Abstract

Introduction: Atherosclerosis complication in the cardiovascular system has been one of the biggest medical problems in recent years and the definitive treatment has not been found. Trimethylamine-N-oxide (TMAO) is a catalytic product of Flavin-Containing Monooxydase (FMO) 3 enzyme and might act as a predispostion factor for atherosclerosis. Indonesia has many herbal plants which can potentially be developed into antiatherosclerosis drug. This was an initial study of drug development which aimed to identify FMO3 inhibitors from Indonesian herbal plants by using molecular docking. Methods: It was a bioinformatics study which utilized all herbal compounds recorded in HerbalDB, had three dimentional structure, and met the criteria for Lipinski's rule of five. Methimazole was used as a standard ligand and hFMO model was determined using FMO protein template from Methylophaga aminisulfidivorans. Herbal compounds were molecularly docked with hFMO3 models using AutodockVina 1.1.2. PyMOL 1.7 dan Chimera 1.10rc were used for visualization of docking results. Binding affinity, binding site, and Lipinski's rule of five criterias were used to determine hFMO3 inhibitor candidates of herbal compounds. Results: Methimazole bound to the hFMO3 model at Asn194 with binding energy average of -3.8 kcal/mol. Droserone, vanillic acid, (s)-(+) abscisic acid, and sebacic acid had lower binding energy, had similar binding site, and had the best drug like property, compared with methimazole. Conclusion: Droserone, vanillic acid, (s)-(+) absicic acid, and sebacic acid become the potential candidates of hFMO3 inhibitor in silico. A future study using flexible ligand and flexible receptor docking methods is needed to get more accurate results. Abstract

Introduction: Lactate dehydrogenase A (LDHA) is an enzyme that catalyzes pyruvate into lactate. LDHA plays an important role in promotion of cancer cells growth through increasing aerobic glycolysis. Because LDHA has a central role in energy metabolism, it become a molecular target for development of anticancer drug. This was a biocomputational study that aimed to identify Indonesian herbal compounds which became a potential candidate of LDHA inhibitor via molecular docking analysis. Methods: Samples in this study were Indonesian herbal compounds that met the following criteria: (1) Registered on Database Herbal Indonesia, (2) had three-dimensional structure, and (3) met the criteria Lipinski rule of five. Oxamate used as a ligand standard and was validated using Autodock Vina software. Herbal compounds were also docked using the same program. Docking results were visualized using PyMOL software. LDHA inhibitor candidate is determined by comparing herbal compounds and standard ligand in terms of binding energy, binding site and Lipinski criteria. Result: Oxamate interacting with LDHA had -4.26 0.06 kcal / mol binding energy and bound to six amino acid residues at Gln 99, Arg 105, Asn 137, Arg 168, His 192, and Thr 247. A lower binding energy was observed in 23 herbal compounds and these compounds bound to LDHA at least five amino acid residues like Oxamate. Herbal compounds Phaseolic Acid, Sebacic Acid, D (-) - Fructose, Suberic Acid and Pimelic Acid interacted with amino acid residues of LDHA as same as Oxamate. The other herbal compounds interacted with less or more than six amino acid residues of LDHA. Based on characteristics of five herbal compounds, Phaseolic Acid, Sebacic Acid and Suberic Acid were probably the best candidates of LDHA inhibitor. Conclusion: Phaseolic Acid, Sebacic Acid and Suberic Acid become biocomputationally the best LDHA inhibitor. Enzymatic assays are needed to investigate whether or not all these compounds can inhibit LDHA enzyme activity. Keywords : Cancer, Inhibitor LDHA, Molecular Docking, Herbal Indonesia

