Abstract

Sampah merupakan salah satu penyebab terjadinya polusi udara di Kota Palembang. Setiap hari terjadi peningkatan volume sampah yang harus diolah, sedangkan kondisi pelayanan pengelolaan sampah baru mencapai sekitar 70% dari total sampah secara keseluruhan. Pengolahan sampah yang dilakukan menggunakan sistem open dumping berdampak terhadap peningkatan terjadinya polusi udara. Gas hidrogen sulfida (H2S) yang diperoleh dari proses penguraian senyawa organik oleh bakteri anaerob pada tumpukan sampah dapat mengganggu kesehatan, terutama sistem pernapasan. Penelitian ini bertujuan untuk menganalisis hubungan antara karakteristik (usia, jenis kelamin, status gizi, merokok, dan jarak tempat tinggal) dan besaran risiko konsentrasi H2S udara ambien terhadap kapasitas vital paru penduduk di sekitar tempat pembuangan akhir (TPA) sampah. Penelitian ini menggunakan desain potong lintang dengan sampel 78 orang penduduk yang berada di sekitar TPA. Analisis data menggunakan uji regresi logistik ganda. Hasil menunjukkan nilai status gizi (nilai p = 0,022, OR = 12,085) dan RQ (nilai p = 0,016; OR = 7,547) berhubungan secara signifikan terhadap kapasitas vital paru penduduk yang berada di sekitar TPA. Penduduk yang berada di sekitar TPA dengan nilai status gizi buruk dan nilai RQ lebih rendah dari median berpotensi mengalami gangguan kapasitas vital paru. Variabel dominan yang secara signifikan memengaruhi kapasitas vital paru penduduk yang berada di sekitar TPA sampah Sukawintan adalah status gizi. Waste is one of factors causing air pollution in Palembang City. Volume of waste that should be processed increases every day, while condition of waste management service is still 70% of the total volume. The waste processing is managed by using open dumping system, which affects on the increase of air pollution. One of gases that exist as effect of the process of organic compound decomposition of anaerobic bacteria from garbage is sulfide hydrogen (H2S) pollutant which can promote health disorders, especially respiratory system. This study aimed to analyze correlation between characteristics (age, sex, nutritional status, smoking, and living distance) and the risk quotient of sulfide hydrogen concentration in air ambient to the lung capacity of people around landfill area. This study used cross-sectional design with the sample of 78 people around landfill area. Data analysis used double logistic regression. Results showed that nutritional status (p value = 0.022, OR = 12.085) and RQ (p value = 0.016; OR = 7.547) significantly related to lung vital capacity of people around landfill area. People around landfill area having worse nutrition and lower RQ than the median were potential to have lung vital capacity disorder. The dominant variable significantly influencing to lung vital capacity of people living around Sukawinatan Landfill is nutritional status.

Abstract

Pada saat kehamilan, terjadi peningkatan kebutuhan zat besi (Fe) yang diperlukan untuk meningkatkan jumlah sel darah merah ibu dan membentuk sel darah merah janin dan plasenta. Apabila kebutuhan zat besi ini tidak terpenuhi, akan terjadi anemia defisiensi besi. Ibu hamil yang tinggal di daerah endemis malaria rentan terinfeksi malaria, yang berdampak terhadap anemia pada kehamilan. Penelitian ini merupakan penelitian deskriptif dengan desain potong lintang yang bertujuan untuk mengetahui gambaran profil zat besi pada ibu hamil yang berada di daerah endemis malaria vivax kota Bengkulu. Pemeriksaan mikroskopis malaria dan pengambilan sampel darah dilakukan untuk pemeriksaan profil zat besi terhadap 55 orang ibu hamil trimester II dan III yang mempunyai riwayat malaria di 5 wilayah kerja puskesmas. Data yang diperoleh kemudian dilakukan analisis univariat. Pada ibu hamil yang tidak terinfeksi malaria tetapi mempunyai riwayat malaria, 90,5% mengalami anemia, 41,5% mengalami penurunan kadar Total Iron Binding Capacity (TIBC), dan 17% mengalami penurunan kadar Fe serum. Ibu hamil yang terinfeksi malaria vivax selain terjadi penurunan kadar hemoglobin juga disertai dengan penurunan kadar Fe serum dan kadar TIBC. Pada ibu hamil yang menderita malaria falsiparum, terjadi penurunan kadar hemoglobin (Hb) dan kadar TIBC tetapi tidak disertai dengan penurunan kadar Fe serum. Ibu hamil yang terinfeksi malaria maupun yang mempunyai riwayat terinfeksi malaria mengalami anemia pada kehamilan. During pregnancy, the need of iron increase significantly. Iron is needed for increasing the amount of mother’s red blood cell and form the red blood cell of fetus and placenta. If this iron needs is not fulfilled, it could cause iron deficiency anemia. Pregnant women who live in endemic malaria will be vulnerable to be infected malaria, and will cause anemia in pregnancy. This research was aimed to know iron profile in pregnant women that live in endemic malaria area. Malaria microscopic examination and blood sampling for examination of iron profile in 55 pregnant women trimester II and III who have a history of malaria in 5 working area health centers. The data obtained was then performed univariate analysis. The results showed pregnant women who have malaria history, 90.5% are suffering anemia, decreasing of Total Iron Binding Capacity (TIBC) level (41.5%) and 17% Fe serum level (17%). Pregnant women who are infected vivax malaria not only experiencing decrease of hemoglobin level but also experiencing decrease of Fe serum level and TIBC level, while pregnant woman who suffers from falciparum malaria haemoglobin (Hb) level and TIBC are decreasing but not in serum Fe levels. Pregnant women with history of malaria infection will suffer from anemia during pregnancy.

