Abstract

Introduction the risk of errors in the perioperative period is very large so that patient safety should be supported and the team is obligated to cooperate in raising awareness toward the occurrence of the incident or event that is not expected. Head nurse control function ensure appropriate planning objectives accomplished. The purpose aims to know the effectiveness of the control function of the head nurse toward the implementation of perioperative patient safety. This research method research design using preexperiment with pretest-posttest design without control. The sample size of 75 nurses that involved in perioperative nursing service. The results data analysis using paired t test represent the effectiveness of the control function of the head nurse with p value = 0.0001 (CI = 120.79 – 127.01) and the improve of implementation of perioperative patient safety by nurses. The conclusion post intervention that indicates positive correlation with p value = 0.0001 (CI = 141.59 – 147.15). Suggestions this research recommends there should be monitoring and evaluation of implementation of the control head room and a development model that is more complete control

Abstract

Background: Alarm fatigue in nurses working in intensive care units is still a critical issue and priority in patient safety. Alarm fatigue is a condition when someone becomes insensitive to safety warnings due to receiving too many alarms. Alarm fatigue can be prevented by implementing effective alarm management. On the other hand, the current implementation of alarm management is still not optimal. Head nurses play an important role in efforts to prevent alarm fatigue incidents and oversee the implementation of alarm management and need direction in its implementation in intensive care units. Purpose: To initiate alarm fatigue management for nurses through alarm management guidelines in intensive care units. Method: A case report study according to Kotter's planned change stages was conducted at the University of Indonesia Teaching Hospital on April 22 - May 24, 2024. The research process included assessment, data analysis, action plan (PoA), implementation, and evaluation. Data collection was carried out through interviews, observations, and filling out questionnaires. Results: The priority problems obtained were the suboptimal planning function related to alarm fatigue management through alarm management and implementation through Kotter's theory which had only been carried out until the third stage. Conclusion: Planned changes can be made in the implementation of implementation in solving problems. The problem found was the suboptimal function of the head of the room in alarm management in the intensive care unit. The alarm management guide that has been created is expected to prevent the occurrence of fatigue alarms in nurses and provide quality services. Suggestion: The draft guide for managing nurse fatigue alarms through alarm management can be approved by the president director and applied in services in the hospital's intensive care unit. In addition, the hospital can organize training and education for nursing staff to improve understanding of fatigue alarms, effective alarm management, and conduct periodic evaluations.   Keywords: Alarm Management; Fatigue Alarm; Intensive Care Unit; Nurses.   Pendahuluan: Alarm fatigue pada perawat yang bekerja pada ruang intensif masih menjadi masalah yang kritis dan prioritas dalam keselamatan pasien. Alarm fatigue adalah kondisi ketika seseorang menjadi tidak peka terhadap peringatan keselamatan akibat menerima terlalu banyak alarm. Alarm fatigue dapat dicegah dengan menerapkan manajemen alarm yang efektif. Di sisi lain, penerapan manajemen alarm saat ini masih belum optimal. Head nurse memainkan peran penting dalam upaya mencegah kejadian alarm fatigue dan mengawasi pelaksanaan manajemen alarm dan membutuhkan panduan dalam pelaksanaannya di ruang intensif. Tujuan: Untuk menginisiasi pengelolaan alarm fatigue perawat melalui panduan manajemen alarm di ruang intensif. Metode: Penelitian case report sesuai tahapan perubahan terencana Kotter’s, dilakukan di Rumah Sakit Pendidikan, Universitas Indonesia pada tanggal 22 April – 24 Mei 2024. Proses penelitian meliputi pengkajian, analisis data, rencana tindakan/plan of action (PoA), implementasi, dan evaluasi. Data dikumpulkan melalui wawancara, observasi, dan pengisian kuesioner. Hasil: Prioritas masalah yang didapat yaitu fungsi perencanaan belum optimal terkait dengan pengelolaan alarm fatigue melalui manajemen alarm dan implementasi melalui teori Kotters, dilakukan baru sampai pada tahap yang ketiga. Simpulan: Perubahan terencana dapat dilakukan dalam pelaksanaan implementasi dalam menyelesaikan masalah. Permasalahan yang ditemukan yakni belum optimalnya fungsi perencanaan kepala ruangan dalam manajemen alarm di ruang intensif. Panduan manajemen alarm yang telah dibuat diharapkan dapat mencegah terjadinya alarm fatigue pada perawat dan memberikan pelayanan yang berkualitas. Saran: Draft pengelolaan alarm fatigue perawat melalui manajemen alarm dapat disahkan oleh direktur utama dan diterapkan dalam pelayanan di ruang intensif rumah sakit. Selain itu, pihak rumah sakit dapat menyelenggarakan pelatihan dan edukasi bagi staf perawat untuk meningkatkan pemahaman tentang alarm fatigue, manajemen alarm yang efektif, dan melakukan evaluasi secara berkala.   Kata Kunci: Alarm Fatigue; Manajemen Alarm; Perawat; Ruang Intensif.