Abstract

Background: Development of new selective anticancer drug is an important necessary. Because of that, some effective way to screen herbal compounds which have good potency for developing drug is needed, one of them is using molecular docking methods. In this study, researchers identify herbal compounds as new thymidylate synthase (TS) inhibitors which more potent than 5-Fluorouracil (5-FU) using molecular docking. Methods: Three-dimensional structure samples of herbal compounds from Indonesian herbal database (herbaldb.ui.ac.id) were docked with three-dimensional structure of TS protein using iGEMDOCK program. Then, the compounds which have lower docking score than fdUMP, as standard ligand, clustered using ChemMine. Results: The docking result showed that 27 herbal compounds have docking score lower than fdUMP in iGEMDOCK program. The 27 compounds have higher affinity to fdUMP and it can form a stable complex with TS protein. Protein-ligand complexes showed that all of 27 compounds have same binding site with fdUMP. But folic acid is excluded from the result because it is a TS ligand. fdUMP has similar structure with UMP, a TS ligand, but folic acid which has lowest docking score is not similar with UMP. Conclusions: The 26 compounds, except folic acid, are more potent than 5-FU as TS inhibitors by docking study. Further study to delineate the effectivity of molecular docking is needed. Abstract

Cervival cancer is one of the top rank of gynecological malignancy in the world, leading to high morbidity and mortality rates. Cisplatin is a chemotherapeutic agent that is generally used to treat cervical cancer but the use of this drug is limited because of serious side effects. Metformin, a diabetic drug, decreases not only blood glucose levels but also cell viability of some cancer cells. The aim of this study was to investigate the anti-proliferative effect of combination metformin and cisplatin in HeLa cells (cervical cancer cell line). Anti-proliferative effect of these combined drugs was analized using MTT assay, combination index assay and HeLa cell morphology. Inhibitory concentration (IC50) of cisplatin and metformin was determined before performing combination index assay. Administration of 10 mM metformin showed inhibition of HeLa cell proliferation and it reached 50% inhibition of cell proliferation at 60 mM. Whilst, cisplatin showed a stronger anti-proliferative effect with initial inhibition dose at 12 μM and IC50 dose at 44 μM. Combination of 30 mM metformin and 5 μM cisplatin indicated the strongest anti-proliferative effect on HeLa cell. In conclusion, metformin may become a promising drug for treatment of cervical cancer in future which enhances anti-proliferative effect of cisplatin.Key words: Anti-proliferative effect, combination index, cervical cancer, cisplatin, metformin Peningkatan Efek Anti-poliferatif Cisplatin oleh Metformin pada Cell Line Kanker Serviks Kanker serviks merupakan salah satu keganasan ginekologi tertinggi di dunia, dengan tingkat morbiditas dan mortalitas yang tinggi. Cisplatin merupakan obat kemoterapi yang umum digunakan untuk terapi kanker serviks, namun penggunaannya relatif terbatas karena menyebabkan beberapa efek samping yang serius. Metformin merupakan obat anti diabetik yang mampu menurunkan kadar glukosa darah dan juga mampu menurunkan viabilitas beberapa jenis sel kanker. Penelitian ini bertujuan untuk mengetahui efek anti-proliferasi kombinasi metformin dan cisplatin pada sel HeLa (cell line kanker serviks). Efek anti-proliferasi kombinasi kedua senyawa tersebut dianalisis melalui MTT assay, combination index assay dan morfologi sel HeLa. Nilai inhibitory concentration (IC50) metformin dan cisplatin pada sel HeLa ditentukan lebih dahulu sebelum melakukan combination index assay. Pemberian metformin 10 mM mulai menunjukkan penghambatan proliferasi sel HeLa dan penghambatan proliferasi sel mencapai 50% pada dosis 60 mM. Cisplatin menunjukkan efek anti-proliferasi yang lebih kuat dengan dosis awal penghambatan sebesar 12 μM dan IC50 sebesar 44 μM. Kombinasi antara metformin 30 mM dan cisplatin 5 μM memperlihatkan efek anti-proliferatif terkuat pada sel HeLa. Sebagai kesimpulan, metformin kemungkinan menjadi obat yang menjanjikan untuk terapi kanker serviks di masa mendatang dengan cara meningkatkan efek anti-proliferasi cisplatin.Kata kunci: Cisplatin, combination index, efek anti-proliferatif, kanker serviks, metformin

