Abstract

Diet quality and glycemic control in adults with type two diabetes mellitusBackground: As assessed by Alternate Healthy Eating Index (AHEI), diet quality is correlated with glycemic control through its effect on glucose-insulin homeostasis. Studies are evaluating the diet quality index in Indonesia, especially its relation to glycemic control in those with type 2 diabetes mellitus (T2DM), are lacking. Objective: To analyze the association between diet quality and glycemic control T2DM in adults. Methods: This research was a cross-sectional study using data from "Cohort study of non-communicable disease risk factors" in 2016 by the Indonesia Ministry of Health. Hundred and five (105) subjects diagnosed with T2DM, aged 25-65, were recruited in this study. A 24-hour dietary recall was collected, and a diet quality index referred to the USA adapted AHEI-2010 was assessed. The portion of food components was modified based on the Indonesian Dietary Guidelines. Glycemic control was evaluated according to fasting blood glucose (FBG) and 2-hours-post-75-g glucose load (2h-PG). Spearman correlation test was applied in this analysis. Results: A mean age of subjects was 51 years, and 77.1% out of 105 subjects were women. Mean value of FBG 123.5 and 2h-PG 228.9 mg/dL. The total score of AHEI was 53.8. There was not a significant correlation between the total score of AHEI with FBG (r= -0.079) and 2h-PG (r= -0.069) (p>0.05). However, there was a significant association between the fruits score and 2h-PG (r= -0.224) (p<0.05). Most of the subjects consumed fruits containing low-moderate fructose, such as bananas, oranges, papayas. Conclusions: In T2DM, there was no significant association between the total score of current diet quality and poor glycemic control, except for the fruits component.

Abstract

Background:   The compliance of Alternate Healthy Eating Index (AHEI) 2010 influences their risk of complications in type two diabetes mellitus (T2DM). In Indonesia, AHEI-2010 has not been widely used in evaluating the diet quality in people after diagnosed T2DM.Objectives: To analyze changes in diet quality of adults patients with T2DM. Methods: This study was analyzed using a secondary data from “Cohort Study of Non-Communicable Diseases Risk Factors” by Indonesian Ministry of Health, on 105 adults newly diagnosed with T2DM. The diagnosis of T2DM was assessed based on the results laboratory tests of fasting blood glucose (FBG) ≥126 mg/dL and 2-hours post-75-g glucose load (2h-PG) ≥200 mg/dL. Dietary intake data was collected twice (at the beginning and the end of monitoring) using a 24-hour recall. The assessment of diet quality uses modified AHEI-2010 USA according to the Indonesians Dietary Guidelines, especially in the portion of the food components.   Results: The total score for diet quality was higher at the beginning of monitoring 54.9 than the end of monitoring 53.3 and there was no statistical significance differences (p≥0.05). The total score from diet quality decreased 1.1 points, 53.4% of subjects showed score deterioration (deteriorating diet quality) and 46.7% showed score improvement (improved diet quality). There was a significant differences at the beginning and the end of monitoring only to components score of red/processed meat (p <0.05).Conclusions: Changes in diet quality that deteriorating over time during monitoring, characterized by a decrease in the total score for diet quality.

Abstract

Background: Overweight and obesity are the accumulation of excess fat accumulation which has become a serious problem at the global level, one of which is an abnormality of the lipid profile (dyslipidemia) which is a trigger for cardiovascular diseases such as heart disease and stroke. Physical inactivity is one of the causes of health problems such as obesity and other metabolic disorders. Walking is a moderate-intensity aerobic physical activity suitable for all ages and has a variety of health benefits, especially in less active obese adults. Objectives: The purpose of this study was to examine and analyze walking exercise against changes in the lipid profile of overweight women Discusion: This study is a literature review conducted through online database searches of Scopus, Google Scholar, PubMed, and Science Direct to examine relevant research results regarding the benefits of walking exercise to improve lipid profile. The results showed that walking exercise was able to reduce levels of TC, TG, LDL, and increase serum HDL in overweight and obese patients if it was done with the recommended intensity, duration, and frequencyConclusions: walking exercise is an aerobic physical activity that is suitable for overweight and obese patients and has the potential to improve lipid profiles. This study is expected to be the basis for recommendations to increase daily physical activity for the prevention of cardiovascular disease.