Abstract

Ibu hamil yang berada di daerah endemis malaria sangat rentan terhadap infeksi malaria selama kehamilan. Gejala malaria pada kelompok ini sering asimptomatik atau bahkan tidak terdeteksi sama sekali karena adanya efek imunitas protektif melalui infeksi yang berulang. Adanya peningkatan kadar tumor necrosis factor-alpha (TNF-α) dapat dijadikan indikator terjadinya infeksi malaria. TNF-α berperan penting dalam respons imun pada malaria akut yang menghambat terjadinya eritropoesis. Penelitian ini bertujuan untuk mengetahui hubungan antara kadar TNF-α dengan kejadian anemia pada ibu hamil didaerah endemik malaria vivax. Penelitian ini menggunakan desain potong lintang, dilakukan pada bulan Januari - Februari 2014 di lima wilayah kerja puskesmas Kota Bengkulu. Sampel penelitian adalah ibu hamil di daerah endemis malaria vivax yang diambil secara accidental sampling. Dilakukan pengambilan darah untuk pemeriksaan mikroskopis malaria, kadar TNF-α dan kadar hemoglobin (Hb). Hasil penelitian menunjukkan seluruh ibu hamil memiliki riwayat pernah terinfeksi malaria vivax, walaupun hasil pemeriksaan slide negatif. Terjadi peningkatan kadar TNF- α dengan rerata 6,90 ± 2,48 pg/mL dan penurunan kadar Hb dengan rerata 9,75 ± 0,88 g%. Uji korelasi Spearman didapatkan korelasi negatif yang kuat (r = -0,734) dan bermakna (nilai p < 0,05) antara Kadar TNF-α dengan kadar Hb. Terdapat hubungan yang bermakna antara kadar TNF-α dengan kejadian anemia. Pregnant mothers living in malaria - endemic area are very susceptible to malaria infection during pregnancy. Malaria symptoms in this group are often asymptomatic or even not detected at all due to protective immunity effect through repeated infections. Any elevation of tumor necrosis factor-alpha (TNF-α) level can be used as indicator of malaria infection. TNF-α takes an important role in immune response on acute malaria that hinders occurence eritropoesis process. This study aimed to find out relations between TNF-α level and anemia occurrence among pregnant women living in malaria vivax - endemic areas. The study used cross-sectional design conducted on January to February 2014 in five working areas in Bengkulu city. Sample of study was pregnant mothers in malaria vivax - endemic areas which was taken through accidental sampling. Blood was taken for malaria-microscopic examination, TNF-α and haemoglobine (Hb) level. The results showed that all of pregnant mothers have malaria vivax - infected record, although slide examination showed negative result. Any TNF-α level elevation with average 6.90 ± 2.48 pg/mL and decrease of Hb level with average 9.75 ± 0.88 g%. Spearman correlation test showed strong negative correlation (r = -0.734) and significant (p value < 0.05) between TNF-α level and Hb level. There was significant relation between TNF-α level and anemia occurrence.

