Abstract

Introduction: Pain is one of the conditions experienced by patients after Coronary Artery Bypass Graft (CABG). It affects physical function, professional life, relationships and family life, social life, sleep patterns, and mood. Foot massage which is one of the complementary therapies can be done by pressing the reflective area of the foot to release endorphins so that pain will be reduced. This study aimed to review the effect of foot massage on the pain level of patients with post-CABG patients. Methods: Systematic review was used using 5 databases namely: ProQuest, ScienceDirect, Google Scholar, ClinicalKey Nursing, and Sage Journal. Articles were selected with the criteria of having been published within the 2018-2022, using English or Indonesian, and patients in the first phase. The Articles were evaluated using JBI and ended up with 5 articles. Results: The results showed that the total number of respondents in this study was 331 people with the majority is male (64.35%). Five articles showed a significant effect between foot massage and pain in patients after CABG with a p-value (p < 0.05). Conclusion: Foot massage can improve hemodynamic status and reduce pain in CABG patients. Feelings of relaxation and decreased stress are associated effectively to affect hemodynamic status and pain.

Abstract

Congestive Heart Failure (CHF) is a chronic complex syndrome characterized by the inability of the heart to support the circulation of the peripheral organs and may cause negative effects on the whole human body systems. CHF survivors may develop poor adaptation and quality of life also death. Roy's adaptation theory model-based intervention aid in improving patient adaptation to the fulfillment of physiological needs, self-concept, role function, and interdependence. The purpose of this case study was to apply Roy's adaptation theory model to patients with CHF. The results showed that adaptive mal behavior becomes the priority problem of low cardiac output with focal stimuli: increased afterload, preload, contractility, and changes in heart rate. The history of hypertension, diabetes mellitus, and non-adherence were included in the contextual stimulus. The history of smoking is included in the residual stimulus. Two mechanisms of regulatory and cognitive control are used as nursing interventions. Results showed that the patient performed physiological-physical adaptation had no diuretic injection, was capable to endure fluid restriction, and showed adequate fluid balance. Moreover, patients were also adapted with lifestyle changes supported by their families. It is recommended to provide Roy's adaptation theory model-based nursing intervention to the chronic disease patients in the clinical setting.

Abstract

Fatigue is the major symptoms in patients with coronary artery disease, during recovery period after cardiac events and during cardiac rehabilitation. Benson’s relaxation is one of relaxation as modalities therapy to reduce fatigue, however few studies related to this technique in planned intervention. This study was to measure the effectiveness of Benson’s relaxation in fatigue of coronary artery disease patients during cardiac rehabilitation. It was a quasi-experimental pretest posttest control group design. This study included 29 respondents in Dr.M. Djamil Hospital were assigned to intervention group which receiving Benson’s relaxation technique (n=15) and control group with routine care (n=14). Benson’s relaxation technique was administered for 5 days 2 times a day, each 20 minutes to intervention group. Fatigue was measured using Maastricht Questionnaire (MQ). The result indicated significant reduction in mean of fatigue between intervention and control group (p value < 0,001). The study concluded that Benson’s relaxation technique is an effective non-pharmacological intervention to reduce fatigue in coronary artery disease patients. Abstract

Background. Diabetic foot ulcers has several negative impacts on individuals, since it disrupts one’s physical activity and psychological condition, as well as the financial loss for the treatment. The purpose of this study was to find the relationship between blood glucose control and foot ulcers. This research used observational analytic study with case-control methods. It applied purposive sampling in34 diabetes mellitus patients with foot ulcers and 34 diabetes mellitus patients without foot ulcers. Theresearch instruments were documentation study about HbA1c and incidence of diabetic ulcers. Theresults indicated there was a relationship between blood glucose control and the incidence of foot ulcers in the patients at Dr. Moewardi Hospital. The respondents with poor blood glucose control will have a chance to suffer from foot ulcers 5.800 times than those who have good blood glucose control.