Abstract

Obesity is one of the nutritional problems in Indonesia. Meanwhile, based on data from Basic Health Research in 2018, the prevalence of overweight was 13.6% and obesity was 21,8%. This study aimed to analyze sleep quality, stress, physical activity, and food consumption with nutritional status. A cross-sectional study design was applied with 60 subjects aged 20-30 years old selected by cluster random sampling method. Characteristics of subjects, sleep quality, stress levels, physical activity, and food consumption data were collected through interviews by structured questionnaires and direct measurements. The results showed no significant between age, gender, allowance, food expenditure, physical activity, and food diversity with nutritional status (p>0,05). In contrast, sleep quality and stress levels were significant to nutritional status (p<0,05), and stress level was the risk factor for the obesity incidence, OR= 6,2 [95% CI 1,9-20,3]. Thus, stress levels are a risk factor that affects individual nutritional status.

Abstract

Penilaian kualitas diet dengan metode Healthy Eating Index (HEI) versi Indonesia belum banyak digunakan. Metode HEI penting digunakan untuk melihat kualitas diet seseorang dalam keadaan sakit atau sehat yang nantinya bisa untuk menunjukan status kesehatan masa kini dan masa depan yang dilihat dari segi gizi. Tujuan penelitian adalah menganalisis faktor-faktor yang berpengaruh terhadap perubahan kualitas diet pada penderita Penyakit Jantung Koroner (PJK). Penelitian ini menggunakan data Studi Kohort Faktor Risiko Penyakit Tidak Menular periode tahun 2013-2016. Ditemukan 124 kasus baru PJK dari hasil EKG. Konsumsi pangan menggunakan metode 1x24 jam setahun sebelum PJK dan setahun setelah PJK. Kualitas diet dengan US-HEI yang dimodifikasi jumlah porsinya sesuai dengan pedoman gizi seimbang Indonesia. Perubahan kualitas diet dinilai dari selisih skor HEI setelah PJK dengan sebelum PJK. Terdapat perbedaan perubahan dan peningkatan kualitas diet yang signifikan yaitu pada skor total kualitas diet 7.9%, secara spesifik pada skor komponen buah-buahan 51.9%, kacang-kacangan 16.9%, dan sodium 10.6%. Faktor-faktor yang memengaruhi peningkatan kualitas diet pada penderita PJK yaitu pendapatan keluarga cukup, dan aktivitas fisik yang cukup. Sedangkan faktor yang memengaruhi penurunan kualitas diet yaitu kadar kolesterol HDL yang tinggi. Hasil ini menegaskan bahwa untuk pencegahan PJK harus diikuti dengan meningkatkan aktivitas fisik, disertai dengan dukungan pendapatan yang cukup.

Abstract

Healthy growth and development of children is the right of every child, both physically and mentally. Stunting is one of the features of delayed growth and development as a result of chronic malnutrition. The purpose of this study was to see the association maternal feeding patterns and quality of food consumption on stunting in children aged 18-24 months in Bogor City. This study uses a nested case-control study design by analyzing data from a cohort study of child development in five villages in Bogor. A total of 90 subjects aged 18-24 months were selected in this study consisted of 45 stunting subjects and 45 non-stunting subjects. The variables analyzed in this study maternal feeding patterns and quality of food consumption as measured by the energy density and other nutrients. Chi-square test was used to analyze the data. The results showed that there was a significant relationship between maternal feeding patterns (p=0.002) and stunting in children aged 18-24 months. There was a significant relationship between iron density measurement 2 (p=0.017) and calcium density measurement 2 (p=0.004) and stunting in children aged 18-24 months. Energy density and protein density did not show a significant relationship with stunting. Poor food parenting, as well as low intake of iron and calcium can increase the occurrence of stunting in children aged 18-24 months.

