Abstract

Breastfeeding behavior to infants is influenced by knowledge, attitude, and practice. The PuskesmasSidoharjo has an exclusive coverage of low ASI (38.3%) when compared to the target of the strategic plan (80%). The objective of the study was to describe breastfeeding behavior in low socioeconomic mother in Sidoharjo sub-district, Sragen. The research was qualitative research with descriptive method. Key informants are 4 mothers who have 7 month old babies each from low socioeconomic group, working and not working and give exclusive and non exclusive breastfeeding. Triangulation informants are family members who live with mothers and village midwives. The results showed that women with low socioeconomic status, working or not working, still provide exclusive breastfeeding because they are aware of health status, smooth milk flow, and economical reasons. Mother who are working or does not working but does not give exclusive breastfeeding because it has a nipple problem that is not prominent. It is concluded that good knowledge and attitudes about exclusive breastfeeding are not always applied in exclusive breastfeeding practices, because there are problems of organic abnormalities and practical reasons for working mothers.

Abstract

Background: The brain releases biochemical substrates, such as S100β protein, into circulation in response to ischemic conditions as a sign of damage in nerve cells and disruption of the blood-brain barrier’s integrity. Thrombolytic therapy has led to the development of many neuroprotective therapies such as protein, phosphatidylcholine, phosphatidylserine, and inulin, which can be added to food products. Protein, phospholipids, and inulin, have a neuroprotective impact on nerve cells in the brain and blood-brain barrier.Objective: To prove the effect of protein, phosphatidylcholine, phosphatidylserine, and inulin on S100β levels and clinical outcomes in patients with acute ischemic stroke.Materials and Methods: This study was done in a single-blind RCT. Eighteen ischemic stroke patients were randomly divided into nine subjects for the intervention group and nine subjects for the control group. The Control group received 250 ml conventional formula milk (11.8 g protein) 3 times/day. The intervention group received 250 mL commercial milk 3 rimes/day which contained 15 g protein with 128 mg phosphatidylcholine, 32 mg phosphatidylserine, and 3 g inulin. All of the groups were given hospital-standard therapy for ischemic stroke. S100β levels were measured at pre and post-intervention.Results: Pre and post S100β levels in intervention and the control group did not show any statistically difference (p = 0.777 and p = 0.096), but there was a trend of decreasing  levels of S100β in the intervention group (-24.6 + 252.0 pg/mL) versus control group (135.8 + 216.2 pg/mL).Conclusions: The addition of protein, phosphatidylcholine, phosphatidylserine, and inulin did not have a significant effect on S100β levels.

