Abstract

Purpose: This study aims to evaluate the influence of traditional markets in rural areas and the relationship between definitive factors that influence through bivariate and multivariate statistical analysis. Methodology: This study used a cross-sectional design to analyze the effect of age, BMI, and the presence of traditional markets on blood pressure using data from visits to the Gatak District Health Center, Sukoharjo Regency (May-July 2024) with bivariate (Chi-Square) and multivariate (logistic regression) methods. Results: This study analyzed 9,639 respondents which showed that age 50 years and above (aOR = 4.647, p = 0.000), overweight/obese BMI (aOR = 1.677, p = 0.000), and the absence of a traditional market around the residence (aOR = 0.779, p = 0.004) were significantly associated with an increased risk of hypertension. This study states that traditional markets can function as a protective factor, while age and obesity increase the risk of hypertension in rural communities. Applications/Originality/Value: This study highlights the role of traditional markets as a protective factor against hypertension in rural Indonesia, which has rarely been discussed in previous studies; This study highlights the role of traditional markets as a protective factor against hypertension in rural Indonesia, a perspective rarely discussed in previous studies. It provides insights into their contribution to public health strategies and preventive measures.

Abstract

Purpose: This study aims to identify risk factors, clinical problems and provide patient management with the implementation of holistic family doctor services based on an evidence-based medical approach with an imaginative approach to patients and families. Methodology: This research is a case report. Primary data were obtained through anamnesis (directly from the patient and indirectly from family members), physical examination, and home visits to complete family, psychosocial, and environmental data. Secondary data was obtained from the patient's medical records at the puskesmas. Assessment is based on a holistic diagnosis from the beginning, process, and end of the quantitative and qualitative studies. Results: A 36-year-old woman was diagnosed with schizophrenia. The patient acknowledged the presence of mental disorder in herself. The results of the evaluation showed that patient complaints and worries were reduced, patient and family knowledge about schizophrenia was lacking and needed further education. Conclusion: Schizophrenia that occurs in patients is caused by a lack of attention from parents and also families who do not socialize with neighbors and family.

Abstract

Purpose: Obesity is a significant global health issue, including in Indonesia, where its prevalence continues to rise. This study investigates the influence of Base Transceiver Station (BTS) towers, age, and gender on obesity in rural Indonesian communities. A cross-sectional study analyzed secondary data from 9,639 patient visits to the Gatak Subdistrict Health Center between May and July 2024. Body Mass Index (BMI) was categorized into normo/underweight and overweight/obese groups, while BTS tower data were obtained from regional statistics. Statistical analyses were conducted using bivariate (Pearson's Chi-square) and multivariate (binary logistic regression) methods. Results showed that the presence of BTS towers and age did not significantly influence obesity. However, gender was a significant factor, with women exhibiting a higher prevalence of overweight/obesity compared to men (aOR = 0.796, p = 0.000). Most patients lived in areas with fewer than three BTS towers, and the majority were classified as normo/underweight. In conclusion, gender significantly influences obesity prevalence, with women being more affected than men, while the presence of BTS towers and age showed no significant relationship with obesity. These findings underscore the need for further research to explore additional factors contributing to obesity in rural Indonesian populations.

Abstract

Background: The management of infectious solid waste is essential in healthcare, influencing environmental safety and public health. Nurses, as frontline healthcare practitioners, are integral to the management of medical waste, encompassing segregation, storage, and disposal. Efficient waste management mitigates the risk of infections and environmental pollution; nonetheless, nurses may encounter difficulties stemming from inadequate training, limited resources, and exposure to work dangers. Aim: This narrative review analyzes the roles of nurses in the management of infectious solid waste, their contributions to patient safety, and the problems encountered in executing good waste management techniques. Methods: A narrative review was performed utilizing academic sources (PubMed, Scopus, Google Scholar) to discover research on nurses' roles, education, and difficulties in hospital waste management. Studies were chosen for their pertinence to waste management techniques within healthcare environments. Results: Nurses are essential in the secure segregation, storage, and disposal of infectious waste. They mitigate infection risks by guaranteeing accurate classification and secure waste containment. Challenges such as limited resources, insufficient training, excessive workloads, and exposure to occupational risks impede effective waste management, despite its crucial importance. Nurses assist to infection prevention by implementing proper hygiene practices and utilizing personal protective equipment (PPE).

