Abstract

Background: Quality nutrition is a deter­mi­nant of children's survival, health, and growth. Nutritional problems and lack of nutrient intake are also about whether bad in the environment or commonly known as clean and healthy behavior. Social capital refers to social relationships and connections between indivi­duals because it is more of a relationship between individuals than an individual attri­bute. This study aimed to examine the relation­ship between clean and healthy behavior and social capital with the nutritional status of children under five at the Waingapu City Health Center.Subjects and Method: This was a cross-sectional study conducted at the Waingapu Health Center, East Sumba, East Nusa Teng-gara, Indonesia, in April 2021. The sample was 200 mothers, and children under five were selected randomly. The dependent variable was child nutritional status (WAZ). The indepen­dent variables were social capital, clean and healthy behavior, gender, maternal age, mater-nal occupation, and maternal education. Data were collected by using a questionnaire and analyzed by multiple logistic regression.Results: Clean and healthy lifestyle improved good nutritional status (OR= 5.50; 95% CI= 1.19 to 25.43; p= 0.029). High social capital in­creased good nutritional status (OR= 2.28; 95% CI= 1.15 to 4.51; p= 0.017).Conclusion: Healthy, clean living behavior, and high social capital improve good nutria­tional status.Keywords: clean and healthy behavior, social capital, nutritional statusCorrespondence: Stevani Rambu Kudu. Abstract

Background: Obesity has reached epidemic proportions worldwide, making obesity a serious global public health challenge. Obesity is not only found in adults but also in children and adoles­cents which can lead to various physical and mental health problems that are detrimental to the quality of life and are very risky into adulthood. Obesity in children and adolescents today is caused by a lifestyle that makes a person increase in consuming fast food, lack of sleep and the longer duration of screen time. This study aims to analyze the effect of screen time and fast food on obesity in children and adolescents.Subjects and Method: This was a systematic review and meta-analysis. Population= children and adolescents, Intervention= screen time and fast food, Comparison= no screen time and no fast food, Outcome= obesity. Article searches through journal databases include: PubMed, Science Direct, Google Scholar and Springerlink. The articles used in this study are articles that have been published from 2011-2021. The keywords used are obesity OR obese OR overweight AND “fast foods” OR snacks OR “fried foods” AND “social media” OR “screen time” OR television AND child OR adolescent. Articles were selected with the help of PRISMA flow diagrams. The inclusion criteria included full-text articles with a cross-sectional study design. The analysis used logistic regression with adjusted odds ratio and published in English. Articles that have met the requirements are analyzed using the Revman 5.3 application.Results: Fifteen articles came from Nepal, China, Pakistan, Canada, Darussalam, Ethiopia, Italy, Australia, Indonesia. Meta-analysis of 8 cross-sectional studies showed that screen time 3 hours/ day can affect obesity in children and adolescents 2.4 times compared to screen time < 3 hours/ day. The results of the meta-analysis in 8 cross-sectional studies showed that fast food 3 times/ week had an effect on obesity in children and adolescents by 2.74 times compared to fast food < 3 times/week.Conclusion: The long duration of screen time and the frequency of consuming fast food often increase the risk of obesity in children and adolescents.Keywords: obesity, screen time, fast foods, meta-analysisCorrespondence:Salwa Annisaa. Abstract

