Abstract

Postoperative complications following general anesthesia, especially those affecting the respiratory system, present a significant challenge in healthcare services, as they can lead to oxygen desaturation and hypoxemia, ultimately impairing vital organ function. One intervention to improve oxygen saturation is the balloon blowing technique. This community service activity aimed to provide education on balloon blowing breathing relaxation techniques to enhance oxygen saturation in postoperative patients under general anesthesia. The method included a 5-minute educational session and a 3–5 minute demonstration of the balloon blowing technique. Measurements of knowledge levels and oxygen saturation were conducted before and after the intervention. The results showed a significant improvement: prior to the education, 70% of participants (21 individuals) experienced decreased oxygen saturation, while after the intervention, all participants (100%) showed increased oxygen saturation. Knowledge levels also improved, with 70% of participants initially in the low category, increasing to 100% in the good category after the education.

Abstract

Choking is a medical emergency caused by a foreign object obstructing the airway, potentially leading to hypoxia or even death if not treated promptly. One recommended emergency intervention is the back blow technique, which involves applying forceful blows to the victim’s back to dislodge the obstruction. This community service activity aimed to increase public knowledge and skills in handling choking emergencies using the back blow technique. The method consisted of health education through lectures and live demonstrations, followed by pre- and post-tests to assess participants' knowledge and skills using structured questionnaires and skill checklists. The results indicated significant improvement. Prior to the intervention, 43% of participants had low knowledge and 33.3% had moderate knowledge. Post-intervention, all participants (100%) showed improved knowledge. In terms of skills, 73.3% were categorized as having low skills before training, while after demonstration, all participants (100%) reached a good skill level. The outputs of this program included an educational pocketbook and a journal publication.

Abstract

Pendahuluan: Salah satu faktor yang mempengaruhi penurunan saturasi oksigen dalam aliran darah adalah merokok Perokok menurut WHO untuk sekarang adalah mereka yang merokok setiap hari untuk jangka waktu minimal 6 bulan selama hidupnya. Merokok adalah kegiatan mengkonsumsi rokok dengan cara membakar rokok dan atau menghisap asap rokok. Penelitian ini bertujuan untuk memperoleh gambaran saturasi oksigen pada pasien perokok dengan general anestesi inhalasi di recovery room.Metode: Penelitian ini adalah deskriptif kuantitatif dengan pendekatan cross-sectional. Teknik sampling menggunakan nonprobability sampling dengan purposive sampling sebanyak 60 responden. Hasil: Penelitian menunjukkan bahwa dari total 60 responden, responden memiliki frekuensi merokok sedang, yaitu sebanyak 41 peserta (68,3%), dan frekwensi merokok ringan berjumlah 19 peserta (31,7%). Saturasi oksigen, sebagian besar yaitu 48 peserta (80,0%), memiliki tingkat saturasi oksigen yang normal, yaitu di atas 95%. Sementara, 12 peserta (20,0%) menunjukkan tingkat saturasi oksigen yang rendah, yaitu di bawah 95%. Kesimpulan: Pada pasien paska general anestesi inhalasi riwayat frekuensi merokok memberi pengaruh pada kadar saturasi oksigen pasien sebanyak 20%.

Abstract

Mobilisasi dini merupakan salah satu intervensi penting pada pasien pasca operasi dengan spinal anestesi untuk mempercepat proses pemulihan dan mengurangi risiko komplikasi akibat imobilisasi berkepanjangan. Kegiatan Pengabdian kepada Masyarakat (PkM) ini bertujuan meningkatkan pengetahuan pasien mengenai mobilisasi dini pasca spinal anestesi melalui edukasi kesehatan menggunakan media leaflet di UPTD RSUD dr. M. Thomsen Nias. Metode pelaksanaan menggunakan pendekatan edukatif partisipatif melalui ceramah, diskusi, pembagian leaflet, dan edukasi langsung kepada pasien serta keluarga pasien. Kegiatan dilaksanakan pada tanggal 4 Februari 2026 sampai 10 Februari 2026 dengan jumlah responden sebanyak 30 pasien pasca spinal anestesi. Evaluasi dilakukan menggunakan kuesioner pre-test dan post-test untuk mengetahui tingkat pengetahuan pasien sebelum dan sesudah edukasi. Hasil kegiatan menunjukkan adanya peningkatan pengetahuan pasien setelah diberikan edukasi. Pada pre-test, responden dengan kategori pengetahuan kurang sebanyak 47%, cukup 40%, dan baik 13%. Setelah edukasi diberikan, hasil post-test menunjukkan peningkatan kategori baik menjadi 70%, kategori cukup 20%, dan kategori kurang menurun menjadi 10%. Hasil menunjukkan bahwa edukasi menggunakan media leaflet efektif meningkatkan pengetahuan pasien mengenai mobilisasi dini pasca spinal anestesi. Kesimpulan kegiatan ini adalah edukasi kesehatan mengenai mobilisasi dini mampu meningkatkan pengetahuan pasien pasca spinal anestesi sehingga diharapkan dapat membantu mempercepat proses pemulihan dan mendukung pengurangan lama rawat inap pasien.