Abstract

Background: Diabetes mellitus (DM) is a serious chronic disease with the tendency to deteriorate. The DM cases in worldwide in 2015 was 415 million people and it is estimated to increase as many as 642 million people affected in 2040. Indonesia ranked 7 in the world with 10 million cases of DM in 2015, about 90% of which were Type 2 Diabetes Mellitus. This study aimed to determine the effect of biopsychosocial factors and calorie intake on the blood glucose control in patients with Type 2 DM.Subjects and Method: This was an analytic and observational study with case control design. This study was carried out at Internal Medicine Polyclinic, Department of Endocrinology, Dr. Moewardi Hospital, Surakarta, Central Java, from October to November 2016. A total sample of 135 cases of Type 2 DM were selected for this study by fixed disease sampling. As many as 106 of these study subjects at HbA1c ?6.5% and 29 cases of Type 2 DM had HbA1c <6.5%. The dependent variable was HbA1c level. The independent variables were educational level, family income, psychological stress, calorie intake, and Body Mass Index (BMI). The data were collected by a set of questionnaire. HbA1c was measured by High Performance Liquid Chromatography (HPLC), which was recorded in the medical record. Calorie intake was measured by 24 hour food recall. Psychological stress was measured by International Physical Activity Questionnaire (IPAQ). The data was analyzed by path analysis on STATA 13.Results: Psychological stress (b=0.99, 95%CI=0.07 to 1.92, p=0.034), calorie intake (b= 1.84, 95%CI= -0.24 to 3.92, p=0.083), and BMI (b= 1.15, 95%CI= 0.22 to 2.08, p=0.016), had positive and statistically significant effect on HbA1c. Calorie intake increased BMI (b= 2.35, 95%CI=0.31 to 4.39, p=0.024), education decreased calorie intake (b=-2.26, 95%CI= -3.38 to -1.14, p<0.001), and family income increased calorie intake (b= 1.23, 95%CI= 0.26 to 2.21, p= 0.013). Conclusion: Calorie intake, BMI, psychologi stress, and s family income are associated with increase in HbA1c level. Education decreases HbA1c level via decreased calorie intake. Type 2 DM patients need to pay attention to these biopsychosocial factors and calorie intake in order to control blood sugar.Keywords: Biopsychosocial factors, calorie intake, HbA1c.

Abstract

ABSTRAKStatus gizi lebih dan anemia merupakan gangguan gizi yang sering muncul pada remaja putri. Remaja putri berisiko tinggi terjadi gangguan gizi tersebut karena cenderung mengalami peningkatan kebutuhan zat gizi, pembatasan pola konsumsi dan penyimpangan pola konsumsi. Asupan protein yang tinggi akan diubah menjadi lemak dan disimpan menjadi jaringan adiposa. Pada status gizi lebih, inflamasi kronik terjadi di jaringan lemak dan terjadi peningkatan sekresi sitokin proinflamasi, seperti interleukin 6 (IL-6). Kadar IL-6 yang tinggi meningkatkan kadar hepsidin, yang dapat menurunkan kadar besi (Fe) dalam sirkulasi. Penelitian ini  bertujuanmenganalisis hubungan antara asupan protein dan kadar IL-6 dengan kadar hemoglobin (Hb) pada remaja putri status gizi lebih.  Data asupan protein menggunakan metode food recall 24 jam dan SQ-FFQ (semi quantitativefood frequency questionare). Kadar IL-6 diukur dengan pemeriksaan ELISA. Kadar Hb diukur dengan metode cyanmethaemoglobin. Hasil penelitian ini menunjukkan bahwa asupan protein dan kadar IL-6 tidak berhubungan dengan kadar Hb (p=0,132) dan (p= 0,607). Penelitian selanjutnya diharapkan untuk menganalisis hubungan status gizi lebih pada remaja putri dengan kejadian anemia, seperti kadar hepsidin, globin dan feritin. Kata kunci: Asupan Protein, Kadar IL-6, Kadar Hb, Remaja Putri, Status Gizi Lebih ABSTRACTOvernutritional status and anemia are nutritional disorders that often occur in female adolescents. Female  adolescents are at high risk of nutritional disorders because they tend to experience an enhanchement nutritional needs, restrictions on consumption patterns and eating disorder. High protein intake will be converted into fat and stored into adipose tissue. In overnutrition adolescents, chronic inflammation occurs in fat tissue and also increasing proinflamation sitokin secretion, such as interleukin 6 (IL-6). High levels of IL-6 increases hepsidin levels, and decreases iron (Fe) level in circulation. This study aims to analyze the corelation of protein intake and IL-6 level with haemoglobin level in overnutrition female adolescents. Data of protein intake is taken through 24 hours food recall and SQ-FFQ (semi quantitative-food frequency questionare). IL-6 levels were measured by ELISA while Hb level is obtained thorugh cyanmethaemoglobin method. The results of this study there is no corelation between protein intake and IL-6 level toward Hb level (p=0,132) and (p= 0,607). Abstract