Abstract

ABSTRACT Health staff job satisfaction has an impact on performance. The Puskesmas has periodically measured customer satisfaction but staff job satisfaction has never been done. From the preliminary study, it was found that the job satisfaction of puskesmas staff in Kotawaringin Barat Regency was still low. To find out the difference in job satisfaction of inpatient puskesmas staff with outpatient health centers. Cross sectional research design with a quantitative approach. The study population was inpatient and outpatient puskesmas staff in Kotawaringin Barat Regency. Subject recruitment was done through the puskesmas. From 14 outpatient health centers, 3 puskesmas were selected and from 4 inpatient puskesmas 2 were selected. A total of 40 subjects filled out a questionnaire that was distributed electronically. Data analysis was carried out using an independent t test to distinguish job satisfaction. High job satisfaction at outpatient health centers is on the factor of supervision, and policies respectively by 62%, motivation 57%, while low satisfaction in the physical work environment 19%, on factors of safety and work safety, incentives, capitation services each 33%, and work culture 38%. High job satisfaction at inpatient health centers in the form of work culture factors, and capitation services each by 68%, work results 63%, and 58% each on co-workers factors, work security and safety, opportunities to improve skills and skills, while low satisfaction in the physical work environment 16%, incentives 16%, workload 37%, supervision 42%, and policy 42%. There was no significant difference between job satisfaction at outpatient puskesmas and inpatient puskesmas (p=0.317). The job satisfaction of outpatient clinics is on the factors of supervision, the policy of the Head of the Puskesmas, and motivation. At inpatient health centers in the form of work culture, capitation services, work results, colleagues, work security and safety, opportunities to improve skills and expertise. Keywords: Job Satisfaction, Puskesmas, Inpatient, Outpatient, Health Workers  ABSTRAK Kepuasan kerja staf kesehatan berdampak pada kinerja. Puskesmas secara periodik sudah melakukan pengukuran  kepuasan pelanggan tetapi kepuasan kerja staf belum pernah dilakukan. Dari studi pendahuluan didapatkan bahwa kepuasan kerja staf puskesmas di Kabupaten Kotawaringin Barat masih rendah. Tujuan untuk mengetahui perbedaan kepuasan kerja staf puskesmas rawat inap dengan puskesmas rawat jalan. Desain penelitian cross sectional dengan pendekatan kuantitatif. Populasi penelitian staf puskesmas rawat inap dan rawat jalan di Kabupaten Kotawaringin Barat. Rekrutmen subjek dilakukan melalui puskesmas. Dari 14 puskesmas rawat jalan dipilih 3 puskesmas dan dari 4 puskesmas rawat inap dipilih 2 puskesmas. Sebanyak 40 subjek mengisi angket yang disebarkan secara elektronik. Analisa data dilakukan dengan uji independent t test untuk membedakan kepuasan kerja. Kepuasan kerja yang tinggi pada puskesmas rawat jalan adalah pada faktor pengawasan, dan kebijakan masing-masing sebesar 62%, motivasi 57%, sedangkan kepuasan yang rendah pada lingkungan kerja fisik 19%, pada faktor keamanan dan keselamatan kerja, insentif, jasa kapitasi masing-masing 33%, dan budaya kerja 38%. Kepuasan kerja yang tinggi pada puskesmas rawat inap berupa faktor budaya kerja, dan jasa kapitasi masing-masing sebesar 68%, hasil pekerjaan 63%, dan masing-masing 58% pada faktor rekan kerja, keamanan dan keselamatan kerja, kesempatan meningkatkan keahlian dan keterampilan, sedangkan kepuasan yang rendah pada lingkungan kerja fisik 16%, insentif 16%, beban kerja 37%, pengawasan 42%, dan kebijakan 42%. Tidak ada perbedaan yang signifikan antara kepuasan kerja di puskesmas rawat jalan dengan puskesmas rawat inap (p=0,317). Kepuasan kerja puskesmas rawat jalan ada pada faktor pengawasan, kebijakan Kepala Puskesmas, dan  motivasi. Pada puskesmas rawat inap berupa budaya kerja, jasa kapitasi, hasil pekerjaan, rekan kerja, keamanan dan keselamatan kerja, kesempatan meningkatkan keahlian dan keterampilan Kata Kunci: Kepuasan Kerja, Puskesmas, Rawat Inap, Rawat Jalan, Tenaga Kesehatan

Abstract

Cakupan pelayanan kesehatan lansia di Kabupaten Seluma tahun 2020 sebesar 26,7% menurun  dibandingkan sebelum pandemi, data tahun 2019 (47,31%). Penelitian bertujuan mendeskripsikan implementasi program posyandu lansia masa pandemi Covid-19. Jenis penelitian deskriptif dengan pendekatan kualitatif. Subjek penelitian terdiri dari 12 informan yang diplih secara purposive. Pengumpulan data melalui wawancara mendalam dan observasi dengan triangulasi sumber untuk menguji keabsahan data. Analisis data dengan metode analisis isi. Hasil penelitian menunjukkan implementasi program posyandu lansia belum optimal, terdapat ketidakpatuhan terhadap protokol kesehatan, partisipasi stakeholder minim, belum terdapat pengembangan model KIE, pelayanan posyandu lansia belum lengkap, jangkauan sasaran homecare belum maksimal karena tidak didukung upaya pemantauan kader.

