Abstract

Background: Parents of newly diagnosed cancer children often experience significant concerns about the implications of the diagnosis. They find themselves in challenging decision-making situations, navigating through uncertain conditions following the disclosure. Uncertainty is a prevailing issue in healthcare, yet its interpretation and application within the context of health and nursing practice exhibit multiple meanings and variations.   Objective: This study aimed to clarify the concept of parental uncertainty arising from having children with cancer.                                                              Methods: The concept analysis followed the steps outlined by Walker and Avant. A thorough literature search was conducted using the Embase, CINAHL, and MEDLINE databases to identify articles published between 1980 and 2022. Results: The literature search yielded 147 articles from various disciplines, including health, nursing, medicine, psycho-oncology, and management. After analysis, 20 articles were selected for inclusion. This study identified four key attributes of parental uncertainty associated with having a child with cancer: (1) emotional disturbance, (2) unpredictability, (3) insufficient information, and (4) problems with decision-making. The antecedents of parental uncertainty encompassed the complexity of the disease, the diagnostic and treatment processes, the hospital environment, communication, and support from credible authorities. The consequences of parental uncertainty included the pursuit of new information, parental adaptability to their role, and parental distress. Conclusion: Understanding the four attributes of parental uncertainty, along with their antecedents and consequences, can facilitate the future development and operational translation of the concept of uncertainty in nursing practice. This concept is valuable in the evidence-based provision of nursing care to parents of children with cancer, enabling comprehensive recognition and assessment of parental uncertainty and the implementation of appropriate interventions that are sensitive to parental experiences. Ultimately, this approach promotes the adaptation of both the child and the parent.

Abstract

Background: Comfort level in breast cancer patients is one of the most critical factors influencing patient’s treatment commitment. The COVID-19 pandemic has affected various sectors, especially health status. Objectives: This study aims to describe patients' comfort levels during the COVID-19 pandemic and to investigate the relationship between patients’ characteristics and comfort levels. Methods: This study involved 108 respondents using a consecutive sampling technique at one time (cross-sectional). The research instrument was Comfort Assessment Breast Cancer Instrument (CABCI). The data were analyzed using Chi-Square. Results: The results showed that the comfort level in this study is categorized as comfortable. The best comfort aspect is the environmental aspect and the least is the physical aspect. In addition, there are two variables related to comfort, treatment (p = 0.007; α = 0.05) and treatment room (p = 0.000; α = 0.05). The best comfort level was found in chemotherapy treatment and outpatient rooms. Patients feel comfortable with a neat, clean, conducive hospital environment and the implementation of health protocols. These aspects could effectively prevent the spread of COVID-19. Conclusions: The results of this study recommend nursing services to optimize all aspects of comfort, which could still be improved. Interventions that maintain or improve comfort will optimize patients’ health conditions.

Abstract

Background: Cancer diagnosis in children impacts not only the children themselves but also their parents, and the role of providing support during the treatment process is crucial. The illness’s complexity, the diagnosis process, and the course of treatment can have various effects on parents and family life. To cope with the challenges of caring for their chronically ill children, families adopt a normalization process to restore their routines. Therefore, this study aims to investigate the implications of a childhood cancer diagnosis on families and how parents normalize their daily lives in the face of this difficult situation. Methods: The study was a qualitative study conducted in a pediatric cancer shelter house in Bandung, Indonesia, from May to August 2018. Ten parents who had children with cancer were included in the study. Data were collected through interviews and thematically analyzed.  Results: This study yielded five themes: 1) parents’ negative feelings when children are first diagnosed with the illness, 2) various changes in the family’s life, 3) the impact on the family’s quality of life, 4) family’s extra effort to overcome the illness’s impact and 5) parents’ expectations for social support. Conclusions: Cancer in children has a significant impact on the lives of the entire family, particularly parents. Normalizing the lives of parents who have children with cancer entails several adaptation or adjustment tasks. Nurses can assist families in adapting by guiding how to deal with the changes they are experiencing and accepting this as the new normal. Nurses can help families normalize their daily routines, provide social support, improve coping strategies, increase family closeness, and identify the source of support from other families or the community.

Methods: The selection of the studies refers to Preferred Reporting Items for Systematic Reviews and Meta-analysis (PRISMA) guidelines and JBI's checklist was used to assess the quality of the study. The study selection came from 7 databases Scopus, ScienceDirect, Sage Journals, ProQuest, Emerald Insight, Pubmed dan Taylor & Francis. The number of studies included in the review was 7 articles, 5 articles used an RCT design, and 2 articles were quasi-experimental. Results: The finding showed 2 articles that provide interventions in the form of DSME, 1 article emphasizes self-management interventions based on self-efficacy theory that is oriented towards active family involvement, 1 article specifies interprofessional-based DSM through telemonitoring media, 1 article focuses on community-based self-management interventions, and 1 article specializes in the use of weblogs in conducting self-management. Special 2 articles emphasize nurse leadership in managing interventions during the study. Five articles showed a significant effect of self-management on quality of life and 2 articles had no effect.
