Abstract

Hemodialysis (HD) patients with chronic kidney disease takes a long time and requires patient compliance. This will give the patient a psychological stressor which will then affect the quality of life. Apart from having an effect on quality of life, therapy hemodialysis also used a high cost, especially if the patient has complications. The purpose of this study was to determine the amount of hemodialysis costs, utility index, and the effect of complications on costs and utility index in hemodialysis patients. This study was a cross-sectional analytical research with the perspective of hospital. The research subjects were outpatient CKD who underwent hemodialysis at the HD Unit of Adji Batara Agung Dewa Sakti Hospital in September-Oktober 2020. Data were obtained use the SF-6D instrument, medical records, and financial financing. Data analysis used independent sample t-test and Kruskal-Wallis.The results showed that the average real cost of therapy in hemodialysis patients for a month was Rp. 12,014,488±0, while the real cost per hemodialysis episode is Rp. 1,316,722±273,989. The mean value of patient utility index as measured by the SF-6D was 0.717±0.143. The statistical test results show a significant value of 0.049 <0.05 on the utility index, which means that there is a relationship between complications on the utility index and a significant value of 0.125> 0.05 on costs, which means that there is no relationship between complications and costs in CKD patients with hemodialysis Keywords: Hemodialysis, Cost analysis, Index utility, SF-6D

Abstract

The lean approach can make hospitals become more organized so they can improve the quality of services to patients by reducing waste, including the occurrence of errors, movements, transportation and poor service flow that leads to long service waits which certainly is not expected by patient. The purpose of this study was to identify the waste and the root causes of waste that occurred in the Pharmacy Installation at RSUD X Jombang. This research method is a qualitative descriptive study. The stages of the research are the observation of the drug management process flow which is made in the form of value stream mapping, focus group dicussion, and fishbone diagrams to determine the root cause of the problem. The results showed that the waste in the procurement process of 96.07% included in the category of necessary but non-value added, the acceptance process amounted to 62,59% of activities included in the category of necessary but non-value added, in the storage process there was a waste of 53, 39% included in the category of non value added, and in the process of using waste by 36.22% included in the category of non value added. The root cause of the problem of procurement is the length of time it takes to request the approval of PPK and PPTK, at the reception there is no specific place for receipt of goods, in storing all records and reporting is still manual and in use there is only one prescription receipt and delivery of drugs. In the future state mapping it is known to save time by 45.60% on procurement, 58% on revenue, 53.39% on storage, and 69% on usage.

Abstract

ABSTRACTIbuprofen is a derivative of propionic acid, a class of non-steroidal anti-inflammatory drugs (NSAIDs). Ibuprofen has side effects on the gastrointestinal tract. So that topical use can be a solution to reduce the side effects of oral use because of the release of topical drugs through the stratum corneum which makes it selective in certain places. The preparation chosen was a topical microemulsion because it is more thermodynamically stable and has no effect on the gastrointestinal tract. This study aims to determine the effect of the ratio of surfactant concentrations of various ratios of tween 20:tween 80 and cosurfactant propylene glycol to the penetration rate of ibuprofen across the membrane and to determine the physical characteristics of topical microemulsions. Selection of tween surfactant and propylene glycol cosurfactant because it has high physical stability. This research is an experimental study, namely by making four formulations with various ratios of surfactant and cosurfactant with a difference of 2.5% in each formulation of topical ibuprofen microemulsion with a concentration of 5% ibuprofen using variations of surfactant tween 20 and tween 80, cosurfactant propylene glycol with black cumin oil phase. Evaluation of the preparation by organoleptic test, pH test of the preparation, examination of the type of emulsion, particle size measurement, microemulsion release test in vitro using the Franz diffusion method. The results show that the concentration ratio of surfactant tween 20:tween 80 and cosurfactant propylene glycol can affect particle size, pH and penetration velocity (flux) in Franz diffusion. The results of the study showed that in F1 using surfactant tween 20:tween 80 with a concentration of 71.6% (6:4) had the smallest particle size of 11.5 nm and the largest flux value was 128.45 g.cm-2.H- 1. In conclusion, variations in surfactant and cosurfactant concentrations can affect the pH of the preparation, particle size, penetration speed (flux). The results obtained that the cumulative percent and penetration speed of all formulas had significant differences (p0.05).

