Abstract

Lean is a continuous effort to eliminate waste and increase the value added of both goods and services in order to provide value to customers. The purpose of this study was to identify critical wastes that affect patient satisfaction at the Pharmacy Installation at Abdul Wahab Sjahranie Hospital, Samarinda. This qualitative research method was a case study. The stages of the research were observing the flow of the service process in the form of value stream mapping, distributing questionnaires about value waste and patients, conducting interviews using the 5-why method to find out the root causes and critical waste that occurs. The results showed that the lead time for concoction recipes was 100.4 minutes and the VAR for recipes was 67%, while the lead time for non-concocted recipes was 45.2 minutes and the VAR for non-concocted recipes was 48%. This shows that drug prescription service activities at the Outpatient Pharmacy Installation at Abdul Wahab Sjahranie Hospital Samarinda were included in the lean category. Critical waste was waste motion (53.3%). It could be concluded that critical waste is motion and waiting waste which is caused by several cases of drug shortages that often occur. What can be suggested is to improve drug planning or procurement in order to minimize drug vacancies.Keywords: pharmaceutical installation; lean hospital; critical waste; service improvement ABSTRAK Lean adalah suatu upaya terus menerus untuk mengeliminasi pemborosan (waste) dan meningkatkan value added produk baik barang maupun jasa agar memberikan nilai kepada pelanggan. Tujuan penelitian ini adalah mengidentifikasi waste kritis yang berpengaruh terhadap kepuasan pasien di Instalasi Farmasi RSUD Abdul Wahab Sjahranie Samarinda. Metode penelitian kualitatif ini adalah studi kasus. Tahapan penelitian yaitu observasi alur proses pelayanan dalam bentuk value stream mapping, penyebaran kuisioner tentang value waste dan pasien, melakukan wawancara dengan metode 5-why untuk mengetahui akar penyebab dan waste kritis yang terjadi. Hasil penelitian menunjukkan bahwa lead time resep racikan adalah 100,4 menit dan VAR resep adalah 67%, sedangkan lead time resep non racikan adalah 45,2 menit dan VAR resep non racikan adalah 48%. Ini menunjukkan bahwa aktivitas pelayanan resep obat di Instalasi Farmasi Rawat Jalan RSUD Abdul Wahab Sjahranie Samarinda termasuk dalam kategori lean. Waste kritis adalah waste motion (53,3%). Disimpulkan bahwa waste kritis adalah waste motion dan waiting yang disebabkan oleh adanya beberapa kasus kekosongan obat yang sering terjadi. Hal yang dapat disarankan adalah memperbaiki perencanaan atau pengadaan obat agar dapat meminimalisir terjadinya kekosongan obat.Kata kunci: instalasi farmasi; lean hospital; waste kritis; perbaikan pelayanan

Abstract

Syzygium polyanthum leaf extract and Syzygium cumini herbs extract have been reported to have antidiabetic activity. This study aimed to predict the molecular target of chemical constituents of S. polyanthum and S. cumini as well as study their interactions with various macromolecular targets of an antidiabetic agent. Molecular docking of all ligands was studied using the Autodock Vina program in PyRx, and the results are presented as binding affinity values (kcal/mol) of ligand against the protein. PyMOL is used to visualize the 3D molecular of docked conformation and ligand-protein interactions. The predicted pharmacokinetic parameters were obtained by SwissADME. Delphinidin-3-gentiobioside and isoquercitrin are predicted to have good interaction with DPP-4 and α-glucosidase, respectively. However, they are predicted to have poor absorption properties. Quercetin and kaempferol are predicted to have good interaction with PTP1B and glucokinase and showed good pharmacokinetic properties.

