Abstract

Daun pandan wangi (Pandanus amaryllifolius Roxb.) salah satu tumbuhan yang memiliki khasiat sebagai obat. Aksi farmakologis senyawa tersebut bervariasi dan mencakup kualitas antibakteri, antioksidan, antikanker, dan antidiabetes. Flavonoid, alkaloid, tanin, dan polifenol adalah beberapa komponen kimia yang diketahui terdapat dalam ekstrak daun pandan wangi. Beberapa di antaranya, seperti flavonoid dan tanin, memiliki sifat antidiare. Dalam penelitian ini, mencit yang diberikan minyak jarak (Oleum ricini) digunakan untuk menguji efek antidiare dari ekstrak etanol 96% daun pandan wangi. Metode penelitian ini menggunakan studi eksperimental. Hasil penelitian yang diperoleh menunjukkan bahwa pemberian ekstrak etanol daun pandan wangi dengan dosis 300 mg/kgBB, 600 mg/kgBB dan 1200 mg/kgBB mempunyai efek sebagai antidiare dalam penurunan konsistensi feses, frekuensi dan lama terjadinya diare pada mencit. Pada pengujian aktivitas antidiare ekstrak etanol 96% daun pandan wangi (Pandanus Amaryllifolius Roxb.) memiliki aktivitas antidiare terhadap mencit, dengan dosis yang efektif sebagai antidiare adalah dosis 1200 mg/kgBB.

Abstract

Sabun Antibiotik merupakan obat yang digunakan dalam mengobati infeksi bakteri. Namun, penggunaan yang tidak tepat dan tidak berdasarkan anjuran medis dapat menyebabkan resistensi antibiotik. Penelitian ini bertujuan untuk mengetahui hubungan antara tingkat pengetahuan masyarakat dengan penggunaan antibiotik yang rasional di Kabupaten Brebes. Penelitian dilakukan dengan pendekatan deskriptif kuantitatif menggunakan teknik random sampling terhadap 100 responden yang pernah menggunakan antibiotik. Data dikumpulkan menggunakan kuesioner tertutup dan dianalisis secara deskriptif serta uji chi-square. Hasil menunjukkan bahwa sebagian besar masyarakat memiliki pengetahuan yang baik (57%) dan penggunaan antibiotik yang cukup (81%). Uji statistik menunjukkan adanya hubungan signifikan antara pengetahuan dan penggunaan antibiotik (p = 0,006). Temuan ini mengindikasikan bahwa edukasi berkelanjutan dan pemahaman yang baik tentang antibiotik penting untuk mendorong penggunaan yang lebih rasional serta mengurangi risiko resistensi di masa depan.

Abstract

Kulit sering terpapar radikal bebas dari polutan dan sinar ultraviolet, yang dapat merusak struktur kulit. Kulit buah manggis (Garcinia mangostana L.) mengandung senyawa antioksidan seperti xantone yang berpotensi melindungi kulit. Lotion, sebagai aplikasi lokal, efektif dalam memberikan manfaat perawatan kulit. Penelitian sebelumnya menunjukkan bahwa lotion ekstrak kulit manggis memiliki aktivitas antioksidan yang baik, tetapi masalah daya sebar dan viskositas perlu diperbaiki. Penelitian ini bertujuan mengoptimalkan formulasi body lotion ekstrak kulit manggis untuk memperoleh stabilitas fisik dan manfaat yang maksimal. Penelitian ini menggunakan desain faktorial 2² dengan dua faktor: konsentrasi asam stearat (6–10%) dan trietanolamin (TEA) (2–3%). Empat formula (F1, F2, F3, F4) diuji berdasarkan variasi konsentrasi tersebut untuk optimasi formulasi. Formula optimum body lotion berbasis ekstrak kulit manggis terdiri dari asam stearat 10% dan TEA 2%, dengan konsentrasi ekstrak sebesar 1,5%. Formula F2 menunjukkan karakteristik fisik terbaik dengan hasil prediksi dan validasi yang sangat mendekati, yaitu pH 7,43, daya lekat 2,27 detik, daya sebar 6,5 cm, dan viskositas 17.440 cP.

