Abstract

Background:Preeclampsia is the second leading cause of maternal death, largely due to the failure to identify the condition early. Prevention behaviors for preeclampsia in pregnant women can be developed through the Social Cognitive Theory (SCT) and the Information-Motivation-Behavioral Skills (IMB) Model. Utilizing religious organizations with unique values and cultures to foster high levels of compliance, a Religious-Based Peer Education approach can optimize these preventive efforts. Objective:This research aims to analyze the influence of knowledge, skills, motivation, observational learning, husband's support, and self-efficacy on the formation of preeclampsia prevention behaviors. Methods:A non-equivalent pre- and post-test design was conducted in Bojonegoro district from August 2022 to February 2023. The sample consisted of 179 pregnant women in the intervention group and 168 in the control group. The intervention involved Religious-Based Peer Education, with peers being members of the Muslimat NU religious organization, while the control group received routine assistance from existing health cadres. Data were analyzed using path analysis. Results:The mean scores for knowledge, skills, and behavior in the intervention group were significantly higher than those in the control group (13.81 vs. 12.34; 2.37 vs. 1.95; 30.33 vs. 28.19; p < 0.001). The strongest factors influencing the formation of preeclampsia prevention behavior were screening skills (b = 0.74, p = 0.010; 95% CI: 0.03–0.23) and self-efficacy (b = 0.36, p = 0.010; 95% CI: 0.23–0.48). Conclusion:The SCT and IMB models, when applied through Religious-Based Peer Education, have proven effective in improving preeclampsia prevention behaviors. Ongoing training for peers and consistent support for pregnant women are essential to identify high-risk pregnancies and prevent maternal complications. Abstract

Mitral stenosis is the most common rheumatic heart disease (RHD) disorder worldwide, including in Indonesia. This pathological condition causes left atrial pressure, leading to left atrial fibrosis that affects the structure and function of the left atrial as well as the clinical condition. The aim of this study was to assess the correlation between circulating fibrosis biomarkers with net atrioventricular compliance (Cn) as a parameter of left atrial function, and left atrial volume index (LAVI) as a parameter left atrium structure of changes. A cross-sectional study was conducted at Panti Rahayu Hospital and Permata Bunda Hospital, Purwodadi, Central Java, with a total of 40 RHD patients with severe mitral stenosis. The ELISA was used to measure the levels of carboxy-terminal propeptide of type I procollagen (PICP), matrix metalloproteinase I (MMP-1), tissue inhibitor matrix metalloproteinase 1 (TIMP-1), and transforming growth factor-β1 (TGF-β1). The left atrial function was assessed by measuring Cn, and the LAVI parameters were measured to assess left atrium structure/size. The mean levels of circulating fibrosis biomarkers were as follows: PICP 153.96±89.12 ng/mL; MMP-1 1.44±2.12 ng/mL; MMP-1/TIMP-1 ratio 0.38±0.54 and TGF-β1 2.66±1.96 pg/mL. From the echocardiographic evaluation, the mean Cn was 5.24±1.93 mL/mmHg and the mean LAVI was 152.55±79.36 mL/m2. There were significant correlation between MMP-1 and MMP-1/TIMP-1 ratio with Cn (r=0.345 and r=0.333, respectively; both had p<0.05). PICP and TGF-β1 biomarkers did not significantly correlate with Cn (p>0.05). Meanwhile, none of the biomarkers had a significant correlation with LAVI (p>0.05). This study highlights that MMP-1 and MMP-1/TIMP-1 ratio are potentially to be used as markers to determine the Cn in RHD patients with severe mitral stenosis. However, further studies with a higher sample size are needed to confirm this finding.

