Abstract

ABSTRAKSelama 4 tahun terakhir, di Rumah Sakit Islam Nahdlatul Ulama Demak (RSI NU Demak) telah terjadi penurunan BOR dari 71% pada tahun 2009 menjadi 49% pada tahun 2012. Penurunan terjadi akibat rujukan ke RS lain (76,5% pada tahun 2012) karena tidak tersedianya ICU. Saat ini di Kabupaten Demak baru tersedia 10 TT ICU dari kebutuhan ideal minimal 22 TT ICU. Tujuan penelitian ini adalah melakukan studi kelayakan pendiran ICU RSI NU Demak ditinjau dari aspek manajemen SDM dan aspek pasar. Metode kualitatif dengan indepth interview digunakan untuk menjelaskan aspek manajemen SDM dan aspek pasar terkait rujukan. Informan utama aspek manajemen SDM adalah Ketua Yayasan, manajemen RS dan informan triangulasi dokter spesialis anasthesi dan dokter spesialis penyakit dalam.Informan utama aspek pasar terkait rujukan medis adalah 10 dokter umum yang berpraktek di sekitar RSI NU Demak . Metode kuantitatif melalui wawancara kepada 30 pasien dan atau keluarga di Poliklinik Penyakit Dalam dan Bedah RSI NU Demak dengan kuesioner terstruktur digunakan untuk menghitung ATP dan WTP. Studi banding dilakukan di RSUD Sunan Kalijaga dan RS Pelita Anugerah Demak. Pengolahan data kualitatif dilakukan dengan analsis isi (content analysis) dan kuantitatif secara deskriptif. Hasil penelitian aspek manajemen SDM menunjukkan jenis tenaga kesehatan sudah memenuhi, jumlahnya memerlukan penambahan 13 perawat. Semua tenaga kesehatan belum memiliki sertifikat pelatihan bantuan hidup lanjut dan sertifikat ICU. Pada aspek pasar, pendirian ICU mendapat dukungan dari dokter di sekitar RSI NU Demak dalam hal rujukan medis. Hasil ATP 1(kemampuan membayar terendah) sebesar Rp. 93.225,00 dan ATP 2 (kemampuan membayar tertinggi) sebesar Rp. 723.000,00 dan WTP sebesar Rp. 300.000,00. Disimpulkan bahwa dari aspek manajemen SDM, ICU di RSI NU Demak belum layak didirikan. Dari aspek pasar, ICU di RSI NU Demak layak didirikan.Kata kunci : Studi Kelayakan, ICU, Rumah SakitABSTRACTIn the past four years, bed occupancy ratio (BOR) in Nahdlatul Ulama Islamic hospital (RSI NU) Demak decreased from 71% in 2009 to 49% in 2012. The decrease was because of referral to other hospitals (76.5% in 2012) due to no intensive care unit (ICU) facility. Currently, the availability of number of beds in the ICU in Demak district was 10; the minimal ideal requirement of ICU beds was 22. Objective of this study was to conduct feasibility study for the establishment of an intensive care unit of RSI NU Demak viewed from human resource and market aspects. Qualitative method with in-depth interview data collection technique was used to explain human resource management aspect and market aspect related to referral. Main informants for human resource management aspect were a director of the foundation and management of the hospital. Triangulation informants were anesthesiologists and internists. Main informants for market aspect related to medical referral were 10 general practitioners who conducted their private practice in surrounding RSI NU Demak. Quantitative method was implemented through in-depth interview to 30 patients and/or family member who visited internal disease and surgery polyclinics of RSI NU Demak. Structured questionnaire was used to calculate ATP and WTP. Benchmarking to Sunan Kalijaga Demak district general hospital (RSUD) and Pelita Anugerah hospital Demak wasconducted. Content analysis was applied in the qualitative data analysis; quantitative data was analyzed descriptively. Results of the study showed that viewed from human resource management aspect: health worker type had fulfilled the expectation; however, additional 13 nurses were needed. All health workers did not have training certificate of life saving for elders and ICU certificate. In the market aspect: the development of ICU of RSI NU Demak obtained medical referral supports from physician surrounding RSI NU Demak. ATP1 (the lowest ability to pay the fee) was Rp. 93,225.00, and ATP2 (the highest ability to pay the payment) was Rp. 723, 000,00, WTP was Rp. 300,000.00. In conclusion, viewed from human resource management, ICU of RSI NU Demak is not feasible to be built. Viewed from market aspect, ICU of RSI NU Demak is feasible to be built.Keywords : feasibility study, ICU, hospital

