Abstract

Introduction the risk of errors in the perioperative period is very large so that patient safety should be supported and the team is obligated to cooperate in raising awareness toward the occurrence of the incident or event that is not expected. Head nurse control function ensure appropriate planning objectives accomplished. The purpose aims to know the effectiveness of the control function of the head nurse toward the implementation of perioperative patient safety. This research method research design using preexperiment with pretest-posttest design without control. The sample size of 75 nurses that involved in perioperative nursing service. The results data analysis using paired t test represent the effectiveness of the control function of the head nurse with p value = 0.0001 (CI = 120.79 – 127.01) and the improve of implementation of perioperative patient safety by nurses. The conclusion post intervention that indicates positive correlation with p value = 0.0001 (CI = 141.59 – 147.15). Suggestions this research recommends there should be monitoring and evaluation of implementation of the control head room and a development model that is more complete control

Abstract

Background: Alarm fatigue in nurses working in intensive care units is still a critical issue and priority in patient safety. Alarm fatigue is a condition when someone becomes insensitive to safety warnings due to receiving too many alarms. Alarm fatigue can be prevented by implementing effective alarm management. On the other hand, the current implementation of alarm management is still not optimal. Head nurses play an important role in efforts to prevent alarm fatigue incidents and oversee the implementation of alarm management and need direction in its implementation in intensive care units. Purpose: To initiate alarm fatigue management for nurses through alarm management guidelines in intensive care units. Method: A case report study according to Kotter's planned change stages was conducted at the University of Indonesia Teaching Hospital on April 22 - May 24, 2024. The research process included assessment, data analysis, action plan (PoA), implementation, and evaluation. Data collection was carried out through interviews, observations, and filling out questionnaires. Results: The priority problems obtained were the suboptimal planning function related to alarm fatigue management through alarm management and implementation through Kotter's theory which had only been carried out until the third stage. Conclusion: Planned changes can be made in the implementation of implementation in solving problems. The problem found was the suboptimal function of the head of the room in alarm management in the intensive care unit. The alarm management guide that has been created is expected to prevent the occurrence of fatigue alarms in nurses and provide quality services. Suggestion: The draft guide for managing nurse fatigue alarms through alarm management can be approved by the president director and applied in services in the hospital's intensive care unit. In addition, the hospital can organize training and education for nursing staff to improve understanding of fatigue alarms, effective alarm management, and conduct periodic evaluations.   Keywords: Alarm Management; Fatigue Alarm; Intensive Care Unit; Nurses.   Pendahuluan: Alarm fatigue pada perawat yang bekerja pada ruang intensif masih menjadi masalah yang kritis dan prioritas dalam keselamatan pasien. Alarm fatigue adalah kondisi ketika seseorang menjadi tidak peka terhadap peringatan keselamatan akibat menerima terlalu banyak alarm. Alarm fatigue dapat dicegah dengan menerapkan manajemen alarm yang efektif. Di sisi lain, penerapan manajemen alarm saat ini masih belum optimal. Head nurse memainkan peran penting dalam upaya mencegah kejadian alarm fatigue dan mengawasi pelaksanaan manajemen alarm dan membutuhkan panduan dalam pelaksanaannya di ruang intensif. Tujuan: Untuk menginisiasi pengelolaan alarm fatigue perawat melalui panduan manajemen alarm di ruang intensif. Metode: Penelitian case report sesuai tahapan perubahan terencana Kotter’s, dilakukan di Rumah Sakit Pendidikan, Universitas Indonesia pada tanggal 22 April – 24 Mei 2024. Proses penelitian meliputi pengkajian, analisis data, rencana tindakan/plan of action (PoA), implementasi, dan evaluasi. Data dikumpulkan melalui wawancara, observasi, dan pengisian kuesioner. Hasil: Prioritas masalah yang didapat yaitu fungsi perencanaan belum optimal terkait dengan pengelolaan alarm fatigue melalui manajemen alarm dan implementasi melalui teori Kotters, dilakukan baru sampai pada tahap yang ketiga. Simpulan: Perubahan terencana dapat dilakukan dalam pelaksanaan implementasi dalam menyelesaikan masalah. Permasalahan yang ditemukan yakni belum optimalnya fungsi perencanaan kepala ruangan dalam manajemen alarm di ruang intensif. Panduan manajemen alarm yang telah dibuat diharapkan dapat mencegah terjadinya alarm fatigue pada perawat dan memberikan pelayanan yang berkualitas. Saran: Draft pengelolaan alarm fatigue perawat melalui manajemen alarm dapat disahkan oleh direktur utama dan diterapkan dalam pelayanan di ruang intensif rumah sakit. Selain itu, pihak rumah sakit dapat menyelenggarakan pelatihan dan edukasi bagi staf perawat untuk meningkatkan pemahaman tentang alarm fatigue, manajemen alarm yang efektif, dan melakukan evaluasi secara berkala.   Kata Kunci: Alarm Fatigue; Manajemen Alarm; Perawat; Ruang Intensif.

