Abstract

Oxidative stress is a condition that can damage human cells and tissues and has been linked to a number of illnesses, including cancer, cardiovascular disease, autoimmune disorders, and neurological diseases. Oxidative stress conditions can be brought on by pollution, radiation exposure, and an unhealthy lifestyle. Antioxidants are substances that can be used to both prevent and treat oxidative stress. This study aimed to identify and quantify (+)-catechin levels and antioxidant activity of the stem bark of Sterculia quadrifida R. Br extracted by the infusion method, a method similar to traditional medicine processing generally in the community. Determination of (+)-catechin and antioxidant activity of S. quadrifida were evaluated by HPLC and DPPH assay, respectively. Quantification of (+) catechin content by HPLC system with wavelength 280 nm and antioxidant activity by spectrophotometry method with wavelength 517 nm. The results show that the mean value of (+)-catechin level was 7.786% and the IC50 value of the antioxidant activity was 51.5 ug/mL having a moderate antioxidant activity category. S. quadrifida stem bark infusion can be utilized as a medication candidate for the prevention or treatment of a variety of disorders caused by oxidative stress.

Abstract

Heart failure is a health problem in Indonesia. The 2013 Basic Health Research Data showed that the estimated heart failure in Indonesia was 530,068 people. Echocardiography examination, which has been a routine examination of heart failure patients is not necessarily available in all hospitals, so that a more applicable and inexpensive alternative examination is needed. Previous studies have shown an increase in C3c levels associated with improved survival and better cardiac remodeling. On this basis, this research needs to be performed in order to determine the correlation between C3c complement levels, NT-proBNP and LVEF in heart failure patients. A cross-sectional study was conducted at the Dr Soetomo Hospital between August 2018 to September 2018 with 30 samples. Samples were taken consecutively from patients with heart failure who were treated at the integrated heart service center. Examination of C3c complement, NT-proBNP and echocardiography (LVEF data) was carried out in all patients. The result of this study showed no significant correlation between C3c complement and NT-proBNP (r = -0.253, p = 0.177). The correlation between C3c complement and LVEF was also not significant (r = -0.074, p = 0.696). A significant moderate correlation between NT-proBNP and LVEF was found (r = -0.444, p = 0.014). The C3c complement could not be used as an alternative examination for NT-proBNP and LVEF. The limitations of the study were heterogeneous sample characteristics. A further study with more stringent criteria is needed to minimize the bias of examination results.

Abstract

Increased tuberculosis in the world is caused by increased HIV-infected and antituberculous drugs (rifampicin) resistant individuals. IL-6 and TNF-α play an essential role in explaining the different degrees of inflammation in Rifampicin Resistant (RR) and Rifampicin Sensitive (RS) pulmonary tuberculosis patients, and healthy people. The research aimed to analyze the differences in plasma IL-6 and TNF-α levels in healthy people, Rifampicin Resistant (RR), and Rifampicin Sensitive (RS) pulmonary tuberculosis patients. A cross-sectional study was conducted from July-September 2017. Thirty-nine subjects were classified into RR pulmonary tuberculosis (n=15), RS pulmonary tuberculosis (n=12) based on GeneXpert examination and treated by antituberculous drugs ≤ 1 month, and healthy people (n=12) based on AFB results, Thorax X-ray, and tuberculin tests. IL-6 and TNF-α were done in all subjects using ELISA U-CyTech®(Biosciences, Inc.). Anova analyzed differences of IL-6 and TNF-α levels between groups. The mean IL-6 levels (pg/mL) in RR and RS pulmonary tuberculosis patients, and healthy people were 54.56±59.13, 27.05±37.04, 4.42±2.83, respectively. The mean TNF-α levels (pg/mL) in RR and RS pulmonary tuberculosis patients, and healthy people were 263.54±327.58, 250.25±314.20, 9.04±5.89, respectively. The mean differences between  IL-6 and TNF-α levels (pg/mL) between RR pulmonary tuberculosis patients and healthy people were 50.14±15.29 (p<0.05) and 254.59±8460 (p<0.05). Significant differences of mean IL-6 and TNF-α levels were found between RR pulmonary tuberculosis patients and healthy people.