Abstract

Infections of the nervous system, such as acute bacterial meningitis, pose serious health problems that require immediate intervention. In experimental animals, exposure to lipopolysaccharide (LPS) is used to induce meningitis. Aside from drug intervention to reduce inflammation in meningitis, aerobic exercise helps to maintain the regulatory mechanisms of brain homeostasis through anti-inflammatory mechanisms. The aim of this study was to evaluate the effect of aerobic exercise on malondialdehyde (MDA), nuclear factor-kappa B (NF-κB), insulin-like growth factor 1 (IGF-1), endothelial nitric oxide synthase (eNOS), brain-derived neurotrophic factor (BDNF), apoptosis, and spatial memory. A four-week experimental study was conducted using 18 rats, which were randomly divided into three different groups (six rats per group): healthy rats as negative controls (non-meningitis), a treatment group treated with antibiotic treatment (meningitis group), and a third group (aerobic exercise group) treated with antibiotics and aerobic exercise following LPS-induced meningitis. Data were analyzed using a one-way analysis of variance (ANOVA) test, and the comparison between groups used the Bonferroni post-hoc test. The results showed that aerobic exercise significantly reduced MDA (p<0.001), NF-κB (p=0.035), and apoptosis (p=0.020) while increasing the serum levels of IGF-1 (p<0.001), eNOS (p=0.011), and BDNF (p=0.001) levels. Improvement in spatial memory was significant in the aerobic exercise group (p<0.001). This study suggested that aerobic exercise could be a promising adjunct therapy in meningitis management strategies, particularly due to its effect on improving spatial memory. Further clinical trials are needed to confirm these findings for clinical use.

Abstract

The surveillance system is a source of information for policymaker to address the dengue outbreak. However, there was a lack of information on the effectiveness of dengue surveillance system in reporting the outbreak. This study aims to assess dengue surveillance activities in Bantul District, Special Region of Yogyakarta, Indonesia, one of the districts with a high burden of dengue, to provide information on current effectiveness and need assessment for further improvement for dengue surveillance system. This qualitative study was conducted in Bantul District, Special Region of Yogyakarta. The assessment approach was based on the input-processoutput-outcome (IPOO) model. The study participants were selected based on purposive sampling. Data were collected through interviews. Eight program holder surveillance were interviewed based on the interview guide. The surveillance system in Bantul Regency already utilizes electronic reporting. Reporting cases of dengue uses the system for surveillance of outbreak events (SISKLB). Analysis and interpretation are done to monitor progress in cases. The follow-up evaluation ceses is vector control to prevent the spread of the case. The incidence of dengue is still higher than the national rate. The case fatality rate (CFR) for dengue is <1%; CFR has hit its target. The larvae-free rate hasn't reached its target yet. Optimizing electronic reporting can improve the quality of information used as a reference in decision-making. SISKLB was created for timely reporting and monitoring of dengue cases. Monitoring available data laboratory can be used as a quick response to take action to prevent an increase in cases.

Abstract

Stem cells are unspecialized cells that differentiate into more specialized cells for various tissues in the human body. Stem cells have the characteristics and ability to become themselves by performing various mitoses and differentiating different cells. Stem cells also have great potential for tissue regeneration. Detection and isolation of specific cells is very important in understanding biological processes such as development, aging, regeneration, and cell pathogenesis. Therefore, stem cells are present as an invaluable therapeutic option. Before stem cells can be applied, it is important to know the characteristics of the specific function cells or phenotypic markers. Isolation and characteristics of stem cells on culture morphology or immunotaining on specific markers. Immunohistochemistry can provide a general description of the expression of markers in a cell under certain conditions. Surface markers are the basis for identifying the characteristics and isolation of stem cells. The basic characteristics of stem cells are one that influences the outcome of the therapy to be carried out. The presence of marker cell surface expressions can also be a differentiator in searching for the origin of the stem cells used6. MSC culture method was performed with DMEM + 5% HPL. UC-MSCs with phenotype differentiation were found to be CD73+, CD90+, CD105+, Lin- while the parameters found were IL-6, SDF-1, BDNF, FGF, NGF, IL-12p70, IL-23, IL-18, IL-10 , VEGF, EGF, IL-17A, IFN-, IL-1β, PDGF, MCP-1.The results obtained are in accordance with the standard content of UC-MSC cells which can be continued as cell research.

