Abstract

Background: Bullying victimization among adolescents is a significant public health issue in Indonesia, with limited research on factors such as self-esteem and peer pressure within local cultural contexts. Objective: This study aimed to identify the factors influencing bullying victimization among early adolescents in junior high schools. Methods: A cross-sectional study was conducted with 401 junior high school students from March to May 2024. Participants were selected through stratified random sampling, and data were collected using validated instruments, including the Revised Olweus Bully/Victim Questionnaire, Rosenberg Self-Esteem Scale, Peer Pressure Inventory, Depression Anxiety Stress Scales for Youth, and Parent Authority Questionnaire. Descriptive statistics, chi-square tests, and ordinal logistic regression were employed for analysis. Results: Among participants, 23.2% reported being victims of bullying. Multivariate analysis identified high peer pressure as the strongest predictor of bullying victimization (OR = 5.64, 95% CI = 2.12–15.03, p <0.001), while authoritarian parenting style emerged as potentially protective (OR = 0.34, 95% CI = 0.13–0.87, p = 0.024) compared to authoritative parenting. Self-esteem, depression, and social media use did not demonstrate significant independent associations with victimization in the final model, despite showing significant bivariate relationships. Conclusion: Peer pressure emerged as the critical predictor of bullying victimization among early adolescents, with authoritarian parenting demonstrating an unexpected protective effect within this cultural context. School-based interventions should prioritize peer-focused strategies while considering culturally specific parenting dynamics. These findings highlight the importance of developing prevention approaches that account for Indonesia's collectivist social framework.

Abstract

Background: Medication adherence remains a critical challenge in tuberculosis (TB) control, particularly in Indonesia, which ranks second globally in TB incidence. While previous studies have examined adherence factors worldwide, limited research has simultaneously evaluated the combined influence of sociodemographic, clinical, and psychosocial determinants in Indonesian settings, particularly using validated Indonesian-language instruments.Purpose: This study aimed to identify and quantify key predictors of medication adherence among pulmonary TB patients in Palembang, Indonesia, with particular emphasis on sociodemographic factors, clinical variables, and psychosocial determinants. Methods: A cross-sectional analytical study was conducted involving 150 pulmonary TB patients recruited through consecutive sampling at five public healthcare centers in Palembang. Data were collected using validated instruments, including the 8-item Morisky Medication Adherence Scale (MMAS-8) for adherence, the Knowledge About Tuberculosis Questionnaire (KATUB-Q) for TB knowledge, a structured questionnaire for side effects, the Treatment Motivation Questionnaire, the Multidimensional Scale of Perceived Social Support (MSPSS), and the TB-related Stigma Scale. Statistical analyses included descriptive statistics, Chi-square tests, and ordinal logistic regression to evaluate relationships between variables and adherence levels. Results: Only 20% of participants demonstrated high adherence, while 50.7% exhibited low adherence. Significant predictors of adherence included early adulthood (OR = 0.061, 95% CI = 0.004–0.857, p = 0.038) and middle adulthood (OR = 0.052, 95% CI = 0.005–0.565, p = 0.015), indicating lower adherence compared to late elderly. Other predictors were poor TB knowledge (OR = 0.316, 95% CI = 0.154–0.650, p = 0.002), low motivation (OR = 0.244, 95% CI = 0.108–0.553, p < 0.001), limited family support (OR = 0.470, 95% CI = 0.232–0.952, p = 0.036), insufficient healthcare worker support (OR = 0.349, 95% CI = 0.204–0.840, p = 0.015), and the presence of drug side effects (OR = 5.294, 95% CI = 2.134–13.126, p < 0.001). Younger adults showed lower adherence rates compared to older populations, while patients with better knowledge and stronger support systems demonstrated higher adherence. Conclusion: Key predictors of medication adherence were age, TB knowledge, motivation, family support, healthcare worker support, and drug side effects. Younger patients, those with poor knowledge, low motivation, weak support systems, and severe side effects demonstrated significantly lower adherence rates. These findings highlight the need for targeted, multifactorial interventions to improve TB treatment outcomes in Indonesia.