Abstract

Abstrak Secara global, kegagalan pertumbuhan linier merupakan bentuk paling umum dari kekurangan gizi  pada masa anak-anak, dimana sekitar 165 juta anak balita mengalami stunting. Akan tetapi, masih terdapat perdebatan mengenai kaitan antara stunting pada anak-anak dengan kejadian kegemukan maupun obesitas pada usia-usia selanjutnya. Penelitian ini bertujuan untuk menganalisis trajektori IMT/U dari anak-anak hingga remaja pada anak yang memiliki riwayat stunting dan normal. Penelitian ini merupakan penelitian kohort retrospektif, dengan menggunakan data sekunder Indonesia Family Life Survey (IFLS) yang merupakan kolaborasi dari Research and Development (RAND) dan Survey Meter. Subjek pada penelitian ini adalah 314 (43.6%) anak yang memenuhi kriteria inklusi dan eksklusi. Pendekatan yang digunakan untuk mempelajari trajektori IMT/U adalah Individual Growth Curve (IGC) model. Analisis IGC dilakukan dengan prosedur Linear Mixed Model pada IBM SPSS Statistics versi 25. Hasil penelitian ini menunjukkan bahwa terdapat selisih nilai z-score IMT/U sebesar 0.505 (β= -0.505; SE= 0.138; p<0.01) antara anak yang mengalami stunting dan normal pada awal pengukuran. Nilai z-score IMT/U anak stunted akan mengalami penurunan 0.078 kali lebih kecil (β= -0.078; SE= 0.011; p<0.01), akan tetapi kemudian mengalami peningkatan 0.011 kali lebih tinggi (β= 0.01; SE= 0.002; p<0.01) dibandingkan dengan anak normal. Penelitian ini menunjukkan bahwa terdapat perbedaan trajektori IMT/U anak yang memiliki riwayat stunting dan normal.Kata kunci: IMT/U; stunting; trajektori Abstract  Globally, linear growth failure is the most common form of childhood malnutrition, with around 165 million children under five are stunting. However, the link between stunting in children with the risk of overweight or obesity at later ages are still debated. This study aimed to analyze the trajectory of BAZ from children to adolescents in children who were stunted and normal. This study was a retrospective cohort study, using secondary data from the Indonesia Family Life Survey (IFLS) which is a collaboration of RAND and Survey Meter. The subjects in this study were 314 (43.6%) children who met the inclusion and exclusion criteria. BAZ trajectory approach used was the Individual Growth Curve (IGC) model. IGC analysis was carried out by Linear Mixed Model procedure using IBM SPSS Statistics version 25.  This study indicated that there was a 0.505 (β= -0.505; SE= 0.138; p<0.01) difference in the z-score of BAZ between children who were stunted and normal at the initial measurement. The decreasing of stunted child’s BAZ was 0.078 times smaller (β= -0.078; SE= 0.011; p<0.01), but then the BAZ would increase 0.011 times higher (β= 0.01; SE= 0.002; p<0.01) than the normal children. Abstract

Anemia pada anak sekolah masih menjadi masalah yang harus segera ditangani di Indonesia. Menurut laporan Riset Kesehatan Dasar (Riskesdas) 2013, prevalensi anemia untuk anak usia sekolah di Indonesia adalah 26.4%. Penelitian ini bertujuan untuk menentukan faktor-faktor risiko (karakteristik subjek, karakteristik keluarga, densitas protein dan asupan zat besi, kecukupan protein dan zat besi, frekuensi konsumsi pangan sumber heme, pola konsumsi enhancer dan inhibitor zat besi serta frekuensi sarapan) terhadap kejadian anemia. Desain penelitian yang digunakan adalah cross sectional yang dilakukan pada bulan Januari - Juni 2018. Penelitian ini dilakukan pada anak sekolah dasar kelas 4 - 6 di Temanggung. Sampel sebanyak 318 siswa dipilih menggunakan metode simple random sampling. Analisis data dilakukan secara univariat, bivariat dengan uji chi square dan multivariat dengan regresi logistik. Hasil analisis multivariat menunjukkan bahwa frekuensi sarapan, frekuensi konsumsi telur, frekuensi konsumsi hati ayam, dan konsumsi obat cacing merupakan faktor risiko terjadinya anemia pada anak sekolah dasar di Temanggung.

