Abstract

The Emergency Department is a high-intensity clinical setting that manages critically ill patients who are highly susceptible to sudden clinical deterioration. Early detection through the implementation of the Early Warning System plays a crucial role in preventing unexpected adverse events, with nurses serving as the primary implementers of this system. This review aims to evaluate current evidence on the implementation of the Early Warning System by nurses in the Emergency Department and its impact on the prevention of unexpected adverse events. This study employed a systematic review approach by applying the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines during the article selection process and using the 2023 edition of the Joanna Briggs Institute (JBI) Critical Appraisal Tool to assess the quality of included studies. Search for articles through the ScienceDirect, ProQuest, PubMed, BMJ Journal, and EBSCO databases from 2020 to 2025. and 10 articles were analyzed. The majority of studies reported that the use of the Early Warning System consistently enhances the early detection of patient deterioration, accelerates clinical response, improves triage accuracy, and assists in predicting the need for advanced care such as admission to intensive care units. Implementation by nurses was also associated with improved compliance in vital signs monitoring and a reduction in patient mortality.

Abstract

Emergency Department (ER) services are highly complex and carry significant risks to patient safety. Effective communication during the handover process is critical to improving patient safety and care quality. Bedside handover is a strategic form of communication between healthcare workers, ensuring that vital patient information—such as vital signs and urgent care needs—is conveyed accurately and directly, with involvement from both patients and families. The ISBAR communication tool provides a clear structure that helps reduce the risk of miscommunication. Objective to develop a guideline that serves as a consistent reference for nurses in performing bedside handover, fostering a culture of safety and high-quality care at Hospital X. Method: This study used a case report approach with both qualitative and quantitative methods to examine the nursing service system at Hospital X. Data were collected through interviews, document reviews, observations, and questionnaires, and analyzed using the nursing management function approach: planning, organizing, directing, and controlling. Questionnaires were distributed to unit leaders, unit coordinators, and all staff nurses working in Room X, with a total of 48 respondents with total sampling. In the Unfreezing Phase, many ER nurses resisted the bedside handover method, perceiving it as time-consuming and burdensome. Observations revealed continued reliance on traditional handover methods. In the Changing Phase, a Plan of Action (POA) was developed, including drafting supervision guidelines, conducting socialization, and evaluating bedside handover implementation. In the Refreezing Phase, bedside handover was expected to become standard practice, though implementation was limited by time constraints. The implementation of bedside handover in the ER, guided by Lewin’s Change Theory and structured through ISBAR, is a strategic step to ensure care continuity and improve patient safety.

Abstract

ABSTRACT Psychological problems are prone to occur in nurses and can be prevented by increasing nurse resilience. Nurse resilience can help overcome psychological problems and affect the quality of nursing care provided to clients. This scientific work aims to implement evidence-based nursing management practice (EBP), namely the application of the Zinger model to the nursing management direction function in improving the resilience of nurses who serve in the inpatient room. Measurement of the level of resilience using the Adult Resilience Measure-Revised (ARM-R) questionnaire before and after implementation. The results of measuring the level of resilience of nurses found that there were differences in the level of resilience, especially in the low and medium categories. There was a 4.5% decrease in the level of resilience in the low category from 22.7% to 18.2%, while in the moderate category there was an increase of 4.6% from 54.5% to 59.1%. Future research is expected to continue the application of the Zinger Model to improve nurse resilience with more interventions and for a longer time, 1-5 months for positive results. Keywords: Inpatient, Nurse, Resilience, Zinger Model  ABSTRAK Masalah psikologis rentan terjadi pada perawat dan dapat dapat dicegah dengan meningkatkan resiliensi perawat. Resiliensi perawat dapat membantu mengatasi masalah psikologis dan mempengaruhi kualitas asuhan keperawatan yang diberikan kepada klien. Karya ilmiah ini bertujuan untuk mengimplementasikan praktik manajemen keperawatan berbasis bukti/ Evidence Based Practice (EBP) yaitu penerapan Zinger model pada fungsi pengarahan manajemen keperawatan dalam meningkatkan resiliensi perawat yang berdinas di ruang rawat inap. Pengukuran tingkat resiliensi menggunakan kuesioner Adult Resilience Measure-Revised (ARM-R) sebelum dan sesudah dilakukan implementasi. Hasil pengukuran tingkat resiliensi perawat didapatkan ada perbedaan pada tingkat resiliensi terutama kategori rendah dan sedang. Ada penurunan 4,5% tingkat resiliensi kategori rendah dari 22,7% menjadi 18,2%, sedangkan pada kategori sedang ada peningkatan 4,6% dari 54,5% menjadi 59,1%. Penelitian selanjutnya diharapkan dapat melanjutkan penerapan Zinger Model untuk meningkatkan resiliensi perawat dengan intervensi yang lebih banyak dan dalam waktu yang lebih lama, 1-5 bulan untuk hasil yang positif. Kata Kunci: Perawat, Rawat Inap, Resiliensi, Zinger Model.