Abstract

Introduction: The research aimed to investigate the correlation between health literacy levels, medication adherence, and quality of life among hemodialysis patients with chronic kidney disease. Methods: The research employed a cross-sectional design. A random sample of 164 hemodialysis patients from two hemodialysis facilities in Padang, Indonesia, was collected between May and June 2023. Inclusion criteria were patients undergoing hemodialysis twice a week and aged over 18 years. Patients who experienced a decrease in consciousness and memory disorders such as dementia were excluded from this study. The questionnaires used to measure variables in this study were the demographic data questionnaire, the HLS-EU-Q16 health literacy questionnaire, the MMAS-8 medication adherence questionnaire, and the KDQOL-36 quality of life questionnaire for chronic kidney disease patients. Results: The high health literacy group (N = 73) exhibited a markedly superior quality of life compared to those in the mid-level (N = 48) and low-level (N = 43) health literacy groups. Multiple regression analysis revealed that, of the three components of health literacy, health promotion has the greatest influence on patients' quality of life, contributing 25.1%, while the aspect of healthcare has the greatest influence on medication adherence, contributing 43.3%. Conclusions: Health literacy is essential in improving medication adherence and the patient's quality of life in a more optimal direction. Healthcare professionals should create plans for tailored and customized health information with the goal of providing patients with the knowledge about critical components they need to manage chronic kidney disease.

Abstract

The effectiveness of implementing tiered clinical supervision has a significant impact on improving nurses’ competency in nursing care delivery while also enhancing the safety and quality of care. This study aimed to explore nurses’ comprehension of tiered clinical supervision in optimizing the effective implementation of tiered clinical supervision at RSU 'Aisyiyah Padang. This study used a qualitative method with a phenomenological approach. Data collection was conducted through observation, document analysis, and in-depth, semi-structured interviews with 11 participants, namely nursing associates and three levels of nursing managers in the inpatient room. The interview data were transcribed, edited, coded, and analyzed thematically. The results showed that the understanding of 1) An insufficiently precise description in defining clinical supervision, with no evaluation element of this definition; 2) Supervision is considered to be best practice for all staff nurses both for new (novice) and senior (expert) nurses; 3) For nurses, the objectives and benefits of tiered clinical supervision include improving understanding, competence, skills, as well as confidence and self-assurance, thus preventing practice errors (malpractice). Also for patients, this clinical supervision has a significant benefit in patient safety improvement. It is important to provide training or educate staff related to tiered clinical supervision to enhance comprehension, in optimizing the effective implementation of tiered clinical supervision.

Abstract

Adherence to tuberculosis (TB) treatment is crucial for achieving cure and preventing drug resistance. Low adherence can lead to treatment failure and increase the risk of infection transmission. Therefore, assessing medication adherence in TB patients is necessary to determine the extent to which patients follow prescribed treatment regimens. This study aims to identify the level of medication adherence among pulmonary TB patients in Kota Lahat. A descriptive analytic design was used in this study. The population consisted of pulmonary TB patients undergoing treatment at five public health centers (PHC) in Lahat City between July 26 and August 5, 2024. A non-probability total sampling technique was employed. Medication adherence was measured using the Morisky Medication Adherence Scale (MMAS-8). Of the total respondents, 51% showed low adherence, 29% moderate adherence, and 20% high adherence. Demographic characteristics revealed that the majority of respondents were male (64.7%), with the largest age group being pre-elderly (42.7%). Most respondents had a bachelor's degree (40.7%) and were employed (70.7%). More than half of the respondents (63.3%) were smokers, with a smoking history of ≥10 years. The level of medication adherence among pulmonary TB patients remains low. Collaborative efforts are needed to enhance education and support for patients to improve adherence, thereby increasing treatment success.

Abstract

Diabetes mellitus is a chronic disease that can cause depression in sufferers. Depression is often associated with feelings of despair. This study aimed to investigate the relationship between hopelessness and depression among patients diabetes mellitus. This research is analytical research with a cross-sectional study approach. 122 people with type 2 diabetes mellitus at Pauh and Andalas Public health center were designated as samples in this study with inclusion criteria not having a history of mood disorders or mental disorders (based on medical diagnosis), aged 25 - 65 years and willing to sign an informed consent form. Data collection was conducted from September to October 2016 using the SDS (Self-rating Depression Scale) questionnaire and BHS (Beck Hopelessness Scale) questionnaire. Data were analyzed using the Chi-Square Test. There is a significant relationship between hopelessness and depression among diabetes mellitus patients (Pv = 0.001). Patients with diabetes mellitus who experience despair have a risk of 7.03 times depression compared to patients who do not experience despair.Despair in patients with diabetes mellitus occurs due to negative thoughts about the disease in the future, lack of motivation in controlling blood sugar and negative expectations of the disease condition. These feelings make the patient depressed. Based on this, education needs to be given regarding diabetes mellitus and its treatment so that hopelessness can be anticipated.
