Abstract

ABSTRACT The minimum service standard is a benchmark for the type and quality of services provided to the community at a minimum. The coverage of TB suspects receiving services in Pesawaran District has not reached the national target (100%). To analyze the implementation of minimum service standard policies in the health sector for people suspected of having TB. This research is a qualitative research with a case study approach. The research locations were carried out at the Pesawaran District Health Office, Kedondong Health Center, Kota Dalam Health Center, Kalirejo Health Center, and Roworejo Health Center. Primary data was obtained from in-depth interviews and observations, secondary data was obtained from document review. There were 11 informants in this study consisting of 3 main informants and 9 triangulation informants. The implementation of the policy on minimum service standards in the health sector for people suspected of having TB has not run optimally. There are variables that are not maximized so that they hinder policy implementation, namely resources, communication between organizations and implementing activities, characteristics of implementing agencies, dispositions of implementing agencies, and environmental, social and economic conditions. Implementation of minimum service standard policies in the health sector for people suspected of having TB can be carried out optimally by increasing cooperation and coordination in both government and private circles. Keywords: Qualitative, Minimum Service Standards, Tuberculosis  ABSTRAK Standar pelayanan minimal merupakan tolak ukur jenis dan mutu pelayanan yang diberikan kepada masyarakat secara minimal. Cakupan terduga TB yang mendapatkan pelayanan Kabupaten Pesawaran belum mencapai target nasional (100%). Untuk menganalisis implementasi kebijakan standar pelayanan minimal bidang kesehatan (SPM-BK) orang terduga TB. Penelitian ini merupakan penelitian kualitatif dengan pendekatan studi kasus (case study). Lokasi penelitian dilakukan di Dinas Kesehatan Kabupaten Pesawaran, Puskesmas Kedondong, Puskesmas Kota Dalam, Puskesmas Kalirejo, dan Puskesmas Roworejo. Data primer diperoleh dari hasil wawancara mendalam dan observasi, data sekunder diperoleh dari telaah dokumen. Informan penelitian ini berjumlah 11 orang yang terdiri dari 3 informan utama dan 9 informan triangulasi. Implementasi kebijakan SPM-BK orang terduga TB belum berjalan maksimal. Terdapat variabel yang belum maksimal sehingga menghambat implementasi kebijakan, yaitu sumber daya, komunikasi antar organisasi dan kegiatan pelaksana, karakteristik badan pelaksana, disposisi pelaksana, serta kondisi lingkungan, sosial, dan ekonomi.  Implementasi kebijakan SPM-BK orang terduga TB dapat dilakukan secara optimal dengan meningkatkan kerjasama dan koordinasi baik dilingkungan pemerintah maupun swasta. Kata Kunci: Kualitatif, Standar Pelayanan Minimal, Tuberkulosis.

Abstract

Stunting is a picture of stunted growth caused by long-term malnutrition. The prevalence of stunting in Palangkaraya City is 23.59%, and the highest percentage of stunting in Palangkaraya City is in the Pahandut Palangkaraya Health Center (32.37%). One of the factors causing stunting is environmental sanitation facilities that do not meet health requirements. This study aims to determine the risk factors for stunting in environmental sanitation facilities in the working area of the Pahandut Palangkaraya Health Center. The type of research used is an analytical survey with a case-control design. The study was conducted in January-February 2023. The under-five population is 1,499 children. The study subjects were children aged 6-24 months, consisting of 38 stunted and 38 non-stunted children. Child body length data were measured using an infantometer and calculated using anthropometric standard deviation. Data on environmental health facilities were obtained from interviews and observations using questionnaires. The Odd Ratio test analyzed the data. Results of analysis of drinking water sources (OR=2.9, 95%CI=1.086-7.744), clown feces handling (OR=3.03, 95%CI=1.174-7.831), sewerage (OR=3.37, 95CI%=1.17 -7.831), toilet facilities (OR=3.2, 95%CI=1.23-9.23). Drinking water that does not meet health requirements is a risk factor, respondents do not have septic tanks, or sewerage is a risk factor for stunting, private wastewater disposal facilities and toilets that do not meet the requirements, and poor handling of clown faeces is a risk factor for stunting in the work area of the Pahandut Palangkaraya Health Center. Advice for public health centers is expected to monitor newborn children up to the first 1000 days of life to prevent malnutrition that causes stunting and aspects of stunting prevention in children.Keywords: Environmental, facilities, sanitation, stunting

