Abstract

Diagnosis tuberkulosis (TB) di Indonesia menurut Kemenkes RI 2014 masih mengandalkan pemeriksaan mikroskopis Basil TahanAsam (BTA) dari hapusan dahak namun memiliki kepekaan dan kekhasan diagnostik yang rendah. Kepekaan diagnostik kulturMycobacterium tuberculosis (M.tuberculosis) pada media Lowenstein Jensen (LJ) lebih tinggi daripada mikroskopis BTA namun hasilnyamemerlukan waktu 6-8 minggu. Uji cepat Fastsure TB DNA menggunakan metode Cross Priming Amplification (CPA) merupakanuji deteksi kualitatif DNA M.tuberculosis yang diamplifikasi pada satu suhu tetap dan menggunakan nucleic acid lateral flow stripdalam perangkat plastik tertutup. Penelitian ini dilakukan untuk menilai nilai diagnostik uji cepat Fastsure TB DNA untuk diagnosistuberkulosis paru. Metode penelitian secara observasional potong lintang selama Juni sampai November 2015. Total 58 spesimen dahakdari 33 orang terduga tuberkulosis paru dan 25 orang non-tuberkulosis dari RS Paru Karang Tembok Surabaya. Setiap spesimendilakukan pemeriksaan mikroskopis BTA, kultur M.tuberculosis pada media LJ sebagai standar baku emas dan ekstraksi DNA danamplifikasi pada uji cepat Fastsure TB DNA. DNA terekstraksi dimasukkan ke dalam tabung kemudian ke cartridge yang tersedia dalamperangkat. Hasil diamati dalam waktu 30 menit berupa munculnya garis uji dan pembanding pada pita. Berdasarkan penelitian ini,diperoleh kepekaan dan kekhasan diagnostik uji cepat Fastsure TB DNA masing-masing sebesar 84,8% dan 92% dengan koefisien kappaadalah 0,757 menunjukkan kesesuaian yang cukup baik. Uji cepat Fastsure TB DNA merupakan pemeriksaan yang cepat, praktis, relatiftidak mahal untuk diagnosis M.tuberculosis dari spesimen klinis khususnya pada BTA negatif.

Abstract

Tuberculosis (TB) is an infectious disease in the world causing a global problem. Vaccination with Purified Protein Derivative (PPD) still cannot prevent tuberculosis in Indonesia. Interferon-gamma (IFN-γ) produced by CD8+-T lymphocyte has an important role in eliminating Mycobacterium tuberculosis. The vaccine candidate antigenic test was done to observe the inducible ability of IFN-γ as a main protection cytokine. This study aim was to research the difference of IFN-γ expression CD8+-T lymphocyte percentage with ESAT-6-CFP-10 fusion antigen stimulations as a TB vaccine candidate. This research is a quasi-experimental study design in the laboratory by ESAT-6-CFP-10 fusion antigen-stimulated Peripheral Blood Mononuclear Cells (PBMC) culture in vitro in TB patients, latent TB, and healthy subjects’ groups. IFN-γ expression CD8+-T lymphocyte percentage were examined by flow cytometry BDFACSCalibur with results: without antigen fusion stimulation IFN-γ expression CD8+-T lymphocyte percentage mean (TB patients 2,560, latent TB 2,173, and healthy people 2,153) and with antigen fusion (TB patients 3,039, latent TB 2,471, and healthy people 2,405). There was no significant difference in fusion antigen stimulation PBMC between TB patients, latent TB, and healthy subjects’ group, and also within groups.

