Abstract

Lansia merupakan fase dimana terjadinya berbagai penurunan fungsi organ tubuh yang dapat menyebabkan banyak permasalahan. Salah satu masalah yang sering dialami lansia adalah jatuh. Kejadian jatuh yang terjadi pada lansia tidak jarang dan berakibat fatal, seperti cedera kepala, cedera jaringan lunak, patah tulang bahkan kematian. Oleh karena itu, perlu dilakukan upaya-upaya untuk mengendalikan angka kejadian jatuhdan mencegah risiko jatuh pada lansia yang bisa diterapkan oleh komunitas. Kegiatan ini bertujuan untuk meningkatkan pemahaman caregiverlansia di komunitas ALZI terkait keseimbangan dinamis dan risiko jatuh pada lansia. Metode yang digunakan pada kegiatan ini dikemas dengan edukasi (ceramah) dengan media materi menggunakan power point dan masing-masing peserta melakukan demonstrasi.  Evaluasi pengetahuan caregiver lansia tentang keseimbangan dinamis dan risiko jatuh pada lansia dilakukan menggunakan kuesioner baik sebelum maupun sesudah proses penyampaian materi. Berdasarkan pada respon partisipan kegiatan, yaitu pada pre test yang dilakukan sebelum penyuluhan didapatkan hasil 46,4% jawaban iya dan 53,6% jawaban tidak. Sedangkan untuk hasil post test yang dilakukan setelah penyuluhan didapatkan sebanyak 89,5% jawaban iya dan sebanyak 10,5% jawaban tidak. Kesimpulannya terdapat peningkatan pengetahuan sebesar 43,1% pada caregiver lansia  komunitas ALZI Solo Raya mengenai keseimbangan dinamis dan risiko jatuh pada lansia. Edukasi dan penyuluhan risiko jatuh dan keseimbangan dinamis memiliki dampak yang baik dalam membantu meningkatkan pengetahuan caregiver dikomunitas ALZI Solo Raya terkait keseimbangan dinamis dan risiko jatuh pada lansia.

Abstract

Chronic low back pain et causa spondylosis and spondylolisthesis is a rare case. One of the physical therapy interventions, in this case, is William's flexion exercises that are proven to relieve pain and increase range of motion. The purpose of this case report is to show William's flexion exercise can maintain or even reduce signs and symptoms of the patient’s condition, increase functional activity measured by Oswestry Disability Index. Our patient is a 56-years-old-woman with Chronis low back pain et causa spondylosis and spondylolisthesis grade 1 condition after 10 treatments have changed the walking distance caused by William's flexion exercise.

Abstract

Introduction: Carpal tunnel syndrome (CTS) is the most common entrapment neuropathy of the upper extremity caused by compression of the median nerve with an overall prevalence in the general population of 5.3% for women and 2.1% for men. In general, CTS will cause pain, neurological symptoms, and decreased functional ability and can interfere with daily activities. Treatment of carpal tunnel syndrome (CTS) can be done by giving a physiotherapy program, one of which is exercise. The purpose of this study was to determine the effectiveness of the combination of tendon and nerve gliding exercise with neurodynamic mobilization on increasing functional activity in patients with Carpal Tunnel Syndrome (CTS). Case Presentation: A 19-year-old woman came with a diagnosis of Carpal Tunnel Syndrome Dextra. She had a tingling sensation in his 1,2,3 and 4th half finger with carpal compression and a positive Phalen test. The patient was given a combination of tendon and nerve gliding exercise with neurodynamic mobilization for 6 weeks face-to-face and was evaluated. Management & Outcome: The patient showed good adherence to the program and achieved the desired goal within 6 weeks of therapy. The patient showed improvement (in pain, range of motion, and hand function. Tendon and nerve gliding exercise combined with neurodynamic mobilization was effective in improving clinical symptoms in patients with CTS. Discussion: Several recent pieces of literature state nerve and tendon gliding exercise as an alternative intervention in conservative management of CTS. Nerve and tendon gliding exercise is a joint and tendon mobilization exercise program that can be applied by performing several movements of the hands and wrists. This case report combines neurodynamic mobilization. This Exercise Program was carried out for 6 sessions and after 6 weeks of close monitoring and guidance of a combined training program. Overall, the results of this case report for CTS-related disorders and clinical symptoms showed significant results for improving hand function in CTS patients. Conclusions: A combination of tendon and nerve gliding exercise with neurodynamic mobilization is effective for improving hand function in CTS patients.

