Abstract

The caesarean section method is recognized recently with the concept of ERACS (Enhanced Recovery After Cesarean Surgery). The surgery process is more convenient because there is less pain and the postoperative recovery process is faster than the traditional method. The ERACS concept was developed from the ERAS (Enhanced Recovery After Surgery) concept and applied to gastrointestinal surgery. It has been shown to minimize hospital admissions and complications after surgery, which results in improved patient satisfaction. The design of this study was quantitative non- experimental using descriptive methods conducted in September 2023 with a population of all adult patients undergoing surgery under general anesthesia and a sample of 70 respondents. This method is used to study populations by collecting data to clarify and test hypotheses. This study focuses on describing the types and induction drugs with the ERACS method. in post-cesarean section patients at Hermina General Hospital Yogyakarta. Data were collected using observation techniques recorded in the observation sheet and then analyzed using the frequency distribution test using SPSS version 23. The results showed that most of the respondents in this study were women aged 26-35 years as many as 38 respondents (54.3%). Based on other respondent characteristics, it was also found that the results of blood pressure <130 and <90 mmHg were (52.9%), which is 37 respondents, followed by a pulse of 80-100x/min as many as 40 respondents (57.1%), spo2> 95% as many as 65 respondents (92.9%) ASA 1 as many as 46 respondents (65.7%), and the average use of morphine and fentanyl was (100%) using a combination of morphine drugs with a dose of 0.1mg and fentanyl with a drug dose of 25mcg. Abstract

Background: Hypotension is a common complication associated with spinal anesthesia, often resulting from sympathetic nerve block. Factors such as the type of drug, patient positioning, injection site, dosage, and age influence the incidence of hypotension. Purpose: This study aimed to evaluate the incidence of hypotension in patients receiving spinal anesthesia at the lumbar 3-4 (L3-L4) and lumbar 4-5 (L4-L5) injection sites. Methods: A quantitative descriptive study with a cross-sectional approach was conducted, involving 190 respondents selected via purposive sampling. Univariate data analysis was performed. Findings: Findings indicated that patients receiving spinal anesthesia at the L3-L4 site experienced an average systolic blood pressure decrease of 24.69% and a diastolic decrease of 16.17%. In contrast, patients with L4-L5 injections showed a lesser reduction, with systolic pressure dropping by 15.51% and diastolic pressure by 12.31%. Additionally, the pulse rate decreased progressively within the first 20 minutes following induction. The average pulse rate at the 20th minute was 75.06 for patients with L3-L4 injections and 75.65 for those with L4-L5 injections. Conclusion: The study highlights that the incidence of hypotension varies based on the spinal injection site, with more significant decreases in blood pressure observed in patients receiving injections at L3-L4 compared to L4-L5. These findings have important implications for anesthesia management in clinical practice.

Abstract

Background: General anesthesia is the most widely practiced anesthetic technique, and mishandling of the airway may lead to serious and even life-threatening complications. Intubation success is maximized when the LEMON assessment is applied to cases where the anesthesia assessment also assesses for prediction of intubation difficulty.  Purpose: This study aims to find out the relationship between the LEMON scoring and the prediction of intubation difficulty encountered during general anesthesia. Methods: This quantitative study follows a cross-sectional design with data being collected within one single period. The sample consisted of 52 individuals undergoing general anesthesia with endotracheal intubation (ETT). Chi-square testing was performed with Fisher's Exact Test as the alternative for statistical analysis. Results: The analysis yielded a p-value of 0.019, which is below the 0.05 cutoff, demonstrating a significant association between the LEMON score and the prediction of intubation difficulty in patients undergoing general anesthesia at RSUD Dr. Soedirman Kebumen. Conclusions: A statistically significant association was found between the LEMON score and the prediction of intubation difficulty. The implication is that LEMON scoring can be another useful tool for anesthetic pre-evaluation in predicting probable airway management concerns during general anesthesia in patient.

Abstract

Background: Hypotension is a frequent complication seen in spinal anesthesia. During spinal anesthesia, hypotension is more common with increasing block height, and an elevated body mass index (BMI) is considered a risk factor. Purpose: The aims of this research were to study the incidence of hypotension associated with spinal anesthesia for cesarean section and to relate block height to BMI. Methods: This is a cross-sectional correlational analytic study. A purposive sampling technique was used in picking 42 participants. The main instrument used for data collection was observation sheets. Results: The Spearman rank test for bivariate data showed that 35.7% of participants with a BMI greater than 25 experienced hypotension. In addition, 64.3% of participants with mild block elevation also developed hypotension. The Spearman rank test indicated a correlation value of -0.483 with a p-value of 0.001, signifying a strong and negative statistically significant relationship between hypotension and elevation of sympathetic block. Conclusion: There exists a significant relation between BMI and incidence of hypotension, with correlation values of -0.461 and a p-value of 0.002, further asserting that both BMI and block height are important characteristics that can be utilized in predicting the hypotension following spinal anesthesia in cesarean section patients