Background: Acute Lymphoblastic Leukemia (ALL) is the most common type of blood cancer in children. Treatment for ALL may affect the patient's quality of life. Factors such as nutritional status, maternal knowledge of nutrition, and dietary compliance play a role in determining the quality of life of children with ALL. This study aims to analyze the relationship between nutritional status, maternal knowledge, and dietary compliance with health-related quality of life (HRQoL) in children with ALL. Subjects and Method: This study used a cross-sectional design with a sample of 52 children with ALL at the Pediatric Hematology Clinic of Dr. Moewardi Surakarta Hospital in October-November 2024. The independent variables were nutritional status, maternal knowledge, and dietary compliance, while the dependent variable was HRQoL. Data were collected using anthropometric instruments and questionnaires, then analyzed with single logistic regression and path analysis. Results: Single logistic regression analysis showed that nutritional status (OR= 4.60; 95% CI= 1.311 to 16.139; p = 0.017) and dietary compliance (OR= 4.82; 95% CI= 1.39 to 16.78; p= 0.013) were significantly associated with HRQoL. Maternal knowledge had no significant effect (OR= 1.88; 95% CI = 0.35 to 10.18; p = 0.467). Path analysis showed that nutritional status, dietary compliance, and body mass index had a direct positive association with the child's quality of life. The path analysis model showed a good fit with the data (goodness of fit p = 0.594). Conclusion: Nutritional status and dietary compliance are major factors in improving the quality of life of children with ALL. Although maternal knowledge does not have a direct effect, the mother's role remains important through improving the child's dietary compliance. Multidisciplinary interventions that integrate nutrition education and family support are needed to enhance HRQoL in pediatric patients with ALL.

Abstract

Adolescent obesity is a growing global health problem, with the risk of developing hypertension due to activation of the sympathetic nervous system (SNS) and sodium regulation. Psychosocial stress and unhealthy diets, including consumption of Ultra-Processed Foods (UPF), contribute to increased blood pressure. UPF high in sodium, fat, and sugar are associated with hypertension, while stress can trigger emotional eating, increasing consumption of high-calorie foods. Adolescent diets tend to be low in fiber and high in UPF, supported by the school environment. This study aims to analyze the relationship between stress levels and UPF consumption on blood pressure in obese adolescents to understand the risk factors for hypertension in this group. This study used an analytical observational design with a cross-sectional approach. A sample of 75 students was selected by purposive sampling. Data were collected through interviews and measurements, then analyzed using SPSS version 29 statistically. The majority of respondents were 16 years old, male, with high UPF consumption. UPF consumption was significantly associated with diastolic blood pressure (p=0.036, r=0.255). Stress level was not significantly associated with blood pressure (p>0.05, r=0.1–0.2). Stress levels did not have a significant relationship with either systolic or diastolic blood pressure. UPF consumption was not significantly related to systolic blood pressure, but UPF consumption was significantly related to diastolic blood pressure.

Abstract

Cardiovascular fitness, quantified as maximal oxygen consumption (VO₂Max), is a significant indicator of exercise tolerance and cardiovascular health. Sedentary behavior and aging are determinants that contribute to endothelial dysfunction, arterial stiffness, and reduced cardiac output, leading to a decline in VO₂Max. Cocoa Flavanols (CFs), rich in (−)-epicatechin, have been found to enhance endothelial function, nitric oxide bioavailability, and reduce blood pressure and hence might be beneficial for cardiovascular adaptation and VO₂Max improvement. This scoping review explores the potential of CFs to enhance cardiovascular function and exercise tolerance. Method: Two randomized controlled trials were employed in this scoping review. The first trial examined the effect of 30-day CF supplementation (1000 mg/day) on cardiorespiratory fitness in healthy elderly adults. The second trial examined the effect of 7-day CF supplementation (400 mg/day) on pulmonary oxygen uptake kinetics and exercise tolerance in middle-age sedentary adults. Results: In older adults, CF supplementation improved VO₂peak, oxygen pulse, exercise capacity, and endothelial function and lowered blood pressure in middle-aged sedentary adults, CFs improved VO₂ kinetics during moderate-intensity exercise, indicating enhanced oxygen delivery, but not exercise tolerance during severe-intensity exercise. Such results highlight CFs' positive effects on vascular health and efficiency of moderate-intensity exercise. Conclusion: Cocoa flavanols may enhance cardiovascular adaptation and exercise performance by improving endothelial function, reducing arterial stiffness, and optimizing oxygen uptake during exercise. Further studies are required to evaluate their effect on maximal exercise performance and long-term cardiovascular outcomes.