Abstract

Introduction: Stroke is one of the health problems that causes disability. According to the World Stroke Health Organization, there are 13.7 million new cases of stroke that cause a decrease in functional ability every year. Physiotherapy interventions based on the Bobath Concept are one of the interventions that are widely used to treat disability problems in post-stroke patients, although the results of these interventions may vary. A study is needed in the management of handling disability in post-stroke using the Bobath Concept, in order to improve functional abilities. This aim can be achieved through exercises that refer to postural improvement, movement analysis, selective movement and the role of sensory information to achieve functional targets. Case Presentation: A 65-year-old man was diagnosed with a right ischemic stroke in December 2020 with a condition of hemiparesis on his left side of the body, 1 week after that the patient was referred for physiotherapy at the Halmahera Medika Main Clinic. The patient reported having hypercholesterolemia. During the initial stroke, the patient feels a cold and limp sensation on the side of the lesion. The results of the examination showed that the patient had impaired postural control, decreased selective movement ability, which led to a decrease in functional ability. Management and Outcome: Patients undergo a therapy program every 3 times a week and each session lasts 40-50 minutes. Physiotherapy with the Bobath concept includes facilitation to activate postural muscles and to suppress the synergistic pattern of stroke, pelvic mobilization, hip mobilization, active assisted exercise, positioning how to sleep to sitting, balance and coordination exercises, walking exercises and facilitation of weight bearing. At the end of the study the patient experienced an increase in functional ability,especially transfer and walking catagory. From the results we can state that there is an increase in the patient's functional status based on the Barthel Index, especially in the ability to transfer and walk on a flat road. Discussion: Two articles have been selected to support and compare with this study, the findings of which are in line with the research of Besios et al (2019), and Mikołajewska (2013). Base on the functional movement analysis and response to facilitation, which is part of the assessment, it shows that the given exercises improve postural orientation and posture stabilization in sitting, standing, and walking. This is the reason how the functional abilities in our patients developed after the intervention especially the transfer and ambulation abilities of walking. Conclusion: The conclusion of this case report is that there is an increase in functional ability, especially the patient's ability to transfer or ambulate independently on one of the Barthel index items, several other items also experienced an increase although not as much as an increase in transfer ambulation.

Abstract

Introduction: Describe clinical reports of pre- and postneurofunctional interventions in cases of agenesis of the corpus callosum. Clinical case: The baby was born spontaneously with a weight of 3100 g, 40 weeks of gestation and required a stimulus of applause for 10s until finally the baby cried for the first time. The baby was born with the thumb congenitally entwined bilaterally. At the initial evaluation, the child has fluctuating tone in the upper and lower extremities. Spasm in the upper trapezius, tibialis anterior and facial muscles (frontal, masseter, obricularis oris), very weak central muscles and hypotonia of the upper and lower extremities. The goals of neurofunctional intervention are to improve motor skills, improve head control, improve core ability in sitting position, and readiness to crawl. the child did not show any orthopedic deformities, but there was limited range of motion and muscle weakness and changes. Conclusion: Functional focused motor interventions proved to be effective, as GMFM showed an improvement in gross motor function after a short intervention period from 31 points to 48 points in the total dimension B.

Abstract

Ischialgia/sciatic pain adalah kondisi dimana pasien mengalami nyeri atau paresthesia pada saraf skiatik atau akar saraf lumbosacral.Tujuan yang ingin di capai yakni untuk mengetahui manfaat dari Trans Electrical Nerve Stimulation dan Neural mobilization untuk mengurangi nyeri pada ishialgia/sciatic pain dextra. Hasil yang di peroleh setelah melakukan terapi sebanyak 5 kali pertemuan terdapat penurunan nyeri tekan pada ischiadicus dextra T1: 4 menjadi T5: 2, adanya penurunan paresthesia pada n.ischiadicus dextra T1: 6 menjadi T5: 4.Untuk peningkatan kemampuan aktivitas fugsional didapatkan T1: 40% menjadi T5: 30%. Pemberian modalitas Transcutaneus Electrical Nerve Stimulation dan neural mobilization exercise dapat mengurangi nyeri dan paresthesia n.ischiadicus serta meningkatkan kemampuan aktivitas fungsional pada kasus ishialgia/sciatic pain dextra.

