR 9.072,- and metformin-glimepiride therapy of IDR 126.298,- and IDR 18.634,-. It can be concluded that the average direct medical cost of metformin therapy is lower than metformin-glimepiride therapy.

Abstract

Metformin and metformin-glimepiride are cost-effective therapy options and are most commonly prescribed to treat type 2 diabetes mellitus (DM). This study aims to determine the cost-effectiveness of using metformin and metformin-glimepiride in patients with type 2 DM at Nene Mallomo Sidenreng Rappang General Hospital. This analytical descriptive observational research employed a retrospective cohort design. The data of type 2 DM patients, new and old patients, were obtained from the medical record unit. The target population was data of outpatients for the period January-December 2019. The parameter of the therapeutic effectiveness is the random blood sugar levels of the patients after receiving treatment. The data on cost from the hospital perspective were obtained from the administration and finance unit. Cost-effectiveness analysis employed calculations of the average cost-effectiveness ratio (ACER) and the incremental cost-effectiveness ratio (ICER). Meanwhile, the risk ratio (RR) employed the analytical chi-squared method to determine the relationship between the two types of therapies and their effectiveness. This study has revealed that 30 patients met the inclusion criteria; 14 patients received metformin therapy and 16 patients received metformin-glimepiride therapy. Metformin therapy is more effective (64.29%) and more costly (IDR120,736). The metformin's ACER value is 1877.99, and its ICER value is -3107.26. The cost-effectiveness analysis has revealed that metformin therapy is more cost-effective than metformin-glimepiride therapy. Meanwhile, the chi-squared analysis has discovered no relationship between the two therapies and their effectiveness. The RR value of 1.080 concludes that metformin has 1.080 as much therapeutic effectiveness as the metformin-glimepiride.
