Abstract

Background: Leprosy is chronic infectious disease caused by Mycobacterium leprae. Endemic areas stillexisted with stable number of new cases including new cases of pediatric leprosy and female leprosy cases.Close contact and women’s role in household increase the risk of transmission of leprosy to their children.Population lived in endemic areas are prone to contract leprosy due to dysregulation of immune system. Thisstudy aims to analyze correlation of hematological parameters and immunity ration in maternal and pediatricleprosy in endemic areas.Methods: This is a cross-sectional study in endemic areas in Tuban Regency, East Java Province,Indonesia. The blood sample was taken from the subjects and underwent complete hematological parametertest (hemoglobin, red blood cells, white blood cells, platelets, hematocrit) and measurement of T cellsactivityratio(Th1/Th2 ratio and Th17/Treg ratio) by ELISA. Correlation test was done between blood testresultsand the immunity ratio of maternal and pediatric leprosy in endemic areas.Results: 33 pairs of maternal and pediatric leprosy cases were analyzed. This study found correlationbetween RBC (p value= 0.029) and HCT (p value= 0.038) with Th17/Treg ratio in children population. Nosignificant results observed in mothers population.Conclusion: The result of this study shows that hematological paramaters (HB and HCT) related to theTh17/Treg ratio and could play roles in the incidence of dysregulation of immune system in children leprosy.

Abstract

Background: Skin Prick Test (SPT) is good diagnostic tool of allergic disease such as Atopic Dermatitis (AD). Imported allergen extracts are expensive and not always available in Indonesia. New local SPT allergen extracts are potential as substitute. Objective: To compare SPT results of new local and imported allergen extracts (house dust mite, chicken, egg white, cow’s milk, and shrimp), and specific IgE in AD patients. Methods: This was clinical trial of 45 non lesional AD patients. The SPT of imported allergen extracts was performed in the first week, followed by the new local and specific IgE examination in the next one week. Result: Sensitivity (Sn) of new local compared with imported allergen extracts in this study was between 15.38%-84.61%, the highest point was house dust mite extract. Specificity (Sp) was ranged from 81.48%-93.75%, the highest rank was shrimp extract. Compared with IgE results, the Sn of new local and imported house dust mite extracts was good (85.7%; 92.9%). The Sp was good for egg white and cow’s milk new local extracts (86.4%; 84.4%). Conclusion: New local allergen extracts should be considered as a substitute of the imported diagnostic tool.

Abstract

In the National Standard for Accreditation of Hospitals (SNARS) 2018, patient safety is one of the standards of assessment. The purpose of this study was to assess the fair and open culture in the patient safety culture survey, with the aim of assessing the culture of reporting and learning in Dr. Soetomo Hospita, Surabaya. This research was an observational study with a total sample of 406 employees spread across 34 installations and 11 fields / sections, with the same proportion. Data was collected using Hospital Survey on Patient Safety Cuture issued by AHRQ (Agency for Healthcare Research and Quality), which consisted of 12 dimensions. The results showed that the majority of employees had assessed the 12 AHRQ dimensions well, but there were several dimensions that still received poor ratings such as openness of communication, bad = 25.4%, frequency of incident reporting, rarely = 27.8% and staffing, less 39.6%; it is necessary to develop a culture of not blaming if someone reports an incident, and changes in staffing to suit the workload. Tujuan penelitian ini adalah untuk menilai budaya adil dan terbuka dalam survei budaya keselamatan pasien, dengan tujuan menilai budaya pelaporan dan pembelajaran di RSUD Dr. Soetomo Surabaya. Penelitian ini merupakan studi observasional dengan total sampel 406 karyawan yang tersebar di 34 instalasi dan 11 bidang/bagian, dengan proporsi yang sama. Data dikumpulkan menggunakan Hospital Survey on Patient Safety Cuture yang dikeluarkan oleh AHRQ (Agency for Healthcare Research and Quality), yang terdiri atas 12 dimensi. Hasil penelitian menunjukkan bahwa sebagian besar karyawan telah menilai baik pada 12 dimensi AHRQ, namun terdapat beberapa dimensi yang masih mendapat penilaian buruk seperti keterbukaan komunikasi, buruk=25,4%, frekuensi pelaporan insiden, jarang=27,8% dan staffing, kurang=39,6%; maka perlu dikembangkan budaya tidak menyalahkan jika ada yang melaporkan insiden, dan perubahan susunan kepegawaian agar sesuai dengan beban kerja. Abstract