Abstract

Hypotension and bradycardia are common complications following spinal anesthesia due to extensive sympathetic blockade, which can lead to hemodynamic instability and decreased perfusion of vital organs. A simple non-pharmacological preventive measure is maintaining a sitting position immediately after induction to limit the upward spread of hyperbaric anesthetic agents toward the thoracic region. This study aimed to determine the effect of maintaining a sitting position for three minutes after spinal anesthesia induction on the incidence of hypotension and bradycardia at PKU Muhammadiyah Gombong Hospital. The research employed a quasi-experimental design with a pre–post test with control group approach involving 69 respondents selected through consecutive sampling. The intervention group (n = 34) maintained a sitting position for three minutes, while the control group (n = 35) was immediately placed in a supine position. Blood pressure and heart rate were observed at 5, 10, and 15 minutes after induction, and the data were analyzed using Chi-Square and binary logistic regression tests. The results showed that the incidence of hypotension and bradycardia was significantly lower in the intervention group (p < 0.05), indicating that the sitting position acted as a protective factor. In conclusion, maintaining a sitting position for three minutes after spinal anesthesia induction effectively reduces the incidence of hypotension and bradycardia and is recommended as a simple strategy to enhance patient hemodynamic stability.

Abstract

Cholelithiasis with comorbid anemia and thrombocytopenia poses significant challenges in perioperative management, particularly when laparoscopic cholecystectomy must be converted to open laparotomy due to severe inflammation and adhesions. This case report describes the anesthetic management of a 57-year-old male patient diagnosed with cholelithiasis, anemia (Hb 10.8 g/dL), thrombocytopenia (96 ×10³/µL), and leukopenia, who underwent surgery under balanced general anesthesia. The procedure was initiated laparoscopically but converted to open laparotomy due to gallbladder wall thickening and distorted anatomy. Intraoperatively, bleeding of approximately 500 mL occurred, requiring transfusion of packed red cells and platelets. Hemodynamic fluctuations in the form of hypotension were successfully managed with fluid therapy and transfusion, ensuring adequate tissue perfusion. Postoperatively, the patient experienced delayed emergence, which improved following antagonization with atropine and neostigmine. The patient was subsequently transferred to the ICU for intensive monitoring given the risks associated with anemia, thrombocytopenia, and suboptimal recovery. This case highlights the importance of comprehensive preoperative evaluation, vigilant intraoperative monitoring, individualized anesthetic strategies with balanced anesthesia, and multidisciplinary collaboration to minimize complications in biliary surgery patients with hematological comorbidities

Abstract

Hemodialysis is a complex and long-term renal replacement therapy that requires strict adherence to multiple medical recommendations, including fluid restriction, medication adherence, dietary regulation, and scheduled therapy sessions. Non-adherence to the hemodialysis regimen can worsen clinical conditions and increase the risk of complications, morbidity, and mortality. Therefore, effective and sustainable intervention strategies are needed to improve adherence among hemodialysis patients. This narrative review aims to integrate recent findings from studies that examine various adherence-enhancing interventions, including interactive education, psychosocial support, behavioral interventions, and physiological monitoring. A narrative synthesis was conducted through a literature review of ten recent studies evaluating the effectiveness of different approaches, such as teach-back methods, nurse-led follow-up calls, behavioral self-regulation strategies, psychosocial support, and physiological monitoring, in improving adherence to fluid restriction and therapeutic regimens. The findings indicate that interactive educational interventions, particularly teach-back techniques and nurse follow-up calls, significantly improve patient understanding and adherence to treatment protocols. Behavioral interventions, including self-regulation and positive coping skills training, support patients’ psychological adaptation to long-term therapy. In addition, psychosocial support from family and healthcare professionals enhances motivation and promotes sustained adherence. Routine physiological monitoring, such as assessment of interdialytic weight gain, further supports adherence to prescribed regimens. Overall effectiveness

