Abstract

Chalazion is a granulomatous inflammation with lipogranuloma or chronic non-infectious inflammation of the eyelid, in the form of a nodule formed due to inflammation and obstruction of the tarsal meibomian gland or zeis. This chalazion often occurs in all age groups, but the chalazion occur more commonly in the productive age. We report the case of a 20-year-old man with complaints of a lump on the upper eyelid, at the left and right eye for 4 months before entering the hospital. The lump is initially small but gets bigger over time, becomes red, feels painful and lumpy. This complaint has been repeated since 10 months ago. Currently the lump no longer feels painful, itchy or hot, but the lump is now permanent. On ophthalmological examination, it was found that the superior eyelid of the right eye had a nodule measuring ±5 mm and the superior eyelid of the left eye had a nodule measuring ±3mm. The diagnosis made was acute exacerbation of chalazion of right eye, then the patient was given non-medicamentosa therapy, medicamentosa, and operative treatment.

Abstract

Pada tahun 2020, insiden Out of Hospital Cardiac Arrest (OHCA) sering terjadi di rumah sebesar 73,9%, tempat umum 15,1% dan panti jompo 10,9%. Orang dewasa mengalami OCHA disaksikan orang awam sebesar 37,1% kasus, tim penyelamat 12,8% kasus dan sama sekali tidak disaksikan 50,1% kasus. Oleh karena itu masyarakat umum perlu pengetahuan Bantuan Hidup Dasar (BHD), yang merupakan tindakan darurat untuk membebaskan jalan nafas, dan mempertahankan sirkulasi darah tanpa alat bantu. Kegiatan pengabdian ini memberikan penyuluhan dan pelatihan BHD di Desa Cipadang Kecamatan Gedong Tataan, Kabupaten Pesawaran, Lampung. Evaluasi kegiatan menunjukkan bahwa tingkat pengetahuan BHD yang sebelumnya < 60% meningkat menjadi 61-80%. Peningkatan pengetahuan masyarakat dapat membantu meningkatkan keselamatan dan mengurangi jumlah korban dengan memberikan pertolongan yang tepat.

Abstract

Digestion organs and brain communicate through specific pathway that involves nervous, endocrine, and immune system onto it. Guts microbiota especially probiotics holds a crucial matter in communicating with brain, that’s why it can affect one’s mood and cognitive functions by producing neurotransmitters that are related to brain through endocrine and autonomy nervous system. The consumption of probiotics is known to be able to improve the quality of guts microbial environment, where this matter has a profound impact in increasing mood so that a person can regulate their emotion better. A good emotion regulation has a beneficial impact in order to prevent and solve stress, anxiety, and depression. Psychosocial changes can be a stressor for individual so that the ability to regulate mood and emotion is needed to help a person adapt well. The aims of this study is to know how beneficial probiotics can be in individual mood changes. The relation between probiotics consuming and mood changes have been such an interest topics to be researched in order to gain knowledge and truth how it may give an enormous impact on people’s health by consuming it regularly. It is also proved by many articles and prior literatures that show the benefits of it in physical and mental aspects yet individual quality of life, vice versa it has negative impacts if it happens in a long-term period. Daily stressors are inevitable so that by knowing what could help to at least manage it. Managing how to face stressor isn’t easy for several people, thus it also can be fatal if it isn’t treated well, such as depression or even suicide.

Abstract

The bacterium Streptococcus pyogenes is a gram-positive bacterium that causes various diseases in humans. Pharyngitis is the most common infection caused by Streptococcus pyogenes, occurring in 15-30% of children and 5-10% of adults. The extract of mangrove bark contains bioactive compounds that can serve as an antibacterial alternative.  This research is an experimental study. The mangrove bark was extracted using 96% ethanol solvent through the maceration method and subjected to phytochemical testing. The treatment consists of 96% ethanol extract preparations with concentrations of 25%; 50%; 70%; 90%; and 100%, as well as a negative control using aquades and a positive control using amoxicillin. Each treatment was repeated 4 times. Antibacterial activity testing was conducted using the well method on Muller Hinton Agar. The results of the phytochemical test showed that the 96% ethanol extract positively contained flavonoid, phenol, alkaloid, tannin, saponin, steroid, and triterpenoid compounds. The results of the antibacterial activity study from 25%, 50%, 70%, 90%, and 100% concentrations of 96% ethanol extract produced zone diameters of 4.33 mm; 5.68 mm; 6.61 mm; 7.72 mm; and 9.02 mm. These results indicate that the 96% ethanol extract of the mangrove bark has an inhibitory effect on the growth of Streptococcus pyogenes. Data analysis of the 96% ethanol extract of the mangrove bark using the One-Way ANOVA test showed p< 0.05.