Abstract

Data tentang leadership kepala ruangan yang menunjukkan bahwa adanya persepsi yang kurang terhadap kemampuan leadership dan quality control, hal tersebut  merupakan bagian dari standar kompetensi yang harus dimiliki oleh kepala ruangan. Pelaksanaan pendidikan bagi kepala ruangan belum optimal dan belum berkesinambungan. Penelitian dilakukan dengan rancangan penelitian survei. Data dikumpulkan melalui wawancara, observasi, Focus Group Discussion (FGD) dan Survei dalam bentuk kuesioner. Data penelitian dianalisis untuk mendiagnosis masalah dan merumuskan metode pemecahannya. Rumusan masalah yang didapat adalah belum optimalnya program pendidikan berkelanjutan bagi kepala ruangan. Penyelesaian masalah berupa Kegiatan dengan metode pendekatan action research yaitu dengan menerapkan metode pelatihan berbasis kurikulum yang disusun sesuai dengan standar kompetensi kepala ruangan dengan jumlah peserta pelatihan sebanyak 112 peserta. Hasil dari action research ini menunjukkan peningkatan pengetahuan dari kepala ruangan. Evaluasi melalui pre dan post test tentang materi pelatihan yang sudah diberikan. Nilai rata-rata pre dan post test peserta meningkat dari rata-rata 53.88 menjadi 80.36,  meningkat 26.48 poin.

Abstract

Implementation of quality improvement in hospitals is still unsatisfactory. One of the causes of this problemis the inadequate implementation of nursing audits by the nursing committee. This study aims to identify theimplementation of the nursing profession audit at Hospital A, West Jakarta. The method used is a pilot studyin the nursing room with the involvement of manager nurses and clinical nurses as many as 140 nurses. Theanalysis from the assessment stage, interviews, observation, distributing questionnaires and focus groupdiscussions, and fishbone analysis found that the nursing profession audit was the task of the nursingsubcommittee that was being improved. The implementation carried out is the latest creation, the auditorteam, audit tools and the implementation of nursing audits. Result: The audit was carried out by determiningthe topic of handover between shifts. The not yet optimal implementation of handover between shifts is theresult of the nursing audit. The conclusion has been that the preparation of a new audit of nursing qualityhas been carried out in RS A, the formation of an auditor team and the compilation of tools in implementingnursing audits. The nursing profession quality audit has been carried out on the topic of handover betweenshifts and the result is that the handover between shifts is optimized. Recommendations for the latest nursingaudits validation, conducting nursing audits on topics according to the needs of the nursing committee andperiodic re-audits.

Abstract

Falls is an incident that must be prevented when patients treated in the hospital. Nurses must comply withStandard Operating Procedures (SOP) to prevent the incidents of patient safety. Nursing supervision is oneway to improve nurse compliance. The method used is a pilot project by innovating. Implementation consistedof making drafting supervision work instructions, supervision instrument drafts, patient safety pocket books,innovation’s sosialization to 40 nurses consist of nursing services, nursing comitee, head nurses, head ofinstallations, leaders and associate nurses. The results of the study through the input project evaluation inthe form of interviews with 2 team leaders and 3 implementing nurses showed that headnurse’s instrumentsand work instructions supervision of falling risk reduction will help increase nurses' knowledge of StandardOperating Procedures (SOPs) for Falling Risk Reduction in Hospitals. Conclusion: the increase in nurses'knowledge about the reduction in fall risk is influenced by the supervisory function of the headnurse as thefirst-line nurse manager. Recommendation: this study is the basis for the implementation of supervision ofpatient safety targets, especially the reduction in the risk of falls.