Abstract

Background: Hypoglycemia and diabetic ketoacidosis (DKA) are the two most common complications in diabetes patients. Monitoring blood glucose levels to prevent acute complications is an important part of diabetes management. In the era of increasingly advanced technology, technology can be used to monitor blood glucose, namely continuous glucose monitoring (CGM).Purpose: To provide an overview of the effectiveness of using CGM devices as diabetes management to reduce the incidence of hypoglycemia and DKA in diabetes patients receiving insulin therapy.Method: Literature review research to identify through a database all articles related to the topic of the effectiveness of continuous glucose monitoring (CGM). Literature search using boolean search with population "Diabetes Type 1" OR "Diabetes Patients" OR "Diabetes Type II", Intervention "Flash Glucose Monitoring" OR "Continuous Glucose Monitoring" OR "Real Time Glucose Monitoring", and outcome: "Hypoglycemia" OR “Diabetic Ketoacidosis” OR “Complications” OR “Diabetes Management”.Results: Identified 360,710 articles and found 15 suitable articles, of which 7 articles discussed that CGM can reduce the incidence of hypoglycemia, 7 articles discussed that CGM installation was related to reducing HbA1C, and 2 articles proved that CGM was related to reducing the incidence of DKA.Conclusion: The use of CGM effectively and efficiently helps nurses in monitoring blood glucose levels while in the inpatient room in the management of diabetes patients so that it is easier to detect the possibility of hypoglycemia and prevent DKA due to ongoing hyperglycemia.Suggestion: It is hoped that further research can compare the effectiveness of using CGM equipment based on various types or models. Keywords: Blood Glucose Monitoring; Continuous Glucose Monitoring; Diabetic Ketoacidosis; Hyperglycemia. Pendahuluan: Hipoglikemia dan diabetic ketoacidosis (DKA) merupakan dua komplikasi paling sering terjadi pada pasien diabetes. Pemantauan kadar glukosa darah sebagai pencegahan komplikasi akut menjadi bagian penting dalam manajemen diabetes. Di era teknologi yang semakin maju, penggunaan teknologi dapat digunakan sebagai pemantauan glukosa darah yaitu continuous glucose monitoring (CGM).Tujuan: Untuk memberikan gambaran efektivitas penggunaan alat CGM sebagai manajemen diabetes untuk menurunkan kejadian hipoglikemia dan DKA pada pasien diabetes yang mendapat terapi insulin.Metode: Penelitian literature review untuk melakukan identifikasi melalui basis data dari semua artikel berkaitan dengan topik efektivitas continuous glucose monitoring (CGM). Pencarian literatur menggunakan boolean search dengan population “Diabetes Type 1” OR “Diabetes Patient” OR “Diabetes Type II”, Intervention “Flash Glucose Monitoring” OR “Continuous Glucose Monitoring” OR “Real Time Glucose Monitoring”, dan outcome: “Hypoglycemia” OR “Diabetic Ketoacidosis” OR “Complications” OR “Diabetes Management”.Hasil: Mengidentifikasi sebanyak  360.710 artikel dan mendapatkan 15 artikel yang sesuai dimana 7 artikel membahas CGM dapat menurunkan kejadian hipoglikemia, 7 artikel membahas pemasangan CGM berkaitan dengan penurunan HbA1C, dan 2 artikel membuktikan CGM berkaitan dengan penurunan kejadian DKA.Simpulan: Penggunaan CGM efektif dan efisien membantu perawat dalam melakukan pemantauan kadar glukosa darah selama di ruang rawat inap dalam manajemen penanganan pasien diabetes sehingga lebih mudah dalam mendeteksi kemungkinan terjadinya hipoglikemia dan pencegahan DKA akibat hiperglikemia berkelanjutan.Saran: Diharapkan penelitian selanjutnya dapat membandingkan efektivitas penggunaan alat CGM berdasarkan dari berbagai jenis atau modelnya. Kata Kunci: Continuous Glucose Monitoring; Diabetic Ketoacidosis; Hiperglikemia; Pemantauan Glukosa Darah.

Abstract

Introduction: Patients recovering from cardiac surgery often experience pain, anxiety, and sleep disturbances. The Benson Relaxation Technique, a muscle relaxation method that influences the hypothalamus and reduces sympathetic and parasympathetic activity, has shown positive effects on respiratory function, heart rate, and cardiac workload, ultimately promoting better sleep quality.The purpose of this study was to assess the effectiveness of the Benson Relaxation Technique combined with slow deep breathing exercises in improving sleep quality among patients following cardiac surgery.Methods: This quasi-experimental study involved 50 post-cardiac surgery patients, divided into two groups: 25 in the intervention group and 25 in the control group. The intervention group practiced the Benson Relaxation Technique combined with slow deep breathing exercises, performed three times. Sleep quality was assessed using the GSQS (Groningen Sleep Quality Scale) questionnaire. Data were analyzed using an independent t-test with SPSS software.Results: Statistical analysis showed a significant improvement in sleep quality in the intervention group, with a p-value of 0.001. The independent t-test yielded a p-value of 0.002 and a mean difference of 0.960 between the intervention and control groups, indicating a meaningful effect of the relaxation technique on sleep quality.Conclusion: The Benson Relaxation Technique combined with slow deep breathing is effective in improving sleep quality among post-cardiac surgery patients. This non-pharmacological intervention can be recommended as a supportive therapy during recovery to enhance patient well-being and accelerate healing.

Abstract

Acute postoperative pain following cardiac surgery, typically localized in the thoracic region, can adversely affect cardiopulmonary function, inhibit effective coughing, and increase the risk of complications such as atelectasis and pneumonia, ultimately contributing to prolonged hospitalization. Multimodal pain management strategies incorporating acupressure—an intervention that modulates both the nervous and inflammatory systems—offer a non-pharmacological means of reducing pain perception and minimizing opioid requirements. This literature review aims to evaluate the effectiveness of acupressure therapy in managing postoperative pain among cardiac surgical patients. This study employed a literature review method by analyzing articles retrieved from databases including ScienceDirect, PubMed, and Scopus, published between 2015 and 2024. The review revealed that six out of seven studies reported that acupressure therapy significantly reduced pain levels in postoperative cardiac patients. Therefore, acupressure can be considered a complementary intervention in managing postoperative pain in cardiac patients.