The elderly usually accompanied by many impairments due to decline in organ functioning which have a great impact on their daily intake and leads to anemia. Many elderlies also have mobility impairment which make them become more dependent and need supports from their family. The aim of this study is to investigate the relationship of family support, the daily intake of protein, iron, vitamin C with hemoglobin levels in elderly. This observational analytic study with cross-sectional design was performed in 185 elderlies aged 60-79 years determined using the OpenEpi program. Subjects collected by simple random sampling from chosen Posyandu in Klaten and Sragen districts. The data on daily intake and family support in fulfilling healthy nutritious foods were collected by using a 2x24-hour food recall form and a modified questionnaire from Hanim’s study. Hemoglobin levels were measured from venous blood using Cyanmethhemoglobin method. The data were analyzed using the Pearson test and Multiple Linear Regression test. The prevalence of mild anemia was found in 10.27% women and 5.41% men. Family support (b=0.09, 95%CI:0.003- 0.18), protein intake (b=0.41, 95%CI:0.25-0.57), iron intake (b=0.19, 95%CI:0.96-0.29), vitamin C intake (b=0.10, 95%CI:0.02-0.41) were significantly associated with hemoglobin levels (p<0.05). To conclude, sufficient family support, adequate intake of protein, iron and vitamin C are associated to higher hemoglobin levels.

Abstract

Nutritional problem that occurs in adolescents due to higher food intake and less physical activity leads to over- nutrition. Other nutritional problems occur among them is micronutrient deficiency including anemia, mostly in female adolescents. Higher consumption of energy-dense snacks, low physical activity and more pocket money  have contributed to adolescent’s body weight. This study aims to determine the correlation of frequency of snacking, hemoglobin levels, physical activity, and pocket money with the nutritional status of female adolescents. This cross- sectional study was carried out in 117 female adolescents in Karanganyar Regency, Central Java Province. Data of snacking frequency was collected using the Food Frequency Questionnaire (FFQ) and hemoglobin level were measured using a Hematology Analyzer. Physical activity data were collected using the IPAQ questionnaire (International Physical Activity Questionnaire) and were converted to Metabolic Equivalent Tasks (METs). Normally distributed data then analyzed using Pearson test and Spearman test for not normally distributed data to determine correlation between independent with dependent variables and followed by the multiple linear regression test if the p-value < 0.25. Most female adolescents had normal nutritional status, but 6.8% and 17.9% female adolescents were overweight and obesity, respectively. The frequency of snacking (r = 0.207, p = 0.025), physical activity (r = -0.201, p = 0.030), pocket money (r = 0.283, p = 0.002) and hemoglobin level (r = 0.150, p = 0.107) were not correlated with nutritional status. In conclusion, frequency of snacks, physical activity, and pocket money were significantly correlated with nutritional status in female adolescents but no correlation with hemoglobin level.

