Abstract

Latar belakang: Pekerja bengkel las berisiko tinggi mengalami luka bakar, gangguan mata, masalah pernapasan, hingga kecacatan akibat paparan sinar las, percikan logam, debu, dan asap. Upaya pencegahan melalui penggunaan alat pelindung diri (APD) belum optimal karena tingkat kepatuhan yang rendah, dipengaruhi keterbatasan pengetahuan, sikap kurang positif terhadap APD, serta praktik kerja yang tidak aman, sehingga meningkatkan kemungkinan terjadinya kecelakaan kerja. Penelitian ini bertujuan menganalisis efektivitas penyuluhan dengan metode diskusi dan ceramah terhadap peningkatan pengetahuan, sikap, dan tindakan pekerja bengkel las mengenai APD. Metode: Penelitian ini menggunakan desain quasi experiment dengan rancangan non-randomized pretest–posttest control group design. Penelitian dilaksanakan di Kelurahan Tanjung Gusta, Kota Medan, Provinsi Sumatera Utara, pada periode 2–10 April 2022. Subjek penelitian mencakup seluruh pekerja bengkel las berjumlah 36 orang yang tersebar di lima bengkel. Pengambilan sampel menggunakan teknik total sampling sehingga seluruh populasi dijadikan responden. Peserta dibagi menjadi dua kelompok, yaitu kelompok intervensi (18 orang) dan kelompok kontrol (18 orang). Pengumpulan data dilakukan menggunakan kuesioner, sedangkan analisis data menggunakan uji Wilcoxon, Mann–Whitney, dan N-Gain dengan tingkat signifikansi α = 0,05. Hasil: Terdapat perbedaan bermakna pada pengetahuan (p < 0,001), sikap (p < 0,001), dan tindakan pekerja bengkel las terkait APD sebelum dan sesudah intervensi penyuluhan. Penyuluhan dengan metode diskusi lebih efektif dalam meningkatkan perilaku pekerja bengkel las dibandingkan penyuluhan dengan metode ceramah. Kesimpulan: Penyuluhan menggunakan metode diskusi terbukti lebih efektif dalam meningkatkan perilaku pekerja bengkel las tentang penggunaan APD dibandingkan penyuluhan yang dilaksanakan dengan metode ceramah

Abstract

Patient satisfaction will encourage high utilization of health services. Patient satisfaction is one of the measures of service quality that will determine hospital performance. Quality and patient satisfaction is one of the factors that make patients come back for treatment because of the feeling of satisfaction they receive or even never come again. This study aims to evaluate the quality of medical services by examining the following dimensions: tangible evidence, reliability, timeliness, confidence, and compassion towards the well-being of dental clinic patients at Haji Amri Tambunan Regional General Hospital. Analytic survey methodology with a cross-sectional design was used in this study. With a sample size of 377, the study population consisted of all dental clinic patients at the Haji Amri Tambunan Regional General Hospital in Deli Serdang from July to December 2023, totaling 1,543 people. Questionnaires were used to collect data, and the Chi-Square test was used to statistically test the findings. Based on the results of the study, it can be stated that there is a significant relationship between the quality of health services based on the dimensions of physical evidence (tangibles) (p-value=0.000), reliability (p-value=0.001), responsiveness (p-value=0.034), assurance (p-value=0.037), and empathy (empathy) (p-value=0.005) to patient satisfaction. Dental polyclinic patient satisfaction is most influenced by the physical evidence variable which is one aspect of health service quality. The dimension of responsiveness is the best dimension of health service quality, while the dimension of physical evidence has the most dominant influence on dental polyclinic patient satisfaction, it is hoped that the Haji Amri Tambunan Deli Serdang Regional General Hospital can improve the reliability dimension, namely providing services as promised accurately and reliably. Abstract

