Abstract

Introduction. Major depression criteria can be found in more than 20% of coronary heart patients and more than 30% acute coronary syndrome patients have significant depressive symptoms. Several studies have shown that reduction of hearth rate variability (HRV) and enhancement of neutrophil lymphocyte ratio (NLR) can cause enhancement of morbidity and mortality in myocardial infarction patients. On the other hand, depression can cause reduction of HRV, enhancement of NLR and reduction of quality of life. This study aimed to determine differences in HRV, NLR and quality of life between acute myocardial infarction (AMI) patients with depressive symptoms and AMI patients without depressive symptoms. Methods. A cross-sectional study was conducted among AMI patients who were treated during the period of March to July 2021 at the Intensive Cardiac Care Unit (ICCU) Cipto Mangunkusumo Hospital Jakarta. Samples were taken consecutively until the amount of sample is met. Data were analyzed using unp aired T test. Results. There was a total of 46 subjects AMI patients in this study, ten subjects with depressive symptoms and 36 subjects without depressive symptoms. From the analysis, it was found that AMI patients with depressive symptoms had lower standard deviation normal to normal (SDNN) average (p=0.447), lower low frequency/high frequency (LF/HF) (p=0.323), and higher NLR average (p=0.438) than AMI patients without depressive symptom, although it was not statistically significant. Analysis showed that the mean value of SF36 total scores (p<0.001), physical function domain scores (p<0.001), energy domain scores (p<0.001), mental health domain scores (p<0.001), social function domain scores (p=0.028), bodily pain domain scores (p=0.002), and general health domain scores (p=0.002) were lower in AMI patients with depressive symptoms. Conclusion. AMI patients with depressive symptoms had lower SDNN, lower LF/HF, and higher NLR than AMI patients without depressive symptoms, although it was not statistically significant. AMI patients with depressive symptoms significantly have lower quality of life than AMI patients without depressive symptoms.

Abstract

Introduction. Cardiovascular disease (CVD) is the common cause of mortality in end-stage renal disease (ESRD) patients undergoing hemodialysis (HD). Causes of CVD in ESRD patients are multifactorial. Ventricular arrhythmia is known as the leading cause of sudden death in dialysis patients. QT dispersion, which is measured from electrocardiography (ECG), is known as one of the parameters to assess the risk of arrhythmia in HD patients. This study aimed to determine the proportion of pre-dialysis prolonged QT dispersion in chronic twice-weekly HD patients and the associated factors. Methods. A cross-sectional study was conducted among HD patients age ≥18 years with dialysis duration of at least three months in Cipto Mangunkusumo Hospital during May–June 2019. Electrocardiography, echocardiography, and blood samples were taken before the dialysis session. Bivariate analysis using Chi-square and Fisher’s exact test was performed on each variable. Significant results were further analysed using logistic regression. Results. A total of 142 subjects were included in this study. The mean age of subjects was 49 (standard deviation (SD) 15) years. Prolonged QT dispersion was found in 51 subjects (35.9%). Multivariate analysis showed prolonged QT dispersion was significantly correlated with systolic dysfunction (PR=1.875 (CI 95% 1.234-2.848); p=0.006) and left ventricle hypertrophy (PR=2.361 (CI 95% 1.032-5.403); p=0.019). Age, diabetes mellitus, hypokalemia, hypomagnesemia, and diastolic dysfunction were not significantly correlated with QT dispersion (p>0.05). Conclusion. The proportion of pre-dialysis prolonged QT dispersion in twice-weekly chronic hemodialysis patients in RSCM is 35.9% and associated with systolic dysfunction and left ventricle hypertrophy