Abstract

Adolescents experience physiological and psychological growth to establish the basis for maturity. The biological well-being of adolescents requires improved nutrition. Intake of foods rich in micronutrients such as iron, zinc, and vitamin A, which are relevant to supporting biological metamorphosis in adolescents, can influence growth and well-being. The aim of this study is to holistically identify the main risk factors affecting adolescents that contribute to anemia in developing country developing.A systematic review was conducted following the guidelines provided by the Preferred Reporting Items for Systematic Reviews and Meta-Analysis (PRISMA). The current review defines adolescents as individuals ranging in age from 10 to 19 years. A total of 2,252 articles were retrieved from the database, of which 618 duplicates were removed and 1,492 records were issued after filtering by title and abstract. 142 articles were fully assessed for notability, and 127 articles were also excluded based on inclusion and exclusion criteria. Finally, 15 studies were included in the systematic review. The review showed that food intake practices, parasite infections, menstruation, the increasing age of adolescent girls, and the low educational status of guardians are the main risk factors for iron deficiency anemia among adolescents in most developing countries.

Abstract

One of the aspects which describes the services’ quality at the Health Center is reflected by the result of human resources’ performance as the service implementer. Many factors influence the employee’ performance at the Health Center in which among them are workload, motivation, leadership and Occupational Safety and Health (K3) The purpose of this study is to analyze factors that influence employee performance in community health centers. This article aims to review the available literature about the influence of the workload, motivation, leadership, and Occupational Safety and Health (K3) on the health centers employee’ performance. The methods used are Systematic Literature Review using channel Preferred Reporting Items for Systematic Literature Review (PRISMA) by collecting articles from Google Scholar and PubMed databases with the keywords workload, motivation, leadership, and Occupational Safety and Health (K3). From the analysis results, 17 relevant articles are selected with the relevant criteria. The conclusion is that workload, motivation, leadership, and Occupational Safety and Health (K3) have a significant influence on health centers employee’ performance.

Abstract

Correlation between fluid intake, potassium, sodium, and interdialytic weight gain (IDWG) in routine hemodialysis patientsBackground: Chronic kidney failure patients undergo hemodialysis regularly to help kidney function. One of the problems that often occurs in patients with chronic kidney failure when undergoing hemodialysis is an increase in interdialytic weight gain (IDWG).Objective: This study aims to determine the correlation between fluid, potassium, and sodium intake with IDWG in hemodialysis patients at RSUP Dr. Mohammad Husein Palembang.Method: This study employs a quantitative approach with a cross-sectional design. The sampling technique used was purposive sampling, resulting in 62 respondents who met the inclusion and exclusion criteria. The data collected included the characteristics of the respondents, as assessed through a questionnaire. Additionally, data on fluid, potassium, and sodium intake were collected using a 2x24-hour food recall form. Body weight data were measured before and after hemodialysis using digital scales. Bivariate analysis was carried out using the Spearman correlation test.Results: There was a significant correlation between fluid intake (p-value = 0.022) and sodium intake (p-value = 0.049) with IDWG, with a positive correlation direction, indicating a relationship in the same direction. The correlation was sufficient for fluids with IDWG, but weaker for sodium with IDWG. There was no relationship between potassium intake (p-value = 0.062) and IDWG.Conclusions: The unexpected increase in IDWG in hemodialysis patients is related to fluid and sodium intake; therefore, the higher the sodium and fluid intake, the greater the increase in IDWG.