Abstract

Background: Patients with chronic diseases generally experience repeated hospitalizations. Cultural diversity can affect communication and engagement from health care providers and recipients. An interactive culturally sensitive telenursing application is needed as a form of continuous nursing intervention when patients are at home. Purpose: To build and test the culturally sensitive telenursing application of Suster Kita. Method: Using the ADDIE (Analysis, Design, Development, Implementation, Evaluation) process design. The research was conducted from August 2022 to February 2024 and tested on 20 respondents in the DKI Jakarta area. The evaluation of the effectiveness of the Suster Kita culturally sensitive telenursing application at the implementation stage was carried out using a questionnaire on the use of the application developed by the research team. The final results of the application are evaluated 2 weeks after the respondent uses the application. Results: the use of the application showed that the average knowledge of chronic diseases and their treatment consisting of 10 knowledge components increased before and after the trial of the use of the culturally sensitive application of Suster Kita. Conclusion: the intervention is acceptable to users and is worth using. Application suggestions are integrated with laboratory results.

Abstract

Background: Burnout is characterized by emotional exhaustion, depersonalization, and decreased personal accomplishment, often occurring in workers with intense social interactions, such as healthcare workers. Nursing is a profession vulnerable to burnout, given the high job demands, long working hours, and significant responsibility in patient care. Burnout can cause physical, emotional, and mental exhaustion, impacting the quality of healthcare services and nurses' well-being. Purpose: To determine the relationship between mindfulness, job satisfaction, and personality factors with burnout among nurses in hospitals. Method: A descriptive-analytical method with a cross-sectional design was applied. The target population included all 1,989 nurses employed at Central Hospitals in Indonesia, from which a sample of 328 respondents was drawn. Data collection was carried out using a questionnaire. For data analysis, the chi-square test was employed in the bivariate analysis, while multiple logistic regression was used for the multivariate analysis. The variable most strongly associated with the outcome was job satisfaction (OR = 17.874), whereas mindfulness showed the weakest association (OR = 4.932). Results: The logistic regression analysis revealed a significant relationship between mindfulness, job satisfaction, and personality factors with work-related fatigue (burnout) among hospital nurses, indicated by a p-value of 0.001. Conclusion: There is a relationship between mindfulness, job satisfaction, and personality factors with work fatigue (burnout) among hospital nurses.

Abstract

Outpatient nursing services are an important element for hospitals to produce many patients, particularly inpatients. Outpatient nursing services are currently still not optimal. Nurses are responsible for providing good service to increase patient satisfaction. Aim: This case report aims to describe the implementation of planned changes to optimize the outpatient nursing service planning function. Method: The method used is a case report. Data collection uses interviews, questionnaires, and field observations. The results of data exploration are then analyzed using SWOT (strengths, weaknesses, opportunities, threats) and fishbone diagrams, then formulating a plan of action (PoA), followed by implementation and evaluation. Results: The results of the study showed that the priority problem in nursing services was that the implementation of the planning function was not optimal. Making outpatient nursing service guidelines is a strategy for solving problems in developing outpatient nursing services. Implementation of problem-solving uses a theory of change approach by the stages of Kotter's eight-step plan. Recommendation: Kotter's eight-step plan theory of change approach can be continued by hospital management as a form of initiation of service development.

Abstract

An Analytical Study Of Managers' Efforts In The Realisation Of Strategic Planning Of RSUD Kabupaten Empat Lawang Through The Balanced Scorecard Approach. Although strategic planning has been prepared in such a way as to choose the best approach, in practice, many managers have difficulty realising the plan. This study aims to analyse the steps and efforts in realising strategic planning at RSUD Empat Lawang Regency through the BSC approach. The method used is a case study through SWOT analysis (Strength, Weakness, Opportunity, and Threat). Data were obtained through a document review of the hospital's profile and achievements. The data were then analysed for external and internal situations using the SWOT method, then inputted into the Internal Factor Evaluation (IFE) matrix and External Factor Evaluation (EFE) matrix to be given weight and scale, and determined the position of the organisation through the Internal-External (IE) matrix. Then strategies were formulated through Threat, Opportunity, Weakness, and Strength (TOWS) followed by the determination of Balanced Scorecard (BSC) perspectives and Key Performance Indicators (KPIs). The results of the analysis show that the RSUD is in cell IV, which means that the RSUD is best controlled with a growth and build strategy so that 10 targets are obtained along with the manager's efforts in realisation which include measuring Value For Money, collaborating with other parties, developing innovations and preparing education and training plans. The managers' efforts in this study can be used as a reference in evaluating the realisation of strategic plans in health service organisations.