Abstract

Introduction. Cardiovascular complication of liver cirrhosis is relatively obscure. Pathophysiology of liver cirrhosis, involving portal hypertension made the possibility of cirrhosis complication manifested as left ventricular diastolic dysfunction. This study aims to determine proportion of left ventricular diastolic dysfunction among liver cirrhotic patients according to American Society of Echocardiography-European Association of Echocardiography (ASE-EAE) 2009 and conventional approach, to determine any correlation between left ventricular diastolic dysfunction severity stages with severity stages of liver dysfunction in cirrhotic patients represented by Child Turcotte Pugh (CTP ) score, also to asses relationship between severity stages of parameters of diastolic function according to ASE-EAE 2009 with liver cirrhosis severity evaluated by numerical CTP score. Methods. A cross sectional study was conducted among 96 liver cirrhotic patients within age of 18-60 years old consecutively due to any causes who admitted to ambulatory unit of Hepatology and Internal Medicine Cipto Mangunkusumo General Hospital wards into intended sample. The study started in November 2013 until proper sample size was obtained. Echocardiography examination was performed by two operators. Interobserver validity was assesed with level of Kappa aggrement and mean difference. Data were extracted to determine prevalence, normality test, Spearman correlation test, and multivariate linear regression test using SPSS for Windows. Results. Left ventricular diastolic dysfunction proportion among liver cirrhotic patients according to ASE-EAE 2009 was 34,3% and 21,9% of normal diastolic function subgroup has left atrial volume index ≥34 mL/m2. Meanwhile, conventional approach resulted in 68,8% of diastolic dysfuncation. All diastolic parameter showed abnormalities on CTP B 8-10. Spearman’s r values of stage of diastolic dysfunction severity according to ASE-EAE 2009 with severity of numerical CTP score was 0,42 (p<0,001) and 0,54 based on conventional approach. Exclusion of diabetic patients and spironolactone treated patients resulted in r=0,51 (p <0,001) based on ASE-EAE 2009 and 0,63 (p<0,001) based on conventional approach.. Parameters of diastolic function that had relation with liver dysfunction severity in cirrhosis measured by numerical CTP were Ar-A (p=0,004), left atrial volume index (p=0,005), and laterale e’ (p=0,026). Conclusion. Severity of left ventricular diastolic dysfunction with severity of liver cirrhosis is positively correlated. Diastolic parameters relate with severity of liver cirrhosis are diastolic ventricular filling pressure with left ventricular chamber stiffness, left atrial remodelling and regurgitant of pulmonary venous flow velocity to oppose filling pressure. Abstract

Tuberculosis (TB) has become a resurgent global problem with increasing numbers of patients. TB infection is still common and remains an important cause of morbidity and mortality, particularly in underdeveloped and developing nations. Intestinal tuberculosis (intestinal TB) rates are rising, consistet with the overall trend. In 2014 Indonesia has the second highest TB prevalence in South East Asia after Timor Leste. Intestinal TB is the sixth highest manifestation of extrapulmonal TB. Manifestations can be non-specific and mimic many conditions, including malignancies causes’ intestinal TB diagnosis more difficult to be accurately determined. Findings from endoscopy and radiological imaging are countless, and depend on the stage of the disease and the time at which investigations are carried out. Hence, diagnosis can be difficult. Until recently there is no single method to identify intestinal TB accurately, various investigative methods have been used to aid in the diagnosis of intestinal TB. Early diagnosis and initiation of antituberculous therapy and surgical treatment are essential to prevent morbidity and mortality. Combined clinical assessment and some modalities examinations are needed to determine intestinal TB. Patient whom has been diagnosed with intestinal TB will be given anti tuberculosis therapy and surgery if any complications occur.