Hospitals face increasing pressure to provide timely and high-quality care amid rising patient demand, limited resources, and complex service pathways. Inefficient patient flow between emergency and inpatient services often results in prolonged waiting times, suboptimal bed utilization, and reduced patient satisfaction. Lean management has been applied in healthcare to improve operational performance through the elimination of non-value-added activities. This study evaluated the impact of lean management implementation on operational efficiency in the emergency department to inpatient care pathway at Royal Prima Hospital, Medan. A quantitative quasi-experimental study with a pre-test and post-test design was conducted. Lean management interventions were implemented across the emergency department to inpatient admission process, including waste identification, workplace organization, value stream mapping, and continuous improvement activities. Data were obtained from hospital operational records and structured questionnaires administered to healthcare personnel. Operational efficiency indicators included patient waiting time, bed occupancy rate, average length of stay, turnover interval, bed turnover, and patient satisfaction. Data were analyzed using descriptive statistics, paired statistical tests, Pearson correlation, and simple linear regression at a 95 percent confidence level. Lean management implementation significantly improved operational efficiency. Total waiting time from emergency department admission to inpatient transfer decreased from 135 minutes to 74 minutes, representing a 45 percent reduction. Bed occupancy rate increased from 78 percent to 82%, average length of stay decreased from 4.8 days to 4.1 days, turnover interval declined from 1.9 days to 1.3 days, and bed turnover increased from 42 to 48 cycles. Patient satisfaction increased from 72 percent to 86 percent. Pre-test and post-test analysis showed a statistically significant improvement in operational efficiency (p < 0.05). Lean management significantly enhanced patient flow, bed utilization, and patient satisfaction without additional resource investment. These findings support lean management as a sustainable strategy for improving hospital operational efficiency in high-demand clinical settings.

Abstract

Type 2 Diabetes Mellitus (T2DM) is a chronic metabolic disorder with substantial psychophysiological components. Integrated therapeutic approaches combining physical exercise and mindfulness practices may address both glycemic and psychological aspects, yet comprehensive evidence remains limited. This systematic review evaluated the effectiveness of combined physical exercise and mindfulness interventions on glycemic regulation and psychophysiological outcomes in patients with T2DM. A systematic search of PubMed, ScienceDirect, and Google Scholar identified experimental and quasi-experimental studies published between 2021 and 2025. Eligible studies included adult patients with T2DM who received combined physical exercise and mindfulness interventions. Outcomes assessed included glycemic control (HbA1c, fasting blood glucose) and psychophysiological parameters (stress, depression, quality of life). Narrative synthesis and comparison tables were used for qualitative analysis, and risk of bias was evaluated using the Cochrane RoB 2 tool and the Newcastle–Ottawa Scale. Five studies met the inclusion criteria (n = 386 participants). Combined interventions produced greater improvements than single-component or control conditions. Mean HbA1c reduction ranged from 0.7% to 1.1%, fasting blood glucose decreased by 12.5 to 23.07 mg/dL, and depression or stress scores declined by 20% to 45%. Additional benefits included better nerve conduction velocity, self-efficacy, medication adherence, and quality of life. Proposed mechanisms involved modulation of the hypothalamic-pituitary-adrenal axis, enhanced insulin sensitivity, and increased neuroplasticity. In conclusion, combined physical exercise and mindfulness interventions demonstrate superior effectiveness over single-modality or standard care approaches in improving glycemic control and psychophysiological well-being in T2DM. This integrated strategy represents a promising adjunct for T2DM management. Further randomized controlled trials with larger cohorts and extended follow-up are needed to substantiate clinical recommendations.

Abstract

Stunting merupakan gangguan pertumbuhan fisik pada anak yang disebabkan oleh kurangnya asupan gizi dalam jangka waktu lama sehingga mengakibatkan masalah gizi kronis. Pencegahan stunting sebaiknya dimulai sejak dini termasuk pada remaja putri yang nantinya akan menjadi ibu. Penelitian ini bertujuan untuk mengetahui seberapa efektif penyuluhan dengan media e-learning berbasis audio visual terhadap pengetahuan dan sikap remaja putri tentang stunting di SMAN 1 Kampung Rakyat. Penelitian ini menggunakan pendekatan kuantitatif dengan desain pra eksperimen jenis One Group Pretest-Posttest. Sampel penelitian sebanyak 109 remaja putri yang dipilih melalui teknik purposive sampling. Hasil penelitian menunjukkan rata-rata skor pengetahuan sebelum penyuluhan sebesar 5,88 (58,8) dan meningkat menjadi 8,50 (85) setelah dilakukan intervensi. Untuk sikap rata-rata skor awal sebesar 21,66 (54,15) meningkat menjadi 30,18 (75,45) setelah dilakukan penyuluhan. Uji statistik Wilcoxon T-Test menunjukkan nilai p sebesar 0,000 yang berarti terdapat pengaruh yang signifikan antara penyuluhan audio visual e-learning terhadap pengetahuan remaja putri tentang stunting. Oleh karena itu, metode penyuluhan audio visual e-learning dapat digunakan sebagai alternatif penyuluhan kesehatan yang efektif untuk mencegah stunting sejak remaja.