Abstract

Diabetes mellitus is a non-communicable disease that has increased significantly in Indonesia and worldwide. Dietary arrangements through diet therapy are beneficial for improving the performance of the pancreas in producing and stimulating the work of insulin. This study aimed to determine the effect of the order of vegetable and carbohydrate consumptions on the decrease of energy intake. Method used randomized control trial with pre-post design.  A total of 24 people with non-complicating type-2 diabetes mellitus aged 18-65 years were assigned into 3 groups ; control group (C), vegetable before carbohydrate group (P1), and vegetable with carbohydrate group (P2) .  Energy intake before and after the intervention was measured using a 24-hour food recall. To compare pre-post intervention, paired T-test were used, meanwhile to compare difference among groups data were analysed using ANOVA.  A significant decrease in energy intake was found in P1 where the mean values of C and P1 were 17.75 ± 12.40 calories and 155.27 ± 23.40 calories (p < 0.001), and mean values of P1 and P2 were 155.27 ± 23.40 calories and 4.54 ± 48.99 calories (p < 0.001). The order of vegetable before carbohydrate consumption decreases energy intake of type-2 diabetes mellitus patients.

Abstract

Latar belakang: Anemia defisensi besi (ADB) merupakan salah satu masalah nutrisi pada remaja putri di seluruh dunia. Penyerapan zat besi di usus halus dipengaruhi oleh adanya vitamin C, kalsium, dan zink pada makanan. Penelitian ini bertujuan untuk mengetahui hubungan pola konsumsi vitamin A, vitamin C, kalsium dan zink terhadap kejadian ADB pada remaja putri. Metode: Penelitian ini merupakan penelitian case control yang melibatkan 60 remaja putri dengan ADB dan 58 remaja putri tanpa ADB. Diagnosis ADB berdasarkan kadar Hb dan indeks eritrosit, dan semikuantitatif FFQ digunakan untuk menentukan asupan vitamin dan mineral. Semua data dianalisis menggunakan test chi square dan tes regresi logistik ganda dengan p<0.05. Hasil: Semua subjek penelitian mempunyai asupan vitamin A dan C harian yang cukup tetapi asupan kalsium dan zinknya tergolong kurang (dalam mg). Namun, mereka semua memiliki frekuensi harian yang berbeda dalam mengkonsumsi mikronutrien tersebut. Remaja putri dengan asupan vitamin A yang jarang (OR=2.67; CI95%=1.10-6.50; p=0.03) dan asupan kalsium yang sering (OR=2.27; CI95%=0.85-6.03; p=0.10) lebih berisiko terkena ADB dibandingkan dengan remaja putri dengan asupan vitamin A yang sering dan asupan kalsium yang jarang. Akan tetapi hanya asupan vitamin A yang memiliki efek signifikan secara statistik. Kesimpulan: Hasil penelitian ini menunjukkan bahwa tingginya risiko ADB pada remaja putri berkaitan dengan asupan vitamin A yang jarang. Kata kunci: anemia defisiensi besi, asupan mikronutrien, remaja putri Abstract Background: Iron deficiency anemia (IDA) is a nutritional problem that occurs in female adolescents around the world. Iron absorption in the small intestine is influenced by the presence of vitamin C, calcium, and zinc in ingested foods. This study aimed to investigate the relationship of vitamin A, vitamin C, calcium, and zinc intake with IDA in female adolescents. Methods: This case-control study was conducted in 60 anemic and 58 normal female adolescents. IDA diagnosis was determined using Hb levels and erythrocyte indexes and the semiquantitative food frequency questionnaire was used to determine vitamin and mineral intake. All collected data were analyzed using chisquare and multiple logistic regression tests with p<0.05. Results: All groups had an adequate intake of vitamin A and C but they had inadequate intake of calcium and zinc (in mg). However, they all had different frequencies in consuming those micronutrients. Rare intake of vitamin A (OR=2.67; CI95%=1.10-6.50; p=0.03) and frequent intake of calcium (OR=2.27; CI95%=0.85- 6.03; p=0.10) increased IDA, compared with frequent intake of vitamin A and rare intake of calcium but only vitamin A intake had a significant effect. Conclusion: Our findings suggest that a higher risk of IDA in female adolescents is related to a rare intake of vitamin A. Keywords: Iron deficiency anemia; micronutrient intake; female adolescents