Abstract

Konsep asuhan keperawatan klien dengan fraktur sudah banyak yang telah membahasnya. Namun tidak ada salahnya diulas kembali sebagai sumber bacaan bagi perawat lapangan, agar praktik keperawatan yang dilaksanakan dapat berdasarkan ilmu keperawatan praktis. Trauma muskuskeletal, khususnya fraktur memerlukan pemberian asuhan keperawatan yang komprehensif. Asuhan terutama ditujukan untuk memenuhi kebutuhan dasar klien yang terganggu dan mencegah mengurangi komplikasi terutama terhadap immobilisasi. Pendidikan kesehatan juga dapat diberikan untuk mencegah cedera lebih lanjut sehingga klien secara bertahap dapat mengoptimalkan fungsi bio-psiko-sosio-spiritualnya. The concept of nursing care for client with fracture has been discussed by many authors. But it is always interesting to discuss it as reference for nurses working in the hospital.Musculoskeletal trauma, especially fracture requires a comprehensive nursing care. The purpose of nursing care is to meet the disturbed basic human need of the client and to prevent client from further complication caused by immobilization.The purpose of health education is to prevent the impact of injury and to support the client to obtain the optimal level of bio-psycho-sosio-spiritual functioning.

Abstract

Hospital as a health care facility can be a dangerous place and a high risk to safety. Nursing personel who work continuously provide nursing care in hospital continuously for 24 hours every day at risk of illness and accidents. This study aims to identify the characteristics of the relationship of nurses with nurses work safety behavior. This study was a descriptive cross sectional approach. The results showed no association between nurse characteristics include age, gender, education, employment, and training with the nurses work safety behavior. The hospital is expected to provide wider opportunities for nurses to continue their education to a higher level, and organizes regular socialization and training related to safety of nurses in improving the safety behavior of nurses.

Abstract

Cancer is a complex disease associated with a wide range of symptoms, declining quality of life, and financial toxicity (FT).To examine the quality of life and symptoms management in Cancer patients experiencing financial toxicity based on a review of published literature.A systematic review was conducted using five databases: Sage Journals, Science Direct, ProQuest, Clinical Key, and Oxford Academic from 2015-2025 years with keywords Quality of Life, Symptom Management and Cancer Patients. The quality of studies was assessed depending on the study design of the 1,050 articles found, only 11 were reviewed. Findings from 11 studies indicated that the quality of life in cancer patients is influenced by multiple factors. Financial toxicity showed a negative correlation with quality of life (r =0.23, p = 0.0057), indicating that higher levels of financial toxicity are associated with lower quality of life. Psychological symptoms such US anxiety and depression were positively associated with FT (r = 0.27, p = 0.0151; r = 0.18, p = 0.0336). High symptom burden, both physical and psychological, significantly reduced quality of life, while social support played a positive role in enhancing well-being (r = -0.17, p = 0.0412). Integrative therapies improved quality of life by 82%, and cost related health literacy was found to reduce financial toxicity (OR = 0.823, p= 0.039). Holistic interventions and attention to the financial challenges faced by cancer patients are essential for improving quality of life, particularly for those affected by financial toxicity. Effective symptom management involves the use of various tools (ESAS, HADS, FACT-G, SESI, EORTC QLQ-C30) and approaches such as palliative care, integrative therapies, symptom cluster management, acupuncture, financial support, social support, enhanced cost-related health literacy, improved psychological resilience, and identification of financial toxicity risks.The quality of life in Cancer patients is influenced by a complex interaction of factors, including financial condition, symptom management, social support, and multidisciplinary care. Enhancing health literacy—especially regarding financial aspects—with comprehensive, evidence-based, and accessible information such as insurance, medication coverage, and financial resources, is critically important.

Abstract

The success of health organizations depends on the quality of human resources including nursing resources. Health organizations can grow and develop if they have staff or nurses with demonstrated high performance. Good performance is directly proportional to the quality of services provided. Performance appraisal is very important to evaluate, assess and measure, to enhance employee capabilities and professionalism. This assessment aims to ensure that organizational goals can be achieved optimally. Furthermore, strategic planning and the role of managers are essential in performance appraisal. With good strategic planning, supported by the optimal role of managers, can enhance staff performance and facilitate the achievement of organizational objectives. Managers who have good abilities in planning, organizing, staffing, actuating and controlling the vision, mission and goals of the organization can be achieved. Objective: This study aims to determine the relationship strategic planning and the role of managers on the performance of nursing resources. Methods : This systematic review was prepared based on Preferred Reporting Items for Systematic Reviews and Meta-Analyzes (PRISMA). Used databases Scopus, ProQuest, Science Direct, Pubmed, Clinicalkey and Google Scholar. Articles used from 2023-2025. Keywords Strategic Planning AND Manager Roles AND Performance Appraisal for Nurses. Joanna Briggs Institute (JBI) and Critical Appraisal Skills Program (CASP) analysis. From 78,200 articles found, 8 articles were analyzed. The results of systematic analysis in the study showed that with appropriate strategic planning and good manager roles can improve nurse performance which has an impact on improving the quality of service and the goals and vision of the organization's mission can be achieved. The results of the study can be concluded that strategic planning and the role of managers have a significant effect on improving performance so that organizational goals can be achieved optimally.