Abstract

Sepsis is a cause of non-cardiac death in the hospital. Early and rapid diagnosis of septic patients is a challenge to increase the expectancy of life. IL-6 and TNF-α are groups of pro inflammatory cytokines that initiate an initial inflammatory response. Procalcitonin is a specific marker of bacterial infection. This study aimed to analyze the correlation of serum cytokine IL-6, TNF-α, procalcitonin and leukocyte count in suspected sepsis patients. This was a cross-sectional observational study consisting of 45 patients with suspected sepsis with procalcitonin level > 0.5 ng/mL. Procalcitonin level was measured with Enzyme-Linked Fluorescent Assay (ELFA) (VIDAS), IL-6 and TNF-α levels were measured with theU-CyTech Human Elisa kit (Bioscience, INC) and leukocyte counts were measured with SYSMEX-XN 1000. It was found in this study that IL-6 levels ranged in 0 pg/mL – 73.29 ng/mL (mean 29.43 ng/mL), TNF-α levels were 0 pg/mL – 390.5 pg/mL (mean 27.62 pg/mL), and the mean value of leukocytes was 20,139/μL. There was no correlation between leukocyte counts with IL-6 (p=0.798 and r=0.040), TNF-α (p=0.304 and r = -0.160), and procalcitonin (p=0.323 and r = 0.154). There was no correlation between IL-6 levels with TNF-α levels (p=0.871 and r = -0.025), and procalcitonin levels (p = 0.466 and r = 0.112). There was a weak negative correlation between TNF-α level and procalcitonin levels (p=0.006 and r = -0.403) and there was a weak negative correlation between procalcitonin and TNF-α levels in suspected sepsis patients.

Abstract

Chronic Kidney Disease (CKD) is not an uncommon issuein children. Chronic kidney disease is the abnormality of structure or function of the kidney that occurs for more than three months. The presence of a longitudinal decline in Glomerulus Filtration Rate (GFR), proteinuria, and hypertension Are the characteristics of CKD. One of the recommendations of nutritional supplementation as the prevention of CKD is by the administration of oral Branched-Chain Amino Acid (BCAA). To date, there has been no research to analyze the effects of the BCAA on children with stage 2-4CKD. This study aimed to analyze the effect of BCAA in inhibiting the progressivity of stage 2-4 CKD in children and improving nutritional status. A study with randomized pre-post test controlled trial design was performed in the Outpatient Clinic of Pediatric Nephrology in Dr. Soetomo Hospital with stage 2-4CKD. The subjects were divided into two groups, such as the BCAA and placebo, and were monitored for eight weeks to be evaluated the GFR, albumin, proteinuria, blood pressure, and nutritional status.Sixteen children with stage 2-4 CKD dominated by 71.4% of male patients were enrolled in this study. The mean age was 12.5 (SD 2.90) years. Approximately 50% (p=0.767) stage 2 chronic kidney, 50% (p=1.000) moderate malnutrition, and 64.28% (p=1.000) short stature were found, with nephrotic syndrome as the most common underlying cause of CKD (p=0.149). In BCAA group, decrease of GFR -5.08±7.13 (p=0.055), increase of serum albumin 0.20±0.23 (p=0.062), decrease of delta systole -11.57±15.08 (p=0.565) and diastole -4.85±16.25 (p=0.708), weight loss -0.07±1.01 (p=0.828), an increase of height 0.14±0.24 (p=0.771), and a decrease in BMI -0.03±0.74 (p=0.389) were reported. It was concluded that branched-chain amino acid (leucine, isoleucine, and valine) supplementation did not provide a significant effect to inhibit progressivity of stage 2-4CKD in children and improvement of nutritional status.