Abstract

Pneumonia remains a major infectious disease contributing to significant morbidity and mortality, especially among adults and elderly patients. Antibiotic therapy as the main treatment for pneumonia must be rational to ensure therapeutic effectiveness, reduce antimicrobial resistance, and improve clinical outcomes. Objective: This study aimed to analyze the rationality of antibiotic use in hospitalized pneumonia patients at Universitas Gadjah Mada Academic Hospital during January–June 2025 based on the National Guidelines for Adult Pneumonia Management 2023. This study used a descriptive non-experimental design with a retrospective approach. A total of 92 patients were selected using purposive sampling. Rationality evaluation was conducted based on appropriate patient, indication, drug, dose, and route of administration. Most patients were aged >60 years (56.5%), male (53.3%), and had comorbidities (97.8%). Antibiotic use evaluation showed 100% appropriate patient selection, 100% appropriate indication, 95.9% appropriate drug selection, 93.8% appropriate dosing, and 100% appropriate route of administration. Overall, rational antibiotic use was found in 86 patients (93.5%), while 6 patients (6.5%) were categorized as irrational. Antibiotic use in pneumonia patients at RSA UGM was generally rational, although inappropriate drug selection and dosing remained issues requiring attention to optimize antibiotic therapy.

Abstract

Infeksi Candida albicans pada rongga mulut dapat menimbulkan ketidaknyamanan, sementara penggunaan antifungi sintetis jangka panjang berisiko menyebabkan efek samping dan resistensi. Daun sirih merah (Piper crocatum) mengandung metabolit sekunder seperti flavonoid, alkaloid, tanin, dan saponin yang berpotensi sebagai antifungi. Penelitian ini bertujuan memformulasi sediaan mouthwash spray ekstrak etanol daun sirih merah serta mengevaluasi aktivitas antifungi terhadap Candida albicans menggunakan metode difusi sumuran pada konsentrasi 5%, 10%, dan 15%. Evaluasi sediaan meliputi uji organoleptik, pH, viskositas, kejernihan, dan pola penyemprotan. Hasil penelitian menunjukkan bahwa seluruh formulasi memenuhi parameter evaluasi fisik yang meliputi uji organoleptik (warna, bau, dan bentuk), pH, viskositas, kejernihan, serta pola penyemprotan. Aktivitas antifungi ditunjukkan melalui zona hambat yang meningkat seiring kenaikan konsentrasi ekstrak, yaitu masing-masing sebesar 4,57 mm; 5,56 mm; dan 6,66 mm. Kontrol negatif tidak menunjukkan adanya aktivitas, sedangkan kontrol positif ketokonazol menghasilkan zona hambat sebesar 12,03 mm. Konsentrasi 15% menunjukkan hasil paling optimal dalam menghambat pertumbuhan Candida albicans dalam sediaan mouthwash spray. Hasil ini menunjukkan peningkatan aktivitas antifungi seiring kenaikan konsentrasi ekstrak. Ekstrak etanol daun sirih merah dapat dikembangkan sebagai bahan aktif mouthwash spray antifungi alami.

