Abstract

Background: Continuity of care (CoC) is a key indicator that emphasizes the therapeutic relationship, service coordination, and continuity of information. Implementing EMR is expected to improve CoC, and the implementation of CoC can be influenced by aligning people, organizations, technology, and digital workloads. Objective: to analyze the impact of EMR type on continuity of care and assess the role of HOT-Fit factors and digital workload in community health centers. Methods: This study used an analytical, observational, cross-sectional design, involving 198 health workers from eight community health centers in Tasikmalaya City who use the ePus Klaster and Medisy systems. Respondents from nine professions were selected through multistage sampling. Data were collected using structured, reliable, and validated questionnaires based on the concepts of continuity of care, HOT-Fit, and cognitive workload (Cronbach’s alpha ≥0.70). The data were analyzed using bivariate and multivariate tests, including linear and logistic regression analyses. Results: Multivariate analysis shows that technology is the strongest predictor with the highest odds of increasing continuity of care across all dimensions, with results regarding informational continuity (β=0.51; p<0.001); (aOR=143.3; 95% CI=10.51–1954.2), relational continuity (β=0.53; p<0.001); (aOR=162.5; 95% CI=12.9–2035.3), and management continuity (β=0.48; p<0.001); (aOR=333.1; 95% CI=15.1–7343.8). Conclusion: Continuity of care is more influenced by technological and organizational factors than by the type of electronic medical record system itself. Therefore, strengthening technological capacity, organizational readiness, digital literacy, and the alignment of EMR systems with clinical workflows, rather than focusing solely on EMR adoption.

Abstract

Pendahuluan: Rendahnya cakupan skrining kanker serviks metode IVA di Indonesia menunjukkan perlunya instrumen yang valid dan reliabel untuk mengidentifikasi faktor perilaku yang memengaruhi partisipasi wanita. Penelitian ini bertujuan menguji validitas dan reliabilitas instrumen berbasis Health Belief Model (HBM). Metode: Penelitian kuantitatif dengan desain cross-sectional dilakukan pada 30 wanita usia subur (30–50 tahun) di wilayah kerja Puskesmas Sukorejo pada bulan Agustus. Instrumen berupa kuesioner meliputi karakteristik biososiodemografi, pengetahuan, dan konstruk HBM (perceived susceptibility, perceived benefits, perceived barriers, cues to action, dan self-efficacy). Uji validitas menggunakan korelasi Pearson product moment, sedangkan reliabilitas menggunakan Cronbach’s alpha. Hasil: Sebagian besar item dinyatakan valid (p < 0,05). Konstruk perceived benefit (α = 0,945), cues to action (α = 0,776), self-efficacy (α = 0,895), dan ketertarikan skrining (α = 0,781) menunjukkan reliabilitas baik. Namun, konstruk pengetahuan (α = 0,364), perceived susceptibility (α = 0,432), serta hambatan akses dan sistem layanan menunjukkan reliabilitas rendah bahkan negatif, serta terdapat beberapa item tidak valid yang perlu direvisi. Diskusi: Instrumen menunjukkan validitas yang baik, namun reliabilitas belum optimal pada beberapa konstruk. Hal ini mengindikasikan perlunya revisi item, terutama pada aspek hambatan dan pengetahuan, agar instrumen lebih konsisten dan sesuai konteks.  Kata Kunci: kanker serviks, IVA, validitas, reliabilitas, Health Belief Model

Abstract

Stunting is still one of the priorities for a maternal and child health in Indonesia. The stunting prevalence rate in Boyolali Regency in 2023 reached 6,86%, relatively lower compared to the national level. However, the focus on handling stunting is still specifically for pregnant women and toddlers. There is a need to optimize the role of Family Support Team cadres to be able to supervise the prospective bride, as an effort to prevent stunting. This service was carried out in the Department of Population Control Service, Family Planning, Women’s Empowerment and Child Protection (DP2KBP3A) Boyolali Regency with a target of 20 family support team cadres’, who represent rom each sub-district in Boyolali Regency. The aim of this service was to increase cadres knowledge on stunting, including factors that cause stunting, pregnancy planning, and family assistance for prospective brides. The majority of cadres have completed high school education (70%), have been cadres for more than 10 years (60%), and have 1-3 jobs (60%). There was an increase in cadres’ knowledge before and after being given training (p<0,05). Result shows that activities carried out were the provision of training as an effort to complement the knowledge and understanding of family support team cadres, especially for prospective brides, was effective, demonstrated by an increase in cadres knowledge before and after training.