Abstract

Pada tahun 2018, keberhasilan ASI eksklusif di Indonesia sebesar 61,33%, DIY sebesar 74,9% pada 2017 dan Bantul sebesar 74,27% pada tahun 2017. Tujuan penelitian adalah untuk menganalisis faktor risiko kegagalan pemberian ASI eksklusif. Jenis penelitian observasi analitik, dengan desain case kontrol. Analisis data menggunakan regresi logistic. Hasilnya menunjukkan variable yang bukan merupakan faktor risiko kegagalan ASI eksklusif adalah pelaksanaan IMD, pengetahuan dan dukungan keluarga. Faktor risiko kegagalan ASI eksklusif adalah keikutsertaan Kelas Ibu Hamil, keikutsertaan KP ibu dan dukungan suami.

Abstract

Nurse job satisfaction is a considerable concern among health practitioners due to the diverse results of the study. From a preliminary study, it was found that the job satisfaction of public health centers (PHC) staffs in West Kotawaringin Regency were still low. The purpose of the study was to analyze demographic factors that affected the PHC of nurse job satisfaction. This research was conducted using a cross-sectional design. The research population of nurses in health centers was affordable to a stable internet network. Subjects’ recruitment was carried out in 10 from 18 existing PHC in West Kotawaringin Regency. A total of 95 subjects who met the inclusion criteria filled out the questionnaires which was distributed electronically. Data analysis was performed by multiple logistic regression. PHC job satisfaction was 41,1%. Higher job satisfaction was found in male nurses, undergraduate education levels/Ners, service periods of 11 to 20 years, nurses with civil service status, and worked in inpatient PHCs. There was no significant association between gender (0,161), education (0,757), length of work (0,761), employment status (0,104) with job satisfaction. There was a significant relationship between the type of health center (0,019) and job satisfaction. Factors related to job satisfaction together are employment status and PHC type.

Abstract

The hospital is a service organization for patients (consumers). Service is expected to provide comprehensive consumer desires. Previous research has not much elaborated on the balanced scorecard and analysis of the constituent factors of customer and employee satisfaction. This study aims to analyze the balanced scorecard, the constituent factors of customer and employee satisfaction in dr. Kariadi Hospital. The number samples used as many as 72 consumer respondents who use hospital services and 27 hospital employees. The research method uses a balanced scorecard and analysis of the constituent factors of customer and employee satisfaction. The analysis results show that the human resource factor still requires improvement compared to the customer factor. This study confirms that efforts to provide optimal service have been made to satisfy customers. But on the other hand, efforts to provide satisfaction to employees are still lacking; therefore, efforts are needed to provide satisfaction.

Abstract

Surveillance and Response Maternal Perinatal Audit (AMP-SR) is a new guideline that is used as a reference for District/City, Provincial and National Health Offices in improving the quality of Maternal Perinatal Audit (AMP) in Indonesia, but the guideline has not worked well so it has an impact in the increase in cases of maternal mortality in Indonesia over the last three years from 87.93/100,000 Live Births (LB) in 2019 to 166.5/100,000 Live Births in 2021. Objective is to analyze evidence of AMP-SR implementation in districts/cities in Indonesia through scoping review. Articles were selected from six databases: Pubmed, Clinicalkey, Scopus, Springerlink, Science Direct, and Garuda with the inclusion criteria of original articles, full text, 2012-2022, Indonesian and English, research in Indonesia, qualitative, and relevant to research purposes. The keywords used are maternal death, policy, implementation, audit, Indonesia. Article screening was carried out with PRISMA. The SPIDER method was used to construct research questions.Of the 499 articles found, only seven articles met the inclusion criteria with a high eligibility value. Seven articles discuss the implementation of AMP-SR in Regencies/Cities in Indonesia with optimal Identification 57.14%, not optimal 42.86%. Reporting is optimal 28.57%, not optimal. The optimal review is 14.29%, not yet optimal 85.71%. The optimal response is 28.57%, not yet optimal 71.43%. Most of the implementation of AMP-SR in districts/cities in Indonesia is not optimal and is still constrained, so it is necessary to strengthen the role and commitment of health workers in implementing AMP-SR so that it is better and more comprehensive. Keywords:  Audit,  Implementation, Maternal Death, Policy