Abstract

Anxiety in the third-trimester pregnancy can affect the physical and psychological aspects of the mother and fetus. Until now, there has been no study comparing yoga sequences that are effective in reducing anxiety levels in third-trimester primigravida. The yoga sequence will cause the heart rate to return to normal after finishing exercising, and the body will feel more comfortable. This study aims to determine the effect of antepartum yoga sequences on anxiety levels, vital sign values, and fetal heart rate in third-trimester primigravida. This study was quasi experiments with a pretest posttest control group design conducted in January-April 2022. The sampling technique used cluster sampling to determine the intervention group and control group. The intervention group was given antepartum yoga sequences for six sessions with a duration of 60 minutes, consisting of 18 respondents in A group model and 18 respondents in B group model. The control group consisting of 18 respondents, received pregnancy exercise for six sessions with a duration of 60 minutes. Data analysis using T-test. The results showed differences in the control group for anxiety (p-value = 0.042), pulse (p-value = 0.021) and respiratory frequency (0.032). In the intervention group model A, there were differences in anxiety, diastolic blood pressure, pulse, and respiratory rate with p-values ​​of 0.032, 0.005, 0.005, and 0.005, respectively. While in the intervention group model B, there were significant differences in anxiety, systolic blood pressure, diastolic blood pressure, pulse, respiratory rate, and fetal heart rate, namely p-values ​​of 0.005, 0.002, 0004, 0.005, 0.005, 0.005. The results of this study can be used by midwifery service providers as one of the complementary treatments for reducing anxiety experienced by third-trimester primigravida mothers, namely by giving antepartum yoga, which is done for 60 minutes.

Abstract

Hospital-based health technology assessment (HB-HTA) is very beneficial for the use of hospital medical technology, cost management, quality improvement, and protection of patient safety, and its usage is rising daily around the world. The main challenge of modern hospitals is purchasing medical technologies. Hospital-based health technology assessments (HB-HTAs) are used in healthcare facilities around the world to support management boards in providing relevant technologies for patients. Although some jurisdictions have used local/hospital-based HTA for more than two decades, little is known about its impacts and influence on hospital budgets, clinical practices, and patient outcomes. We performed a review using a mixed-methods approach with the goal of synthesizing existing information about the impacts and impact of community/hospital-based care. This study uses a narrative review, with many articles on the same topic. Articles were taken from the Scopus, Science Direct, ProQuest, SpringerLink, Google Scholar, Nature, JSTOR, and Emerald Insight databases with a total of 19 articles used. The purpose of this study was to determine does the implementation of HB-HTA in hospitals have a good impact on the progress of the hospital, and also discuss what are the benefits that will be obtained if implementing HB-HTA in hospitals.  Abstrak: Penilaian teknologi kesehatan berbasis rumah sakit (HB-HTA) sangat bermanfaat untuk penggunaan teknologi medis rumah sakit, manajemen biaya, peningkatan kualitas, dan perlindungan keselamatan pasien, dan penggunaannya meningkat setiap hari di seluruh dunia. Tantangan utama rumah sakit modern adalah membeli teknologi medis. Penilaian teknologi kesehatan berbasis rumah sakit (HB-HTA) digunakan di fasilitas kesehatan di seluruh dunia untuk mendukung dewan manajemen dalam menyediakan teknologi yang relevan bagi pasien. Meskipun beberapa yurisdiksi telah menggunakan HTA berbasis lokal/rumah sakit selama lebih dari dua dekade, sedikit yang diketahui tentang dampak dan pengaruhnya terhadap anggaran rumah sakit, praktik klinis, dan hasil pasien. Kami melakukan tinjauan menggunakan pendekatan metode campuran dengan tujuan mensintesis informasi yang ada tentang dampak dan dampak perawatan berbasis komunitas/rumah sakit. Penelitian ini menggunakan review naratif, dengan banyak artikel dengan topik yang sama. Artikel diambil dari database Scopus, Science Direct, ProQuest, SpringerLink, Google Scholar, Nature, JSTOR, dan Emerald Insight dengan total 19 artikel yang digunakan. Tujuan dari penelitian ini adalah untuk mengetahui apakah penerapan HB-HTA di rumah sakit berdampak baik bagi kemajuan rumah sakit, dan juga membahas apa saja manfaat yang akan diperoleh jika penerapan HB-HTA di rumah sakit.