Abstract

Pendahuluan. Prevalensi diabetes global diprediksi akan meningkat sebesar dua kali lipat pada tahun 2030 dari 2,8% menjadi 4,4% (berkisar 366 juta) dengan angka komplikasi kaki diabetes berkisar 10%. Ulkus diabetes dan amputasi ekstremitas bawah merupakan komplikasi diabetes yang akan meningkatkan mortalitas dan morbiditas Setelah lima tahun amputasi pertama, 28-51% pasien akan menjalani amputasi kedua. Penelitian ini dilakukan untuk mengetahui proporsi dan profil pasien yang mengalami reamputasi di Rumah Sakit Cipto Mangunkusumo tahun 2008-2012. Metode. Disain studi ini adalah potong lintang dengan 80 subjek pasien kaki diabetes yang dirawat di RSCM tahun 2008 sampai 2012. Dilakukan pencatatan data yang didapat dari rekam medik berupa data demografis (jenis kelamin, rerata usia, pendidikan, pekerjaan, pembiayaan) dan klinis (rerata lama diabetes, hipertensi, merokok, neuropati, Peripheral Artery Disease (PAD), anemia, komorbid dan gagal ginjal kronik). Hasil. Angka amputasi pada kaki diabetes dari tahun 2008-2012 sebanyak 128 subjek (20,3%) pasien dan dari 128 subjek terdapat 80 subjek yang statusnya lengkap dan yang menjalani reamputasi sebanyak 47 subjek (58,7%). Subjek yang menjalani reamputasi lebih sering pada perempuan sebanyak 25 orang (53,2%), dengan rerata usia 58,2 tahun, memiliki pendidikan SMA kebawah 97,8%, menggunakan biaya gakin (72,6%). Lama menderita DM rata-rata 7 tahun, kadar HbA1C ≥ 7% sebesar 46,8%, hipertensi pada 63,8% subyek, merokok sebanyak 14 orang (29,7%), neuropati sebanyak 41 orang (87,2%), PAD sebesar 27,6%,, anemia sebanyak 46 orang (97,8%), kadar albumin rerata 2,5 gr/dl dan terdapat 19 orang (40,45%) dengan eGFR(40,4%). Komorbid yang ada pada subjek yang menjalani reamputasi adalah pneumonia (34%), penyakit arteri koroner (21,2%), penyakit ginjal kronik (17%), stroke (10,6%), keganasan (8,6%) dan gagal jantung kongestif (8,6%) Simpulan. Proporsi reamputasi pada kaki diabetes di RSCM tahun 2008 – 2012 sebesar 58,7%. Angka reamputasi pada kaki diabetes lebih tinggi pada perempuan, usia lebih lanjut, pendidikan SMA kebawah, menggunakan biaya umum dan gakin, kadar HbA1C ≥ 7%, hipertensi, neuropati, PAD, anemia, kadar albumin rendah, dengan eGFRbanyak

Abstract

Pendahuluan: Prevalensi sirosis cukup tinggi di Indonesia yang mayoritas populasinya adalah muslim. Pada saat menjalani puasa Ramadhan yang merupakan kewajiban umat muslim terjadi berbagai proses metabolik seperti penurunan glikokogenesis, peningkatan glukoneogenesis dan ketogenesis, dapat mempengaruhi keadaan klinis, nutrisi dan bokimiawi pasien sirosis hati yang juga mengalami proses hiperkatabolik. Penelitian tentang pengaruh puasa Ramadhan pada pasien sirosis hati di Indonesia belum pernah dilakukan. Berdasarkan kenyataan diatas perlu diteliti bagaimana perubahan terhadap status nutrisi, status fungsi hati, pembentukan badan keton dan keseimbangan nitrogen pada pasien sirosis hati yang menjalankan puasa Ramadhan. Metode: Penelitian ini merupakan studi pre dan post, yang menilai perubahan pada pasien sirosis hati yang berpuasa Ramadhan: saat Ramadhan dan pasca Ramadhan. Pada subjek penelitian dilakukan pengambilan data-data dasar, anamnesis dasar. Pada minggu ke 4 saat puasa Ramadhan dilakukan pengukuran antropometrik, yaitu timbang berat badan, indeks massa tubuh (IMT), ketebalan triceps skinfold (TSF), dan mid-upper arm circumference (MUAC). Pada subjek juga dilakukan pengambilan darah vena untuk pemeriksaan laboratorium dan pengukuran ekskresi nitrogen urin yang ditampung dalam 24 jam. Pada minggu ke empat pasca Ramadhan subyek dievaluasi kembali dengan prosedur yang sama seperti saat Ramadhan. Hasil: Rerata (SD) indeks massa tubuh saat puasa Ramadhan adalah 25,112 (4,05) kg/m2 dan pasca Ramadhan 25,25 (4,01) kg/m2 (p = 0,438). Didapatkan rerata (SD) midarm muscle circumference (MAMC) saat Ramadhan 25,77 (3,077) cm dan rerata (SD) pasca Ramadhan 25,96 (3,42) cm (p=0,228). Tidak ada perbedaan bermakna secara statistik antara kadar 3-β-hidroksi butiran darah saat puasa Ramadhan dan 4 minggu pasca Ramadhan. Rerata (SD) saat puasa Ramadhan 2,44 (2,93) gram/24 jam, sedangkan rerata (SD) keseimbangan nitrogen pasca Ramadhan 0,51 (3,16) gram/24 jam (p=0,037). Simpulan: Tidak ada perubahan status fungsi hati yang dinilai dengan Skor Child Pugh saat puasa Ramadhan dan pasca Ramadhan. Indeks massa tubuh dan ketebalan TSF membaik pasca Ramadhan. Tidak ada perbedaan kadar 3-β-hidroksi butirat darah saat puasa Ramadhan dan pasca Ramadhan. Terdapat keseimbangan nitrogen yang lebih positif saat puasa Ramadhan dibandingkan pasca Ramadhan.