Abstract

Background: Preeclampsia can lead to maternal and fetal complications due to its ability to cause multiple organ disorders. Interestingly, N-terminal pro-brain-type natriuretic peptide (NT-proBNP) levels were higher in preeclampsia than in non-preeclampsia, representing cardiovascular mal¬function as the potential cause. Moreover, NT-proBNP also has a potential role in predicting complications that will arise in preeclampsia. This systematic review was performed to determine the role of NT-proBNP plasma levels in predicting maternal and fetal complications in preeclampsia.Subjects and Method: This systematic review was conducted based on PRISMA-P by previous observational study from scientific databases, namely Hinari, Cochrane Library, ScienceDirect, Scopus, and grey literature in OCLC’s OAISTER between 2006 and 2021. The search keyword used were ((NTproBNP) OR (NT-proBNP) OR (N-terminal pro-BNP) OR (N-terminal pro-Brain Natriuretic Peptide)) AND ((preeclampsia) OR (pre-eclampsia)) AND (pregnancy complications). Newcastle - Ottawa Quality Assessment Scale (NOS) was used to assess quality of included studies.Results: After study selection, five studies from 156 studies were considered eligible and selected in this systematic review. The results showed that pre-eclampsia complications occurred with NT-proBNP levels above 500 mg/dL, which cardiovascular complications may occurred above 700 mg/dL. NT-proBNP levels were higher in women with maternal complications such as placental abruption, HELLP (Hemolysis, elevated liver enzyme levels, and low platelet levels) syndrome, eclampsia, pulmonary oedema, congestive heart failure, cerebrovascular accident, renal dysfunction, and hypertensive retinopathy. Furthermore, increased NT-proBNP levels were associated with fetal growth restriction, resulting in low birth weight. NT-proBNP was significantly higher in pregnant women due to a combination of pre-existing volume overload and NT-proBNP clearance dysfunction in kidney. Conclusion: NT-proBNP levels were associated with adverse outcomes in preeclampsia. NT-proBNP serum levels could be used to predict maternal-fetal complications in preeclampsia.

Abstract

The high prevalence of stunting in West Aceh Regency can be caused by low public awareness to prevent stunting early on through various activities or programs to improve community attitudes, especially in prospective brides as a determining factor that contributes to the problem of stunting due to the age of marriage too early, the low education of prospective brides who are in rural areas, and the lack of information about the importance of stunting prevention by prospective brides. The purpose of this study was to determine the relationship between age, education, region, and knowledge with attitudes to prevent stunting in prospective brides. This research was conducted at the Religious Affairs Office (KUA) of West Aceh Regency, namely the KUA of Meureubo, Kaway XVI, Johan Pahlawan, and Samatiga Districts. This study used a cross-sectional design. The research sample was prospective brides who were selected proportionally according to each location using purposive techniques based on the results of sample calculations on unknown populations according to the Lemeshow formula with the acquisition of 108 respondents. Data collection used a questionnaire that had been validated using validity and reliability tests for the variables of knowledge and attitudes to prevent stunting. The data that has been collected is analyzed using the chi-square test to conclude the problem under study. The results of statistical analysis showed that age and education, both of which had a P.Value smaller than the alpha probability of 0.05, showed that age and education were not associated with stunting prevention attitudes in prospective brides. Meanwhile, region and knowledge obtained a P.Value value greater than the probability alpha 0.05, indicating that region and knowledge are significantly related to the attitude of stunting prevention in prospective brides at the KUA of West Aceh Regency.