Abstract

ABSTRACT Indonesia is one of the countries with the highest burden of TB in the world. The DOTS strategy has been applied to control TB disease, but the implementation of the DOTS strategy has not been optimal due to the obstacles that have arisen. To analyze the implementation of DOTS strategy as tuberculosis control. This research is a systematic literature review using indexed articles on Google Scholar, Science Direct, Pub Med, and Garuda. Articles were selected using PRISMA (Preferred Reporting Items for Systematic Review and Meta-Analyses). Analysis of article quality using the Critical Appraisal Skills Program (CASP). Eight articles were obtained that were in accordance with the research, indicating that the implementation of the DOTS strategy as tuberculosis control on political commitment in terms of funding, case detection, drug taking supervisor (PMO), as well as recording and reporting has not run optimally, while the drug distribution has been going well. Implementation of the DOTS strategy can run optimally by increasing cross-sectoral collaboration in tuberculosis control. Keywords: Directly Observed Treatment Short-Course Strategy, Qualitative, Tuberculosis Control.  ABSTRAK Indonesia menjadi salah satu negara yang memiliki beban tertinggi TB di Dunia. Telah diterapkan strategi DOTS untuk penanggulangan penyakit TB, akan tetapi implementasi strategi DOTS belum optimal disebabkan oleh adanya hambatan yang muncul. Untuk menganalisis implementasi strategi DOTS sebagai pengendalian tuberkulosis. Penelitian ini merupakan kajian pustaka sistematik dengan menggunakan artikel terindeks pada Google Scholar, Science Direct, Pub Med, dan Garuda. Artikel yang dipilih menggunakan PRISMA (Preferred Reporting Items for Systematic Review and Meta-Analyses). Analisis kualitas artikel menggunakan Critical Appraisal Skills Program (CASP). Diperoleh delapan artikel yang sesuai dengan penelitian, menunjukkan bahwa implementasi strategi DOTS sebagai pengendalian tuberkulosis pada komitmen politik dalam hal pendanaan, deteksi kasus, pengawas minum obat (PMO), serta pencatatan dan pelaporan belum berjalan optimal, sedangkan pada pendistribusian obat sudah berjalan baik. Implementasi strategi DOTS dapat berjalan optimal dengan meningkatkan kerjasama di lintas sektor dalam pengendalian tuberkulosis. Kata Kunci: Directly Observed Treatment Short-Course Strategy, Kualitatif, Pengendalian Tuberkulosis.

Abstract

Obesity is excessive accumulation of fat due to an imbalance between energy intake and energy expenditure. This study aims as a community diagnosis to determine the respondent's body mass index and to determine the risk factors that have a reciprocal effect on the nutritional status of the community. This study uses a cross sectional method with a descriptive approach and using a backward logistic regression method: Wald. Data collection is carried out to the population was 55 women who met the inclusion criteria aged 21-65 years and were willing to become respondents. The results of this study, obtained information that the age of the respondent has a significant influence on the obesity status of the community with a significant value of 0.043 with an opportunity of 1.072 times. Likewise, the respondent's work has a significant value of 0.015, this provides information that the more the respondent do the physical activity, can protect the body from obesity status by 1.940 times or almost 2 times for the better. Obesity has a chance of 0.065 times affecting the incidence of hypertension with p-value 0,011. Not only that, a person's stomach circumference is closely related to the incidence of obesity with a p-value of 0.019 and an odds ratio value of 5.019. Eating packaged food such as instant noodles which is often done by respondents has a significant effect on obesity status of 0.059. Instant food will have a negative effect of 3 times of obesity.