Abstract

Determinant factors of cardiometabolic risk in adolescents aged 15-18 yearsBackground: Cardiometabolic diseases are increasing worldwide, and the risk of developing them starts as early as adolescence. In the long term, they may lead to comorbidities. Evidence suggests the risk factors of cardiometabolic diseases among adolescents are obesity, excessive food intake, socioeconomic status, breakfast habits, and physical activity, among others. Objective: This study aimed to determine the determinants of cardiometabolic risk in adolescents aged 15-18. Methods: This was a cross-sectional study in Al-Abidin High School Surakarta with 146 study subjects. Cardiometabolic risks were classified using Cardiometabolic Disease Staging (CMDS), waist circumference was measured with Medline, physical activity was measured using a Physical Activity Questionnaire for Adolescents (PAQA), and blood pressure was measured with a sphygmomanometer. Socioeconomic, transportation, and breakfast habits were obtained from the general questionnaire. Chi-square, ANOVA, and logistic regression were then performed using SPSS 23. Results: Gender was significantly associated with cardiometabolic risk (p=0.001). A total of 41.3% of boys were at risk of cardiometabolic (OR=3.964). Socioeconomic, breakfast habits, transportation, and physical activity were not associated with cardiometabolic risk (p>0.05).Conclusion: Boys 15 – 18 years were at a higher risk of cardiometabolic, by 3.694 times greater than girls. Socioeconomic status, breakfast habits, transportation, and physical activity were not correlated with cardiometabolic risk among adolescents.

Abstract

Background: Farmers have a great potential danger to exposure and poisoning of pesticides due to the application of occupational safety and health, which is still low because it does not use personal protective equipment (PPE). This study aimed to examine the relationship of knowledge using PPE in farmers with a history of goiter disease in the Kismantoro Subdistrict, Wonogiri Regency.Subjects dan Method: This was a cross-sectional study. Sampling techniques used purposive sampling with inclusion criteria that are willing to participate in this study, reside and become a farmer in Kismantoro Subdistrict for more than ten years, and have a history of goiter disease work actively as a farmer. The dependent variables were complete PPE and PPE masks in farmers. The independent variable was knowledgeable. Population: All farmers who were declared positively affected by goiter and recorded in the Kismantoro Subdistrict Health Center in 2020. Sample: The samples were 54 respondents. Data collection was done directly from respondents with observation techniques or observation into the field and questionnaire filling. The data were analyzed using chi-square test.Results: There was a relationship between the knowledge of farmers with a history of goiter disease with the use of complete PPE when spraying with p= 0.005 and (OR=0.77; 95%CI= 0.62 to 0.97). And there was a relationship between the knowledge of farmers with a history of goiter disease with the use of masks at the time of spraying with p= 0.002 and (OR= 6.43; 95%CI= 1.93 to 21.39).Conclusion: There is a relationship between knowledge and complete PPE and masks PPE in farmers with a history of goiter disease in Kismantoro Subdistrict, Wonogiri Regency.Keywords: knowledge, the use of PPE complete, the use of PPE mask.Correspondence:

Abstract

Background: Breast cancer is the most common type of cancer experienced by women which can cause the most deaths in both developed and developing countries. The risk of breast cancer severity in women with BMI obesity category increased up to 30% compared with women who had normal weight. This study aims to analyze the effect of obesity on breast cancer mortality in women, with a meta-analysis of primary studies conducted by previous authors. Subjects and Method: This study is a systematic review and meta-analysis with the following PICO, population: breast cancer women. Intervention: obesity. Comparison: not obese. Outcome: mortality. The articles used in this study were obtained from four databases, namely Google Scholar, Pubmed, Science Direct and Springerlink. Keywords to search for articles “Obesity” OR “Obese Overweight” AND “Mortality” OR “Death” AND “Women with Breast Cancer” OR “Breast Cancer” OR “Breast Cancer Mortality” AND “Adjusted Hazard Ratio”. The included articles are full-text English with an observational cohort study design from 2012 to 2021 and report on the aOR in multivariate analyses. Article selection is done by using PRISMA flow diagram. Articles were analyzed using the Review Manager 5.3 application. Results: A total of 12 observational cohort studies from the United States, Denmark, Netherlands, UK, Spain, Malaysia and China were selected for systematic review and meta-analysis. The data collected showed that obesity increased the mortality risk of breast cancer women and was statistically significant (aOR = 1.23; 95% CI = 1.00 to 1.52; p = 0.050). Abstract

