Abstract

Tuberculosis (TB) is an infectious disease caused by infection with Mycobacterium tuberculosis complex, which is the most frequently found bacteria and is transmitted between humans via the air route. The incidence of TB in Indonesia in 2020 was 301 per 100,000 population. Therefore, family doctors play an important role in promoting health and preventing TB disease, identifying risk factors, clinical problems, and providing management to patients by implementing holistic family doctor services based on evidence based medicine through a patient centered and family approach, the main problem This patient did not wear PPE when in direct contact with other patients while working at PKM Tanah Pasir. In this case report, primary data was obtained through anamnesis (autoanamnesis), physical examination and home visits to complete family data, psychosocial and environmental data. Patient Mr. R, 45 years old, underwent routine TB treatment at the Tanah Pasir Community Health Center and was diagnosed two months ago. The patient has normal nutritional status. Pharmacological and non-pharmacological interventions are carried out for patients in the form of education about TB disease for patients and their families. The results of the evaluation of the intervention are that the patient has started to do physical activity and the family has supported the patient's routine treatment.

Abstract

Patient An MM was brought by his parents to the Tanah Pasir Community Health Center Children's Polyclinic with complaints of itching on both hands for the last 3 weeks. The initial complaint was that it felt like itching accompanied by the appearance of reddish spots, and over time, pimples appeared between the fingers. Complaints felt worse in the last week until the patient felt itchy all day and complaints of itching felt worse at night until the patient had difficulty getting to sleep because he kept scratching the part of the body that felt itchy. Based on the anamnesis of complaints obtained after contact with the patient's older brother who experienced similar complaints. A physical examination was carried out and the examination results were within normal limits. Primary data was obtained through anamnesis and physical examination by conducting home visits, filling in family folders, and filling in patient files. The assessment is carried out based on a holistic diagnosis at the beginning, process and end of the visit quantitatively and qualitatively. The interventions carried out include education about the causes of scabies in the family, education on environmental sanitation and cleanliness and management of the disease as well as explaining complications that may arise from the patient's disease so that the patient's family maintains better personal hygiene and environmental cleanliness.

Abstract

Latar Belakang: Tuberkulosis (TB) adalah penyakit menular yang disebabkan oleh bakteri Mycobacterium tuberculosis. Penyakit ini menjadi epidemik di dunia. Indonesia merupakan Negara dengan jumlah penderita TB Paru terbanyak kedua di dunia setelah india. Pada tahun 2018, terdeteksi 8.471 kasus tuberkulosis di Provinsi Aceh. Kota Lhokseumawe dengan CNR kasus TBC tertinggi berada di Provinsi Aceh, yaitu sebesar 351 per kasus 100.000 penduduk. Di Kabupaten Muara Dua jumlah penderita tuberkulosis yaitu 201 per 100.000 penduduk. Tujuan: Mengetahui gambaran pengaruh media promosi kesehatan tentang pengetahuan dan pencegahan penyakit tuberkulosis pada masyarakat desa uteunkot kecamatan muara dua kota Lhokseumawe. Metode: Penelitian ini melibatkan 90 masyarakat dengan Jenis penelitian quasi eksperimen one group pretest-posttest design. Data dikumpulkan dengan menggunakan lembar check list dan observasi. Hasil: Sebagian besar responden pengetahuan cukup tentang pengetahuan dan pencegahan penyakit tuberkulosis sebelum diberikan edukasi terbanyak pada kategori baik 14 orang (15,5%), cukup 54 (60%) dan kurang 22 orang (24%) setelah diberikan edukasi yang terbanyak baik 19 orang (21,1%), cukup 70(77,8%) dan kurang 1 orang (1,1%). Berdasarkan uji stastistik adanya pengaruh pengetahuan terhadap pengetahuan dan pencegahan penyakit tuberculosis didapatkan p-value = 0,000 (p<0,05). Kesimpulan penelitian ini ada pengaruh media promosi pengetahuan terhadap pengetahuan dan pencegahan penyakit tuberculosis. Diharapkan kepada responden agar dapat meningkatkan informasi tentang penyakit tuberkulosis.Kata Kunci: Penyakit tuberkulosis, Pengetahuan, PencegahanBackground: Background: Tuberculosis (TB) is an infectious disease caused by the bacterium Mycobacterium tuberculosis. Results: Most of the respondents had sufficient knowledge about knowledge and prevention of tuberculosis before being given education, mostly in the good category, 14 people (15.5%), enough 54 (60%) and lacking 22 people (24%) after being given education, most of them were good 19 people (21.1%), enough 70 (77.8%) and less 1 person (1.1%). Based on statistical tests, the effect of knowledge on knowledge and prevention of tuberculosis was obtained p-value = 0.000 (p <0.05). The conclusion of this study is that there is an influence of knowledge promotion media on knowledge and prevention of tuberculosis. Abstract