Purpose: This community service activity aims to improve the performance of the immunization program through strengthening leader behaviors and management processes at Puskesmas Pujud. Design/methods/approach: This activity used a descriptive approach through community service implementation. Data were collected using observation, interviews, questionnaires, and documentation. The intervention included counseling, training, and mentoring for health workers. A total of 20 participants were involved. Findings/results: The results showed an improvement in participants’ understanding of leadership and management concepts. Team coordination improved, particularly in task distribution and communication. The implementation of management functions became more structured, especially in planning, implementation, and supervision. Furthermore, the performance of the immunization program improved, indicated by increased immunization coverage, timely implementation, and improved recording and reporting systems. Conclusions: Community service activities focusing on strengthening leadership and management processes can effectively improve the effectiveness of immunization programs at primary healthcare centers.

Abstract

Abstract Hypertension is one of the non-communicable diseases that has become a major public health problem because it increases the risk of cardiovascular diseases and premature death. Informal sector workers have diverse job characteristics with varying levels of activity and working conditions that may increase the risk of hypertension. This study aimed to analyze the relationship between several risk factors and the incidence of hypertension among informal sector workers in the working area of Darussalam Public Health Center in 2025. This study used a quantitative research design with a cross-sectional approach. The population consisted of all informal sector workers in the working area of Darussalam Public Health Center, with a total sample of 60 respondents selected using the total sampling technique. Data were collected through questionnaires and blood pressure measurements. Data analysis was conducted using univariate and bivariate analysis with the Chi-Square test at a 95% confidence level (Ethical approval number: 048/SP/UNPRI.FI.9.E/SPPKI.1/VIII/2025). The results showed that there was no significant relationship between family history (p=0.113), age (p=0.362), gender (p=0.957), salt consumption (p=0.282), caffeine consumption (p=0.297), physical activity (p=0.241), obesity (p=0.258), work stress (p=0.251), type of occupation (p=0.405), working duration (p=0.492), and income (p=0.100) with the incidence of hypertension. However, there was a significant relationship between length of employment and the incidence of hypertension (p=0.048). In conclusion, length of employment is a factor associated with the incidence of hypertension among informal sector workers. Therefore, preventive efforts are needed through proper work time management, increasing workers’ awareness to routinely check blood pressure, and adopting healthy lifestyle behaviors to reduce the risk of hypertension. Abstract

The purpose of this study is to determine the effect the need for information and educational media in supporting self-management of people with diabetes mellitusat RSU Royal Prima Medan. This type of research is a quantitative research with a cross sectional approach. The population in this study isout patient diabetes mellitusat RSU Royal Prima Medan from January 2022 to December 2022 who have been diagnosed based on criteria totaling 548 patients. Sampling used the Taro Yamane formula with a total sample of 100 samples. Data analysis in this study used univariate analysis and bivariate analysis. The research results show that the majority of diabetes mellitus patients in this study were aged 31-40 years and the majority of patients were male, the quality of self-management of people with diabetes mellitus at RSU Royal Prima Medan was good for 89 patients and 11 patients who were not good, Information needs play a role and influence in supporting self-management of people with diabetes mellitus at RSU Royal Prima Medan p value <0.05, educational media play a role and influence in supporting self-management of people with diabetes mellitus at RSU Royal Prima Medan p value <0.05. Suggestions for RSU Royal Prima Medan should be to maximize all forms of educational activities as well as information regarding self-management of people with diabetes mellitus, such as routinely giving seminars to people with diabetes mellitus on how to maintain good self-management for people with diabetes mellitus

Abstract

Nutritional problems in hospitals are assessed according to individual conditions which directly or indirectly affect the healing process. The trend of increasing cases of nutrition-related diseases in all vulnerable groups, from pregnant women, babies, children, teenagers, to the elderly, requires special nutritional management. Therefore, quality food/nutrition services or administration are needed to achieve and maintain optimal nutritional status and accelerate healing. This research is viewed from the intervention carried out on the research object, which is pre-experimental research using descriptive evaluative methods, observational approaches and interviews. The results of this research show that the ratio of procurement of dry food ingredients at the Nutritional Installation of RSUP H. Adam Malik Medan is efficient. It is proven that the TOR value obtained is 1.79, which means that the stock of dry food ingredients is used and updated properly, so that the supply is sufficient to meet patient needs and prevent excessive overstocking