Abstract

Inflammation is a response to tissue damage due to various adverse stimuli, both chemical and mechanical, infections and foreign bodies such as bacteria and viruses. This study aims to determine the effect of the anti-inflammatory effect of ethanol extract of karamunting stems. Karamunting stems were extracted by maceration method using 96% ethanol solvent. Each anti-inflamatory test was divided into five groups, namely the negative control group (CMC Na), the positive control group (Diclofenac Na), the Karamunting stem extract test group (dose of 100 mg/kg BW, 200 mg/Kg BW, and 400 mg/kg). kg BW). Anti-inflammatory test using the Rat hind paw edema method induced by 1% carrageenan. Data analysis used the Shapiro-Wilk test and then continued with a one-way ANOVA test. The results showed that the ethanol extract of karamunting stems had an anti-inflammatory effect at doses of 200 mg/Kg BW and 400 mg/Kg BW.the karamunting stem could provide an analgesic effect at a dose of 200 mg/Kg BW.

Abstract

Drug management in the hospital is one of the important hospital management. Its inefficiency can have a negative impact on the hospital, both medically and economically. The purpose of this study was to evaluate drug management at the pharmacy installation of Ki Ageng Selo General Hospital by using efficiency indicators and implementing improvement strategies using the Hanlon method. This study uses a descriptive design for data that is retrospective and concurrent. Data in the form of quantitative and qualitative accompanied by interviews with related parties. All stages of drug management at the Pharmacy Installation of Ki Ageng Selo General Hospital were measured for their level of efficiency using indicators from the Ministry of Health, Permenkes, and WHO, then compared with standards and described based on priority analysis of action plans using the Hanlon method. The results of the study according to the standards were: the suitability of the available drug items with the RS Formulary (92.21%), the frequency of delayed payments (5 times), the percentage of correctness between the drug and the stock card (100%), the level of drug availability (12 months), average time spent serving prescriptions to patients (non-concocted 7.7 minutes, concocted 17 minutes), percentage of prescriptions with generic drugs (84.9%). Stages of drug management that are not in accordance with standards, namely: the percentage of available capital/funds with all the funds needed (202.14%), the percentage of drug procurement funds allocation (20.17%), the percentage of expired drug value (8.1%), the number of drug items per prescription sheet (4.27). Priorities for handling problems are as follows: 1) Monitor drugs for a maximum of 6 months before the ED, 2) Increase the role of KFT in evaluating and coordinating with doctors regarding the use of drugs to avoid polypharmacy, 3) Evaluation is needed every 3 months or 6 months for budget revisions, 4) carry out an evaluation at the end of each year to determine the budget and better coordination between drug managers and hospital fund providers.

Abstract

Background: Polycystic ovary syndrome (PCOS) is an endocrine disorder. Parameters characteristic of the disorder include weight gain, insulin resistance and malondialdehyde (MDA). The traditional medicine used is Moringa oleifera. Objectives: The aim was to determine the effect of weight loss and blood glucose levels, MDA levels and histopathological features of the ovarian follicles in the rat model PCOS-insulin resistance induced by testosterone. Methods: Extraction process, followed by fractionation using n-hexane, ethyl acetate and water, identification of compound content using TLC, and rats were grouped into 7 groups (n=5), namely normal group, negative group, positive group, extract, fraction water, ethyl acetate fraction and n-hexane fraction by looking at the characteristic parameters and ovarian histopathology. Data analysis using ANOVA and Kruskal-wallis. Results: The yield of the extract was 30.4%, the water fraction was 85.59%, the ethyl acetate fraction was 6.64% and the n-hexane fraction was 4.05%. Positive for flavonoids, tannins, alkaloids in the ethyl acetate fraction, the water and extract fractions were positive for tannins, the n-hexane fraction was positive for steroids. The modeling sample group obtained extract body weight 195.40 g, water fraction 195.80 g, ethyl acetate fraction 194.00 g, n-hexane fraction 196.00 g, blood glucose level extract 83.00 mg/dL, water fraction 84.27 mg/dL, ethyl acetate fraction 80.00 mg/dL, n-hexane fraction 122.85 mg/dL, MDA extract content 2.704 nmol/mL, water fraction 3.547 nmol/mL, 1.685 nmol/mL, 5.308 nmol /mL and can improve ovarian histopathology. Conclusion: The most effective value is the ethyl acetate fraction because it has the highest decrease in PCOS characteristics.