Abstract

BACKGROUND: Inflammation plays an important role in the atherosclerotic process. High-sensitivity C-reactive-protein (hs-CRP) is commonly used as inflammatory biomarker. It is well known that regular physical activity lowers hs-CRP levels, while prolonged exercise induces hs-CRP elevations. However, the relationship of training and exercise characteristics with hs-CRP levels remains not well elucidated. We evaluated baseline and post-exercise hs-CRP levels and its association with training and exercise characteristics.METHODS: Eighty-eight male endurance cyclists were involved. Demographic data, health condition and training characteristics were collected. Baseline and postexercise blood-samples were collected to determine hsCRP concentrations. A hs-CRP cut-off point of 3 mg/L was used. Blood-cell count and biochemical parameters were measured at baseline. Heart rate (HR) was measured during exercise.RESULTS: Cyclists performed 7.3 hours (interquartilerange (IQR) = 5.4-7.5) of endurance exercise at intensity of 81.8 % (IQR = 74.9-85.8). Cyclists with baseline hsCRP ≥ 3 mg/L reported higher body mass, body mass index (BMI), waist-circumference and total-cholesterol. An increase in hs-CRP was following endurance exercise. Cyclists with any elevation of hs-CRP reported a higher BMI, HR during exercise and exercise intensity. Binary logistic regression analysis showed BMI (OR = 1.24, 95% CI = 1.04-1.48) and cycling distance (OR = 0.22, 95% CI = 0.06-0.76) were associated with post-exercise hs-CRP elevations.CONCLUSION: Body mass, BMI, waist-circumference, total- and HDL-cholesterol are associated with baseline hsCRP, whereas BMI and cycling distance were associated with hs-CRP elevations. These findings suggest that anthropometry parameters and lipid levels attributed to baseline hs-CRP, while anthropometry parameters and cycling intensity attributed to post-exercise hs-CRP elevations.KEYWORDS: C-reactive-protein, exercise, endurancecycling, inflammation, acute-phase-response

Abstract

Latar Belakang: Penyakit kusta merupakan masalah di Indonesia yang menempati urutan ke-3 di dunia. Anak dari pasien kusta mempunyai risiko tinggi tertular penyakit kusta. Kadar antibodi IgM anti Phenolic Glicolipid-1 (PGL-1) dapat digunakan untuk mendiagnosis kusta subklinik. Tujuan: Mengevaluasi hubungan lingkungan fisik dan nonfisik rumah dengan kadar antibodi IgM anti-PGL-1 pada anak dari pasien kusta di perkampungan kusta Jepara. Metode: Penelitian observasional analitik potong lintang dengan subjek penelitian 41 anak dari pasien kusta yang berusia 6-14 tahun dan metode consecutive sampling. Pengumpulan data mengunakan kuesioner terstruktur untuk menilai intensitas kontak dan lama kontak, mengukur kelembapan, pencahayaan, suhu dan kepadatan rumah, dan pemeriksaan sampel darah untuk diperiksa kadar antibodi IgM anti-PGL-1 secara Enzyme Linked Immuno Assay (ELISA). Analisis data mengunakan korelasi rank Spearman dan Fishers’s Exact test. Hasil: Terdapat 10 anak (24,4%) yang mengalami kusta subklinik (kadar antibodi IgM anti-PGL-1 ≥ 600 m/ML). Terdapat hubungan lingkungan fisik rumah: kelembapan (r=-0,366 ; p=0,009) dan suhu rumah (r=0,342 ; p=0,014) dengan kadar antibodi IgM anti-PGL-1. Pencahayaan, kepadatan rumah, ventilasi, jenis lantai, dinding rumah, dan lingkungan nonfisik tidak berhubungan dengan kadar antibodi IgM anti-PGL-1. Simpulan: Faktor lingkungan fisik rumah suhu dan kelembapan berhubungan dengan kadar antibodi IgM anti-PGL-1, semakin tinggi suhu rumah semakin tinggi kadar antibodi IgM anti-PGL-1 dan semakin rendah kelembapan rumah semakin tinggi kadar antibodi IgM anti-PGL-1.

