Abstract

Common bile duct stricture is a common complication following open cholecystectomy. Approximately 80% of benign strictures occur following injury during a cholecystectomy. We report a female aged 70 years old, with a history of open cholecystectomy due to gallbladder stones. She was admitted to Cipto Mangunkusumo Hospital with symptoms and signs of obstructive jaundice. Endoscopy retrograde cholangio-pancreaticography (ERCP) examination revealed proximal common bile duct (CBD) stricture. She underwent open laparotomy, during which, a narrowing of proximal CBD was seen due to a fibrosis involving part of the duodenum. The patient then underwent a hepatic-duodenostomy bypass surgery. Abstract

Background: Approximately 3% of the world population is infected with hepatitis C virus (HCV). Protein of hepatitis C virus modulates apoptosis and steatosis, liver cell injury, activates liver stellate cells and liver fibrosis. Hepatitis C virus infection will cause injury to the hepatocytes. This injury to the hepatocyte will activate liver stellate cells. Stellate cells have a huge role in the development of liver fibrosis. The objective of this study is to evaluate the difference of active CD38+ liver stellate cells in various degree of fibrosis as well as its relation with aspartate transaminase (AST), alanine transaminase (ALT), and quantitative amount of hepatitis c virus ribonucleic acid (HCV RNA) in chronic hepatitis C.Method: This study was a cross-sectional study performed in 32 patients with chronic hepatitis C who had undergone liver USG, did not suffer from hepatoma, had undergone liver biopsy. Paraffin block of patients’ liver tissue was further stained using Haematoxylin and Eosin technique to identify the Metavir degree which is categorized into mild-moderate or severe degree. Special staining is performed to evaluate liver stellate cells that were then counted in averagely in five fields of view.Results: In this study, we found significant difference in the amount of CD38+ stellate liver cells between severe and mild-moderate fibrosis (p 0.001), there was no association between CD38+ stellate liver cells with AST (p = 0.2) or ALT (p = 0.7), and there was association between CD38+ stellate liver cells with quantitative HCV RNA (r = -0.372).Conclusion: Total amount of CD38+ stellate liver cells in severe fibrosis was higher compared to the total amount of CD38+ liver stellate cells in mild-moderate fibrosis. There was no association between the value of AST, ALT, and quantitative HCV RNA with the number of CD38+ stellate liver cells.

Abstract

Patients with gastric cancer which infiltrates distal esophagus often complain of dysphagia. Stenting is currently the therapy of choice for malignant dysphagia. Self expanding metallic stent (SEMS) placement has become the standard stenting therapy.We reported a case of 63 year old male patient with an advanced gastric adenocarcinoma which infiltrated distal esophagus who complained of dysphagia. The diagnosis was made based on esophago gastro duodenoscopy (EGD), histopathology study and contrast enhanced abdominal computed tomography (CT) scan. Patient underwent esophageal stenting successfully. This case report demonstrates SEMS placement as an effectivepalliation therapy in patient with an advanced gastric cancer which infiltrated distal esophagus.Keywords: gastric cancer, self expanding metallic stents, malignant dysphagia

Abstract

Microbiota and the body's immune system have a two-way interaction that is interconnected. The microbiota plays a role in the formation and maturation of the immune system, on the other hand, the immune system shapes the composition and function of the microbiota. This interaction is important in maintaining the body’s immune system’s homeostasis. The human intestine contains various types of microbes that form the gut microbiota. Various studies have found that changes in the gut microbiota are associated with immunity in distal tissues such as the lung. This gives rise to the concept of a feedback relationship between the gut and lung organs called the gut-lung axis. Through this axis, changes in the composition of the gut microbiota not only cause abnormalities in the gut but also affect microbiota in the lungs and can cause disease. One of the important routes in the transmission of substances from the intestine is through the mesenteric lymphatics. Probiotics can help improve the gut’s defense mechanism. A high-fiber diet can also reduce pathogenic bacteria by increasing levels of short-chain fatty acids. Therefore, a healthy lifestyle is needed to maintain the balance of the microbiota.

Abstract

The use of antithrombotic medicine has increased along with the growing number of cardiovascular disease in the world. Endoscopy procedure often done to the patients who use antithrombotic medicine. Eventhough most of endoscopies are safe for those patients, there are some main things related to antithrombotic treatment management that have to be noted such as the kind of antithrombotic that being used, stratification of bleeding risk on the procedure, the risk of future thromboembolic, and the process of antithrombotic treatment re-initiation. So that, patient’s safety in undergoing endoscopy procedure is guaranteed.

Abstract

Introduction. Frailty syndrome, the newest elderly health predictor, associated with higher morbidity and mortality. PPI are often used in elderly due to presence of upper gastrointestinal complaints, and relates with polypharmacy as one of the risk factor for frailty syndrome. There is no study of the relationship between long term PPI use and frailty syndrome in elderly. Methods. A case control study included subjects 60 years and above with good cognitive status. All subjects with history of hypersensitivity of PPI were excluded. Elderly who were frail based on FI-40 item were defined as cases, while individuals that were not frail were classified as control. Primary data (included frailty status) was collected on March-June 2013 by Seto E and Sumantri S, et al. Secondary data used in this current study were gathered from the primary data of previous research and from the medical record taken from geriatric and diabetic outpatient clinics Cipto Mangunkusumo Hospital. Results. There were 225 subjects collected (75 cases: 150 controls), 59,6% were female (mean age 72,14 years old, SD ± 6,4 years) and 47,1% with higher education. Lower education, divorced, poor nutrition, dependent, needed caregiver, economically insufficient, more comorbidity and poor health condition were seen in frail group. The proportion of long term PPI use were 40,9%. Long term PPI medication increased the risk of frailty syndrome (Crude OR 2,154; CI 95% 1,225-3,778; p<0,007) with adjusted OR 1,83 (CI 95% 1,02-3,37) after adjusting with nutrition and smoking variables. Conclusions. Long term use of PPI significantly increase the risk of frailty syndrome compared to the non-users.