Abstract

Background: Nurse burnout can reduce performance and quality of service. Nurses who experience burnout syndrome will lose motivation for work due to prolonged responses to emotional, physical, and mental exhaustion. Purpose: To analyze the effectiveness of mindfulness in reducing burnout in implementing nurses. Method: Systematic review using protocols and rules in accordance with the Preferred Reporting Items for Systematic Review (PRISMA), using databases from Scopus, Proquest, Pubmed, SAGE, and Science Direct in 2019-2024, found 220 articles and 8 selected articles. Results: Mindfulness training reduces fatigue levels, produces lower scores for emotional exhaustion and depersonalization, and is higher for personal achievement. Conclusion: Mindfulness in implementing nurses is an effective effort to choose what nurses will think and maintain the attention needed to maintain awareness of current experiences or in providing nursing care, thereby helping implementing nurses reduce stress due to burnout in the long term. Suggestions: This review has the potential to be biased due to the lack of studies and high heterogeneity. Practical mindfulness techniques for nurses still need further research and should be guided by professionals to be effective.   Keywords: Burnout Syndrome; Mindfulness; Implementing Nurses.   Pendahuluan: Burnout perawat dapat menurunkan kinerja dan mutu layanan. Perawat yang mengalami sindrom burnout akan kehilangan motivasi terhadap pekerjaan karena respon yang berkepanjangan terhadap kelelahan emosional, fisik, dan mental. Tujuan: Untuk menganalisis efektifitas mindfullness dalam menurunkan burnout perawat pelaksana. Metode: Sistematik review menggunakan protokol dan kaidah yang sesuai dengan Preferred Reporting Items for Systematic Review (PRISMA). Pencarian literatur berasal dari database, Scopus, Proquest, Pubmed, SAGE, dan Sciencedirect tahun 2019-2024, ditemukan 227 artikel dan sebanyak 8 artikel terpilih. Hasil: Pelatihan mindfulness mengurangi tingkat kelelahan, menghasilkan skor yang lebih rendah untuk kelelahan emosional dan depersonalisasi, serta lebih tinggi untuk pencapaian pribadi. Simpulan: Mindfulness pada perawat pelaksana merupakan upaya yang efektif untuk memilih apa yang akan perawat pikirkan dan mempertahankan atensi yang dibutuhkan untuk menjaga kesadaran terhadap pengalaman saat ini atau dalam melakukan asuhan keperawatan, sehingga membantu perawat pelaksana menurunkan stress akibat burnout dalam jangka waktu yang lama. Saran: Tinjauan ini berpotensi bias karena minimnya studi dan tingginya heterogenitas. Teknik mindfulness yang praktis bagi perawat masih perlu diteliti lebih lanjut dan sebaiknya dipandu profesional agar efektif.   Kata kunci: Burnout Syndrome; Mindfulness; Perawat Pelaksana.