Abstract

Staphylococcus aureus is a pathogenic bacterium often associated with skin infections and various other diseases. The increasing prevalence of antibiotic resistance has prompted the search for alternative antibacterial agents derived from medicinal plants. Daun Itch (Laportea aestuans L. Chew), a plant from Eastern Indonesia, known locally as daun itch, contains bioactive compounds such as flavonoids, tannins, saponins, and terpenoids, which are reported to have antimicrobial potential. This study aims to evaluate the antibacterial activity of ethanol extract of daun itch against Staphylococcus aureus using the liquid dilution method. Daun itch was macerated using 70% ethanol for 5 days, phytochemical screening tests were carried out using two methods, namely Thin Layer Chromatography (TLC) and Tube Test. Observation of antibacterial activity by determining the Minimum Inhibitory Concentration (MIC) and Minimum Bactericidal Concentration (MBC) through a series of dilutions in Brain Heart Infusion (BHI) media. The results of phytochemical screening showed that the ethanol extract of daun itch contains flavonoids, saponins, tannins and terpenoids using TLC and Tube Test. Bacterial growth was assessed visually based on turbidity and confirmed by subculture on Mannitol Salt Agar (MSA). The MIC and MBC values ​​of the ethanol extract against Staphylococcus aureus were 300 mg/mL and 600 mg/mL, respectively. These concentrations indicate that the higher the concentration used, the greater the inhibitory effect on bacterial growth. These findings indicate that the extract of daun itch leaves has significant antibacterial activity against Staphylococcus aureus and can serve as a natural source of antibacterial agents. Further research is needed to isolate and characterize the active compounds that act as antibacterials.   ABSTRAK Staphylococcus aureus adalah bakteri patogen yang sering dikaitkan dengan infeksi kulit dan berbagai penyakit lainnya. Prevalensi resistensi antibiotik yang semakin meningkat telah mendorong pencarian agen antibakteri alternatif yang berasal dari tanaman obat. Daun Gatal (Laportea aestuans L. Chew), tanaman dari Indonesia Timur, yang dikenal secara lokal sebagai daun gatal, mengandung senyawa bioaktif seperti flavonoid, tanin, saponin, dan terpenoid, yang dilaporkan memiliki potensi antimikroba. Penelitian ini bertujuan untuk mengevaluasi aktivitas antibakteri ekstrak etanol daun gatal terhadap  Staphylococcus aureus menggunakan metode dilusi cair. Daun gatal dimaserasi menggunakan etanol 70% selama 5 hari, uji skrining fitokimia dilakukan dengan dua metode yaitu Kromatografi Lapis Tipis (KLT) dan Uji Tabung. Pengamatan aktivitas antibakteri dengan menentukan Konsentrasi Hambat Minimum (KHM) dan Konsentrasi Bunuh Minimum (KBM) melalui seri pengenceran dalam media Brain Heart Infusion (BHI). Hasil skrining fitokimia menunjukkan bahwa ekstrak etanol daun gatal mengandung senyawa flavonoid, saponin, tanin dan terpenoid menggunakan uji KLT dan Uji Tabung. Pertumbuhan bakteri dinilai secara visual berdasarkan kekeruhan dan dikonfirmasi dengan subkultur pada Mannitol Salt Agar (MSA). Nilai KHM dan KBM ekstrak etanol terhadap Staphylococcus aureus adalah 300 mg/mL dan 600 mg/mL. Konsentrasi tersebut menunjukkan semakin tinggi konsentrasi yang digunakan maka efek penghambatan pertumbuhan bakteri juga akan meningkat.  Temuan ini menunjukkan bahwa ekstrak daun daun gatal memiliki aktivitas antibakteri yang signifikan terhadap Staphylococcus aureus dan dapat berfungsi sebagai sumber alami agen antibakteri. Penelitian lebih lanjut diperlukan untuk mengisolasi dan mengkarakterisasi senyawa aktif yang berperan sebagai antibakteri.

Abstract

Diabetes mellitus remains a major global health burden, leading to growing interest in herbal formulations as complementary antidiabetic therapies. This study aimed to elucidate the synergistic antidiabetic mechanism of the traditional Kunyit–Asam herbal formulation by integrating LC-HRMS-based metabolomics, in vitro INS-1 pancreatic β-cell assays, and molecular docking analysis. Metabolomic profiling using LC-HRMS identified a diverse array of bioactive secondary metabolites, predominantly phenolic acids, flavonoids, and curcuminoids, which are associated with antidiabetic and antioxidant activities. Functional evaluation in INS-1 cells demonstrated that the Kunyit–Asam extract significantly enhanced glucose-stimulated insulin secretion (GSIS) and improved β-cell viability, indicating insulinotropic and cytoprotective effects. Furthermore, molecular docking analysis revealed that several identified metabolites exhibited strong binding affinities toward key diabetes-related molecular targets, including α-glucosidase, α-amylase, AMP-activated protein kinase (AMPK), insulin receptor substrate-1 (IRS-1), phosphoinositide 3-kinase (PI3K), and GLUT4 regulatory proteins. Integration of metabolomic, biological, and in silico findings suggests that the antidiabetic activity of Kunyit–Asam is mediated through multi-target modulation of insulin signaling pathways, inhibition of postprandial glucose-regulating enzymes, and protection against oxidative stress. These results support a synergistic pharmacological mechanism driven by multiple phytoconstituents rather than a single dominant compound. This study provides scientific evidence supporting the traditional use of Kunyit–Asam as an antidiabetic herbal formulation and offers valuable insights for future herbal-based drug development.