Abstract

Latar belakang : Campak merupakan salah satu penyebab utama kematian pada anak dan balita.Indonesia memiliki angka kesakitan campak kurang lebih 1 juta pertahun dengan 30.000 kematian.Upaya Pemerintah untuk menurunkan angka kesakitan dan kematian akibat campak dengan programvaksinasi campak di Kota Bitung. Cakupan imunisasi campak di Kota Bitung pada tahun 2008-2010menurun berturut-turut 84,48%; 82,01%; 79,02% dan di bawah target nasional yaitu 90%.Tujuan : Untuk mengetahui faktor-faktor yang berhubungan dengan kinerja (performance) petugasimunisasi puskesmas dalam melaksanakan pelayanan imunisasi campak di kota Bitung, ProvinsiSulawesi Utara.Metode : Penelitian ini adalah penelitian observasional dengan pendekatan Cross Sectional.Pengumpulan data menggunakan kuesioner dan observasi. Populasi dan sampel dalam penelitian iniadalah seluruh petugas imunisasi Puskesmas di Kota Bitung sebanyak 49 orang. Analisis datamenggunakan uji Rank Spearman dan uji Regresi Logistik Ganda.Hasil Penelitian : Menunjukan kinerja 73,5% petugas dalam pelayanan imunisasi campak sudahbaik. Pengetahuan tentang imunisasi baik pada 57,1% petugas. Sarana dan prasarana masih kurangbagi 55,1% petugas. Persepsi beban kerja 51% petugas dalam pelayanan imunisasi campak adalahringan. Persepsi 57,1% petugas terhadap kepemimpinan kepala Puskesmas baik. Persepsi 55,1%petugas terhadap supervisi yang dilakukan masih kurang. Persepsi 51% petugas tentang kompensasibaik dan motivasi 53,1% petugas baik. Hasil analisis bivariat menunjukan bahwa ada hubungan antarafaktor pengetahuan, sarana prasarana, persepsi beban kerja, persepsi kepemimpinan, persepsisupervisi, persepsi kompensasi dan motivasi dengan kinerja petugas dalam pelayanan imunisasicampak di Kota Bitung. Hasil analisis multivariat menunjukkan faktor yang paling kuat berhubungandengan kinerja petugas dalam pelayanan imunisasi campak adalah persepsi kompensasi(Exp.B=3,856).Simpulan : Faktor yang berhubungan dengan kinerja petugas dalam pelayanan imunisasi campakadalah pengetahuan, sarana prasarana, persepsi beban kerja, persepsi kepemimpinan, persepsisupervisi, persepsi kompensasi dan motivasi, yang berhubungan paling kuat adalah persepsikompensasi.

Abstract

Infection prevention practices by health workers was still selectively done. ln Kulon Progo district 44.4% of infant mortality was caused by sepsis, 9% by post-delivery fever complication. The majority of delivery assistance (59 %) was done by private Practice Midwife (BPS). Almost a half (47.8%) of BPS has Delima midwives predicate. They should do their work with service of excellence. A preliminary study showed that Delima midwives did not take into account infection prevention and they felt that infection prevention was annoying. The study objective was to identify infection preventive action difference between Delima and non Delima midwives in the delivery care. This was an observational analytic and comparative study with cross sectional approach. Study population was all 90 BPS in Kulon Progo district. Study subjects consist of 34 Delima midwives and 34 BPS non Delima who fulfilled inclusions and exclusion criteria. Data were collected through survey using questionnaire and observation using observation sheet. Results of study showed that Delima midwives had higher value on eight examined variables than non Delima BPS. Result of a difference test showed that there was a significant difference in the implementation of infection prevention (p = 0.003), knowledge (p = 0.037), attitude (p = 0.03), patient satisfaction perceptions (p = 0.016), perception on workload (p = 0.027), support from organization profession (p = 0.012). There was no significant difference in infection preventive training (p = 0.169) and midwives’ motivation (p = 0.325). Result of bivariate test with age stratification showed that two variables namely implementation of infection prevention and patient satisfaction perceptions were not significantly different in both elder age group and young age group. Recommendation for lBl is to establish standard procedure of infection prevention in the delivery care. All members of lBI should obey this standard procedure. lBl should develop Delima midwives’ program to improve BPS service. Recommendations for district health office are to facilitate training implementation on the management of infection prevention, to collaborate with IBI to do supervision to BPS focusing on important of infection prevention especially in the delivery care.