Background Hepatitis B is caused by infection with the Hepatitis B virus, this disease affects 240-280 million people in the world. Hepatitis B virus causes chronic disease that develops in liver damage, cirrhosis and liver cancer, more than 600,000 people die every year. Giving repeated blood transfusions increases a person's risk of developing hepatitis infection. Hemodialysis patients who receive blood transfusion therapy are more at risk of developing hepatitis infection. Objective of this study was to study and estimate the influence of blood transfusion history on the risk of hepatitis B infection in hemodialysis patients based on the results of previous similar studies by meta-analysis. Method is a meta-analysis conducted by searching for articles from the databased Google Scholar, PubMed, Springer Link, and Research Gate. The keywords used were "blood transfusion" OR "risk factor" AND "HBV" OR "hepatitis B infection" OR "TTI" AND "patient hemodyalysis" OR "Patient renal failure". The inclusion criteria in this study were full text articles, from 2000-2020, with a cross sectional study design. Article analysis using RevMan 5.3. Results of data analysis studies 6 articles in the forest plot, giving blood transfusions was able to increase the incidence of hepatitis B infection in hemodialysis patients 1.30 times more at risk than those who did not transfuse, statistically significant (aOR 1.30; 95% CI = 1.02-1.66; p = 0.03) and based on the funnel plot, there was no publication bias. Conclusion blood transfusion affects hepatitis B infection in hemodialysis patients.

Abstract

Prevalensi penderita diabetes melitus (DM) di Jawa Timur menurut Survei Kesehatan Indonesia tahun 2023 meningkat (0,1%) dari sebelumnya tahun 2018, yakni 2,7%. Penelitian ini bertujuan untuk mengembangkan media daftar bahan makanan penukar sebagai media edukasi  diet DM pada penderita DM. Penelitian ini menggunakan metode Research and Development (R&D) dengan model Analyze, Design, Development, Implementation, Evaluation (ADDIE) untuk mengembangkan media edukasi dan pada tahap implementation dilakukan dengan metode quasy experimental dengan rancangan pre post-test with a control group. Penelitian ini melibatkan partisipasi tiga validator ahli dan 10 penderita DM sebagai calon pengguna. Pengumpulan data melalui wawancara dan kuesioner penilaian media. Jumlah subjek pada tahap implementation terdiri atas 40 penderita DM. Analisis data menggunakan teknik deskriptif. Hasil penilaian kelayakan media oleh ahli diperoleh skor rata-rata 91.03% dengan kategori sangat layak dan uji coba media pada kelompok calon pengguna diperoleh skor rata-rata 92.5% dengan kategori sangat layak. Video DBMP dapat digunakan sebagai media edukasi gizi diet DM pada penderita DM. Uji beda skor pengetahuan diet DM menggunakan analisis statistik Paired Sample T-Test. Dari hasil penelitian diperoleh nilai p=0.031 pada kelompok intervensi di tahap evaluation dan nilai p=0,648 pada kelompok kontrol. Hal ini menunjukkan bahwa terdapat perbedaan skor pengetahuan yang signifikan pada kelompok intervensi setelah 3 kali pemberian konseling gizi diet DM. Tahap akhir evaluasi dianalisis menggunakan post hoc Mann Whitney u test, menunjukkan bahwa ada perbedaan yang signifikan (p=<0.001) antara kelompok kontrol dan kelompok intervensi (video DBMP).