Abstract

Ensuring the continuous availability of critical medicines in remote island regions remains a major challenge in decentralized health systems. This study examines the pharmaceutical supply chain in the remote islands of Maluku Province, Indonesia, through a supply chain mapping and vulnerability analysis using a System Dynamics (SD) approach. The findings indicate that fragmented governance structures, combined with extreme geographical dispersion, generate persistent vulnerabilities across procurement, distribution, and service delivery levels. Key reinforcing mechanisms driving medicine stockouts include delayed quantification and procurement processes, logistical bottlenecks in inter-island transportation, and shortages of skilled pharmaceutical personnel. The analysis further shows that interventions focused solely at the health facility level are insufficient to resolve systemic weaknesses. Instead, strengthening medicine security in Maluku requires integrated strategies at higher administrative levels, including investment in human capital, diversification of supply routes, strategic buffer management, and the use of transparent, data-driven logistics information systems. By adopting a systems-based analytical framework, this study provides evidence to support more resilient policy design for improving equitable access to essential medicines in Eastern Indonesia.

Abstract

Multidrug-resistant Pseudomonas aeruginosa poses a significant challenge in chronic wound management due to its robust biofilm formation and antibiotic resistance. Green-synthesized silver nanoparticles (AgNPs) using Tamarindus indica (tamarind) leaf extract offer an eco-friendly and effective approach to combat these infections. The bioactive phytochemicals in tamarind leaves function as both reducing and stabilizing agents, producing biocompatible AgNPs with potent antimicrobial and antibiofilm activities. These nanoparticles disrupt bacterial membranes, generate reactive oxygen species, inhibit virulence factors such as pyocyanin, and compromise biofilm integrity. Incorporation of tamarind-derived AgNPs into hydrogels or composite wound dressings further enhances wound healing by maintaining a moist environment and providing sustained antimicrobial release. Although in vivo studies specifically using tamarind AgNPs against P. aeruginosa are limited, evidence from other plant-derived AgNPs supports their promising potential for treating biofilm-associated wound infections. Green synthesis using tamarind thus represents a sustainable strategy for developing next-generation antimicrobial wound therapies.

Abstract

Losartan potassium, a Biopharmaceutics Classification System (BCS) Class II drug, exhibits poor aqueous solubility and limited oral bioavailability. This study aimed to develop and optimize a Self-Nanoemulsifying Drug Delivery System (SNEDDS) to enhance its dissolution and absorption. SNEDDS formulations were prepared using Capmul MCM C8, Tween 80, and PEG 400 based on solubility screening and ternary phase diagram analysis. The optimized formulation (F3) was evaluated for droplet size, polydispersity index (PDI), in vitro drug release, pharmacokinetics, and stability. Pharmacokinetic studies were conducted in experimental rats Pharmacokinetic studies were conducted in experimental rats (n = 3 per group) to assess bioavailability. The optimized SNEDDS produced nanoemulsions with a droplet size of 68.3 nm, low PDI, and good dilution stability. In vitro studies demonstrated rapid drug release (>96% within 30 minutes), significantly higher than the plain suspension. Pharmacokinetic results showed a 3.09-fold increase in bioavailability and a 2.79-fold increase in Cmax without altering Tmax. Stability studies confirmed good physicochemical stability over six months under ICH conditions. In conclusion, SNEDDS is a promising strategy to improve the oral bioavailability of poorly water-soluble drugs such as losartan potassium.