Abstract

Nefropati diabetik merupakan komplikasi kronis yang sering terjadi di pasien penyakit Diabetus Melitus (DM), perubahan statusglikemik harus diketahui lebih awal untuk mencegah komplikasi lebih lanjut. HbA1c sering digunakan sebagai pengendali glikemikjangka panjang (tiga/3 bulan). Albumin terglikasi merupakan tolok ukur baru yang dikembangkan untuk mengendalikan glikemikdalam waktu yang lebih singkat (2 minggu). Penelitian ini bertujuan untuk mengetahui kenasaban Albumin Glikat (GA) terhadapHbA1c di pasien nefropati diabetik. Dari 89 pasien DM yang diteliti, 34 didiagnosis sebagai diabetes nefropati. Kadar HbA1c diukurdengan metode Turbidimetric Inhibition Immunoassay (TINIA) dan GA diperiksa secara enzimatik. Analisis statistik dilakukan denganmenggunakan kenasaban Pearson, yang bermakna pada p<0,005. Cut off untuk GA: adalah 16% dan untuk HbA1c: 6,5%. RerataGA: adalah 20,73% dan 7,42% HbA1c. Pada penelitian ini, dengan uji Pearson diketahui kenasaban yang baik antara GA dan HbA1c(r=0,785, p-nilai<0,0001). Albumin Glikat memiliki kenasaban yang kuat terhadap HbA1c. Oleh karena itu, GA dapat digunakan untukmendeteksi indeks glikemik dalam jangka waktu singkat (2 minggu) di pasien pengidap nefropati diabetik.

Abstract

Polycystic Ovary Syndrome (PCOS) is the most prevalent endocrine disorder in female of childbearing age. PCOS patients also have metabolic and cardiovascular disorder risks. One of the factors that can affect those disorders is androgen hormone level. Dehydroepiandrosterone, is produced by the adrenal gland, and its metabolite dehydroepiandrosterone sulphate (DHEAS) has a different effect than testosterone. DHEA and DHEAS might have a protective effect to dyslipidemia and cardiovascular disorder. LDL cholesterol is an important parameter for dyslipidemia therapy guide according to The National Cholesterol Education Program Adult Treatment Panel. The aim of this study was to analyze the correlation between DHEAS and LDL cholesterol.This research was a cross sectional observational study. DHEAS and LDL cholesterol were examined in 25 research subjects by chemiluminescent immunoassay (Immulite®, Siemens) and colorimetric enzymatic (Dimension EXL®, Siemens). These subjects were divided into 3 groups according to 2003 Rotterdam’s criteria. Spearman and Pearson’s correlation statistical analysis were done, with a significance of p-value less than 0.05This study showed a moderate negative correlation between DHEAS and LDL cholesterol (r= -0.441, p = 0.027). Group D PCOS (non-hyperandrogenic) had a strong negative correlation between DHEAS and LDL cholesterol (r=-0.717, p= 0.001). DHEA, in some literatures was shown to activate Peroxisome Proliferator Activated Response γ (PPAR γ) receptors and thereby increasing LDL-cholesterol receptors in adipose tissue and LDL-cholesterol uptake from serum. The rise in DHEAS might indicate a better prognosis for PCOS patients in terms of cardiovascular complications, especially in group D PCOS.

Abstract

Sistem imunitas seluler sangat berperan dalam melawan infeksi TB yaitu peran sel limfosit T yang berdiferensiasi menjadi sel Th1 mensekresikan sitokin pro inflamasi IFN-γ dan sel Th2 yang mensekresi sitokin anti inflamasi IL-10. Antigen fusi EC610 bersifat spesifik dan memiliki antigenitas yang kuat terhadap stimulasi sel T. Tujuan penelitian untuk mengetahui perbedaan rerata kadar IFN-γ dan IL-10 setelah stimulasi antigen EC610 pada penderita TB aktif dan TB laten. Desain penelitian ini adalah eksperimen semu secara in vitro dengan kultur PBMC yang distimulasi oleh antigen EC610 pada kelompok TB aktif dan TB laten. Penelitian dan pemeriksaan dilakukan di RSK Paru Provinsi Sumsel dan Laboratorium Pusat Penelitian dan Pengembangan Kesehatan, Jakarta Pusat. Terdapat 21 subjek penderita TB aktif dan 28 subjek penderita TB laten yang sesuai dengan kriteria inklusi. Pengukuran kadar IFN-γ dan IL-10 dilakukan menggunakan ELISA-Reader. Hasil penelitian menunjukkan kadar IFN-γ dan IL-10 setelah stimulasi antigen EC610 lebih tinggi pada TB aktif daripada TB laten. Tingginya kadar IFN-γ pada penderita TB aktif menunjukkan adanya respon imun protektif terhadap kuman M.tb sedangkan tingginya kadar IL-10 menunjukkan perannya sebagai anti inflamasi . Tidak terdapat perbedaan bermakna rerata kadar IFN-γ pada penderita TB aktif dan TB laten (p=0,769) dan terdapat perbedaan bermakna reata kadar IL-10 setelah stimulasi antigen EC610 lebih tinggi pada TB aktif daripada TB laten (p=0,000).

