Abstract

Fire is one of the non-natural calamities that frequently occurs at the household level and has the potential to cause burn injuries with serious health impacts. Hegarmanah Village is considered an area with high vulnerability to fire incidents, which highlights the importance of education to improve community knowledge and preparedness. This activity aimed to educate village women on fire prevention and burn management through the Permata Siaga Women’s School program. The methods applied included interactive lectures, hands-on practice, and evaluation using pre-test and post-test. The program was attended by 15 female participants with diverse ages and educational backgrounds. The results showed a significant notable rise in knowledge and skills, as indicated by the rise in the average score from the pre-test to the post-test. Participants were also able to correctly practice the use of fire extinguishers and implement proper first aid procedures for burn injuries. In conclusion, this educational program was effective in enhancing women’s preparedness in facing fire risks and managing burn injuries in household settings.

Abstract

Background: Encephalitis can quickly progress to a life-threatening, multisystem seizure, accompanied by septic shock, disseminated intravascular coagulation (DIC), and severe respiratory failure. In such conditions, early detection by nurses is essential to recognize clinical deterioration and support timely intervention. Purpose: To analyze the contribution of early detection of nursing in recognizing overt shock in young adult patients with encephalitis who experience complications of septic shock and DIC. Method: The study used a descriptive case report design on a 22-year-old male patient who was treated in the emergency unit and intensive care unit of a tertiary referral hospital in March–April 2026. Data were obtained retrospectively from medical records, nursing records, laboratory results, radiological examinations, blood gas analysis, and therapy documentation. Results: Preliminary findings showed severe hemodynamic instability with blood pressure of 65/36 mmHg, pulse of 128 times/minute, respiratory rate of 29 times/minute, and lactate of 7.47 mmol/L. During treatment, progressive thrombocytopenia, coagulation disorders, decreased neurological status, and refractory hypoxemia were found. Mechanical ventilation and pronation position increased oxygen saturation from 84% to 98%, while lactate levels decreased to 2.26 mmol/L after intensive administration. Conclusion: Continuous nursing monitoring plays a role in accelerating resuscitation, vasopressor administration, ventilation support, and multidisciplinary coordination. Early detection of nursing plays an important role in recognizing overt shock, monitoring multisystem deterioration, and improving clinical stabilization and patient safety.

Abstract

The Hypertensive Heart Disease (HHD) is a chronic condition that arises from uncontrolled hypertension, resulting in myocardial damage and ventricular dysfunction that can increase the risk of cardiogenic shock. Pleural effusion occurs because of cardiac congestion and causes fluid buildup in the pleural cavity. In patients with this condition, unstable electrolyte levels are often found in the body. The combination of hypertensive heart disease, cardiogenic shock, pleural effusion, and electrolyte disturbances is a complex problem, requiring a multidisciplinary approach to prevent fatal complications such as multisystem failure and risk of death. This case report aims to describe nursing management in patients with hypertensive heart disease, cardiogenic shock, pleural effusion, and electrolyte disturbances, then relate it to scientific evidence through a literature review. The method used in this study was a case report. The subject of this case report was a 65-year-old woman with hypertensive heart disease, cardiogenic shock, pleural effusion, and electrolyte disturbances in the High Care Unit. Nursing management of patients with hypertensive heart disease, cardiogenic shock, pleural effusion, and electrolyte disturbances includes comprehensive assessment, collaborative intervention in the administration of inotropics, fluid management, oxygenation management, and collaboration in pleural puncture, as well as evaluation. Patients with hypertensive heart disease, cardiogenic shock, pleural effusion, and electrolyte disturbances present complex problems that can lead to death if not treated quickly and appropriately. Nurses play a major role in nursing management through assessment, problem identification, intervention, and periodic evaluation of hemodynamic status, fluid balance, and patient response to therapy.

