Abstract

Background: Obstructive Sleep Apnea (OSA) in hemodialysis patients results from a shift in the rostral fluid overnight. Compression Stocking is a fairly inexpensive therapy compared to other OSA therapies. Compression Stocking is leg stocking that use a pressure force of 20-30 mmHg, effective in preventing the accumulation of fluid that is installed in accordance with the patient's feet. Objective: to determine the effect of Compression Stockings on Obstructive Sleep Apnea (OSA) hemodialysis patients. Method: Search used several electronic databases such as Google Scholar, Proquest, and PubMed. The inclusion criteria in this literature study were quantitative research papers and experiments in English, full text, peer review, published between 2008-2018. Results: Searching for articles with predetermined keywords obtained 718 articles from the database, Google Schoolar obtained 550 articles, ProQuest obtained 68 articles, and PubMed obtained 100 articles. Articles were screened to 32 articles according to the purpose of the review with inclusion and exclusion criteria. After screening 15 articles that met the criteria were read intensively and then analyzed based on content analysis. Conclusion: An overnight rostral fluid shift is one of the factors contributing to the pathogenesis of OSA in hemodyalysis patients. Compression Stocking can be considered as an alternative therapy to prevent fluid retention in the legs and overnight shift to the neck thus providing evidence for a new therapeutic approach to OSA.

Abstract

Background: Obstruction Sleep Apnea (OSA) in hemodialysis patients is a sleep disorder that involves stopping or decreasing air flow to breathe during sleep due to excess fluid in the leg. Compression Stockings (CS) with 30-40 mmHg are high pressure elastic stockings that are assumed to reduce foot fluid accumulation during the day and to prevent overnight fluid shifting to the neck.Purpose: This study aimed to determine the effects of 30-40 mmHg CS on the risk of OSA in hemodialysis patients.Method: This research used a quasi-experimental pre-posttest with control group design. A purposive sampling was applied to recruit as many as 68 hemodialysis patients. The risk score of OSA was measured using STOP Bang questionnaire. High pressure CS were given for one week to the intervention group with two OSA measurements on the 4th and 7th days. The data then were analyzed using the Post Hoc Wilcoxon test and the Mann Whitney test. Results: There was a significant difference between the risk score for OSA before and after the use of 30-40 mmHg Compression Stocking in the intervention group on day 4 (p=0.000) and day 7 (p=0.000) compared to the control group. Conclusion:  High pressure CS decreased the risk of OSA in hemodialysis patients by avoiding fluid retention in the legs, thus preventing fluid shifting to the neck. CS should be recommended as an alternative to prevent the risk of OSA in hemodialysis patients.

Abstract

Immobilising critically ill patients increases the prevalence of pressure ulcers, which can increase the length of a patient’s stay and rehabilitation, plus leading to increased pain and other complications. An independent intervention of effleurage massage using essential oil is considered to be a preventative measure for integrity anoxia, which causes pressure ulcers. The purpose of this research is to identify the effect of effleurage massage using virgin coconut oil (VCO) on the risk level of pressure ulcers in the intensive care unit. Quasi-experimental one-group pre- and post-tests were performed on 34 patients through purposive sampling using the Braden Scale. The interventions were carried out for 20 minutes on the back of the head, neck, back, glutei, sacrum, hands and feet. A dependent t-test was used in this research because it met the parametric test requirements, followed by a Repeated Measures ANOVA test. The results of the dependent t-test showed that the intervention had a significantly positive effect on pressure ulcers (p=0.0001) if effectively carried out for at least three continuous days. It was concluded that effleurage massage using VCO has a significant effect on reducing the risk level of pressure ulcers in immobilised patients.