Abstract

Introduction: Bell's palsy is a weakness or paralysis of the peripheral facial nerves, is acute, and the cause is unknown (idiopathic). In Indonesia, the exact incidence of Bell's palsy is difficult to determine. Data collected from 4 hospitals in Indonesia in 2010 found that the frequency of Bell's palsy was 19.55% of all neuropathy cases and the highest was at the age of 21-30 years. Problems that arise in right Bell's palsy are facial asymmetry, stiffness and thickness on the affected side of the face, decreased facial muscle strength on the injured side, the potential for tissue spasm and adhesions, and the potential for irritation of the eye on the injured side. Case Presentation: A 23-year-old woman with the status of a student was referred to medical rehabilitation with complaints of slurred speech during a speech, lack of right eyebrow movement, unconscious right eye blinking, inability to chew food, forehead wrinkles, and inability to whistle. Management and Outcome: The Exercise Program is given three times a week for two weeks according to a doctor's referral and each session lasts 30-45 minutes. A home program is recommended to be done every day with a dose of twice a day and each movement is repeated five times. Results were measured using the Ugo Fisch for facial function. After six treatment sessions, the patient showed the improvement are increase in the score on the frown expression by 4 points from 3 points to 7 points and the smiling expression increased by 12 points from 9 points to 21 points. Discussion: Physiotherapy measures in the form of massage and mirror exercise have been given to these patients. massage has a positive effect in improving muscle function and relaxation to improve blood circulation, thereby reducing tension, anxiety and stress. Mirror exercise is a therapeutic intervention in which a mirror is used to convey visual stimuli to the brain by observing the unaffected body part when the individual performs a series of movements. Conclusion: Through our research and previous research, we concluded that massage and mirror exercise can improve the functional ability of facial muscles.

Abstract

Introduction : The Galeazzi dislocation fracture is a unique injury that involves a fracture of the radial diaphysis, together with a dislocation of the distal radioulnar joint (DRUJ). Galeazzi fractures have poor stability due to disruption of the DRUJ (distal radial ulnar joint) and disruption of the interosseous membrane (IOM), the triangular fibrocartilage complex (TFCC) which acts as the main stabilizer of the DRUJ and the dorsal and volar radioulnar ligaments. Case Presentation : A 58-year-old patient had a chief complaint of limited movement in dorsi flexion and palmar flexion of the left wrist. Management and Outcome : In this case, significant changes were found in muscle strength, and an increase in wrist ROM, but no change in functional ability was evident. Discussion : Early mobilization has been shown to maintain physiological viscoelasticity and connective tissue homeostasis. Early movement up to 1-3 weeks after injury appears to significantly improve bone healing at the site of long bone fracture. Controlled movement is essential to avoid unwanted changes resulting from immobilization and to maintain normal homeostasis and connective tissue viscoelasticity. Conclusion : The role of physiotherapy can reduce the adverse effects of immobilization and help the patient return to pre-fracture function. Physiotherapy management post Galeazzi fracture for 4 times intervention with evaluation done once a week 2 times to see the development of joint range of motion and functional activity in the left wrist, was not seen significantly.

Abstract

Introduction : Previous studies have shown that preoperative management of ACL injuries performed at least 3 weeks before surgery can improve postoperative recovery, prevent knee joint effusion, can be fully active and passive range-of-motion, and increase quadriceps muscle strength with strengthening exercises. Case Presentation : A 19-year-old man as an athlete complains of pain in the right knee. The patient reported that the onset of the pain complaint was that in March the patient fell on the stairs with his feet sideways out. After the incident the patient felt pain, swelling and bruising in the right knee area. The results of Magnetic Resonance Imaging (MRI) carried out on May 20, 2021 showed a complete Anterior Cruciate Ligament (ACL) rupture. Management and Outcome : Ultrasound and strengthening exercise with the aim of increasing and maintaining the muscle strength of the lower limbs in preparation before surgery, reducing pain during walking activities and preventing further back pain. Evaluation of pain using VAS and muscle strength using MMT. Discussion : In this case, prior to surgery or reconstruction, a pre-operative rehabilitation program for ACL injuries was given with the aim of focusing on quadriceps strength, reducing pain for normal gait and ADL. Conclusion : Pre-operative physiotherapy management of ACL injuries found an increase in muscle strength in the quadriceps and hamstring muscles. But in pain management there is no reduction in pain during walking activities.

Abstract

The growth and development of children aged 0-6 months is an important stage to achieve optimal quality of life. Parents have an important role in stimulating child growth and development early on in child growth and development. Problems that occur in children of this age, one of which is gross motor development due to parents' lack of understanding of child development. Education in the form of health literacy discusses growth and development and how to stimulate according to age so that parents can apply what has been taught. The aim is to determine changes in the level of understanding of parents when given growth and development education about gross motor development of children aged 0-6 months. This study used a quantitative research design with a type of quay experimental research, a one- group pre-test-post-test approach. The number of samples was 44 respondents with the purposive sampling method. The variables of this study are independent, namely parents' understanding of child growth and development measured by questionnaires, and dependent variables, namely gross motor development of children aged 0-6 months measured by ASQ (Ages and stages questionnaire). Data analysis techniques used were univariate test, normality test, and effect test. The effect test using Wilcoxon showed the sig result (2-tailed) was 0.000 <0.05, meaning there is a significant effect. It can be concluded that there is an effect of growth and development education on parents' level of understanding about the gross motor development of children aged 0-6 months.