Introduction: Acne vulgaris is a common cutaneous inflammatory disorder affecting more than 85% of adolescents worldwide. More than quarter of medical students 2016th had suffers from acne and complaint stress. The stress levels could increase because lot of burden in academy assessment, and other factors that can induced stress like psychosocial psychological stress and triggers by other factors. Stresss levels measured by PSS questionnaire there is mild (≤ 13), moderate (14-26) and severe (>27). Although there is widespread acceptance of a relationship between stress and acne flares, not many studies have been conducted to assess this relationship. Therefore this research encourage, does the stress affect with the severity appearances of acne. Objectives: To correlate stress levels to acne vulgaris stage among students of Faculty of Medicine Universitas Airlangga. Methods: This research design used correlative analysis with cross sectional aproach and total sampling of all students medical students 2016th with the exclusion of students who are using corticosteroid, psychotherapy drugs and non acne vulgaris by staging the acne vulgaris using GAGS and stress level using PSS (perceived stress scale) questionnaires that relate to the research. Also taking pictures for assure the stage of respondent who suffers acne vulgaris, measure and correlate with spearmen correlation. Results: The total of subjects 69 students constituted the study population. This study was attended by 60 male 27,6% of the total number of respondents and 157 female 72,4%. There is 69 people who suffered from acne consist of 46 peoples 66,67% had mild acne stage, 17 peoples 24,63% had moderate acne stage and 6 peoples 8,7% had severe acne stage. The risk factor that mostly induced acne vulgaris stage were final exam (91.3%), eating habbit(87%), and premenstruation acne(71%). The correlation between stress levels and acne vulgaris stage shows .521 using SPSS program. Conclusion: There is significant correlation that the stress levels effect to the skin apearrance and stage of acne vulgaris, also the others factors have been asked on the questionnaire could be increase the acne become more aggressive growth on skin.Keywords: acne vulgaris stage; medical students; stress levels

Abstract

Low back pain is a crucial public health problem that is commonly associated with intervertebral disc de‐ generation and has vast socio‐economic impact worldwide. Current treatments for disc degeneration are conservative, non‐surgical, or surgical interventions, and there is no current clinical therapy aimed at directly reversing the degeneration. Given the limited capacity of intervertebral disc (IVD) cells to self‐repair, treatment aiming to regenerate IVDs is a topic of interest and mesenchymal stem cells (MSCs) have been identified as having potential in this regeneration. Recent studies have revealed that the benefits of MSC therapy could result from the molecules the cells secrete and that play principal roles in regulating essential biologic processes, rather than from the implanted cells themselves. Therefore, the objective of this study is to review the potential use of the MSC secretome to regenerate IVDs. Current evidence shows that the secretome may regenerate IVDs by modulating the gene expressions of nucleus pulposus cells (upregulation of keratin 19 and downregulation of matrix metalloproteinase 12 and matrix Gla protein) and stimulating IVD progenitor cells to repair the degenerated disc.

Abstract

Multidrug therapy (MDT) regiment has been used for leprosy all over the world for more than 20 years. Drug resistances of Mycobacterium have been reported from many areas. The resistance mostly occurred due to mutation on the gene coding protein targeted by anti-leprosy drugs. Two hundreds and seventy M. leprae isolates from some area in Indonesia were examined for studying the profile of mutation among isolates collected during 2003-2011. Drug resistance determining region of the folP1 gene and the rpoB gene was sequenced. The results showed 5 isolates of M. leprae harboured mutation only in the folP1 gene and another isolate harbored mutation in both the folP1 and rpoB gene. The point mutation in the folP1 gene that was found in 2 isolates occurred in codon 53 (ACC→GCC; Thr→Ala). Double point mutations on codon 53 that was found in two isolates were ACC→AGA (Thr→Arg) and ACC→AGG (Thr→Arg). The point mutation in the folP1 geneoccurred in codon 55 were found in two isolates were CCC→CTC (Pro→Leu) and CCC→CGC (Pro→Arg). Whereas mutation in the rpoB gene in one isolate occurred in codon 410 was GAT→TAT (Asp→Tyr). These mutations that altered the amino acids of the protein revealed that isolates of M. leprae were resistant to drug with variable profiles