Abstract

Pelaporan Insiden Keselamatan Pasien (IKP) dapat dijadikan suatu proses pembelajaran dan dapat mencegah terulangnya kesalahan yang pada akhirnya akan meningkatkan keselamatan pasien dan mutu asuhan dan pelayanan keperawatan di rumah sakit. Sebuah tinjauan sistematik dilakukan, yang bertujuan untuk mengidentifikasi faktor yang mempengaruhi pelaporan IKP oleh perawat di suatu rumah sakit. Metode pada penelitian ini menggunakan pencarian artikel melalui database seperti SAGE Journal, ScienceDirect, ProQuest dan hand searching dengan kata kunci: kepemimpinan; motivasi; pelaporan insiden keselamatan pasien; pendidikan; pengalaman kerja; persepsi; sikap. Dari hasil pencarian tersebut didapatkan 682 kemudian dilakukan screening, didapatkan 7 jurnal yang sesuai dengan kriteria inklusi PICOT, masalah dan tujuan penelitian. Hasil penelitian ini menunjukkan berdasarkan 7 jurnal secara umum faktor yang signifikan terhadap pelaporan IKP adalah kurang pengetahuan, takut dan khawatir dan persepsi tidak melapor terhadap pasien tidak cedera.

Abstract

Cancer is a complex disease associated with a wide range of symptoms, declining quality of life, and financial toxicity (FT).To examine the quality of life and symptoms management in Cancer patients experiencing financial toxicity based on a review of published literature.A systematic review was conducted using five databases: Sage Journals, Science Direct, ProQuest, Clinical Key, and Oxford Academic from 2015-2025 years with keywords Quality of Life, Symptom Management and Cancer Patients. The quality of studies was assessed depending on the study design of the 1,050 articles found, only 11 were reviewed. Findings from 11 studies indicated that the quality of life in cancer patients is influenced by multiple factors. Financial toxicity showed a negative correlation with quality of life (r =0.23, p = 0.0057), indicating that higher levels of financial toxicity are associated with lower quality of life. Psychological symptoms such US anxiety and depression were positively associated with FT (r = 0.27, p = 0.0151; r = 0.18, p = 0.0336). High symptom burden, both physical and psychological, significantly reduced quality of life, while social support played a positive role in enhancing well-being (r = -0.17, p = 0.0412). Integrative therapies improved quality of life by 82%, and cost related health literacy was found to reduce financial toxicity (OR = 0.823, p= 0.039). Holistic interventions and attention to the financial challenges faced by cancer patients are essential for improving quality of life, particularly for those affected by financial toxicity. Effective symptom management involves the use of various tools (ESAS, HADS, FACT-G, SESI, EORTC QLQ-C30) and approaches such as palliative care, integrative therapies, symptom cluster management, acupuncture, financial support, social support, enhanced cost-related health literacy, improved psychological resilience, and identification of financial toxicity risks.The quality of life in Cancer patients is influenced by a complex interaction of factors, including financial condition, symptom management, social support, and multidisciplinary care. Enhancing health literacy—especially regarding financial aspects—with comprehensive, evidence-based, and accessible information such as insurance, medication coverage, and financial resources, is critically important.

Abstract

The success of health organizations depends on the quality of human resources including nursing resources. Health organizations can grow and develop if they have staff or nurses with demonstrated high performance. Good performance is directly proportional to the quality of services provided. Performance appraisal is very important to evaluate, assess and measure, to enhance employee capabilities and professionalism. This assessment aims to ensure that organizational goals can be achieved optimally. Furthermore, strategic planning and the role of managers are essential in performance appraisal. With good strategic planning, supported by the optimal role of managers, can enhance staff performance and facilitate the achievement of organizational objectives. Managers who have good abilities in planning, organizing, staffing, actuating and controlling the vision, mission and goals of the organization can be achieved. Objective: This study aims to determine the relationship strategic planning and the role of managers on the performance of nursing resources. Methods : This systematic review was prepared based on Preferred Reporting Items for Systematic Reviews and Meta-Analyzes (PRISMA). Used databases Scopus, ProQuest, Science Direct, Pubmed, Clinicalkey and Google Scholar. Articles used from 2023-2025. Keywords Strategic Planning AND Manager Roles AND Performance Appraisal for Nurses. Joanna Briggs Institute (JBI) and Critical Appraisal Skills Program (CASP) analysis. From 78,200 articles found, 8 articles were analyzed. The results of systematic analysis in the study showed that with appropriate strategic planning and good manager roles can improve nurse performance which has an impact on improving the quality of service and the goals and vision of the organization's mission can be achieved. The results of the study can be concluded that strategic planning and the role of managers have a significant effect on improving performance so that organizational goals can be achieved optimally.