Abstract

Acquired hemophilia A is a rare bleeding disorder that presents as gross hematuria, with or without an initiating factor such as stone or cancer in urinary tract. People with hemophilia may require some surgical procedures in their live. Surgery in patients with congenital or acquired coagulation defects has always been challenging and requires special care with a multidisciplinary approach. Extracorporeal Shock Wave Lithotripsy (ESWL) is a standard procedure performed in patients with kidney stones.

Abstract

Background Enhanced Recovery After Surgery (ERAS) is a surgery protocol that focused on the patient. This protocol designed to reduce stress responses and promoting faster recovery to patient. ERAS protocols have been widely used and demonstrating better result in patient outcomes. There is limited information about its economic impact and cost analysis of ERAS. This review aims to analyze the existing literature on the application of ERAS protocol by comparing the cost of patient care with or without ERAS protocol. Methods This study conducted a literature search using electronic research databases over the past ten years. Data was taken from Scopus, analyzed 99 articles using various analysis techniques and supported by the VOS viewer application. The analysis includes observations of research developments, evaluating authors and research subject areas and than analyzed with descriptive statistics to describe the results. Results This study identified ninety nine potential studies published between 2014 and 2024, resulting ten studies for inclusion in the review. The studies identified from various types of surgeries. Nine from ten articles in this review show that application of ERAS protocol resulted in advantage in financial. The advantage of application ERAS protocol to reduce cost of hospitalization due to shorter Length of Stay (LOS), lower mortality and complication rate in patient. Conclusions Application of ERAS protocol provide benefits and efficiency across various types of surgical specialities. Hospitalization costs in ERAS group consistenly lower compared to non-ERAS group.

Abstract

The study is aimed at comparing the clinical and institutional outcomes of using two postoperative analgesics regimens, Epidural Levobupivacaine and Syringe Pump Fentanyl, and how these drugs interfere with analgesia control and resource consumptions, patient satisfactory, and system performance. A comparative observational research design was used which covered 60 patients of Total Knee Replacement (TKR) who were in a tertiary orthopedic center. The participants were at random allocated to an Epidural Levobupivacaine or Fentanyl protocol. The collected data included pain scores, adverse events, the readiness of the patient to leave, and satisfaction questionnaires and efficiencies of hospitals in terms of data. The data were analyzed using descriptive and comparative approaches. The Epidural group of patients recorded less pain scores, fewer adverse effects, earlier mobilization and shorter length of stay. The scores of the satisfaction on pain control, comfort and responsiveness were significantly more in the Epidural group. The mode of operation also demonstrated better nursing efficiency and discharge planning, and risk indicators were reduced, e.g. escalation to high-dependency care, with the use of the epidural protocol. Epidural Levobupivacaine represents an effective operational and clinical performance that demonstrates that it can be used as a strategical standar in elective orthopedis care based on systemic opioids. In addition to the analgesic effectiveness, the protocol improves institutional efficiency, minimizes the clinical risk, intensifies patient experience and fits the contemporary performance-based healthcare provision. Pain management must therefore be redefined as an impact component that has a cross-functional component of hospital service design.