Abstract

Nurses in hospitals often face complex ethical situations; however, there are still gaps in the understanding and application of nursing ethical principles, and a structured, easily accessible ethical guideline is not yet available. This study aims to develop two main solutions Nursing Ethic Navigation (an ethical guideline web search engine) and the Nurses' Ethical Guideline E-Book using John Kotter’s Eight-Step Change Model approach. The method used is a case study conducted at Hospital X during September–October 2024, encompassing problem identification, action planning, implementation, and evaluation. The respondents in this study were the Head Nurse, Primary Nurse, Nursing Department staff, members of the Nursing Committee, and nurses in the IGD-COT unit at Hospital X. Data were collected through interviews, observations, and questionnaires, then analyzed using problem priority scoring and a fishbone diagram to identify root causes as the basis for developing action plans and change strategies. The results showed a significant improvement in nurses’ knowledge, awareness, and ability to apply ethical principles after the intervention. Digital media such as the e-book and web-based tools proved effective as learning platforms and practical references that can be accessed anytime. This study recommends expanding the implementation of ethical guidelines to other service units, supported by managerial involvement and continuous evaluation, to ensure integration into the organizational culture and daily nursing practice.

Abstract

Strategic planning and management of human resources (HR) play a crucial role in improving the quality of hospital services. This field study aims to analyze human resource development strategies, particularly in nursing, at Hospital. Field study method. Data were collected through interviews at Hospital X and documentation from Hospital X. Data were obtained through group-based analysis of academic documents and assessment of the hospital's existing conditions. The analysis revealed that Hospital X's main strengths lie in HR competencies, recruitment systems, and ongoing training programs. However, it faces challenges in work culture and career development systems. The proposed strategies include digitalizing HR management systems, collaborating with educational institutions to strengthen nursing capacity, and fostering a culture of patient safety. As part of the implementation, a five-year work plan was developed, encompassing CPD training programs, the development of a Human Resources Information System (HRIS), and the enhancement of reward and recognition programs. This strategic approach is expected to enhance the efficiency, productivity, and competitiveness of hospital human resources amid the transformation of healthcare services.

Abstract

Data tentang leadership kepala ruangan yang menunjukkan bahwa adanya persepsi yang kurang terhadap kemampuan leadership dan quality control, hal tersebut  merupakan bagian dari standar kompetensi yang harus dimiliki oleh kepala ruangan. Pelaksanaan pendidikan bagi kepala ruangan belum optimal dan belum berkesinambungan. Penelitian dilakukan dengan rancangan penelitian survei. Data dikumpulkan melalui wawancara, observasi, Focus Group Discussion (FGD) dan Survei dalam bentuk kuesioner. Data penelitian dianalisis untuk mendiagnosis masalah dan merumuskan metode pemecahannya. Rumusan masalah yang didapat adalah belum optimalnya program pendidikan berkelanjutan bagi kepala ruangan. Penyelesaian masalah berupa Kegiatan dengan metode pendekatan action research yaitu dengan menerapkan metode pelatihan berbasis kurikulum yang disusun sesuai dengan standar kompetensi kepala ruangan dengan jumlah peserta pelatihan sebanyak 112 peserta. Hasil dari action research ini menunjukkan peningkatan pengetahuan dari kepala ruangan. Evaluasi melalui pre dan post test tentang materi pelatihan yang sudah diberikan. Nilai rata-rata pre dan post test peserta meningkat dari rata-rata 53.88 menjadi 80.36,  meningkat 26.48 poin.

Abstract

Implementation of quality improvement in hospitals is still unsatisfactory. One of the causes of this problemis the inadequate implementation of nursing audits by the nursing committee. This study aims to identify theimplementation of the nursing profession audit at Hospital A, West Jakarta. The method used is a pilot studyin the nursing room with the involvement of manager nurses and clinical nurses as many as 140 nurses. Theanalysis from the assessment stage, interviews, observation, distributing questionnaires and focus groupdiscussions, and fishbone analysis found that the nursing profession audit was the task of the nursingsubcommittee that was being improved. The implementation carried out is the latest creation, the auditorteam, audit tools and the implementation of nursing audits. Result: The audit was carried out by determiningthe topic of handover between shifts. The not yet optimal implementation of handover between shifts is theresult of the nursing audit. The conclusion has been that the preparation of a new audit of nursing qualityhas been carried out in RS A, the formation of an auditor team and the compilation of tools in implementingnursing audits. The nursing profession quality audit has been carried out on the topic of handover betweenshifts and the result is that the handover between shifts is optimized. Recommendations for the latest nursingaudits validation, conducting nursing audits on topics according to the needs of the nursing committee andperiodic re-audits.