Abstract

Pendahuluan: Sebagai organ terluas yang menyusun tubuh manusia, kulit menjadi salah satu lokasi penting dalam menghantarkan obat atau bahan aktif tertentu ke dalam tubuh manusia. Beberapa bahan aktif berbentuk makromolekul dengan berat molekul lebih dari 500 Da tidak dapat menembus stratum korneum sehingga membutuhkan bantuan penetration enhancer. SPACE (Skin Penetrating and Cell Entering) merupakan rangkaian molekul peptida berukuran kecil yang berpotensi bekerja sebagai enhancer dalam menghantarkan makromolekul dengan cara yang tidak invasif. Namun SPACE merupakan enhancer golongan baru sehingga efek penambahan SPACE terhadap karakteristik dan stabilitas fisik pada formulasi sediaan gel masih belum banyak diketahui. Tujuan: Penelitian kali ini bertujuan untuk mengevaluasi efek pemberian SPACE pada karakteristik dan stabilitas fisik sediaan gel freeze dried Amniotic Membrane Stem Cell-Metabolite Product. Metode: Karakteristik fisik sediaan gel freeze dried AMSC-MP dengan SPACE dievaluasi menggunakan parameter organoleptis (warna, bau, bentuk, tekstur), pH dan diameter sebar sediaan. Stabilitas fisik sediaan akan dievaluasi dengan menguji karakteristik fisik sediaan selama 30 hari penyimpanan. Hasil: Gel freeze dried Amniotic Membrane Stem Cell memiliki pH sekitar 6,1 dan diameter sebar sediaan sekitar 6,0 cm. Seluruh hasil pengujian karakteristik masih memenuhi rentang yang dipersyaratkan. Hasil uji karakteristik dianalisis menggunakan statistika metode ANOVA satu arah dan tidak menunjukkan adanya perbedaan bermakna antar formula. Semua formula F1, F2, F3 dan F4 stabil selama 30 hari penyimpanan. Kesimpulan: Penambahan SPACE tidak mempengaruhi karakteristik dan stabilitas fisik sediaan gel freeze dried Amniotic Membrane Stem Cell-Metabolite Product.

Abstract

Pendahuluan: Penggunaan growth factor dalam kosmetika adalah upaya yang menjanjikan dalam mengatasi masalah penuaan, namun juga menjadi sebuah tantangan tersendiri untuk pengaplikasiannya. Karakteristik dan stabilitas fisik merupakan parameter yang penting dalam formulasi sediaan, utamanya pada sediaan kosmetika yang mengandung growth factor karena dapat berpengaruh langsung terhadap efektifitas kandungan zat aktifnya. Tujuan: Mengevaluasi pengaruh perbandingan Amniotic Membran Stem Cell Metabolite Product (AMSC-MP) dan SPACE peptide (F1 = 1:0; F2 = 1:1; F3 = 1:2; F4 = 1:3) terhadap karakteristik dan stabilitas fisik krim antiaging AMSC-MP yang mengandung growth factor. Metode: Uji karakteristik sediaan dievaluasi dengan parameter organoleptis, pH dan daya sebar. Uji stabilitas fisik dilakukan dengan mengevaluasi karakteristik sediaan selama 21 hari penyimpanan. Hasil: Sediaan krim yang mengandung AMSC-MP dengan SPACE peptide memiliki karakteristik; rentang pH 5,8 - 6,5 hasil uji statistik dengan metoda ANOVA satu arah menunjukan perbedaan bermakna, daya sebar sediaan antara 5,6 - 7,2 cm hasil uji dengan metoda Kruskal Wallis menunjukkan tidak ada perbedaan yang bermakna, bau sedikit amis, warna putih susu dan tekstur lembut. Hasil uji stabilitas terhadap nilai pH dan daya sebar menunjukkan bahwa formula krim dengan penambahan berbagai konsentrasi SPACE peptide stabil selama penyimpanan 21 hari. Kesimpulan: Peningkatan konsentrasi SPACE peptide meningkatkan pH sediaaan namun tidak mempengaruhi daya sebar, bau, warna dan tektur sediaan. Semua formula sediaan krim AMSC-MP dengan  SPACE peptide stabil selama 21 hari penyimpanan.