Abstract

ABSTRACT Burnout is a psychological problem resulting from emotional and interpersonal stress that occurs continuously in the workplace. The high workload of nurses can physical and psychological problems that can lead to burnout. This study amins to describe the incidence of burnout in emergency nurses at Al-Ihsan Hospital, West Java Province. The quantitative descriptive study design with total sampling of emergency nurses at Al-Ihsan Hospital, West Java Province (N=62). The instrument used was the Copenhagen Burnout Inventory (CBI), analyzed using univariate analysis. Data collection was carried out from 4-7 January 2025. The results of this study indicate that most respondents (83.9%) experienced low personal burnout, most respondents (79%) experienced low work-related burnout, and most respondents (83.9%) experienced low client-related burnout. The conclusion of this study is that there are emergency nurses who experience low personal burnout, work-related burnout, and client-related burnout. Based on the results of the study, it is suggested for institution to conduct stress management training programs and conduct training related to communication skills and training to improve the ability to adjust to carrying out multiple roles. It is also recommended for nurses to improve the quality of social support from both fellow nurses and nurse managers and implement workplace stretching exercise interventions to increase muscle flexibility and improve physical fitness. Keywords: Client-Related Burnout, Emergency Department, Nurses, Personal Burnout, Work-Related Burnout.  ABSTRAK Burnout merupakan masalah psikologis akibat dari stres emosional dan interpersonal yang terjadi secara terus-menerus di tempat kerja. Tingginya beban kerja perawat dapat menimbulkan masalah fisik dan psikologis yang dapat berujung pada burnout. Penelitian ini bertujuan untuk mengetahui gambaran burnout pada perawat IGD RSUD Al-Ihsan Provinsi Jawa Barat. Rancangan penelitian deskriptif kuantitatif dengan total sampling perawat IGD RSUD Al-Ihsan Provinsi Jawa Barat (N=62). Instrumen yang digunakan adalah Copenhagen Burnout Inventory (CBI), dianalisis menggunakan analisis univariat. Pengambilan data dilakukan pada tanggal 4-7 Januari 2025. Hasil penelitian ini menunjukkan bahwa sebagian besar responden (83,9%) mengalami personal burnout yang rendah, sebagian besar responden (79%) mengalami work-related burnout yang rendah, dan sebagian besar responden (83,9%) mengalami client-related burnout yang rendah. Simpulan dari penelitian ini adalah personal burnout, work-related burnout, dan client-related burnout pada perawat IGD berada dalam kategori rendah. Berdasarkan hasil penelitian tersebut, disarankan bagi institusi untuk mengadakan program pelatihan manajemen stres dan melakukan pelatihan terkait keterampilan berkomunikasi dan pelatihan untuk meningkatkan kemampuan menyesuaikan diri untuk menjalankan peran ganda. Disarankan juga bagi perawat untuk meningkatkan kualitas dukungan sosial baik dari sesama perawat maupun manajer perawat dan menerapkan intervensi workplace stretching exercise guna meningkatkan fleksibilitas otot dan meningkatkan kebugaran fisik. Kata Kunci: Client-Related Burnout, Instalasi Gawat Darurat, Perawat, Personal Burnout, Work-Related Burnout.