Abstract

Polycystic Ovary Syndrome (PCOS) is the most prevalent endocrine disorder in female of childbearing age. PCOS patients also have metabolic and cardiovascular disorder risks. One of the factors that can affect those disorders is androgen hormone level. Dehydroepiandrosterone, is produced by the adrenal gland, and its metabolite dehydroepiandrosterone sulphate (DHEAS) has a different effect than testosterone. DHEA and DHEAS might have a protective effect to dyslipidemia and cardiovascular disorder. LDL cholesterol is an important parameter for dyslipidemia therapy guide according to The National Cholesterol Education Program Adult Treatment Panel. The aim of this study was to analyze the correlation between DHEAS and LDL cholesterol. This research was a cross sectional observational study. DHEAS and LDL cholesterol were examined in 25 research subjects by chemiluminescent immunoassay (Immulite®, Siemens) and colorimetric enzymatic (Dimension EXL®, Siemens). These subjects were divided into 3 groups according to 2003 Rotterdam's criteria. Spearman and Pearson's correlation statistical analysis were done, with a significance of p-value less than 0.05 This study showed a moderate negative correlation between DHEAS and LDL cholesterol (r= -0.441, p = 0.027). Group D PCOS (non-hyperandrogenic) had a strong negative correlation between DHEAS and LDL cholesterol (r=-0.717, p= 0.001). DHEA, in some literatures was shown to activate Peroxisome Proliferator Activated Response γ (PPAR γ) receptors and thereby increasing LDL-cholesterol receptors in adipose tissue and LDL-cholesterol uptake from serum. The rise in DHEAS might indicate a better prognosis for PCOS patients in terms of cardiovascular complications, especially in group D PCOS.

Abstract

Tuberculosis remains a global health burden. Mycobacterium tuberculosis infection causes humoral and cellular responses. Macrophages of patients with pulmonary tuberculosis evolve M1 polarization that blocks infection or immunosuppressive M2, promoting tissue repair mediated by IL-4, IL-10, and IL-13. Previous research showed a decrease of IL-4R and IL-10 expression in lung macrophages of anti-TB drug resistance. A molecular test can detect rifampicin- resistance. There has been no study, which showed the difference in serum IL-4 levels in rifampicin-sensitive and rifampicin-resistant tuberculosis patients. This study aimed to determine the difference between circulating IL-4 levels in rifampicin-sensitive and rifampicin-resistant pulmonary tuberculosis patients. This cross-sectional observational study consecutively recruited subjects based on positive molecular and acid-fast bacilli microscopic examination from MDR-TB Clinic of the Dr. Soetomo Hospital between December 2018 to March 2019. Subjects were classified into a rifampicin-sensitive and rifampicin-resistant group. On ELISA measurement, IL-4 data were analyzed with SPSS version 17. Mann-Whitney U test and ROC analysis tests were performed, and p < 0.05 was significant for α=0.05 (95% CI). There was significant difference between rifampicin-sensitive group (420±281 pg/mL) and rifampicin-resistant group (253±279 pg/mL) (p=0.014). Receiver operating characteristics analysis showed AUC 0.70, the sensitivity of 81.5%, the specificity of 63.6%, and the cut-off value of 235.6 pg/mL. There was a significantly higher level of circulating IL-4 in the rifampicin-sensitive group than the rifampicin-resistant group. IL-4 level in healthy subjects should be measured as the normal value in the population. Immunology and metabolic parameters should be performed to increase sample homogeneity. Further study was also needed to understand the IL-4 role in rifampicin resistance of lung tuberculosis patients in the Indonesia population.