Abstract

Introduction. The relationship between serum uric acid (SUA) level and ischemic heart disease abides controversial and still has not been established as a cardiovascular risk factor. The cooperative interaction between those two factors is not fully understood. Prior epidemiological evidences of the causal relationship is still argumentative. There were various studies using the same methods yet the outcome were different. This study aims to conduct a meta-analysis to synthesize the results of recent studies in order to obtain data quantitatively and also accurately. Methods. This study protocol was registered with PROSPERO (CRD42020210948), and conducted according to PRISMA guidelines, tracing studies published in vulnerable periods from January 2010 to May 2020. The Cochrane Library, EBSCO, Medline/PubMed, ProQuest and Science Direct are sources of published studies. Meta-analysis was conducted to synthesize the associations between SUA level and severity of coronary artery stenosis, using random effect model to account for possible study heterogeneity. Heterogeneity was assessed using I2 , and the meta-analysis was performed using comprehensive meta-analysis version 3 (CMA3) software. Results. Five studies (n = 601 patients) identified a correlation between serum uric acid levels and Gensini scores (r = 0.548; p <0.001) in ACS patients. Heterogeneity bias was found in the analysis, whereas publication bias was not found. There was a moderate positive correlation between serum uric acid levels and the severity of coronary artery stenosis, with a correlation coefficient of 0.548 (p value <0.001).

Abstract

Celiac disease is a reversible, proximal immune enteropathy resulting from the interaction of dietary gluten with the intestinal immune system. Celiac disease can produce varied manifestations and may develop at any age. New cases have increased substantially, due to increase awareness and better diagnostic tools. Standard diagnosis of celiac disease is based on positive serologic tests and histopathologic examination of duodenal biopsies. Treatment of celiac disease with free gluten diet will improve symptoms, quality of life, reduce risk of malignancy and complication. Nowadays, treatment strategy of celiac disease with non-dietary therapies is still underdevelopment.

Abstract

Introduction. In daily clinical practice, fluid status in Hemodialysis (HD) patients is well correlated with dry weight calculation. Dry weight calculation is commonly practiced by clinical observation, which is not accurate. Because of these, few methods has been suggested to calculate the dry weight non-invasively. Bioelectrical Impedance Analysis (BIA) is widely available in overseas but not readily available in all dialysis center in Indonesia, while inferior vena cava diameter is a relatively inexpensive method, and readily available in all dialysis center because it can be performed with ultrasonography (USG) instrument. Methods. A cross-sectional study was performed in a group of regular HD patients at the Haemodialysis Unit, Cipto Mangunkusumo Hospital in Jakarta, June 2011. Dry weight was evaluated with bioelectrical impedance analysis, while the inferior vena cava collapsibility index was evaluated using USG performed by two different observer. Results. We have recruited 30 HD patients, in which 18 (60%) of the subjects were overload according to the bioelectrical impedance analysis, while 21 (70%) were overload according to the inferior vena cava collapsibility index. The mean age of the subjects is 52 years old with the minimum 24 and maximum 69 years. In this research, we found negative correlation (r = -0.957, P<0.0001) between inferior vena cava colapsibility index and BIA. We found a 94.4% sensitivity and 66.7% specificity for inferior vena cava colapsibility index. Both of USG operators showed a κ coefficient value of 0.92, which reflected a very strong agreement between them. Conclusions. The inferior vena cava colapsibility index have a good role as a screening method in determining dry weight in dialysis patients