Abstract

Salah satu komplikasi mikrovaskuler pada kondisi diabetes yakni retinopati diabetik dimana terjadi akibat kerusakan sel endotel pada pasien diabetes yang dapat meningkatkan ekspresi VEGF. VEGF mampu menginduksi adhesi leukosit sel endotel retina yang mengakibatkan kerusakan retina. Penelitian ini bertujuan guna mengetahui aktivitas ekstrak etanol daun sirih merah (EDDSM) dalam menurunkan kadar glukosa darah serta aktivitasnya pada kadar VEGF plasma tikus putih retinopati diabetik. Daun sirih merah dikeringkan dan dibuat serbuk halus lalu diekstraksi menggunakan pelarut etanol 70% dengan metode remaserasi. Pengujian dilakukan pada 35 ekor tikus yang terbagi dalam 5 kelompok perlakuan yakni kelompok kontrol negatif, kontrol positif (Glibenklamid), EEDSM dosis 50, 100 dan 200 mg/kgBB. Metode uji antidiabetes menggunakan induksi STZ-NA secara intraperitonial kemudian mengukur kadar glukosa, VEGF awal dan setelah perlakuan pada setiap kelompok kemudian dilakukan analisa menggunakan uji oneway Anova. Berdasarkan hasil pengukuran kadar glukosa darah dan VEGF plasma diperoleh perbedaan yang bermakna diantara kelompok kontrol negatif dengan kontrol positif serta EEDSM dengan nilai signifikan (p<0,05). Hasil penelitian diperoleh EEDSM dosis 100 mg/kgBB ialah dosis efektif dalam menurunkan kadar glukosa darah dengan % PKGD yaitu 18,22%. EEDSM dosis 200 mg/kgBB ialah dosis efektif guna menurunan VEGF plasma tikus retinopati diabetik sebesar 31,20%.

Abstract

Loss becomes a problem in the hair that causes baldness. Biotin is a vitamin that nourishes hair and prevents hair loss. Biotin which is formulated in microemulsion tends to be transparent, has low viscosity and high solubility. Formula optimization was carried out to determine the concentration of span 80, tween 80, and PEG 400 in biotin microemulsion hair tonic. The research was conducted on an experimental basis using a factorial design method with 3 factors, namely span 80, tween 80, and PEG 400. The optimum microemulsion formula was determined using Design Expert 13 software using the Factorial Design method. Testing the physical quality of the preparation is carried out at the optimum formula. Optimization of biotin microemulsion hair tonic showed results of span 80 (20%), tween 80 (9.466%) and PEG 400 (5%) with a predicted viscosity of 46.75 cPs and globule size of 2.98 µm. Prediction testing and test results compared to the One Sample T-test showed results that were not significantly different with a significance value of >0.05. The optimum formula has a viscosity value of 47.67 cPs and a globule size of 2.4 µm. The results obtained concluded that the optimal composition composition of the biotin microemulsion formula from the combination of span 80, tween 80 and PEG 400 which produces hair tonic with good effects and physical quality is span 80 20%, tween 80 9.47% and PEG 400 5%.

Abstract

ABSTRACT Inventory Control Analysis Of AVMedicine CategoriesbyABC, VEN and EOQ Method In Pharmacy Installation at Aura Syifa Hospital Kediri.Pharmaceutical logistics management and pharmaceutical services aim to identify, prevent and resolve drug-related problems. Broadness of services and Aura Syifa Hospital Kediri participation in Jaminan Kesehatan Nasional (JKN) requires an efficient pharmaceutical supplies management. Regulations changes in Badan Penyelenggara Jaminan Sosial (BPJS) regarding tiered referral also change the pattern of drug consumption. In addition, arrears of BPJS patient claim payment at Aura Syifa Hospital Kediri during the last few months caused hospital to be slightly constrained in terms of operations and services, especially related to the procurement of medicines. Purpose of this study was to analyze control system and improve the efficiency of managing AV classification medicine using ABC, VEN and EOQ methods inpharmacy installation at Aura Syifa Hospital Kediri.This study wasdescriptive with retrospective data collected, data used in the period January 2018 - December 2018. Medicine analyzed were oral drugs, injections and infusions that were included in group A from the results of ABC analysis, group V from the results of VEN analysis so it was obtained BPJS medicine AV categories. Result of this study were combination of ABC and VEN analysis methods could identify drugs that require strict inventory control and prioritize 12 BPJS medicine in AV category. Pharmacy installation at Aura Syifa Hospital Kediri in 2018 did not meet cost efficiency that is: procurement frequency was higher than the EOQ method. Calculation of ABC, VEN and EOQ methods may improve efficiency in controlling AV classification medicine supplies in BPJS patients at IFRS Aura Syifa Kediri.