Abstract

Introduction: Inadequate intake of energy and macronutrients causes a decrease in an individual's ability to carry out his work activities and makes him feel tired or tired more easily. The purpose of this study was to analyze the relationship between energy adequacy level (EAL) and protein adequacy level (PAL) as well as the percentage of carbohydrate intake and the percentage of fat intake with the fatigue level of female workers.Methods: This research is an analytic observational study with a cross-sectional design. The population in this study were 67 female workers at the Bandarharjo Fish Smoking Center, Semarang City. Research subjects were selected by purposive sampling of 40 subjects, based on inclusion criteria. EAL, PAL data were collected, as well as the percentage of carbohydrate and fat intake obtained through interviews using a 2x24 hour food recall form. Data analysis was performed by Pearson Product Moment correlation test on normally distributed data and Ranks Spearman correlation test on non-normally distributed data.Results: The results showed that the average EAL was 86,6 (±26,3)% energy requirement/day, PAL was 64,5 (±23,5)% protein requirement/day, the percentage of carbohydrate intake was 51,5 (±7,6)% energy intake/day, and the percentage of fat intake was 34,7 (±8,6)% energy intake/day, and the level of work fatigue with an average reaction time of 429,7 (±121,9) milliseconds. There was no relationship between EAL (r=0,127 and p=0,435), PAL (r=0,159 and p=0,327), percentage of carbohydrate intake (r=0,036 and p=0,825), and percentage of fat intake (r=-0,088 and p = 0,590) with the level of work fatigue.Conclusion: It can be concluded that there is no relationship between EAL, PAL, and the percentage of carbohydrate and fat intake with the level of work fatigue in female workers in the Bandarharjo Fish Smoking Center, Semarang City.

Abstract

AbstractIntroduction: Though global trends for maternal and newborn mortality have improved, the COVID-19 pandemic had a negative impact on maternal and newborn health (MNH), increasing risk of complicated births. Indonesia’s Ministry of Health (IMOH) has made significant progress in expanding MNH services; however, challenges remain in improving screening for emergent threats (ET) to MNH, which includes emerging infectious diseases. This review aimed to compare antenatal care (ANC) standards in Indonesia to WHO guidelines and to evaluate the role of laboratory services and ultrasound (USG) for ANC.Methods: A narrative literature review was conducted through PubMed and Google Scholar. Articles were also found from the IMOH and the World Health Organization (WHO). Search terms included antenatal care, laboratory, ultrasound, and doppler, yielding sixteen articles in total. Nine articles published between 2010 and 2023 were included for review.Results: Compared to WHO standards of 8 ANC visits and 1 USG, Indonesia has a lower number of required ANC visits (6), but a higher requirement for USG visits (2); though, conflicting evidence exists for USG recommendations during pregnancy. One article evaluated standard laboratory tests administered during the first ANC visit, finding no records of testing at the puskesmas (primary healthcare center) level, while another cited improved infectious disease (ID) detection using screening questionnaires.Conclusion: In comparison to global standards, Indonesia may benefit from an increased number of ANC visits, ID screening in non-endemic areas, and studies on adherence to national screening guidelines. Further research is needed to evaluate the optimal timing of USG during the prenatal period.

Abstract

Stunting remains a significant public health concern, impacting physical growth and impeding children's development. Mothers, who play a crucial role in stimulating children's development, often encounter barriers in providing effective stimulation, primarily due to limited access to appropriate resources and information. Online education can bridge this gap by offering easily accessible learning. This aim of this study was to determine the effect of online education on maternal knowledge and attitudes in providing developmental stimulation for stunting toddlers. A quasi-experimental research design was employed, comprising a treatment group and a control group, each consisting of 46 mothers with stunting toddlers, sampled purposively. The treatment group received an online developmental stimulation educational intervention through WhatsApp groups for twelve meetings, each lasting 1.5 to 2 hours. Meanwhile, the control group participated in a stunting assistance program provided by the community health centers (Puskesmas). To compare the knowledge and attitudes between groups, Mann-Whitney and independent Student t-test were used. The assessment of intervention effects on knowledge and attitudes was conducted using Wilcoxon and paired Student t-test within each group. Following the educational intervention on developmental stimulation, there was a significant increase in knowledge; however, there was no significant difference in attitudes. The treatment group (mean score 3.9±1.76) had a higher increase in knowledge scores compared to the control group (2.0±2.25) with a p<0.001. Nonetheless, no significant difference in attitudes was observed between the two groups with the mean change scores was 5.8±15.31 in treatment group and 2.5±18.69 for control group, with a p=0.335. This study suggests that providing online education leads to increased knowledge scores but does not impact attitudes significantly. Additional educational approaches should be considered to enhance maternal attitudes.