Abstract

Obesity continues to be a major health problem worldwide and has become an epidemic that has worsened over the past 50 years. Obesity has a prevalence that continues to increase every year due to unhealthy lifestyles and energy intake that is greater than energy expenditure. Obesity is closely related to the habit of consuming high-fat foods, which can increase the risk of increased blood pressure. Several studies have found that a 15% increase in body weight can cause an 18% increase in blood pressure. The results of the 2018 Indonesian Basic Health Research showed that the prevalence of obesity in the Indonesian population aged 18 years and over reached 21.8%. The purpose of this study was to determine the difference in obesity status on blood pressure at the Panjang Health Center, Bandar Lampung City. This study used an observational analytical method with a cross-sectional approach. The population in this study were obese patients registered at the Panjang Health Center, Bandar Lampung City in the period August 2023 to August 2024. The total number of research subjects who met the inclusion and exclusion criteria was 113 research subjects obtained from medical records of obese patients at the Panjang Health Center, Bandar Lampung City in the period August 2023-August 2024. Data are presented in a distribution table and analyzed using the Mann-Whitney test. The results showed a P value of 0.001 which indicated that there was a difference in the effect of obesity status on blood pressure status at the Panjang Health Center, Bandar Lampung City.

Abstract

Sindrom Pickwickian Sindrom juga dikenal dengan sindrom Hipoventilasi pada Obesitas (SHO) adalah kondisi ditemukannya akumulasi dari minimal dua dari gejala yaitu: obesitas, gangguan tidur terkait pernapasan, dan hipoventilasi alveolar kronik. Penegakan diagnosis baru dapat dilakukan setelah menyingkirkan penyebab hipoventilasi lain. Kami melaporkan sebuah kasus yang kami temukan di Rumah Sakit Umum Daerah Abdoel Moeloek Provinsi Lampung, pada tanggal 20 Agustus 2023, dimana wanita 35 tahun datang dengan keluhan utama sesak napas yang semakin bertambah tambah sejak dua hari sebelum masuk rumah sakit. Sesak sering kambuh dalam 6 bulan terakhir dan dirasakan pada malam dan menjelang pagi hari, disertai mengi dan dipengaruhi oleh posisi tidur. Ada riwayat asma pada riwayat penyakit terdahulu pasien sejak kecil. Pasien sering mendengkur sejak 1 tahun dan sering merasa kurang tidur pada siang hari karena tidur malamnya terganggu. Pada pemeriksaan fisik didapatkan pasien obesitas tingkat 2 dengan lingkar leher 37 cm dan skor Apnea Hypopnea Index (AHI) 49. Pemeriksaan penunjang menunjukkan bahwa pasien mengalami peningkatan kadar karbondioksida dalam darah. Karena memiliki tiga tanda yaitu obesitas, hipoventilasi dan OSA maka pasien didiagnosis dengan Sindrom Hipoventilasi pada Obesitas (SHO)

Abstract

Patient satisfaction is a key indicator in assessing healthcare quality, especially in hospital pharmacy services which play a vital role in treatment effectiveness. Hypertension is a major chronic illness requiring strict medication adherence to prevent complications such as heart disease, stroke, and kidney failure. This review examines the relationship between outpatient hypertension patients' satisfaction with pharmacy services and their medication adherence. Findings highlight that satisfaction dimensions such as reliability, empathy, responsiveness, tangibles, and assurance significantly shape patient perceptions. Higher satisfaction correlates with increased adherence. Continuous education, empathetic interaction, efficient service systems, and clear pharmacist communication are essential. Quality pharmacy services not only improve patient outcomes but also support long-term hypertension control.

