Abstract

One of the most complex chronic diseases is diabetes, which requires ongoing medical care aimed at reducing multifactorial risks beyond glycaemic control. Diabetes and stress are interrelated, where stress results in increased cortisol production, cortisol itself inhibits the work of insulin, this triggers an increase in blood sugar levels. One of the ways to control blood sugar levels by providing non-pharmacological therapy is to provide music therapy, relaxation and guided imagery. Progressive Muscle Relaxation (PMR) is one of the relaxations that can be applied to patients with diabetes mellitus. This relaxation technique can speed up the body's metabolism by physiologically increasing blood flow. This study aims to determine the effect of progressive muscle relaxation on blood sugar levels in patients with type 2 diabetes mellitus. This research is a quasi-experimental. The research design used is nonrandomised pretest - posttest with control group design. The research sample was divided into two groups, the intervention group and the control group. Blood sugar level measurements were carried out in the intervention group twice, namely before (pretest) and after (posttest) being given progressive relaxation therapy, while the control group did not receive any actions. The population in this study were patients with type 2 diabetes mellitus at Dr Moewardi Surakarta Hospital. Data analysis using the parametric paired t-test because the data is normally distributed. The blood sugar level in the control group before the relaxation intervention was an average of 237.90 mg/dl and after the relaxation intervention was an average of 218.57 mg/dl. The average blood sugar level in the control group before relaxation was 238.10 mg/dl and after relaxation was 231.43 mg/dl. The p-value for the intervention group based on the paired sample t-test statistic is 0.000 (p < 0.05) with an average decrease in blood sugar level of 19.33 mg/dl. These results indicate that progressive muscle relaxation exercises have an effect on changes in blood sugar levels in patients with type 2 diabetes mellitus. Based on the above description, it can be concluded that progressive muscle relaxation has an effect on changes in blood sugar levels in patients with type 2 diabetes mellitus.

Abstract

End-stage chronic kidney disease requires hemodialysis with vascular access via an arteriovenous fistula (AVF). AVF cannulation is often painful. Cryotherapy, a cold therapy that can reduce pain through gate control theory, is used. Objective to assess the effectiveness of cryotherapy in reducing pain during arteriovenous fistula (AVF) puncture in hemodialysis patients. This study applied an Evidence-Based Nursing Practice approach, including a literature search through PubMed and ProQuest databases. The primary reference used was a systematic review and meta-analysis entitled "The Effect of Cryotherapy on Pain Associated with Arteriovenous Fistula Puncture in Hemodialysis Patients." This study used a single-group pre-test and post-test design. The population consisted of hemodialysis patients with AVFs who met the inclusion criteria, patients undergoing regular hemodialysis, using AVF access, and who had been using AVF access for more than three months. Exclusion criteria included patients who had taken pain medication within three hours before hemodialysis. A total of 19 participants were included. The intervention involved applying ice gel compresses to the cannulation area for 10 minutes before the procedure. Pain levels were measured using a Visual Analogue Scale before and after the intervention. Data were analyzed using paired t-tests and Chi-square tests. Cryotherapy significantly reduced AVF cannulation pain, with a p-value of 0.001. There was a difference in pain levels before and after the cryotherapy intervention.

Abstract

Type 2 diabetes mellitus (DM type 2) is a serious problem worldwide due to the impact of the disease which can cause complications and reduce the quality of life of sufferers. Factors that cause this are due to lack of knowledge, self-management and chronic hyperglycemia. Therefore, it is necessary to carry out continuous educational interventions such as Diabetes Self-Management Education (DSME). The purpose of this implementation is to improve knowledge, self-management and control of random blood sugar levels in patients with type 2 DM. This type of quantitative research uses a one-group pre-test-post-test approach, a sample of 15 respondents and the sampling technique used was purposive sampling. The implementation time is April 21 to May 24, 2025 at Dr. H. Abdul Moeloek Regional General Hospital, Lampung Province. The measurement instruments used are Diabetes Knowledge Questionnaire (DKQ-24), Diabetes Self-Management Instrument (DSMI) and GDS measurement sheets. The analysis used is the paired t-test and the Wilcoxon test. The results of the implementation after being given 4 sessions of DSME showed an increase in knowledge as shown by a p-value of 0.003 (<0.05), an increase in self-management with a p-value of 0.001 (<0.05) and control of random blood glucose values with a p-value of 0.001 (<0.05).0.000 (<0.05). The 4-session ideo-based DSME had a positive impact on knowledge, self-management, and random blood sugar values because DSME is a continuous educational intervention that helps respondents understand their disease better.