Abstract

ABSTRACT Idiopathic Dilated Cardiomyopathy (IDCM) is a condition that can trigger malignant arrhythmias such as Ventricular Tachycardia (VT) and Ventricular Fibrillation (VF), especially if accompanied by severe hyperkalemia. The combination of myocardial dysfunction, impaired renal function, and electrolyte imbalance increases the risk of fatal complications such as cardiac arrest. Objective: To report the case management of an IDCM patient with severe hyperkalemia causing malignant arrhythmias, and to highlight the importance of a multidisciplinary approach in its clinical management. This case study uses a single case approach to a 52-year-old patient treated in the Intensive Care Unit. Data were collected through direct assessment, observation, and supporting examinations during five days of intensive care. The patient experienced severe hyperkalemia (K+ 7.05 mmol/L) which triggered VT and progressed to VF. Emergency management was carried out with ACLS, administration of calcium gluconate, insulin-dextrose, Kalitake, sodium bicarbonate, and inotropic support. Hemodialysis was performed to treat persistent electrolyte disturbances. The patient showed significant clinical improvement after comprehensive intervention. Management of severe hyperkalemia in IDCM patients requires coordination between cardiology, nephrology, and intensive care specialists. Rapid intervention, aggressive medical therapy, and hemodialysis can save lives and restore patient stability. Keywords: Nursing Care, Idiopathic Dilated Cardiomyopathy (IDCM), Hyperkalemia.  ABSTRAK Idiopathic Dilated Cardiomyopathy (IDCM) merupakan kondisi yang dapat memicu aritmia maligna seperti Ventricular Tachycardia (VT) dan Ventricular Fibrillation (VF), terutama jika disertai dengan hiperkalemia berat. Kombinasi antara disfungsi miokard, gangguan fungsi ginjal, dan ketidakseimbangan elektrolit memperbesar risiko komplikasi fatal seperti cardiac arrest.Tujuan: Melaporkan penanganan kasus pasien IDCM dengan hiperkalemia berat yang menyebabkan aritmia maligna, serta menyoroti pentingnya pendekatan multidisiplin dalam manajemen klinisnya.Studi kasus ini menggunakan pendekatan single case terhadap pasien berusia 52 tahun yang dirawat di ICU. Data dikumpulkan melalui pengkajian langsung, observasi, dan pemeriksaan penunjang selama lima hari perawatan intensif. Hasil: Pasien mengalami hiperkalemia berat (K+ 7.05 mmol/L) yang memicu VT dan berkembang menjadi VF. Penanganan darurat dilakukan dengan ACLS, pemberian kalsium glukonat, insulin-dextrose, Kalitake, natrium bikarbonat, dan dukungan inotropik. Hemodialisis dilakukan untuk mengatasi gangguan elektrolit yang persisten. Pasien menunjukkan perbaikan klinis signifikan setelah intervensi komprehensif. Penanganan hiperkalemia berat pada pasien IDCM membutuhkan koordinasi antara spesialis kardiologi, nefrologi, dan perawatan intensif. Intervensi cepat, terapi medis agresif, serta hemodialisis dapat menyelamatkan nyawa dan memulihkan stabilitas pasien. Kata Kunci: Asuhan Keperawatan, Idiopathic Dilated Cardiomyopathy (IDCM), Hiperkalemia.

Abstract

This educational process does not only focus on knowledge transfer; it also develops a wide range of individual potential, including academic abilities, relationships, talents, aptitudes, physical abilities, and the arts. Progressivism education is essential to the understanding and implementation of true education in Indonesia. Progressivism education educates students to face and solve problems in social life. This shows how important education is to produce people who are beneficial to society and the country (Sulkifly, 2020). This research uses a quantitative approach and is classified as quantitative descriptive research. This research method is conducted by collecting numerical data. This process of collecting and analyzing data is known as research design or research design. This indicates that the research includes planning and conducting it. The design starts with observing and evaluating known research until further evidence is needed. In the research design, a pre-test and post-test control group was used. This test is done with pearson correlation or by comparing r count with r table. Data validity can be seen if rcount> rtable with 5%. If rtable < rcount then it is said to be valid. The results of the T table test show that the t count obtained is 14.450 has a significance value of 0.000, which indicates that the seventh grade science subject matter has a significant effect on improving students' critical thinking skills. Based on the results of hypothesis testing, F table obtained 208.806 or greater than F table (208.806 greater than 4.351).