Abstract

The Secretion of IFN-γ and IL-2 After ESAT-6-CFP-10 Fusion Antigen Stimulation in Active and Latent TB Patients. This study held to discover how immune responses work and to know the pathogenesis of active TB and latent TB patients. This study used PBMC to stimulate T Cells with ESAT-6 and CFP-10 antigen fusion, and measure the level of IFN-γ and IL-2 with ELISA antibody sandwich (U-Cytech). 16 ml of blood were drawn to 5 tubes. ESAT-6 CFP-20 inducted one tube with QuantiFERON for IFN-γ assay. The other four tubes were PBMC isolated using Ficoll-Paque, and pre-incubated with stimulation of ESAT-6 CFP-10 fusion antigen for 24-72 hours at 370 C and measured using T-Spot and ELISA reader. We got from this study that there are no significant differences in IFN-γ levels for both groups with active TB and latent TB. Measurement of IL-2 levels showed significant differences between the two group.

Abstract

Coronary heart disease is a highly frequent illness in both developed and developing nations. Non-HDL cholesterol (n-HDL-c) and LDL cholesterol (LDL-c) stages are biomarkers that doctors frequently utilize to assess the risk of Coronary Heart Disease (CHD). This study was a systematic review and meta-analysis to assess the association between n-HDL-c and LDL-c as major risk factors for CHD. Cochrane, PubMed, and Science Direct searches were conducted using the keywords "LDL cholesterol," "non-HDL cholesterol," and "coronary heart disease." Any research that describes the analysis of LDL-c and n-HDL-c as key risk factors for CHD and all studies involving patients diagnosed with CHD are included in the literature. A total of seven papers were involved in the qualitative analysis (systematic review), while five studies were included in the quantitative analysis (meta-analysis). The English-language research includes two RCTs, four case-control studies, and one cohort study, with a total of 68,713 individuals. LDL-c parameters were obtained (MD = 8.45; 95 percent CI = 7.03-9.87 p=0.001) and n-HDLc (p=0.001) (MD = 35.57; 95 percent CI = 33.27-37.88). n-HDL-c may be a more significant parameter of CHD risk because it has a higher MD value.

Abstract

Highlights: The effectiveness of vaccination through the kinetics of the antibody response to the SARS-COV-2 vaccine administration by examining IgG S-RBD levels was aimed. Two doses of the Sinovac vaccine formed antibodies in healthy individuals in the first 3rd month after vaccination. An S-RBD IgG antibody in the 6th-month post-vaccination was significantly different between groups without a history and groups with a history of infection with COVID-19.   Abstract: Since it was declared a pandemic in early 2020, Coronavirus Disease 2019 (Covid-19) has caused high morbidity and mortality in the world. In view of the urgency of the situation, vaccination efforts are needed to break the chain of disease transmission. Various types of vaccines have been successfully developed and obtained approval for emergency use. However, the effectiveness of these vaccines, both in the short and long term, has not been fully known. This study aimed to examine the effectiveness of vaccination through the kinetics of the antibody response to the administration of the SARS-COV-2 vaccine by examining IgG S-RBD levels. This study was an observational analytic study with a prospective cohort approach carried out between January and November 2021 at Dr. Soetomo General Academic Hospital, Surabaya, Indonesia. Fifty health workers who received Sinovac vaccination in as many as 2 doses underwent venous blood taking and measurement of quantitative S-RBD antibody level. Then, quantitative S-RBD IgG antibody levels were measured and recorded in each subject. The mean S-RBD IgG antibody was found to have fluctuation. The titer was found to significantly increase on day 14 and dropped significantly in month 3 (p <0.001). There was a significant difference in S-RBD IgG levels 6 months after vaccination between Covid-19 uninfected groups and Covid-19 infected groups (p <0.001). In a conclusion, two doses of the Sinovac vaccine formed antibodies, although humoral immunity obtained tended to decrease in 3rd month after vaccination to healthy individuals. The average level of S-RBD IgG antibody in the sixth month post-vaccination was found to be significantly different between groups without history and groups with a history of with infection COVID-19.