Abstract

Pendahuluan: Amniotic membran stem cell metabolite product (AMSC-MP) merupakan metabolit produk yang diperoleh dari Amniotic membrane yang diisolasi dan dikultur yang kemudian ditumbuhkan dalam medium terkondisi. AMSC-MP mengandung banyak growth factor dan sitokin yang sangat berguna sebagai antiaging. Growth factor dan sitokin yang pada umumnya berukuran besar lebih dari 20 KDa menyebabkan AMSC-MP membutuhkan formulasi khusus untuk penggunaannya secara topikal. Pada penelitian ini digunakan asam hialuronat dikombinasi dengan SPACE peptide yang memiliki fungsi sebagai enhancer makromolekul untuk formulasi sediaan gel AMSC-MP sebagai antiaging. Untuk memastikan kualitas dan stabilitas suatu sediaan, maka perlu dilakukan uji karakteristik fisik dan uji stabilitas fisik sediaan tersebut. Tujuan: Mengevaluasi pengaruh penambahan asam hialuronat (0; 0,01; 0,02; 0,04 %) yang dikombinasi SPACE peptide terhadap karakteristik dan stabilitas dari formulasi sediaan gel AMSC-MP. Metode: Karakteristik fisik sediaan gel freeze dried AMSC-MP dengan penambahan asam hialuronat yang dikombinasi SPACE peptide dievaluasi dengan menggunakan parameter organoleptis (warna, bau, konsistensi dan tekstur), pengukuran pH dan kapasitas penyebaran. Pengujian stabilitas fisik dievaluasi dengan pengukuran pH dan kapasitas penyebaran sediaan setelah penyimpanan selama 28 hari. Hasil: Hasil uji karakteristik dan stabilitas fisik sediaan gel AMSC-MP menunjukkan penampilan fisik yang baik, gel transparan dengan konsistensi kental, lembut dengan bau yang khas. Nilai pH sekitar 5,26 - 5,37 dan kapasitas penyebaran pada 6,13 - 6,50. Semua formula stabil selama 28 hari penyimpanan. Kesimpulan: Penambahan kombinasi asam hialuronat dan SPACE peptide pada formulasi tidak merusak karakteristik dan stabilitas fisik sediaan gel AMSC-MP pada penyimpanan selama 28 hari.

Abstract

Background: Indonesia's annual increase in leprosy is a serious health problem. In east Java, child leprosy cases spread in several counties/cities, the number of new cases of child leprosy cases has been raised in Gresik District from 2015 until 2019. Purpose: This study analyzed characteristics, BCG vaccine status, nutritional status and home environment, the correlated risk factors to child leprosy in Gresik District. Methods: This study used a design population-based matched case-control design and analysis data using computer computing. The variables for this research were characteristics (age, sex, and education) and BCG vaccine status (BCG scar and without BCG scar), nutritional status classified as Body Mass Index (BMI)(normal and thin) and protein (albumin and Hb serum), and home environment (wall, ceiling, floor, humidity, ventilation, and density of occupancy). It was conducted in October 2019-February 2020 using a questionnaire, observation and measurements. The total sample was 36 respondents, of which the number of the case was 18, and the control was 18. Data were analyzed using the Logistic Regression method entered with a 95% CI and an OR. Results: Home environment had not correlated to leprosy cases of children. The completeness of BCG vaccine status correlated to children's leprosy cases (p=0.02). BMI had correlated to leprosy cases of children (p=0.01). The most influential variable was the completeness protein of albumin serum (Odds Ratio (OR) =9.10; 95%CI=1.11-41.44; p=0.01). Conclusion: The most dominant risk factor correlated with the incidence of leprosy is the serum albumin level.