Abstract

Sudden cardiac arrest is a medical emergency that requires immediate treatment. The general public plays an important role in providing initial treatment in the form of first aid. This activity aims to increase public knowledge of Basic Life Support (BLS) through training for members of the Disaster-Resilient Village (Destana) and the Youth Integrated Health Post (Posyandu Remaja) in Tanjungsari Village. The education was provided to 43 participants using lectures to deliver BLS theory and facilitator-led practical training, supported by BLS mannequins and audiovisual materials, including PowerPoint presentations and videos. The educational program began with a pre-test, followed by a material presentation and practical session, and concluded with a post-test. The results showed a significant increase in participants' knowledge (p < 0.001), with an average knowledge score of 46 ± 20.8 before the activity and 78 ± 16.4 after the activity. This indicates that the education provided had a positive impact on increasing community knowledge of managing cardiac arrest emergencies in disaster situations. This activity also served as an effort to strengthen the community's capacity to provide appropriate initial treatment before professional medical assistance became available. Thus, similar educational activities involving material presentations, video screenings, practical sessions, and simulations can be conducted for a wider community. Kejadian henti jantung mendadak merupakan kondisi kegawatdaruratan yang membutuhkan penanganan dengan cepat. Masyarakat awam memiliki peran penting dalam pemberian penanganan awal dalam pemberian pertolongan pertama. Kegiatan ini bertujuan untuk meningkatkan pengetahuan masyarakat mengenai Bantuan Hidup Dasar (BHD) melalui pelatihan kepada anggota Desa Tanggung Bencana (Destana) dan Posyandu Remaja di Desa Tanjungsari. Edukasi diberikan kepada 43 peserta menggunakan metode ceramah untuk teori BHD dan metode pelatihan yang dipimpin oleh fasilitator untuk praktik, serta menggunakan media manekin BHD serta audiovisual power point dan video. Edukasi dimulai dengan pre-test, sesi pemberian materi dan praktik, serta post-test. Hasil menunjukkan adanya peningkatan secara signifikan pada pengetahuan peserta (p<0,001), dengan nilai rata-rata pengetahuan sebelum kegiatan 46±20,8 dan sesudah kegiatan 78±16,4. Hal ini menunjukkan bahwa edukasi yang dilakukan memberikan dampak positif dalam meningkatkan pengetahuan masyarakat menghadapi kondisi gawat darurat henti jantung pada keadaan bencana. Kegiatan ini juga merupakan upaya untuk memperkuat kapasitas komunitas agar mampu melakukan penanganan awal secara tepat sebelum bantuan medis profesional tersedia. Dengan demikian, kegiatan edukasi serupa melalui pemberian materi, pemutaran video, praktikum, dan simulasi dapat dilakukan kepada masyarakat lebih luas.

Abstract

 Guillain-Barré Syndrome (GBS) is an autoimmune disease affecting the peripheral nervous system, potentially leading to progressive muscle weakness, respiratory failure, and autonomic dysfunction. Severe autonomic dysfunction may result in arrhythmias, blood pressure fluctuations, and vital organ dysfunction, often worsened by sepsis. This report adopted a case report approach of a patient with GBS and severe autonomic dysfunction accompanied by suspected sepsis. Data were thoroughly collected through direct observation, comprehensive review of medical records, and detailed analysis of laboratory, electrocardiogram (ECG), and radiological findings. Analysis was conducted using both clinical and theoretical frameworks. This case report aims to describe autonomic symptom changes in GBS with suspected sepsis and to analyze their clinical implications for nursing care in critically ill patients A 51-year-old male patient presented with progressive extremity weakness, respiratory failure, and required mechanical ventilation. During intensive care, the patient developed second-degree AV block, sinus tachycardia, hypotension, persistent fever, and leukocytosis. These manifestations indicate severe autonomic dysfunction affecting both sympathetic and parasympathetic control on patient. Interventions included mechanical ventilation, intravenous immunoglobulin (IVIg), vasopressors, broad-spectrum antibiotics, and haemodynamic monitoring. The patient’s autonomic symptom changes reflected dysfunction of both sympathetic and parasympathetic nervous systems, as indicated by cardiac rhythm disturbances and blood pressure instability during intensive care. AV block indicated sinoatrial node involvement, while sinus tachycardia suggested sympathetic dominance during sepsis. Haemodynamic instability, observed through fluctuating heart rate and hypotension requiring vasopressor support, illustrated the complex interaction between GBS and sepsis. This case highlights that severe autonomic dysfunction in GBS may lead to AV block and sinus tachycardia, aggravated by sepsis. This case suggests that nurses caring for patients with GBS should maintain a high level of vigilance for autonomic instability including arrhythmias and blood pressure changes, especially in the presence of sepsis. Intensive management and continuous monitoring are critical to stabilize autonomic function and improve clinical outcomes in critically ill patients.