Abstract

Introduction: Hospital-acquired pneumonia (HAP) is a common infection with a poor prognosis. Previous studies on factors influencing HAP outcomes have yielded inconsistent findings. Therefore, further research is needed to determine risk factors that affect HAP outcomes. This study evaluated the factors associated with HAP outcomes to enable timely interventions to reduce mortality, costs, and length of stay (LOS). Methods: This cross-sectional study was conducted at Dr. Moewardi General Hospital, Surakarta, using medical record data from January to December 2022. The data included age, gender, malnutrition, anemia, level of consciousness, comorbidities, bacterial culture, and multidrug-resistant organisms (MDROs). The analyses were performed using the Chi-squared and Mann-Whitney U tests, followed by multiple logistic and linear regression tests to determine the correlation between risk factors and outcomes (recovery, death, and LOS). Results: This study included 102 patients with HAP, the majority being males (64.7%) aged 60 years old and above. Additionally, most patients did not exhibit malnutrition (87.3%) or anemia (96.1%), had normal levels of consciousness (67.6%) and mild comorbidities (60.8%), were not infected with MDROs (66.7%), and were predominantly infected by Klebsiella pneumoniae (21.6%). The multiple logistic regression test revealed that decreased consciousness was significantly associated with increased mortality (p < 0.001). Meanwhile, age 60 years old and above was significantly associated with a shorter LOS (p = 0.05). Conclusion: The majority of HAP cases occurred in men aged 60 years and above. Furthermore, there was a significant relationship between decreased consciousness and increased mortality, as well as between the age of 60 years and above and a shorter LOS.

Abstract

Background: Following a stroke, cognitive ability, and cardiovascular fitness decline, exerting a substantial influence on physical rehabilitation and overall quality of life. Stroke management encompasses a multidisciplinary approach, with emerging studies highlighting the role of exercise in enhancing cognitive ability and cardiovascular fitness post-stroke. This study aims to analyze the effects of exercise on cognitive ability, cardiovascular fitness, and quality of life in stroke survivors, shedding light on potential therapeutic benefits. Subject and Methods: The PICO Model eligibility criteria were employed to execute the article search including P= post-stroke patients; I= exercises C= standard care, O= cognitive ability OR cardiovascular fitness OR quality of life. The study extracted articles from PubMed and Science­Direct databases, with keywords "exercises" AND "cognitive ability" OR "cardiovascular fitness" OR "quality of life" AND "post-stroke patients". Inclusion criteria required full-text articles with rando­mized controlled trial (RCT) designs, focusing on post-stroke patients and evaluating improve­ments in cognitive ability, cardiovascular fitness, or quality of life. PRISMA diagram and Review Manager 5.4.1 tool were used to analyse the selected articles. Results: 25 studies included in the review had 2,110 participants, across Asia; Australia, Europe, North America, and South America eligible for meta-analysis. The result indicated a positive effect of exercise on cognitive ability (SMD=0.68, CI 95% (0.21 to 1.16); p=0.005); cardiovascular fitness (SMD=0.63, CI 95% (0.17 to 1.08); p=0.007) and quality of life (SMD=0.60, CI 95% (-0.08 to 1.27); p=0.080). Conclusion: Engagement in diverse forms and durations of exercise emerges as a crucial element in post-stroke rehabilitation. This intervention signifies a promising avenue toward enhancing cognitive abilities, and cardiovascular fitness, and potentially elevating quality of life among stroke survivors.

Abstract

Proses produksi dengan metode stamping menggunakan pencetak logam berupa dies dan mesin press dengan bahaya keselamatan kerja tertinggi, yaitu proses gerinda dan pengelasan. Bahaya keselamatan kerja ini muncul dikarenakan kurangnya safety awarenes atau kesadaran keselamatan pekerja. Pemanfaatan media digital berupa aplikasi mampu meningkatkan safety awareness pekerja terhadap lingkungan kerjanya. Penelitian ini bertujuan untuk menganalisis peningkatan safety awareness melalui aplikasi Safety Guard pada pekerja produksi PT X. Penelitian berupa quasi experimental design dengan pendekatan nonequivalent control group design. Responden adalah pekerja produksi PT X sejumlah 80 responden dari total 130 pekerja. Penelitian menggunakan teknik purposive sampling dengan pengisian kuesioner mengacu pada Safety Awareness Scale. Hasil penelitian menunjukkan terdapat peningkatan safety awareness melalui aplikasi Safety Guard dengan p-value <0.001 dan nilai pengaruh sebesar 74.9% (R Square = 0.749). Terdapat pengaruh aplikasi Safety Guard, usia, pendidikan terakhir, dan masa kerja secara simultan terhadap tingkat safety awareness pada pekerja produksi PT X.