Abstract

Status gizi merupakan faktor penting pada masa kehamilan yang berpengaruh terhadap kesehatan ibu hamil, perkembangan embrio dan janin. Ibu yang sedang hamil harus mengurangi beban kerja yang terlalu berat karena akan memberikan dampak kurang baik terhadap kehamilanya. Apabila pekerjaan ibu berat maka asupan gizi yang dikonsumsi juga lebih sedikit dikarenakan ibu stres dan pekerjaan menjadi beban yang menyebabkan menurunnya nafsu makan begitu juga sebaliknya, sehingga asupan gizi ibu hamil akan mempengaruhi status gizi ibu selama kehamilan. Tujuan artikel ini yaitu mengkajikan literatur terkait status gizi dan kesehatan keselamatan kerja pada ibu hamil yang bekerja di rumah sakit. Penelitian ini menggunakan metode Literature Review melalui Google Scholar dan Doaj. Hasil penelitian menunjukkan bahwa ibu hamil yang bekerja akan mendapatkan pendapatan tambahan yang mempengaruhi pembelian pangan untuk kebutuhan gizi, ibu hamil yang bekerja di rumah sakit tentunya juga mempunyai pengetahuan yang lebih mengenai gizi, sehingga ibu hamil yang bekerja mengetahui asupan gizi yang akan dikonsumsi serta dapat mengonsumsi makanan yang lebih bervariasi dan bergizi. Sedangkan Ibu hamil yang bekerja di rumah sakit lebih rentan terhadap bahaya pekerjaan tertentu, seperti paparan radiasi, bioagen menular, angkat berat dan jam kerja yang berkepanjangan karena dampaknya terhadap perkembangan janin. Oleh karena itu, diperlukannya peraturan atau kebijakan yang sesuai untuk meningkatkan kenyamanan karyawan yang sedang hamil dan membantu mereka menangani stres terkait pekerjaan serta menjamin kesehatan dan keselamatan kerja ibu hamil agar tidak berpengaruh terhadap kondisi janin ataupun status gizi hamil.

Abstract

Based on Disability Adjusted Life Years (DALYs), there are four main disease burdens, namely ischemic heart disease, cerebrovascular disease (including stroke and hypertension), diabetes and tuberculosis infectious disease. Gestational hypertension is very closely related to CVD (cardiovascular disease) with the potential for preeclampsia to have an impact on premature birth or small babies for the gestational age, stillbirths, low birth weight babies, etc. The research objective was to analyze the risk factors for gestational hypertension and preeclampsia. Methods: This study is a literature review based on the PRISMA guidelines. using electronic databases namely Google scholar, Sciencedirect and Pubmed with a period from 2017 to 2022 with the keywords "Hypertension, risk factors, high risk pregnancy". Results: The prevalence of hypertension and pre-eclampsia varies between countries. In Bangladesh, the prevalence of pre-eclampsia: 14.4%, without history of hypertension, 5.4% prevalence of pre-eclampsia with chronic hypertension, the same as Ethiopia. The incidence of gestational hypertension is 6%, in Ireland, gestational hypertension is 5.9% and 4.6% of pre-eclampsia. In France hypertension was 7.4% and pre-eclampsia was 2.0%, gestational hypertension was 4.2%, HELLP syndrome was 10.4%. Hypertension is associated with risk factors for pregnancy, lifestyle and sociodemographic factors. Conclusion: Prevention, timely diagnosis and prompt management of hypertension in pregnancy should be intensified so as to reduce morbidity and severity of adverse birth outcomes

Based on Disability Adjusted Life Years (DALYs), there are four main disease burdens, namely ischemic heart disease, cerebrovascular disease (including stroke and hypertension), diabetes and tuberculosis infectious disease. Gestational hypertension is very closely related to CVD (cardiovascular disease) with the potential for preeclampsia to have an impact on premature birth or small babies for the gestational age, stillbirths, low birth weight babies, etc. The research objective was to analyze the risk factors for gestational hypertension and preeclampsia. Methods: This study is a literature review based on the PRISMA guidelines. using electronic databases namely Google scholar, Sciencedirect and Pubmed with a period from 2017 to 2022 with the keywords "Hypertension, risk factors, high risk pregnancy". Results: The prevalence of hypertension and pre-eclampsia varies between countries. In Bangladesh, the prevalence of pre-eclampsia: 14.4%, without history of hypertension, 5.4% prevalence of pre-eclampsia with chronic hypertension, the same as Ethiopia. The incidence of gestational hypertension is 6%, in Ireland, gestational hypertension is 5.9% and 4.6% of pre-eclampsia. In France hypertension was 7.4% and pre-eclampsia was 2.0%, gestational hypertension was 4.2%, HELLP syndrome was 10.4%. Hypertension is associated with risk factors for pregnancy, lifestyle and sociodemographic factors. Conclusion: Prevention, timely diagnosis and prompt management of hypertension in pregnancy should be intensified so as to reduce morbidity and severity of adverse birth outcomes.