Abstract

ABSTRAK Diabetes melitus merupakan penyakit tidak menular dengan prevalensi terus meningkat dan berisiko menimbulkan komplikasi kronis, salah satunya kaki diabetes. Upaya pencegahan di tingkat komunitas belum optimal karena keterbatasan pengetahuan dan keterampilan kader kesehatan. Kegiatan ini bertujuan meningkatkan kapasitas kader dalam deteksi dini dan pencegahan kaki diabetes melalui pelatihan berbasis komunitas. Pelatihan dilaksanakan pada 28 kader di Kelurahan Cempaka Baru, Jakarta Pusat, melalui edukasi, demonstrasi pemeriksaan kaki, praktik perawatan, serta evaluasi pre–post test. Setelah pelatihan, kader mampu melakukan pemeriksaan kaki sederhana dan memberikan edukasi pencegahan. Rata-rata skor pengetahuan meningkat dari 70,00 ± 8,01 menjadi 90,54 ± 5,07, dengan selisih 20,54 poin (p 0,001). Temuan ini menunjukkan pelatihan berbasis komunitas efektif meningkatkan pengetahuan dan keterampilan kader. Integrasi kegiatan serupa ke dalam Posyandu Lansia dan Posbindu PTM perlu dilakukan secara berkelanjutan. Kata Kunci: Diabetes Melitus, Kaki Diabetes, Kader Kesehatan, Deteksi Dini, Pelatihan Berbasis Komunitas.  ABSTRACT Diabetes mellitus is a non-communicable disease with increasing prevalence and a high risk of chronic complications, including diabetic foot. Community-based prevention remains suboptimal due to limited knowledge and skills among health volunteers. This program aimed to improve their capacity in early detection and prevention of diabetic foot through community-based training. Conducted among 28 volunteers in Cempaka Baru, Central Jakarta, the training included education, foot examination demonstrations, foot-care practice, and pre–post test evaluation. After the training, participants were able to perform basic foot examinations and provide preventive education. The mean knowledge score increased from 70.00 ± 8.01 to 90.54 ± 5.07, with a mean difference of 20.54 points (p 0.001). These findings indicate that community-based training effectively improved knowledge and skills of health volunteers. Abstract

Fatigue is one of the most common clinical symptoms experienced by patients with chronic diseases and significantly affects their quality of life. The high prevalence of fatigue across various chronic conditions indicates that it is a common, widespread, and complex symptom. However, comparative studies across different chronic diseases remain limited. In addition, the main risk factors and their variations among chronic conditions have not been comprehensively explored. This study aimed to identify differences in the incidence and contributing factors of fatigue among patients with various types of chronic diseases. Methods: This study employed a quantitative cross-sectional design. A total of 128 respondents were recruited using stratified random sampling. The instruments used included the Modified Fatigue Impact Scale (MFIS), the Center for Epidemiologic Studies Depression Scale (CES-D), and the Self-Care Symptom Management Scale. The MFIS has demonstrated excellent internal consistency (Cronbach's α = 0.93), while the CES-D has shown good validity and reliability with Cronbach's α ranging from 0.85 to 0.90 across various populations. The Self-Care Symptom Management Scale also demonstrated satisfactory psychometric properties, with a Cronbach's α of 0.84. Statistical analyses consisted of univariate descriptive analysis and bivariate analysis, including Chi-square tests for categorical variables and logistic regression for categorical dependent variables.Statistical analyses consisted of univariate descriptive analysis and bivariate analysis, including Chi-square tests for categorical variables and logistic regression for categorical dependent variables. Of the 128 respondents, the majority experienced severe fatigue, particularly those with Type 2 Diabetes Mellitus. There was no significant difference in fatigue levels among the five types of chronic diseases (p = 0.976). Severe fatigue was significantly associated with education (p < 0.001), employment status (p < 0.001), income (p < 0.001), self-care practice (p < 0.001), and depression (p = 0.011), but was not associated with age, sex, duration of illness, or comorbidity.Fatigue should be routinely assessed and managed in patients with chronic diseases regardless of disease type, with a focus on improving self-care practices, early detection of depression, and consideration of educational level, employment status, and income to enhance patients’ quality of life.