Abstract

Various strategies for dealing with COVID-19 have been carried out since the WHO declared COVID-19 as an international health emergency. One of the preventive strategies is the development of vaccines. Various vaccines have been developed worldwide. As of April 13, 2021, there were 184 vaccine candidates in the pre-clinical phase and 16 vaccine candidates currently in phase III clinical trials using several platforms, such as inactivated viruses, vector viruses, and protein subunits, and mRNA. Clinical trials of the SARS-CoV-2 vaccine include a screening test consisting of thorough physical examination and laboratory tests. The safety of clinical trials is evaluated based on laboratory test results referring to the standard toxicity grading scale. Immunogenicity assessment at the stage of clinical trials of vaccines includes assessment of humoral and cellular immunogenicity. The humoral immunogenicity test measures the ability of antibodies to neutralize the virus with the live virus neutralization test, Pseudo Virus Neutralization Test (pVNT), and Surrogate Virus Neutralization Test (sVNT) method. The cellular immunogenicity response aims to assess the immune response that leads to the Th1-cell phenotype. The COVID-19 vaccine under development is expected to trigger a helper 1 (Th1) cell response. Th1-producing Interferon-g (IFNg) is formed during acute viral infection, and Th1-type immune response correlates with milder disease. This is one of the considerations in vaccination. Th1-cell phenotype as part of cellular immunogenicity can be evaluated with ELISPOT, interferon-gamma release assay, and flow cytometry using blood samples that have been cultured with the administration of specific SARS-CoV-2 peptides. This literature review aims to study various immunogenicity assessments in the laboratory for clinical trials of COVID-19 vaccines.

Abstract

COVID-19 infection causes severe acute respiratory syndrome and requires immediate action. Therefore, developing safe vaccine efficacy and new therapies has become a global priority for achieving herd immunity. Vaccination is expected to form specific antibodies against the SARS-CoV-2 spike protein that can neutralize the virus, thus preventing it from binding to its specific receptor (ACE 2 receptor). This study aimed to analyze the kinetics of antibody response to the CoronaVac vaccine after administration of the second dose vaccine. An observational analytic study with a prospective cohort approach was conducted from January to November 2021 at Dr. Soetomo General Academic Hospital, Surabaya. Two hundred fifty specimens from 50 health workers who met the inclusion criteria were measured for S-RBD IgG levels using the indirect chemiluminescence immunoassay method on the Snibe Maglumi® device. The SARS-CoV-2 S-RBD IgG levels were measured five times, such as before vaccination (day 0) and day 14, day 28, month 3, and month 6 after vaccination of the second dose of CoronaVac. The median (min-max) of S-RBD IgG levels before and after vaccination of the second dose on day 14, day 28, month 3, and month 6 were 0.43 (0.43–4.07); 109,25 (30.71–1619,42); 136,46 (19.38–725,28); 26.56 (7.64–158,65); 13.11 (0.59–8666,00) BAU/mL, respectively. There was a significant difference in S-RBD IgG levels at six months post-vaccination between the group with COVID-19 infection and those without COVID-19 disease (p < 0.001). Vaccination of the second dose of CoronaVac resulted in antibody formation; however, there was a trend of decreasing humoral immunity in the 3rd month after the second dose of CoronaVac vaccination in healthy individuals.

Abstract

Coronary heart disease is a highly frequent illness in both developed and developing nations. Non-HDL cholesterol (n-HDL-c) and LDL cholesterol (LDL-c) stages are biomarkers that doctors frequently utilize to assess the risk of Coronary Heart Disease (CHD). This study was a systematic review and meta-analysis to assess the association between n-HDL-c and LDL-c as major risk factors for CHD. Cochrane, PubMed, and Science Direct searches were conducted using the keywords "LDL cholesterol," "non-HDL cholesterol," and "coronary heart disease." Any research that describes the analysis of LDL-c and n-HDL-c as key risk factors for CHD and all studies involving patients diagnosed with CHD are included in the literature. A total of seven papers were involved in the qualitative analysis (systematic review), while five studies were included in the quantitative analysis (meta-analysis). The English-language research includes two RCTs, four case-control studies, and one cohort study, with a total of 68,713 individuals. LDL-c parameters were obtained (MD = 8.45; 95 percent CI = 7.03-9.87 p=0.001) and n-HDLc (p=0.001) (MD = 35.57; 95 percent CI = 33.27-37.88). n-HDL-c may be a more significant parameter of CHD risk because it has a higher MD value.