Abstract

Background: The handover process in nursing care is very important because it is directly related to patient safety and continuity of care. A suboptimal handover can increase the risk of medical errors. Purpose: To develop handover guidelines in the inpatient ward of the University of Indonesia Hospital. Method: A case report study on the preparation of handover guidelines was conducted at the University of Indonesia Hospital on September 2 - October 2, 2024. This activity includes assessment, data analysis, action plan (Plan of Action/PoA), implementation, and evaluation. Data collection was carried out through interviews, observations, and questionnaires. The approach used to solve the problem through Kurt Lewin's theory of change. Results: Shows that the implementation of handover is not optimal with variations in methods used, including conferences and handovers at the bedside. The results of the study indicate that the implementation of the handover SOP has not been consistently adhered to by ward nurses and several factors such as time constraints and lack of supervision have a negative effect on the effectiveness of the handover. Conclusion: The implementation of handovers in inpatient rooms has not been optimal due to inconsistencies in the application of SOPs, time constraints, and lack of supervision. The development of handover guidelines with the Kurt Lewin change theory approach is a strategic step to improve the effectiveness and consistency of handovers. Wider dissemination of guidelines and regular monitoring are needed to ensure patient safety and the quality of sustainable nursing care. Suggestion: Handover guidelines need to be updated regularly based on scientific evidence (evidence-based practice) to remain relevant to developments in nursing science. Socialization and implementation activities of handover conferences and handovers at the patient's bedside need to be monitored and evaluated routinely to ensure their effectiveness in improving the quality of patient handovers.   Keywords: Handover; Guidelines; Development; Inpatient; Hospital.   Pendahuluan: Proses serah terima atau handover dalam asuhan keperawatan sangat penting karena secara langsung berhubungan dengan keselamatan pasien dan kontinuitas perawatan. Handover yang tidak optimal dapat meningkatkan risiko kesalahan medis. Tujuan:  Untuk mengembangkan pedoman handover di ruang rawat inap rumah sakit Universitas Indonesia. Metode: Penelitian case report mengenai pengembangan panduan handover, dilakukan di Rumah Sakit Universitas Indonesia pada 2 September – 2 Oktober 2024. Kegiatan ini meliputi pengkajian, analisis data, rencana tindakan plan of action (PoA), implementasi, dan evaluasi. Data dikumpulkan melalui wawancara, observasi, dan pengisian kuesioner. Pendekatan yang digunakan untuk menyelesaikan masalah melalui teori perubahan Kurt Lewin. Hasil: Menunjukkan bahwa pelaksanaan handover belum optimal dengan variasi metode yang digunakan, termasuk conference dan bedside handover. Hasil menunjukan bahwa pelaksanaan SPO handover tidak konsisten dipatuhi perawat ruangan dan beberapa faktor, seperti keterbatasan waktu dan kurangnya supervisi berpengaruh negatif terhadap efektivitas handover. Simpulan: Pelaksanaan handover di ruang rawat inap belum berjalan secara optimal akibat ketidakkonsistenan dalam penerapan SPO, keterbatasan waktu, dan kurangnya supervisi. Pengembangan panduan handover dengan pendekatan teori perubahan Kurt Lewin menjadi langkah strategis untuk meningkatkan efektivitas dan konsistensi handover. Sosialisasi panduan yang lebih luas dan monitoring berkala diperlukan untuk menjamin keselamatan pasien dan kualitas asuhan keperawatan yang berkelanjutan. Saran: Panduan handover perlu diperbarui secara berkala berdasarkan bukti ilmiah (evidence-based practice) agar tetap relevan dengan perkembangan ilmu keperawatan. Abstract