Abstract

ABSTRAKPenilaian konsumsi pangan dengan metode Healthy Eating Index (HEI) belum banyak dilakukan di Indonesia. Tujuan penelitian adalah untuk menganalisis hubungan HEI versi Indonesia dengan biomarker Sindrom Metabolik (SM) pada penderita Penyakit Jantung Koroner (PJK). Penelitian ini menggunakan data Studi Kohor Faktor Risiko Penyakit Tidak Menular periode tahun 2013-2016. Ditemukan sebanyak 124 kasus baru PJK berdasarkan pengukuran EKG. Pengukuran konsumsi pangan menggunakan konsumsi pangan 1x24 jam setahun sebelum subjek didiagnosa PJK. Penilaian kualitas diet dengan metode US-HEI yang dimodifikasi berdasarkan jumlah porsi pada pedoman gizi seimbang Indonesia. Data biomarker SM yang digunakan meliputi tekanan darah, glukosa darah puasa (GDP), glukosa darah pasca pembebanan (GD2PP), High Density Lipoprotein (HDL), Low Density Lipoprotein (LDL) dan trigliserida yang diukur pada saat pertama kali didiagnosis PJK. Rata-rata skor HEI yaitu 58.6, dan sebanyak 1.6% subjek termasuk kategori baik. Komponen HEI yaitu minuman berpemanis berhubungan signifikan dengan GDP (r=0.271), GD2PP (r=0.191), HDL (r=-0.200) (p<0.05); sedangkan skor Polyunsaturated Fatty Acid (PUFA) berhubungan signifikan dengan HDL (r=0.219) (p<0.05). Hasil studi mengonfirmasi kembali bahwa dalam program pencegahan PJK sebaiknya diikuti dengan mengurangi asupan gula dan meningkatkan konsumsi pangan sumber PUFA.Kata kunci:  healthy eating index, konsumsi pangan, penyakit jantung koroner, sindrom metabolikABSTRACTThe food consumption assessment using the Healthy Eating Index (HEI) method has not been widely applied in Indonesia. This study aimed to analyze the relationship between the Indonesian version of HEI and Metabolic Syndrome (MetS) biomarkers on coronary heart disease (CHD) patients. This study used data from the Cohort Study of Non-Communicable Disease Risk Factors for the 2013-2016 period. There were 124 new cases of CHD based on ECG measurements. The measurement of food consumption used a 24-hour dietary recall a year prior to the CHD diagnosis. The diet quality assessment was done using the modified US-HEI method based on the number of servings in the Indonesian balanced nutrition guidelines. Data on MetS biomarkers were blood pressure, Fasting Blood Glucose (FBG), Postprandial Blood Glucose (PBG), High-Density Lipoprotein (HDL), Low-Density Lipoprotein (LDL), and triglycerides at the time of the CHD diagnosis. The mean HEI score was 58.6, and 1.6% of patients were in good categories. The HEI component, sugar-sweetened beverage, was significantly associated with FBG (r=0.271), PBG (r=0.191), HDL (r=-0.200) (p<0.05); meanwhile, Polyunsaturated Fatty Acid (PUFA) score was significantly associated with HDL (r=0.219) (p<0.05). These results confirm that CHD program prevention is better followed by reducing sugar intake and increasing PUFA food sources.Keywords: coronary heart disease, food consumption, healthy eating index, metabolic syndrome

Abstract

The prevalence of obesity in the adult is increasing each year due to increased consumption of fast food and decreased consumption of fiber.  Based on Basic Health Research there was 20% of Indonesians suffered from obesity in 2018. The objective is to analyze changes in dietary fiber intake in adult obese in Bogor City. Methods: This research is conducted using secondary data from Risk-Factor of Non-Communicable Disease Bogor. The research design was prospective cohort.  Total subjects were 138 subjects who were newly suffer from obesity in 2014 and 2016, and had normal BMI in baseline (2011 and 2012). Consumption data were collected using 24-hr recall and FFQ and were processed and analyzed using Nutri Survey, Microsoft excel and SPSS 23.0 version. Univariate analysis was used to see the characteristic of the subjects, while different test was using Wilcoxon test since the data are not distributed normally. Results,most subjects were women (77,5%) aged 35-44 years old. The mean of dietary fiber intake was at about 9,7 g in the beginning and it increased by 1,2 g. Despite the increment in the percentage of the people who consumed dietary fiber at about 58%, the mean was still under the recommendation (<25g) at 96,4%. Conclusions, there was an increasing number of dietary fiber in the obese adult who were newly suffered from obesity in two years follow-ups, but the intake is still under the recommendation.