Pulmonary TB was an infectious disease that was still a health problem both in Indonesia and global, so, it was still one of the goals of sustainable health development. Indonesia at second place with the most pulmonary TB sufferers in the world. In 2023, pulmonary tuberculosis sufferers at Lhokseumawe City reached 833 cases, in a condition of Covid-19 pandemic, which can worsen the condition of pulmonary TB sufferers and people who were risk of being infected with pulmonary TB. Interesting health promotion could be done with short education film media, this media has advantages in displaying pictures, animations, and sounds. The purpose of this study was to find out how short education movie media influences knowledge of pulmonary TB prevention in assisted families during the Covid-19 pandemic. This study involved 51 respondents with quasi-experimental research type using one group pretest-posttest design. Data was collected using questionnaire sheet. The results showed that the characteristics of female respondents was more than male, with the majority being the late adolescent age group (17-25 years). The results showed an increase in knowledge of 100.0% in the good category after the intervention was carried out. The use of short education movie media has an influence on knowledge based on Wilcoxon test with p 0.000. The conclusion of this study was that, there is an influence of short education movie media on knowledge of pulmonary TB prevention in assisted families during the Covid-19 pandemic in Uteunkot village, Lhokseumawe City.

Abstract

Malnutrition and stunting among children under five remain major public health challenges in developing countries, including Indonesia. These conditions are associated with increased morbidity, mortality, impaired cognitive development, and reduced productivity in adulthood. Stunting reflects chronic nutritional deficiencies, while severe malnutrition indicates acute or prolonged inadequate nutrient intake, often exacerbated by infections, poor caregiving practices, and unfavorable socioeconomic conditions. This case report aims to describe the clinical condition, contributing factors, management, and outcomes of a 14-month-old female toddler diagnosed with severe malnutrition and stunting without complications in Blang Dalam Geuteung Village, Nisam Subdistrict, North Aceh Regency. The assessment was conducted using a family medicine approach, incorporating detailed history taking, physical examination, anthropometric measurements, and nutritional status evaluation based on World Health Organization Z-score indicators. Interventions included nutritional rehabilitation through appropriate dietary intake, family education on balanced nutrition and proper feeding practices, routine growth monitoring, and optimization of primary healthcare services. The results showed gradual improvement in the child’s nutritional status, evidenced by weight gain, better appetite, and improved feeding patterns following the intervention. This case highlights the importance of early detection, comprehensive family-centered management, and continuous monitoring in addressing malnutrition and stunting at the primary care level. Strengthening community-based interventions, improving caregiver knowledge, and enhancing collaboration between healthcare providers and families are essential to reduce the burden of malnutrition and support optimal child growth and development.

Abstract

Until now Tuberculosis has become a health problem in Indonesia which causes high mortality found in 8,471 cases. In Lhokseumawe City with the highest CNR of tuberculosis cases in Aceh Province, around 351 per 100,000 population. One of the efforts to prevent TB transmission is by avoiding contact with the community. It is necessary to educate about TB disease in terms of symptoms, transmission and prevention. This research involved around 90 people with a quasi-experimental research type of one group pretest-posttest design. Data was collected using a check list sheet and observation. The results of this activity indicated that there was an increase in respondents' knowledge of knowledge and prevention of Tuberculosis with the less category of 22 people (24.4%). Respondents level of knowledge in the sufficient category as many as 54 people (60.0%). Meanwhile, respondents whose level of knowledge was in the good category were 14 people (15.6%). but after being given education, the level of knowledge of respondents about tuberculosis increased, namely enough with the number of 70 people (77.8%). poor and good categories as many as 19 people (21.1%) while less a little (1.1%). It is hoped that the provision of TB disease education can increase knowledge in efforts to prevent TB transmission will increase for a better quality of life in the health sector.