Background: The use of information technology is increasingly widespread in the health sector, especially for nurses. The intensity of the use of this technology has created a new phenomenon called technostress. The European Risk Observatory categorizes technostress as an increasing occupational health risk caused by the negative psychosocial effects of the use of information technology. These problems are correlated with decreased job satisfaction, productivity, and work-life balance that have the potential to threaten patient safety. Purpose: To analyze the dimensions, measurement instruments, and research results of technostress in nurses. Method: A systematic review was conducted by searching the literature related to technostress in nurses using PubMed, ScienceDirect, EBSCO, Proquest, and Scopus databases. Studies that met the inclusion criteria were assessed using the JBI Critical Appraisal Checklist for Cross-sectional Studies. Results: A total of seven relevant studies were identified with five high-quality studies and two moderate-quality studies. Five main dimensions of technostress were identified: techno-overload, techno-invasion, techno-complexity, techno-insecurity, and techno-uncertainty. Six measurement instruments were identified with moderate levels of technostress in nurses. Conclusion: Based on the results of the study review, six main instruments were obtained that were used to measure technostress in nurses and five main dimensions of technostress in nurses, including techno-overload, techno-invasion, techno-complexity, techno-insecurity, and techno-uncertainty, as well as moderate levels of technostress to impacts that include emotional exhaustion, work-privacy conflict, burnout, decreased job satisfaction, desire to move, and physical and mental health problems.   Keywords: Measurement Instruments; Nurses; Technostress.   Pendahuluan: Penggunaan teknologi informasi semakin meluas di sektor kesehatan, terutama pada perawat. Intensitas penggunaan teknologi ini telah menciptakan fenomena baru yang disebut technostress. European Risk Observatory mengategorikan technostress sebagai risiko kesehatan kerja yang terus meningkat yang diakibatkan efek psikososial negatif dari penggunaan teknologi informasi. Permasalahan tersebutlah yang berkorelasi dengan penurunan kepuasan kerja, produktivitas, dan keseimbangan kehidupan kerja yang berpotensi mengancam keselamatan pasien. Tujuan: Untuk menganalisis technostress  pada perawat dan instrumen pengukurannya. Metode: Penelitian tinjauan sistematis terkait technostress pada perawat, pencarian literatur berasal dari database antara lain PubMed, ScienceDirect, EBSCO, Proquest, dan Scopus. Studi yang memenuhi kriteria inklusi dinilai menggunakan JBI Critical Appraisal Checklist for Cross-sectional Studies. Hasil: Sebanyak tujuh studi relevan diidentifikasi dengan lima studi berkualitas tinggi dan dua studi berkualitas sedang. Lima dimensi utama technostress teridentifikasi: techno-overload, techno-invasion, techno-complexity, techno-insecurity, dan techno-uncertainty. Enam instrumen pengukuran teridentifikasi dengan tingkat technostress  moderat pada perawat. Simpulan: Berdasarkan hasil tinjauan studi, diperoleh sebanyak enam instrumen utama yang digunakan untuk mengukur technostress pada perawat dan lima dimensi utama technostress pada perawat yang meliputi, techno-overload, techno-invasion, techno-complexity, techno-insecurity, dan techno-uncertainty, serta tingkat technostress  moderat hingga dampak yang meliputi kelelahan emosional, konflik kerja-privasi, burnout, penurunan kepuasan kerja, keinginan berpindah, serta masalah kesehatan fisik dan mental.   Kata Kunci: Instrumen Pengukuran; Perawat; Technostress.

Abstract

Latar belakang: Perencanaan strategis sangat diperlukan dalam pengembangan rumah sakit. Perencanaan strategis secara finansial dapat mengoptimalkan kekuatan rumah sakit, sehingga dapat meningkatkan pendapatan rumah sakit serta kualitas layanan terhadap pasien. Tujuan: Studi ini bertujuan untuk menyampaikan hasil eksplorasi data perencanaan keuangan keperawatan RS wilayah Depok Jawa Barat, menganalisis data, dan membuat rekomendasi untuk mengembangkan sebuah rancangan perencanaan strategis. Metode: Penelitian ini menggunakan metode studi kasus dengan melakukan analisis SWOT dan mengembangkan analisis rencana strategis menggunakan analisis matriks TOWS. Hasil: Berdasarkan hasil analisis RS wilayah Depok Jawa Barat pada matriks internal eksternal terletak pada sel II yang artinya growth and build dimana terdapat 5 strategi terpilih meliputi penguatan premi asuransi komersial sebagai upaya (1) peningkatan sarana prasarana layanan. (2) Peningkatan kerjasama dengan mitra asuransi untuk menjaring pasar yang lebih luas. (3) Peningkatan mitra dan jejaring untuk meningkatkan modal pengembangan usaha. (4) Peningkatan benchmarking dan studi banding untuk penentuan tarif layanan unggulan yang kompetitif serta (5) Optimalisasi unit kerja fungsional untuk meningkatkan penyerapan dana BPJS. Kesimpulan: strategi yang harus digunakan antara lain penetrasi pasar dan pengembangan produk karena menurut analisis SWOT dan dilanjutkan dengan pembobotannya, RS wilayah Depok Jawa Barat berada pada sel II yaitu growth and build.

Abstract

Nurses as the person in charge of nursing care must ensure safe behavior while working, one of which is from the occurrence of needlestick injuries. The purpose of this paper is to identify the impact of optimizing nurse practice in the prevention of needlestick injuries. The author observed the behavior of nurses by using Standard Operating Procedures (SPO) to administer drugs through intravenous. Optimization of nurses' practices through interactive discussions on the prevention of needlestick injuries during pre-conferences. The results of optimizing nurses' practices in giving drugs through intravenous complete according to SPO. It is recommended that optimization of nurse practice is important to prevent needlestick injuries.