Abstract

The Telkomedika Health Center (THC) Madiun Clinic requires a good performance evaluation/measurement of the THC Madiun Clinic Pharmacy Installation. Therefore, research is needed which aims to determine the performance of the THC Madiun Clinical Pharmacy Installation using the Balanced Scorecard method based on 4 perspectives, namely finance, internal business processes, growth and learning, and customers. This research was a descriptive study. Data were obtained retrospectively and concurrently through interviews with pharmacists at pharmacy installations, filling out questionnaires by employees and patients, as well as direct observation. Data were analyzed descriptively and compared with applicable standards. The research results showed that: 1) finance was efficient, but still needs to be improved; 2) provision of drug information needs to be improved; 3) employee job satisfaction was in the moderate range; 4) patients were satisfied with the quality of service, but there was a gap between patient expectations and performance. It was concluded that the performance of the Madiun THC Clinic Pharmacy Installation was quite good, but there were still a number of things that needed to be improved. Managers should conduct regular monitoring and evaluation to improve organizational performance.Keywords: performance evaluation; balanced scorecard; clinical pharmacy installation ABSTRAK Klinik Telkomedika Health Center (THC) Madiun membutuhkan evaluasi/pengukuran kinerja Instalasi Farmasi Klinik THC Madiun secara baik. Maka diperlukan penelitian yang bertujuan untuk mengetahui kinerja Instalasi Farmasi Klinik THC Madiun dengan metode Balance Scorecard berbasis 4 perpesktif yaitu keuangan, proses bisnis internal, pertumbuhan dan pembelajaran, serta customer. Penelitian ini merupakan studi deskriptif. Data diperoleh secara  retrospektif dan concurrent melalui wawancara kepada apoteker instalasi farmasi, pengisian kuesioner oleh karyawan dan pasien, serta observasi langsung. Data dianalisis secara deskriptif dan dibandingkan dengan standar yang berlaku. Hasil penelitian menunjukkan bahwa: 1) keuangan sudah efisien, namun masih perlu ditingkatkan; 2) pemberian informasi obat perlu ditingkatkan; 3) kepuasan kerja karyawan berada pada rentang cukup; 4) pasien merasa puas dengan kualitas pelayanan, namun terdapat gap antara harapan dan kinerja pasien. Disimpulkan bahwa kinerja Instalasi Farmasi Klinik THC Madiun cukup baik, namun masih ada beberapa hal yang perlu ditingkatkan. Sebaiknya pengelola melakukan monitoring dan evaluasi secara berkala untuk meningkatkan kinerja organisasi.Kata kunci: evaluasi kinerja; balanced scorecard; instalasi farmasi klinik

Abstract

Lean is a continuous effort to eliminate waste and increase the value added of both goods and services in order to provide value to customers. The purpose of this study was to identify critical wastes that affect patient satisfaction at the Pharmacy Installation at Abdul Wahab Sjahranie Hospital, Samarinda. This qualitative research method was a case study. The stages of the research were observing the flow of the service process in the form of value stream mapping, distributing questionnaires about value waste and patients, conducting interviews using the 5-why method to find out the root causes and critical waste that occurs. The results showed that the lead time for concoction recipes was 100.4 minutes and the VAR for recipes was 67%, while the lead time for non-concocted recipes was 45.2 minutes and the VAR for non-concocted recipes was 48%. This shows that drug prescription service activities at the Outpatient Pharmacy Installation at Abdul Wahab Sjahranie Hospital Samarinda were included in the lean category. Critical waste was waste motion (53.3%). It could be concluded that critical waste is motion and waiting waste which is caused by several cases of drug shortages that often occur. What can be suggested is to improve drug planning or procurement in order to minimize drug vacancies.Keywords: pharmaceutical installation; lean hospital; critical waste; service improvement ABSTRAK Lean adalah suatu upaya terus menerus untuk mengeliminasi pemborosan (waste) dan meningkatkan value added produk baik barang maupun jasa agar memberikan nilai kepada pelanggan. Tujuan penelitian ini adalah mengidentifikasi waste kritis yang berpengaruh terhadap kepuasan pasien di Instalasi Farmasi RSUD Abdul Wahab Sjahranie Samarinda. Metode penelitian kualitatif ini adalah studi kasus. Tahapan penelitian yaitu observasi alur proses pelayanan dalam bentuk value stream mapping, penyebaran kuisioner tentang value waste dan pasien, melakukan wawancara dengan metode 5-why untuk mengetahui akar penyebab dan waste kritis yang terjadi. Hasil penelitian menunjukkan bahwa lead time resep racikan adalah 100,4 menit dan VAR resep adalah 67%, sedangkan lead time resep non racikan adalah 45,2 menit dan VAR resep non racikan adalah 48%. Ini menunjukkan bahwa aktivitas pelayanan resep obat di Instalasi Farmasi Rawat Jalan RSUD Abdul Wahab Sjahranie Samarinda termasuk dalam kategori lean. Waste kritis adalah waste motion (53,3%). Disimpulkan bahwa waste kritis adalah waste motion dan waiting yang disebabkan oleh adanya beberapa kasus kekosongan obat yang sering terjadi. Hal yang dapat disarankan adalah memperbaiki perencanaan atau pengadaan obat agar dapat meminimalisir terjadinya kekosongan obat.Kata kunci: instalasi farmasi; lean hospital; waste kritis; perbaikan pelayanan

Abstract

Syzygium polyanthum leaf extract and Syzygium cumini herbs extract have been reported to have antidiabetic activity. This study aimed to predict the molecular target of chemical constituents of S. polyanthum and S. cumini as well as study their interactions with various macromolecular targets of an antidiabetic agent. Molecular docking of all ligands was studied using the Autodock Vina program in PyRx, and the results are presented as binding affinity values (kcal/mol) of ligand against the protein. PyMOL is used to visualize the 3D molecular of docked conformation and ligand-protein interactions. The predicted pharmacokinetic parameters were obtained by SwissADME. Delphinidin-3-gentiobioside and isoquercitrin are predicted to have good interaction with DPP-4 and α-glucosidase, respectively. However, they are predicted to have poor absorption properties. Quercetin and kaempferol are predicted to have good interaction with PTP1B and glucokinase and showed good pharmacokinetic properties.