Abstract

Tinea corporis represents a common superficial dermatophyte infection in tropical regions, often exacerbated by behavioral, environmental, and metabolic risk factors. This study aimed to comprehensively analyze the management of tinea corporis with grade II obesity in a 50-year-old female patient using a family medicine approach at a primary healthcare facility. The study employed an empirical descriptive case report design integrating clinical findings, structured anamnesis, physical examination, and home visit assessment to identify multidimensional determinants of disease. Results demonstrated chronic pruritic annular lesions with characteristic scaling and central healing, supported by stable vital signs and absence of systemic involvement. Behavioral factors such as habitual scratching, shared personal items, and limited health literacy were identified alongside environmental contributors including household density. Obesity further influenced disease persistence through increased skin moisture. A comprehensive intervention combining antifungal therapy and family-based education improved disease management outcomes. The findings highlight the importance of integrating clinical, behavioral, and familial dimensions to enhance therapeutic effectiveness. The family medicine approach provides a strategic framework for addressing chronic dermatological conditions through holistic, patient-centered care.

Abstract

This study aimed to analyze the implementation of a family medicine approach in managing stage 1 hypertension in a 38-year-old female patient at a primary healthcare facility. The research employed an empirical case report design with a qualitative descriptive approach, integrating clinical assessment, behavioral evaluation, and family-based analysis. Data were collected through in-depth anamnesis, physical examination, home visits, and the use of family medicine instruments, including genogram, APGAR, and SCREEM. The findings demonstrated that comprehensive management combining pharmacological therapy, lifestyle modification, and family involvement contributed to improved blood pressure control and enhanced patient adherence. Behavioral changes were observed in dietary patterns, physical activity, and stress management, supported by strengthened family support systems. The integration of patient-centered care and contextual family assessment enabled more sustainable health outcomes compared to conventional approaches. These results highlight the importance of holistic, continuous, and family-oriented care in managing hypertension at the primary care level, particularly among productive-age patients with modifiable risk factors and complex psychosocial backgrounds.

Abstract

The increasing complexity of competition in the healthcare industry requires hospitals to adopt adaptive and strategic management practices to enhance organizational competitiveness. This study aims to analyze the role of strategic management in improving hospital competitiveness through a Systematic Literature Review (SLR) approach. A total of 41 national and international scientific articles published between 2014 and 2025 were reviewed, sourced from Scopus, Web of Science, Google Scholar, and Sinta databases, following the PRISMA guidelines for literature selection. The synthesis of findings indicates that strategic management plays a significant role in strengthening hospital competitiveness through clear strategy formulation, consistent implementation, and continuous performance evaluation. Key factors contributing to competitive advantage include strategic leadership, adaptive organizational culture, effective human resource management, utilization of information technology, and integration between strategic planning and operational processes. The results emphasize that hospitals implementing strategic management comprehensively are better equipped to respond to environmental dynamics, improve service quality, and sustain competitive advantage in the long term. This study highlights the importance of strategic management as a foundational element in modern hospital governance and provides a conceptual basis for future empirical research in healthcare management.

Abstract

Tuberculosis is still a health problem in Indonesia and the world today. Aceh Province has the highest number of TB cases in Indonesia, and North Aceh district is the number one highest district in Aceh Province. One effort to reduce the morbidity rate caused by TB is TB case detection (Case Detection Rate/CDR) and the success rate of treatment. The high and low rates of TB case discovery are influenced by the performance of all managers of the TB disease control program (P2TB), which all related parties support. This research describes officers' performance finding tuberculosis cases at the Lhokseumawe City Health Center. This type of research is quantitative with a cross-sectional design. Samples were taken from the total population, namely all officers responsible for TB at community health centres in Lhokseumawe, totalling 15 officers. The results of this research showed that the dominant gender was 86.7% female officers, 80% D3 education level, 100% good knowledge, 86.7% sufficient motivation of officers, 73.3% good leadership of the head of the community health centre, 86% good training for TB officers, 7%, case detection was 100% good, work period was more than two years 73.3%, and the achievement of TB case discovery was only one health centre that reached the standard. The conclusion from research conducted at the Lhokseumawe City Health Center is that many factors influence officers' performance in case finding.