Abstract

Kebutaan merupakan permasalahan kesehatan yang signifikan di Indonesia dengan prevalensi mencapai 75% kasus gangguan penglihatan yang disebabkan oleh katarak. Tingginya angka kebutaan akibat katarak berdampak serius pada produktivitas dan kualitas hidup masyarakat, terutama di daerah dengan akses kesehatan terbatas seperti Desa Sanggung. Pengabdian masyarakat ini bertujuan mengidentifikasi dan mengimplementasikan upaya pencegahan kebutaan melalui deteksi dini serta edukasi komprehensif tentang katarak. Metode yang digunakan adalah pendekatan kualitatif deskriptif dengan teknik pengumpulan data melalui observasi, wawancara, dan dokumentasi, melibatkan 41 peserta prioritas lansia di atas 50 tahun. Kegiatan dilaksanakan dalam dua tahap utama: edukasi interaktif tentang patofisiologi, faktor risiko, dan pencegahan katarak, serta pemeriksaan kesehatan mata komprehensif menggunakan peralatan medis standar. Hasil menunjukkan peningkatan signifikan pengetahuan masyarakat, ditandai dengan berkurangnya persentase pengetahuan rendah dari 53,66% menjadi 21,95%, dan meningkatnya kategori pengetahuan tinggi dari 9,76% menjadi 26,83%. Deteksi dini berhasil mengidentifikasi 21 peserta (51,22%) dengan indikasi katarak berbagai tingkat keparahan. Program ini berpotensi meningkatkan kesadaran masyarakat akan pentingnya pemeriksaan mata berkala, serta menurunkan risiko kebutaan total melalui deteksi dan penanganan dini, yang pada gilirannya akan meningkatkan kualitas hidup dan produktivitas masyarakat Desa Sanggung.

Abstract

Ringkasan: Latar Belakang: Digitalisasi rekam medis elektronik (RME) menjadi kebutuhan di rumah sakit, meningkatkan mutu layanan namun implementasinya belum sepenuhnya optimal dan berdampak pada variabilitas kepuasan pengguna. Tujuan: Menganalisis hubungan antara penggunaan RME terhadap kepuasan pengguna sebagai dasar kebijakan pengembangan sistem di rumah sakit swasta. Metode: Studi analitik cross-sectional terhadap 132 responden (dokter, perawat, radiologi, laboratorium, kasir) dari lima RS dengan kuesioner HOT Fit, analisis univariat dan bivariat (uji Chi Square, r). Hasil: Penggunaan RME kategori baik (50,76%) berkorelasi kuat dengan tingkat kepuasan (r=0,609; p<0,05); variabel human, organisasi, teknologi, dan manfaat memiliki hubungan signifikan dengan penggunaan RME. Simpulan: Penggunaan RME berdampak pada peningkatan kepuasan dan efektivitas kerja, khususnya pada pengguna yang aktif dan telah mendapat pelatihan terstandar. Saran: Rumah sakit perlu mempertahankan, mengembangkan fitur sesuai kebutuhan pengguna, dan melakukan pelatihan berkala agar optimalisasi kepuasan tercapai.

Abstract

Stroke is a multi-causal disease, and many factors can lead to stroke. There are also some unseen behavioral factors, such as poor diet. In recent years, poor sleep quality has been considered a potential trigger of stroke. If these factors are not well controlled, it can have an impact on the incidence of ischemic stroke. The aim of this research is to assess the correlation between sleep quality and low-density lipoprotein cholesterol levels with the incidence of ischemic stroke. This study used an analytic observational design with a cross-sectional approach using the PSQI (Pittsburgh Sleep Quality Index) questionnaire and secondary data. A total of 60 respondents were selected with a purposive sampling technique and met the inclusion and exclusion criteria. This research was conducted at Fatmawati Soekarno Hospital, Surakarta City. Statistical analysis showed that the Chi-Square test produced a significance value, with a p value of 0.007 for the correlation of sleep quality with the incidence of ischemic stroke, and a p value of 0.688 for the correlation of low-density lipoprotein cholesterol levels with the incidence of ischemic stroke. It can be concluded that there is a correlation between sleep quality and the incidence of ischemic stroke, but there is no relationship between low-density lipoprotein cholesterol levels and the incidence of ischemic stroke.
