Abstract

Central nervous system (CNS) infections such as encephalitis, meningitis, or myelitis have high morbidity and mortality in Indonesia. High sensitivity C-reactive protein (hs-CRP) is a sensitive marker of acute inflammation, while albumin is the most abundant protein component in plasma and cerebrospinal fluid (CSS). Infection triggers an inflammatory response so that an increase in the hs-CRP/albumin ratio (CAR) can be a predictor of worsening clinical outcome in patients with CNS infections. However, studies examining the predictor value of serum and CSF CAR on worsening clinical outcomes of patients are limited, particularly in CNS infections. The purpose of this study was to prove the CAR as a predictor of worsening clinical outcome in patients with CNS infections.  It was an observational study using a prospective cohort design. Fifty subjects recruited until October 2021 at Dr. Sardjito General Hospital were involved.  The multivariate regression analysis showed that serum CAR (OR=3.604; 95%CI=1.487-8.736; p =0.005) could be a single predictor. However, by combining three variables, namely serum CAR, CSF CAR, and decreased consciousness at admission, could be a stronger predictor of worsening clinical outcome in patients with CNS infection (AUC = 97.1%; 95%CI = 0.929-1.00; p <0.001). The optimal cut-off value for serum CAR was 1.35 (Youden index = 0.88, sensitivity = 96%, specificity = 92%) while for CSF CAR was 0.14 (Youden index = 0.60, sensitivity = 76%, specificity = 84%). In conclusion, a combination predictive model of three variables, namely serum CAR, CSF CAR, and awareness at admission can be a stronger predictor of clinical outcome in patients with CNS infection than serum CAR alone.

Abstract

Introduction: Dysmenorrhea can reduce school productivity and quality of life in adolescent girls. Cat Stretch exercises can reduce pain but have never been taught to junior high school students at SMP Negeri 1 Gondangrejo Karanganyar. Purpose: The purpose of this research is to determine how giving stretch exercise paint reduces dysmenorrhoea in junior high school students. Method: The type of research used is quantitative with a Quasi-Experimental research design with pretest and posttest with group control. Researchers measured pain at the beginning, then provided intervention for one month and measured dysmenorrhea pain again during the next menstruation. The purposive sampling technique obtained a sample size of 50 junior high school female students, of which 25 were in the intervention group and 25 were in the control group. The data analysis technique used was the Independent T-Test. Results: The average pre-test intensity of menstrual pain in the intervention group was 4.04, decreasing to 2.72 during the post-test. There was a significant difference in students before and after the cat stretching intervention (p=0.000). In contrast, the control group showed an average pain intensity of 3.56 in the pre-test and almost did not decrease during the post-test, namely 3.52 with p=0.327. Testing for pain reduction in the intervention group was 1.32 while the control group was 0.04 with significant difference results (p-value 0.014). Conclusion: Providing cat stretch exercises can reduce dysmenorrhoea pain in female students. Therefore, young women can use stretch paint exercises to reduce dysmenorrhoea non-pharmacologically. Abstract

A community-based tourism (CBT) approach has become an integral part of rural and sustainable tourism development strategies. However, CBT has some constraints on being sustainable due to some internal weaknesses and external concerns, including poor marketing competencies. As a top-down government initiative, Malaysian CBT homestay is blessed with numerous assistance to ensure its survival which in the other hand, creates over-reliance toward other parties. This lack of marketing expertise challenges homestay sustainability. This study examines the importance of community education in marketing activities as a lifelong learning towards sustainable tourism in Malaysia. Qualitative case study is employed to gain an extensive and in-depth explanation on homestay's marketing implementation and its issues. A semi-structured interview was taken to 9 homestay operators. This study employs a thematic data analysis to investigate the interview results. Research findings show that in majority, the community enjoys the comfort zone provided by government and travel agents, hence encourage the resistance for change. However, a few homestay demonstrates positive respond to involve lifelong learning through community education to support sustainable tourism. More importantly, partnership between various key stakeholders are the utmost important to ensure policy planning and to improve community education quality.

Abstract

Plastic waste is a pollution problem in coastal areas, and this issue has become a global concern that requires serious attention, especially for archipelagic countries like Indonesia. This community service program aims to reduce plastic pollution at Elak-Elak Beach, West Lombok, and raise environmental awareness among the local community. Conducted in September 2023, the program involved the participation of nearly 200 students and 12 lecturers from the Marine Science Program at the University of Mataram, along with several local community members. Through scheduled beach clean-up activities, the program successfully collected approximately 10 bags, equivalent to 50-70 kg of waste, with plastic dominating the types of waste found. In addition to waste reduction efforts, the program also indirectly educated the local community about the impacts of plastic pollution on marine ecosystems, successfully increasing participants' awareness after the activities. Although the program showed positive results, challenges remain in waste management and the lack of adequate facilities. Therefore, ongoing collaboration between the community, government, and relevant organizations is essential to preserve Elak-Elak Beach and educate the public on the importance of proper environmental management.