Abstract

Kemajuan teknologi kecerdasan buatan (Artificial Intelligence/AI) telah memberikan dampak besar dalam sektor kesehatan, khususnya dalam membantu proses diagnosis penyakit. AI mampu mengolah data medis dalam jumlah besar, seperti gambar radiologi, rekam medis elektronik, hingga data genomik, dengan tingkat efisiensi dan akurasi yang tinggi. Sejumlah penelitian membuktikan bahwa penggunaan algoritma machine learning dan deep learning dalam diagnosis mampu mengidentifikasi penyakit kronis seperti COVID-19, kulit, dan periodontal dengan akurasi yang sangat tinggi. Beberapa kondisi dapat melampaui kinerja tenaga medis manusia dari segi kecepatan dan akurasi. Selain mempercepat proses penegakan diagnosis, AI dapat meminimalkan risiko kesalahan manusia serta meningkatkan mutu pelayanan kesehatan. Di bidang radiologi, teknologi seperti convolutional neural networks (CNN) telah digunakan secara efektif untuk mendeteksi kelainan jaringan melalui CT scan dan MRI dengan hasil yang lebih presisi. AI juga berperan penting dalam sistem pendukung keputusan klinis (Clinical Decision Support System/CDSS), yang mendorong implementasi pengobatan berbasis data dan pendekatan yang dipersonalisasi. Namun, penerapan AI di negara berkembang seperti Indonesia masih dihadapkan pada berbagai tantangan, antara lain terbatasnya data lokal, ketimpangan infrastruktur digital, serta permasalahan etika dan regulasi. Kajian literatur ini bertujuan untuk meninjau manfaat AI dalam bidang diagnosis medis, serta mengidentifikasi hambatan yang perlu diatasi agar teknologi ini dapat diimplementasikan secara optimal dan berkelanjutan dalam sistem layanan kesehatan yang beragam. Abstract

Irritant contact dermatitis (ICD) is a non-immunologic inflammatory skin disorder resulting from direct damage to the epidermal barrier by chemical, physical, or biological agents. ICD is more common than allergic contact dermatitis since most individuals are at risk of repeated exposure to irritants in daily activities, such as detergents. We report the case of a 54-year-old woman presenting with peeling, erythema, pruritus, and stinging pain on both hands after washing clothes with powdered detergent without protective gloves. Dermatologic examination revealed erythematous plaques with thick scales, diffuse borders, and fissures on the dorsal and palmar regions bilaterally. The diagnosis of ICD was established based on a clear exposure history, characteristic clinical features, and the absence of allergic manifestations. Management included ceramide-based moisturizer to restore skin barrier function, high-potency topical corticosteroid (clobetasol propionate 0.05%) to reduce inflammation, and oral antihistamine (cetirizine) to relieve pruritus. Patient education emphasized adherence to therapy, skin hygiene, and strict avoidance of repeated detergent exposure. Marked clinical improvement was achieved within a short period following combination therapy. This case highlights detergent exposure as an important cause of ICD among housewives and underscores the significance of preventive strategies, including the use of personal protective equipment, safer cleaning alternatives, and continuous patient education. Early recognition and comprehensive management are essential to reduce recurrence and improve patient quality of life.

Abstract

Major depressive episodes with psychotic symptoms are a form of major depressive disorder characterized by severe affective symptoms accompanied by delusions or hallucinations. This condition is relatively rare, but is associated with a poorer prognosis, a high risk of recurrence, and an increased suicide rate compared to depression without psychotic symptoms. This case report aims to describe the clinical manifestations, diagnostic process, and management strategies in patients with major depressive episodes accompanied by psychotic symptoms. The patient presented with feelings of depression, sadness, and lack of enthusiasm in performing daily activities for 1 year, which had worsened for 4 months prior to admission to the hospital, since the death of the patient's mother. The diagnosis was established based on a psychiatric interview, mental status examination, and diagnostic criteria from the Diagnostic and Statistical Manual of Mental Disorders (DSM-5). Management consisted of a combination of antidepressants and antipsychotics, as well as intensive psychosocial support. This report emphasizes the importance of early detection, accurate diagnosis, and comprehensive management to reduce the risk of complications, improve the patient's quality of life, and reduce the recurrence rate. Thus, this case study is expected to serve as an additional reference in the clinical management of major depressive disorder with psychotic symptoms.