Abstract

Chronic energy deficiency (CED) in pregnant women is a condition of energy and protein deficiency that lasts for years and causes problems in the mother and fetus. Due to its significant consequences, determining the determinants associated with CED incidence is of utmost importance. The aim of this study was to determine the determinants of the incidence of CED in pregnant women in Indonesia. A cross-sectional study was conducted on pregnant women in Banyumas, Central Java, Indonesia, in 2022. Plausible determinants included maternal age, pregnancy interval, parity, educational attainment, nutritional knowledge, employment, frequency of antenatal care (ANC), and nutritional intake. The Chi-squared test followed by multivariate logistic regression were used to determine the factors associated with the incidence of CED. Our data indicated that 32% of the pregnant women had CED. Univariate analysis found that maternal age (p=0.022), pregnancy interval (p=0.009), educational attainment (p=0.012), knowledge of nutrition and CED (p=0.023), frequency of utilization of ANC services (p=0.028), energy intake (p=0.002), protein intake (p=0.006), vitamin C intake (p=0.016), folate intake (p=0.011), and calcium intake (p=0.004) were significantly associated with CED incidence in the pregnant women. Multivariate analysis indicated that extreme maternal age (OR; 3.49; 95%CI: 1.10–11.05), low educational attainment (OR: 4.12; 95%CI: 1.37–12.33), short pregnancy interval (OR; 7.30; 95%CI: 1.84–28.99), low frequency of ANC (OR: 3.06; 95%CI: 1.01–9.19) and low protein intake (OR: 6.80; 95%CI: 1.62–28.59) were associated with CED incidence. This study underscores the importance of increasing nutritional intake, frequency of ANC, and pregnancy interval among pregnant women to reduce the risk of CED and its adverse health outcomes.

Abstract

Kabupaten Demak merupakan Kabupaten dengan laju pertumbuhan tertinggi (1.5%/tahun), namun jumlah peserta KB IUD mengalami penurunan. Kepesertaan IUD di Kecamatan Wonosalam masih sangat rendah, padahal jumlah peserta KB aktif baru tergolong tinggi.Fasilitas memadai sudah disediakan dan semua bidan puskesmas sudah mendapatkan pelatihan pemasangan dan pencabutan IUD. Menurut Green (1980), perilaku seseorang dipengaruhi beberapa faktor antara lain faktor prediposisi. Faktor yang termasuk dari faktor predisposisi antara lain adalah pengetahuan. Sehingga ada kemungkinan bahwa perilaku pemilihan alat kontrasepsi IUD tersebut juga dapat disebabkan oleh pengetahuan mereka tentang IUD masih rendah. Penelitian ini mengidentifikasi apakah ada hubungan antara pengetahuan dengan pemilihan kontrasepsi IUD di Kecamatan Wonosalam Kabupaten Demak 2014. Hasil penelitian ini menunjukkan bahwa pengetahuan sangat mempengaruhi praktik dalam pemilihan alat kontrasepsi.

Abstract

Pendaki gunung sebagian besar memilih bahan makanan yang dapat diolah secara cepat dan instan tanpa memikirkan kebutuhan nutrisi yang diperlukan saat mendaki gunung. Pendaki lebih banyak membutuhkan nutrisi untuk meningkatkan kadar hemoglobinnya, wanita lebih rentan mengalami defisit hemoglobin dikarenakan wanita harus melewati periode menstruasi setiap bulannya. Penelitian ini bertujuan untuk menganalisis hubungan tingkat kecukupan gizi (protein, zat besi, asam folat, vitamin B12, vitamin A dan vitamin C) dengan kadar hemoglobin pada wanita pendaki gunung. Studi penelitian ini menggunakan desain cross sectional di Basecamp Gunung Prau jalur pendakian Pathak Banteng, dengan jumlah sampel sebanyak 40 orang pendaki wanita yang berusia 15-30 tahun, subjek dipilih menggunakan kriteria inklusi dan ekslusi. Data di kumpulkan dengan melakukan wawancara recall 2x24 jam. Kadar hemoglobin diukur menggunakan alat Quik-Check Hb. Data dianalisis dengan uji korelasi pearson, rank spearman dan regresi linear. Hasil menunjukkan bahwa 28% subjek mengalami anemia. Saat tidak mendaki, subjek memiliki tingkat kecukupan protein (p=0,001) dan zat besi (p=0,001) yang lebih baik daripada saat mendaki. Terdapat hubungan antara tingkat kecukupan zat besi (p=0,008; r=0,417) dan vitamin C (p=0,019; r=0,369) dengan kadar hemoglobin pada wanita pendaki gunung. Tidak terdapat hubungan antara tingkat kecukupan protein, asam folat, vitamin B12, dan vitamin A dengan kadar hemoglobin pada wanita pendaki gunung (p>0,05). Kesimpulan penelitian ini adalah tingkat kecukupan zat besi merupakan variabel yang paling berpengaruh signifikan terhadap kadar hemoglobin (p=0,050).
