Abstract

One innovative nutritional therapy approach for diabetic mellitus (DM) is modulating the gut microbiota by administering probiotics. Soygurt is a probiotic functional food made from fermenting soy milk with lactic acid bacteria (LAB), which includes bioactive peptides and antioxidants. The use of soybean, mungbean, and date juice as sweeteners is expected to increase the product's physicochemical quality as well as the presence of lactic acid bacteria, maximizing its benefits as a functional meal for DM. This study was an experiment that used a completely randomized design with three treatment formulas comparisons of soybeans and mungbean's (% : %) as follows F1 (25:75), F2 (50:50), and F3 (75:25). Protein content was determined using the biuret method, fat content was identified using the mojonieur method, the reduced sugar content was analyzed using the Nelson Smogyi method, crude fiber content was identified using the gravimetric, and LAB were determined using the Total Plate Count (TPC). The best formula was continued for clinical trials on animals. The result of F3 produced protein of 5.46%, fat of 2.36%, reduced sugar of 4.66%, crude fiber of 3.81%, and a pH of 4.139. The lactic acid bacteria (LAB) count in F1 was 1.9 x 108 CFU/mL. It concluded that soygurt mung bean with date juice can potentially control blood glucose levels due to its high protein and low-fat content.

Abstract

Diabetes mellitus is a metabolic disorder characterized by high blood glucose (hyperglycemia) caused by damage or abnormalities in insulin production. Pharmacological therapy used as a treatment for diabetes mellitus in some patients has adverse side effects on the body, thus encouraging exploration to use natural ingredients as an alternative therapeutic source in reducing blood glucose. Therefore, a thorough review is needed to summarize scientific data and investigate the potential use of natural ingredients bekul fruit (Ziziphus jujube Mill.) in its role as an antidiabetic agent. This study used a scoping review with 4 databases: Pubmed, ScienceDirect, DOAJ and Google Scholar. The results of the scoping review of 5 scientific articles found that bekul fruit (Ziziphus jujube Mill.) is proven to have bioactive content that is rich in antioxidants such as phenolic/polyphenolic compounds (ferulic acid, caffeic acid, chlorogenic acid), flavonoids (quercetin, rutin, catechin), phytosterols, and inulin which have the potential as antidiabetic agents with different mechanisms of action in reducing blood glucose levels. Conclusion: bekul fruit (Ziziphus jujube Mill.) has the potential to be an alternative nutraceutical diet therapy in patients with diabetes mellitus.

Abstract

Analisis data menggunakan uji One-way Anova.

Abstract

This study aimed to determine the efficacy of combination of sappan (secang) wood and gotu kola extracts in reducing insulin resistance and Malondialdehyde (MDA) levels in diabetic rats induced by Streptozotocin (STZ) 65 mg/kg Body Weight (BW) and Nicotinamide (NA) 230 mg/kg BW. Forty-two male Sprague Dawley rats weighing ±200 g were divided into 7 groups: 1) control, 2) glibenclamide 0.45 mg/kg BW, 3) sappan wood extract (CS) 250 mg/kg BW, 4) gotu kola extract (CA) 500 mg/kg BW, 5) 1st combination of extracts of sappan wood and gotu kola (CSCA1) 125 mg/kg BW + 750 mg/kg BW, 6) 2nd combination (CSCA2) with 250 mg/kg BW + 500 mg/kg BW, and 7) 3rd combination (CSCA3) with 375 mg/kg BW + 250 mg/kg BW. The insulin resistance levels were measured using the HOMA-IR index based on fasting blood glucose and insulin. The Thiobarbituric Acid Reactive Substance (TBARs) method was used to measure MDA levels. All measurements were taken before treatment, 14 days after treatment, and 21 days after treatment. The group receiving CSCA3 showed significant reduction in insulin resistance (-3.32±0.05) and MDA levels (-2.04±0.37 nmol/ml) on Day 21 after treatment. The CSCA3 treatment did not show statistically different result compared to glibenclamide treatment (p>0.05). Hence, CSCA3 treatment was considered as the best proportion of sappan wood and gotu kola extracts mixture and the result is comparable to glibenclamide. This study shows that the combination of sappan wood and gotu kola extracts has the potential to be developed as a functional drink for people with diabetes.

Abstract

Background:  Type 2 diabetes (T2DM) has a significant impact on patients' quality of life (QoL), including physical, psychological, social, and economic aspects. However, a culturally and linguistically adapted Diabetes Quality of Life Brief Clinical Inventory (DQoL-BCI) for Indonesian patients remains limited. A validated Indonesian version of this instrument is necessary to ensure accurate assessment and effective interventions for diabetes management. Objective: This study aimed to assess the validity and reliability of the Indonesian version of the Diabetes Quality of Life Brief Clinical Inventory (DQoL-BCI) among patients with T2DM in primary care. Methods: A cross-sectional study was conducted among 30 T2DM patients at a primary health care center in Singkawang City, West Borneo, Indonesia. Data were collected through interviews. The DQoL-BCI, a 15-item questionnaire, was used to measure QoL, with responses rated on a 5-point Likert scale. Construct validity and internal consistency were assessed using Pearson’s r and Cronbach’s alpha. Results: All items showed Pearson’s r values greater than 0.361, indicating an adequate correlation with the overall scale and thus validating the items. The highest Pearson’s r values were for satisfaction with the time to determine sugar levels (0.688) and sex life (0.680). The Cronbach’s alpha coefficient for all items was 0.887, exceeding the minimum requirement of 0.70, demonstrating excellent internal consistency. Conclusion: The Indonesian version of the DQoL-BCI is a valid and reliable instrument for measuring the QoL in individuals with T2DM in primary care settings.

Abstract

Background: Proprotein convertase subtilisin/kexin type 9 (PCSK9) inhibitors represent a novel class of medications for managing dyslipidemia. Although previous meta-analyses have confirmed their efficacy in lowering low-density lipoprotein cholesterol (LDL-C), few have evaluated their effects on broader lipid parameters. Moreover, most studies focus on the general dyslipidemic population, provided limited insight into specific subgroups. This study specifically investigated the effects of PCSK9 inhibitors on multiple lipid parameters in individuals with dyslipidemia who were statin-intolerant, statin-resistant, or required intensified lipid-lowering treatment. Methods: This study systematically searched PubMed, ScienceDirect, and the Cochrane Library for phase 3 randomized controlled trials (2013-2023), evaluating PCSK9 inhibitors against placebo or non-statin standard care in dyslipidemic patients aged ≥18 years. The main outcome was the changes from baseline in lipid parameters. Random-effects meta-analyses were conducted using RevMan. Results: Eight studies involving 2,343 participants met eligibility criteria. PCSK9 inhibitors significantly reduced LDL-C (MD -46.8, 95% CI [-53.2; -40.4]), non-HDL-C (MD -41.1 [-46.9; -35.3]), total cholesterol (MD -31.5 [-37.8; -25.2]), triglycerides (MD -11.7 [-15.0; -8.4], Lp(a) (MD -19.2 [-25.7; -12.6]), and ApoB (MD -39.4 [-45.0; -33.7]). PCSK9 inhibitors also significantly increased HDL-C (MD 6.3 [4.7; 7.9]) and ApoA-I (MD 4.1 [2.8; 5.5]).Conclusion: PCSK9 inhibitors significantly improve a broad spectrum of lipid parameters, including non-traditional markers such as non-HDL-C, ApoA-I, ApoB, and Lp(a), underscoring their potential role in managing dyslipidemia, particularly in patients inadequately controlled with standard therapies.