Abstract

Kidney transplantation is the treatment of choice for patients with end-stage chronic kidney disease. The success of this procedure is highly influenced by appropriate perioperative fluid management. Fluid imbalance can lead to serious complications, including delayed graft function, impaired renal perfusion, and increased postoperative morbidity and mortality. This review aims to evaluate the current evidence on perioperative fluid therapy strategies in kidney transplantation, focusing on fluid type selection, monitoring methods, and clinical implications. A narrative review was conducted by synthesizing recent studies on fluid management in patients with chronic kidney disease undergoing kidney transplantation. The findings show that balanced crystalloids are preferred over 0.9% sodium chloride because they improve acid–base balance and renal perfusion. In addition, goal-directed fluid therapy guided by dynamic hemodynamic parameters helps maintain intraoperative hemodynamic stability. The combination of these approaches is associated with reduced postoperative complications and improved graft function. In conclusion, optimal perioperative fluid management plays a crucial role in improving kidney transplant outcomes. An individualized approach based on patient hemodynamic status, surgical phase, and graft characteristics should be applied to achieve better, safer clinical outcomes consistent with current evidence-based practice. The clinical implications highlight the need for standardized perioperative fluid management protocols across transplant centers to improve patient safety, reduce clinical practice variability, and support the consistent implementation of evidence-based guidelines in modern healthcare. This approach is also expected to enhance long-term patient outcomes and healthcare efficiency in referral hospitals through improved coordination of an integrated multidisciplinary clinical team.

Abstract

Chronic kidney disease (CKD) is a major global health burden characterized by progressive loss of renal function and impaired acid–base regulation resulting from reduced hydrogen ion excretion and limited bicarbonate regeneration.These abnormalities frequently lead to chronic metabolic acidosis, contributing to disease progression and increasing perioperative risk in surgical patients.This narrative review evaluates the impact of intravenous fluid choice, specifically 0.9% saline versus balanced crystalloids, on acid–base homeostasis in CKD patients undergoing surgery. A critical analysis was performed using ten peer reviewed articles from reputable journals, focusing on the pathophysiology of acid–base disturbances in CKD, biochemical effects of perioperative fluids, and clinical outcomes related to fluid selection.The findings indicate that 0.9% saline, due to its high chloride concentration, is associated with hyperchloremic metabolic acidosis and may worsen renal function.Conversely, balanced crystalloids contain metabolizable buffers such as lactate or acetate, preserve a more physiological strong ion difference, enhance acid–base stability, and are linked to a lower risk of acute kidney injury.In conclusion, balanced crystalloids are preferable to saline for perioperative fluid management in CKD patients, as they support better acid–base balance and may reduce postoperative complications. Individual patient factors and electrolyte status should guide perioperative therapy decisions.

Abstract

Intravenous fluid management is a critical component of perioperative care in patients with chronic kidney disease (CKD). The type of fluid administered may influence acid–base balance, electrolyte status, and the risk of acute kidney injury (AKI). This article aims to review the latest scientific evidence comparing the effects of 0.9% saline and balanced crystalloids in CKD patients in the perioperative setting. A narrative review was conducted by searching PubMed, Scopus, ScienceDirect, and Google Scholar using keywords related to saline, balanced crystalloids, CKD, perioperative care, and AKI. Eligible articles included randomized controlled trials, meta-analyses, and systematic reviews published between 2015–2024. Results: Evidence from multiple studies indicates that balanced crystalloids are superior in maintaining acid–base stability, reducing chloride levels, and lowering the incidence of AKI compared to saline. Large trials such as SMART, SALT-ED, and BEST-Fluids demonstrated reduced major adverse kidney events and delayed graft function in the balanced group, although mortality outcomes remain inconsistent. Balanced crystalloids provide greater physiological advantages and renal protection compared to saline in CKD patients undergoing surgery. While mortality reduction has not been consistently observed, current evidence supports the use of balanced crystalloids as the preferred perioperative fluid, particularly in patients at high risk of AKI.