Abstract

BACKGROUND: Interleukin (IL)-10 is involved in Wallerian degeneration after peripheral nerve crush injury. Oral thymoquinone was previously observed to decrease local immunoglobulin-G (IgG) in a crush-injured rat model. No study has evaluated the pathway of various thymoquinone dosages on local IgG and IL-10 in this injury.METHODS: This experimental study used 126 Rattus norvegicus Wistar rats that were divided into 18 groups: six groups received a placebo, the other six groups received thymoquinone at 100 mg/kg/day and the last six groups received thymoquinone at 250 mg/kg/day, respectively. Rats were sacrificed at 12, 18, 24, 5x24, 6x24, and 7x24 hours. Matrix metalloproteinase-9 (MMP-9), IL-10, and local IgG levels were assessed by Enzyme-Linked Immunosorbent Assay (ELISA). The nuclear factor KappaB (NF-κB) expressions on Schwann cells were examined by flow cytometry. Path analysis was performed using SmartPLS.RESULTS: The path analysis showed that 100mg/kg/day of thymoquinone significantly decreased NF-κB expression. However, NF-κB did not affect local MMP-9, and MMP-9 had no significant relationship with local IL-10 and IgG. Thymoquinone 250 mg/kg/day also significantly inhibited NF-kB expression, decreased local MMP-9, and, in turn, decreased local IL-10 and IgG.CONCLUSION:  Administration of oral thymoquinone 250 mg/kg/day decreases local IgG and IL-10 levels via suppressing NF-κB expression and MMP-9 levels.KEYWORDS: thymoquinone, crush injury, IgG, IL-10, MMP-9, NF-κB

Abstract

COVID-19 infection causes severe acute respiratory syndrome and requires immediate action. Therefore, developing safe vaccine efficacy and new therapies has become a global priority for achieving herd immunity. Vaccination is expected to form specific antibodies against the SARS-CoV-2 spike protein that can neutralize the virus, thus preventing it from binding to its specific receptor (ACE 2 receptor). This study aimed to analyze the kinetics of antibody response to the CoronaVac vaccine after administration of the second dose vaccine. An observational analytic study with a prospective cohort approach was conducted from January to November 2021 at Dr. Soetomo General Academic Hospital, Surabaya. Two hundred fifty specimens from 50 health workers who met the inclusion criteria were measured for S-RBD IgG levels using the indirect chemiluminescence immunoassay method on the Snibe Maglumi® device. The SARS-CoV-2 S-RBD IgG levels were measured five times, such as before vaccination (day 0) and day 14, day 28, month 3, and month 6 after vaccination of the second dose of CoronaVac. The median (min-max) of S-RBD IgG levels before and after vaccination of the second dose on day 14, day 28, month 3, and month 6 were 0.43 (0.43–4.07); 109,25 (30.71–1619,42); 136,46 (19.38–725,28); 26.56 (7.64–158,65); 13.11 (0.59–8666,00) BAU/mL, respectively. There was a significant difference in S-RBD IgG levels at six months post-vaccination between the group with COVID-19 infection and those without COVID-19 disease (p < 0.001). Vaccination of the second dose of CoronaVac resulted in antibody formation; however, there was a trend of decreasing humoral immunity in the 3rd month after the second dose of CoronaVac vaccination in healthy individuals.