Abstract

Background: Pregnancy hypertension was a complication of pregnancy which became one of the causes of maternal and perinatal morbidity and mortality in the world. This study aimed to determine the effect of individual and contextual factors of integrated health post (posyandu) on hypertension in pregnancy.Subjects and Method: A case control study was conducted in 5 posyandus in Magelang, Central Java, from April to May 2019. A sample of 210 women was selected by simple random sampling. The dependent variable was hypertension in pregnancy. The independent variables were age, parity, nutritional status, family history of hypertension, history of hypertension before pregnancy, history of hormonal contraceptive, and exposure to cigarette smoke. The data were collected by questionnaire and analyzed by a multilevel multiple logistic regression.Results: Hypertension in pregnancy increased with age &lt;24 or &gt; 35 years old (b= 2.10; 95% CI= 0.16 to 4.04; p= 0.033), parity (b= -3.81; 95% CI = - 6.31 to -1.32; p = 0.003), nutritional status (b= 3.35; 95% CI= 1.36 to 5.33; p= 0.001), family hypertension history (b= 3.35; 95% CI= 1.33 to 5.37; p= 0.001), hypertension history before pregnancy (b= 2.46; 95% CI= 1.1 to 3.81; p&lt;0.001), hormonal contraceptive history (b= 2.6; 95% CI= 0.53 to 4.72; p= 0.014), and cigarette smoke exposure (b= 2.87; 95% CI= 1.15 to 4.58; p= 0.001). Posyandu had negligible contextual effect on hypertension in pregnancy with ICC= 7.6%.Conclusion: Hypertension in pregnancy increases with age &lt;24 or &gt; 35 years old, parity, nutritional status, family hypertension history, hypertension history before pregnancy, hormonal contraceptive history, and cigarette smoke exposure. Abstract

Background: SARS-CoV-2 is a new type of coronavirus that has never been previously identified in humans. In severe cases of COVID-19 it can cause pneumonia, acute respiratory syndrome, kidney failure, and even death. Hypertension causes a number of pathophysiological changes in the cardiovascular system such as left ventricular hypertrophy and fibrosis. This can make a hypertensive heart particularly vulnerable to SARS-CoV-2. This study aims to analyze and estimate the effect of comorbid hypertension on mortality in COVID-19 patients.Subjects and Method: This was a systematic review and meta-analysis conducted with PRISMA flow diagram. Article searches through journal databases include: Google Scholar, PubMed, Science Direct, and Springer Link by selecting articles published in 2020 and 2021. The keywords used are “Hypertension” OR “Hypertensive Patient” OR “Blood Pressure”, “Systolic Pressure” OR “Diastolic Pressure” AND “Mortality”, OR “death” OR “loss” AND “COVID-19” OR “SARS-COV-2” OR “coronavirus” AND “multivariate”. The inclusion criteria were full paper articles with observational research methods, the results of the analysis used the aOR value, the exposure given was comorbid hypertension, the research subjects were COVID-19 patients, with mortality outcomes. Eligible articles were analyzed using the Revman 5.3 app. Results: A meta-analysis of 18 articles showed that confirmed COVID-19 patients who had comorbid hypertension had a 1.27 times risk of mortality compared to those without comorbid hypertension and were statistically significant (aOR= 1.27; 95% CI= 1.14 to 1.41; p &lt;0.001).Conclusion: Comorbid Hypertension affects the mortality of COVID-19 patients.