Abstract

Menuju Adaptasi kebiasan baru setelah Pandemi COVID-19 yang saat ini terjadi di Indonesia, khususnya di DKI Jakarta diperlukan peran serta seluruh masyarakat dengan saling membantu dan tolong menolong sesuai dengan kemampuan dan kapasitas yang dimilikinya. Selama masa pandemic kegiatan posyandu lansia dan posbindu tidak berjalan. Hasil wawancara masyarakat yang menderita DM dan hipertensi juga tidak memeriksakan kesehatannya ke pelayanan kesehatan karena takut terkena Covid. Wilayah ini berzona kuning dan ada 2 warga yang meninggal Covid dengan komorbid Hipertensi dan DM. Tujuan dari pengabdian masyarakat ini adalah untuk meningkatkan kemampuan kader dalam melakukan upaya promotif dan preventif pada kelompok yang menderita atau beresiko PTM. Metode pada pengabdian ini adalah dengan melakukan kombinasi beberapa pendekatan yaitu penyegaran kader, peningkatan kemampuan kader dalam skrining kesehatan masyarakat, proses skrining dan peningkatan kader dalam melakukan upaya promotif dan preventif. Dari hasil pengabdian masyarakat yang dilakukan didapatkan hasil terjadinya paningkatan rata-rata pengetahuan dan keterampilan skrining kader sebelum dan setelah dilakukan kegiatan pengabdian masyarakat.

Abstract

 Abstrak HIV/AIDS masih menjadi masalah di Indonesia dengan faktor risiko tertinggi penularan melalui hubungan heteroseksual. Meskipun hubungan  heteroseksual merupakan faktor penyebab utama penularan HIV,  masih ada pasangan ODHA HIV Positif- HIV negatif dapat hidup dengan normal tanpa tertular HIV meski bertahun – tahun tetap melakukan hubungan seksual. Pasangan ini disebut serodiskordan. Studi fenomenologi ini dilakukan untuk mengeksplorasi berbagai pengalaman seksual serodiskordan dalam mempertahankan status HIV negatif di RSPI Prof. Dr. Sulianti Saroso. Sebanyak 6 serodiskordan berpartisipasi dalam studi ini. Data dikumpulkan dengan wawancara mendalam. Empat tema utama teridentifikasi dalam studi ini : (1) Pengetahuan tentang HIV/AIDS, cara penularan dan penyebab suami terinfeksi, (2) Kebutuhan seksual, (3) Pencegahan penularan HIV, (4) Pandangan tentang seksualitas. Pengetahuan yang baik mengenai pencegahan penularan HIV sangat berdampak pada kemampuan serodiskordan dalam mempertahankan status HIV negatifnya selama ini dengan cara penggunaan kontrasepsi dan memberikan dukungan dalam kepatuhan mengkonsumsi ARV. Diperlukan edukasi dan konseling yang berkelanjutan pada serodiskordan dalam meningkatkan pengetahuannya mengenai HIV/AIDS sehingga dapat mengurangi terjadinya kasus baru  HIV. AbstractHIV/AIDS is still a problem in Indonesia with the highest risk factors for transmission through heterosexual relations. Although heterosexual intercourse is the main cause of HIV transmission factors, there are still a couple people living with HIV can live normally without being infected with HIV even though many – year fixed sexual intercourse. The couple called serodiscordant. Phenomenology study was conducted to explore various sexual experience serodiscordant in maintaining negative HIV status at RSPI, Prof. Dr. Sulianti Saroso. As many as 6 serodiscordant participated in this study. Data collected by in-depth interviews. The four main themes identified in this study: (1) knowledge of HIV/AIDS, transmission method and causes the infected husband, (2) sexual Needs, (3) the prevention of HIV transmission, (4) views about sexuality. A good knowledge about the prevention of HIV serodiscordant ability badly in maintaining its negative HIV status during this time by means of the use of contraceptives and provide support in compliance to consume ARVS. Required education and ongoing counseling on improving his knowledge in serodiscordant about HIV/AIDS so as to reduce the occurrence of new cases of HIV.