Abstract

Background: Type 2 diabetes mellitus can cause complications, one of which is diabetic nephropathy. Parameters that indicate damage to the kidneys are the increase in creatinine and albumin levels. One of the traditional medicines used in the treatment of DM is  red fruit (Pandanus conoideus Lam). Objective: The purpose of this study was to determine the antihyperglycemic activity and the effect of red fruit oil administration on creatinine levels, microalbumin, and renal histopathology in STZ-NA-induced rats. Methods: This study used 30 male Wistar rats conditioned with type 2 DM with STZ-NA induction. The rats have grouped into 6 groups: group I, the normal control, group II, the negative control, group III, the positive control (pioglitazone 15 mg/kg BW), and groups IV, V, and VI, the red fruit oil respectively 1.35 mL/kg BW,2.7 mL/kg BW, and 5.4 mL/kg BW. Red fruit oil is made in traditional way and prepared for 2 days. Parameters tested in the study include blood glucose levels, creatinine, microalbumin, and kidney histopathology. Data analysis used the ANOVA method followed by Tukey's post hoc test. Results: The results showed that a red fruit oil dose of 5.4 mL/kg BW was an effective dose in reducing blood glucose levels, microalbuminuria, and serum creatinine, and repairing damage to the kidneys of rats. The percent activity of a red fruit oil dose of 5.4 mL/kgBW for blood glucose levels, microalbuminuria and serum creatinine were 84.69%, 76.30%, and 92.20% respectively. Conclusion: Red fruit oil can reduce blood glucose levels, creatinine levels, microalbumin and can repair kidney damage.

Abstract

The Covid-19 pandemic has had an impact on various aspects of life, one of which is the workload of health workers. This study aims to describe the workload of health workers and determine the relationship between the characteristics of the research subjects and the workload during the Covid-19 pandemic. This research is an analytic observational type with a Cross-Sectional approach, the instrument used is a workload questionnaire. Data were obtained from distributing questionnaires to health workers at health service facilities owned by the Government of Tana Tidung Regency, North Kalimantan in October 2021. The results showed that public health workers had the highest workload score of 26,8 ± 9,32 and physical therapy workers had the lowest workload score was 18,8 ± 4,55. The average score of the workload of health workers is 23,43 ± 5,81. The p-value of the Chi Square analysis between age and workload is 0,013. The workload of health workers in health care facilities during the Covid-19 pandemic is in the medium category. There is a significant relationship between the characteristics of the profession category/occupation and workload, the difference in the profession/occupation determines the level of workload.

Abstract

Hiperglikemia menyebabkan metabolisme karbohidrat dan lemak terganggu. Gangguan sekresi insulin terjadi disebabkan karena adanya radikal bebas akibat pemberian induksi. Hiperglikemik menyebabkan kelainan metabolisme lipid yang ditandai dengan perubahan pada profil lipid berupa kenaikan kadar LDL, trigliserida, kolesterol total dan penurunan kadar HDL. Penelitian ini bertujuan untuk mengetahui efek ekstrak batang brotowali terhadap penurunan kadar glukosa darah dan perbaikan profil lipid. Penelitian ini menggunakan 7 kelompok tikus, dan masing-masing kelompok diinduksi high fat diet (HFD) dan propiltiourasil (PTU) selama 21 hari dilanjutkan dengan pemberian Streptozotocin-nikotinamid   (45/110 mg/kgBB) selama 3 hari, kecuali pada kelompok 1 yaitu kelompok kontrol normal. Kelompok 2 sebagai kelompok negatif (HFD + DM), kelompok 3 dan 4 sebagai kontrol positif (Metfomin dan Simvastatin), kelompok 5, 6 dan 7 diberikan variasi dosis ekstrak batang brotowali 100, 200  dan 400 mg/kgBB. Bahan uji diberikan secara oral selama 14 hari, kemudian dilihat peningkatan berat badan tikus, penurunan kadar glukosa darah, dan perbaikan profil lipid, serta perbaikan profil histopatologi organ pankreas dan hati dengan pewarnaan Hemaktosilin-Eosin (HE).  Hasil penelitian ini menunjukkan bahwa variasi dosis ekstrak batang brotowali mampu menurunkan kadar glukosa darah. Pada perbaikan profil lipid semua variasi dosis ekstrak juga mampu  memperbaiki profil lipid, akan tetapi dosis 400 mg ekstrak merupakan dosis yang paling efektif dalam menurunkan kadar LDL, meningkatkan kadar HDL dan menurunan kadar trigliserida. Pada perbaikan profil histopatologi pankreas dan hati menunjukkan bahwa dengan pemberian variasi ekstrak mampu memperbaiki organ pankreas dan hati.