Abstract

Introduction. Helicobacter pylori (H. pylori) infection risk is associated with many factors related to host-agent-environment. Ethnicity is one of the host factors which was the most studied factor overseas. The prevalence of H. pylori infection was found higher in certain ethnic such among Chinese. Based on migration and transmission theory, it was suspected that H. pylori infection was transmitted by people migrating from areas with a high prevalence of infection to the destination area. Chinese in West Borneo are originated from South China region where the prevalence of H. pylori infection is high. It is estimated that there are differences in the prevalence of H. pylori infection among Chinese compared to the native people of West Borneo, the Dayaknese. Methods. This was a cross-sectional study to determine the differences in the prevalence of H. pylori infection. The study was conducted at St. Antonius General Hospital Pontianak from December 2014 to June 2015 with consecutive sampling method. H. pylori infection prevalence is presented in percentage numbers, while the epidemiological characteristics and endoscopic finding differences among Chinese and Dayaknese were analyzed by bivariate analysis using the chi-square with significance value (p) = 0.05. Results. From a total of 203 subjects in this study, consisted of 102 Chinese subjects and 101 Dayaknese subjects, the prevalence of H. pylori infection was 40.8%. The prevalence among Chinese is higher than Dayaknese, which is 48.0% and 33.7%, respectively. There is no difference in the epidemiological characteristics and endoscopic findings in both ethnic groups. Conclusions. The prevalence of H. pylori infection among the Chinese (48.0%) is higher than among Dayaknese (33.7%). There is no difference in the epidemiological characteristics and endoscopic findings among both ethnic groups.

Abstract

Introduction. Medicine is a long study time. Medical student will take five years study to accomplish the education. Because the lot of task and some taken test, some medical student will face irritable bowel syndrome in their study periods. This study aims to identify the prevalence of IBS in medical student and related factors.Methods. A cross-sectional study of 350 medical student Universitas Indonesia from first grade until fifth grade was done during November-December 2016. This study used stratified randomized sampling. The Rome IV criteria was used for diagnosing IBS. Interview with questionnaire guided was applied to all subjects. A bivariate analysis was done to know relationship between IBS and its related factors using chi-square, unpaired t-test, and their alternatives. Multivariate analysis was done using logistic regression test.Results. The IBS proportions were 18.3% in the female medical students and 9.7% in the male medical students (95% CI 1.11- 3.91). The highest subtype was diarrhea subtype (53.1%). Both student-life stress inventory score (stressor and response to stressor) were higher in IBS group than in non-IBS group (For stressor (66.4 (SB 11.4) vs 60.0 (SB 12.2), p=0,001) and for responseto stressor (64.0 (41-97) vs 55,0 (35-88), p<0,0001). Female sex and anxiety were related to IBS ((aOR 2.14; 95% CI 1.12-4.08) and (aOR 2.89; 95% CI 1.43-5.83)).Conclusions. The IBS proportions were 18.3% in the female medical students and 9.7% in the male medical students (95% CI 1.11- 3.91) with predominant diarrhea subtype (53.1%). Anxiety and woman were related to IBS.

Abstract

Pendahuluan. Bioelectric Impedance Analysis (BIA) mulai banyak digunakan dalam mengevaluasi status nutrisi. Belum ada data penelitian nutrisi di Indonesia yang menggunakan BIA. Penelitian ini dilakukan untuk mengetahui perbedaan rerata hasil pemeriksaan BIA antara status nutrisi baik dan malnutrisi pada penderita penyakit gastrointestinal dan hati yang dirawat inap. Metode. Penelitian potong lintang retrospektif terhadap penderita yang dirawat inap di ruang perawatan interna RSCM periode 1 Juni-31 Desember 2013, untuk mengetahui perbedaan rerata hasil pemeriksaan BIA penderita status nutrisi baik dan malnutrisi pada penyakit gastrointestinal dan hati yang dirawat inap. Hasil. Dari 28 penderita dengan status nutrisi baik, 71,57% laki-laki, dan 21,47% wanita. Dari 28 penderita malnutrisi, 53,60% laki-laki, dan 46,40% wanita. Rerata hasil pemeriksaan BIA antara penderita nutrisi baik dan malnutrisi adalah: lean body mass, 49,5 ± 8,59 vs. 39,68 ± 6,28 kg, po(3,73-10,11)o vs 3,46o(0,40-6,51)o; p<0,001.Simpulan. Pada penderita penyakit gastrointestinal dan hati yang dirawat inap dengan status nutrisi baik, memiliki nilaibody mass, body Fell mass,total body water dan phase angle hasil pemeriksaan BIA yang lebih tinggi dibandingkandengan penderita malnutrisi.