Abstract

Pendahuluan: Nyeri pasca bedah jantung merupakan komplikasi umum yang sering terjadi dan menjadi perhatian utama bagi pasien. Pasien akan mengalami tingkat nyeri yang bervariasi, mulai dari sedang hingga berat, yang dirasakan saat istirahat dan meningkat saat aktivitas. Meskipun nyeri ini dapat diatasi melalui kombinasi berbagai obat analgetik, termasuk golongan opioid, NSAID, dan asetaminofen, namun penanganan nyeri yang tidak memadai dapat berpotensi meningkatkan morbiditas dan length of stay (LOS). Terapi Hipnosis sebagai pendekatan komplementer non-farmakologis dapat meningkatkan efektivitas manajemen nyeri pada pasien pasca bedah jantung. Tujuan: Memberikan gambaran melalui literature review mengenai efektivitas terapi komplementer Hipnosis dalam manajemen nyeri pasien pasca bedah jantung. Metode: menggunakan metode literature review melalui online database PubMed, Scopus, Science Direct, Proquest, dan Sage Journals dari tahun 2014 hingga 2024. Hasil: Hipnosis sebagai salah satu terapi komplementer non-farmakologis efektif dalam mengurangi persepsi nyeri. Selain itu Hipnosis juga memberikan efek tidak langsung terhadap penurunan dosis analgetik, penurunan kecemasan, dan meningkatkan mobilisasi dini pada pasien pasca bedah jantung. Teknik Hipnosis yang digunakan antara lain hipnosis dengan terapis langsung berhadapan atau melalui headphone, audio hipnosis (rekaman), dan self-hypnosis/ autohipnosis, menunjukkan hasil yang baik, dengan durasi sesi hipnosis berkisar dari 20 menit hingga 40 menit. Efektivitas penerapan hipnosis dalam manajemen nyeri sangat dipengaruhi oleh durasi prosedur, tingkat hipnotisabilitas pasien, dan kompleksitas prosedur yang diterapkan. Kesimpulan: Hipnosis merupakan salah satu terapi non-farmakologi yang terbukti efektif dapat digunakan sebagai terapi komplementer untuk meningkatkan efektifitas pengelolaan nyeri pada pasien pasca bedah jantung.

Abstract

Understanding the predictors of self-care behavior in heart failure (HF) patients is crucial for designing effective interventions tailored to patient needs. The situation-specific theory of HF self-care emphasizes the interaction between the person, problem, and environmental as factors influencing self-care. This study aimed to identify predictors of self-care behavior in HF patients by examining individual characteristics, disease-related variables, and environment influences. This cross-sectional study was conducted on 110 HF patients at the outpatient cardiac clinic of Kendari City Hospital, selected using consecutive sampling method. Self-care behavior was measured using the European Heart Failure Self-Care Behavior Scale-9. The predictor variables included person-related factors (age, gender, occupation, self-efficacy as measured by the CSES, and spiritual well-being as measured with the FACIT-Sp12), problem-related factors (HF duration and symptom severity), and environmental factors (social support as measured by the 12-item MSPSS). A Binary logistic regression was used to identify the significant predictors. Variables with p-value < 0.05 in multivariate analysis were considered significant predictors of self-care. Among the participants (65.5% male; mean age 62.27 ± 9.10 years), 66.4% reported inadequate self-care. Higher Self-efficacy (AOR = 1.35; 95% CI: 1.15–1.58) and greater social support (AOR = 1.08; 95% CI: 1.01–1.16) were significantly associated with better self-care. In contrast, HF duration ≥ 2 years (AOR = 0.66; 95% CI: 0.46–0.96) was associated with poorer self-care. These findings recommend that nursing interventions should integrate person, problem, and environmental components, with an emphasis on strengthening self-efficacy and enhancing social support to optimize self-care among HF patients.

Abstract

Coronary artery disease (CAD) is a major contributor to global mortality and leads to a high number of cardiac catheterization procedures, including coronary angiography (CAG) and percutaneous coronary intervention (PCI). These invasive procedures often trigger psychological responses such as anxiety and physical responses such as pain. Unmanaged pain may contribute to autonomic dysregulation and worsen cardiovascular outcomes. This systematic review aimed to synthesize scientific evidence on the effectiveness of relaxation interventions on the psychophysiological responses of patients undergoing cardiac catheterization. The review followed PRISMA 2020 guidelines and searched five databases: PubMed, Scopus, EBSCOhost, ProQuest, and Google Scholar. The PICOS framework was used to identify randomized controlled trials (RCTs) published between 2016 and 2026. Article quality was assessed using the Cochrane Risk of Bias tool version 2 (RoB 2). Eight RCTs were included in the analysis. Interventions identified included Benson relaxation, progressive muscle relaxation (PMR), deep breathing exercises, and music therapy. Results showed that relaxation interventions significantly reduced anxiety and pain levels. Although findings on hemodynamic parameters varied, improvements in cardiovascular regulation, heart rate variability, and cortisol levels were observed. Relaxation interventions are safe, effective, and support evidence-based nursing care practices.