Abstract

Rumah Sakit perlu memenuhi Standar Pelayanan Minimal (SPM) rumah sakit, termasuk RSUD Muara Teweh. Penelitian ini bertujuan mengetahui bagaimana pelaksanaan SPM RS terutama di Instalasi Farmasi RSUD Muara Teweh yang meliputi 4 indikator, yaitu waktu tunggu pelayanan resep, kepuasan pasien, tidak adanya kesalahan pemberian obat dan penulisan resep sesuai dengan formularium dan untuk mengetahui faktor apa saja yang menghambat pelaksanaan SPM di RSUD muara Teweh. Penelitian ini merupakan penelitian deskriptif non eksperimental dengan pendekatan secara kualitatif dan kuantitatif. Data diperoleh dari hasil observasi, wawancara pada pihak yang terkait dengan empat indikator dan kuesioner tingkat kepuasan pelanggan kepada pasien. Penelitian ini dilakukan di RSUD Muara Teweh pada bulan Desember 2016 dengan sampel sebesar 110 untuk waktu tunggu pelayanan resep jadi dan waktu tunggu pelayanan resep racikan, untuk kepuasan pasien diambil 110 pasien yang sudah pernah lebih dari satu kali berkunjung ke instalasi farmasi, untuk cek tidak adanya kesalahan pemberian obat dan kesesuaian penulisan resep diambil seluruh pelayanan resep pada bulan Desember. Hasil penelitian menunjukkan bahwa rata – rata waktu tunggu pelayanan obat yaitu 25,1 menit/lembar resep obat jadi, 35 menit/lembar resep obat racikan, kepuasan pasien terhadap pelayanan farmasi 94,88%, tidak adanya kesalahan pemberian obat 100% dan kesesuaian penulisan resep dengan formularium 99%. Faktor yang mempengaruhi pelaksanaan SPM Rumah Sakit adalah SDM dan sarana prasarana.

Abstract

Daun Kemangi (Ocimum sanctum L.) mempunyai kandungan kimia yang dapat digunakan untuk melindungi tubuh dari pengaruh radiasi. Kelemahan pembuatan sediaan topikal menggunakan bahan aktif ekstrak tanaman adalah bentuk, warna, dan bau yang khas. Penelitian ini bertujuan untuk mengetahui pengaruh dari kombinasi Aristoflex® AVC dan Glycerin terhadap stabilitas fisik dan aktivitas sebagai sunscreen agar dapat memperbaiki kelemahan dari ekstrak sebagai bahan aktif sediaan topikal. Ekstrak diperoleh melalui metode maserasi dengan pelarut n-heksan dan ethanol 70%. Paramater yang diuji meliputi daya sebar, daya lekat, pH, viskositas dan nilai SPF. Nilai SPF dihitung secara in vitro menggunakan alat spektrofotometer. Spektrum absorbansi sampel dalam bentuk larutan diperoleh pada kisaran 320-290 nm, setiap interval 5 nm. Nilai SPF sediaan gel dianalisis menggunakan metode Mansur. Hasil yang diperoleh kemudian diamati pengaruh masing-masing kombinasi Aristoflex® AVC dan Glycerin terhadap sifat fisik dan nilai SPF. Penambahan Aristoflex® dapat menaikkan nilai viskositas (317 dPas), daya lekat (2,01 detik), pH (5,10) dan nilai SPF (5,47), namun menurunkan nilai daya sebar (3,0 cm). Penambahan Glycerin dapat menaikkan nilai daya sebar (5,10 cm), dan pH (5,10), namun menurunkan nilai daya lekat (1,10 detik), viskositas (143 dPas) dan nilai SPF (3,18).