Abstract

Background: Postpartum hemorrhage (PPH) remains a leading cause of maternal morbidity and mortality in both developed and developing countries. To prevent postpartum hemorrhage, knowledge of the risk factors were needed.Method: Methos of the research is a case control study. Study population were hospitalized mothers who delivered in three hospitals on January 2015 – July 2016. The subjects were 55 patients who suffered from PPH (as cases) and 55 patients who were not suffered from PPH (as control). Sampling method for cases was consecutive sampling and purposive for controls. Data were analysed by Odds Ratio calculation and logistic regression.Results: The risks factor significantly for PPH are midwife as birth attendant (OR=7.10; 95% CI 2.2 –22.81; p=0.001), poor of obstetric history (OR=5.37; 95%CI 1.53–18.86; p=0.009), pregnancy interval (< 2 years or > 5 years) (OR =4.04; 95%CI 1.48 –11.07; p= 0,007), anaemia trimester III (OR =3,58; 95%CI 1.23-10.43; p=0.019), history of abortion (OR=4.93; 95% CI 1.20-20.31; p=0,027).Conclusion: The risk factors for PPH were midwife as birth attendant, poor of obstetric history, pregnancy interval (<2 years and >5 years), anaemia trimester III, and history of abortion.

Abstract

The dietary acid load was associated with metabolic syndrome and mental health among obese college studentsBackground: The prevalence of metabolic syndrome has been increasing every year in the young population. The western diet is one of the causes that makes excess acid in the body called a dietary acid load. On the other hand, the high dietary acid load was associated with decreased mental health status.Objective: To examine the association between dietary acid load and the risk of metabolic syndrome and mental health.Methods: This was a cross-sectional study conducted with 115 female students in Semarang aged 18-22 years. The dietary acid load was evaluated through a potential renal acid load (PRAL), the risk score for metabolic syndrome was calculated by Metabolic syndrome risk score. Mental health status was assessed through levels of self-esteem and levels of anxiety. The level of self-esteem was measured by the Rosenberg Self-Esteem Scale questionnaire, while the level of anxiety was measured by the State Anxiety Inventory questionnaire. Multiple linear regression models with adjustment for a potential cofounding variable were used to evaluate the association between dietary acid load with metabolic syndrome scores, self-esteem scores, and anxiety scores.Results: The mean PRAL score was 6.11 ± 9.72 mEq/day indicated the intake of the subjects was the dietary acid load. 13% of subjects had metabolic syndrome, and 87% had a pre-metabolic syndrome. 36.5% of the subjects have low self-esteem scores. The anxiety level score calculation shows that 59.1% of the subjects are at high risk of anxiety. Pearson test showed that PRAL scores were correlated with anxiety scores (r=0,669; p<0,001). The linear regression test showed that PRAL scores were correlated with metabolic syndrome scores (β=0.899; p<0.001) and self-esteem scores (β =-0.069; p=0.047).Conclusions: High dietary acid load may be a risk factor related to the development of metabolic disorders and decreased mental health status in females with obesity.

Abstract

Rumah Sakit Brebes belum menggunakan sistem informasi dalam layanan pemeriksaan medis sementara itu hanya digunakan di bagian pendaftaran. Selain itu, hasil pemeriksaan medis masih diproses secara manual, sehingga dapat diselesaikan dalam waktu 3 hari hingga 1 minggu. Penelitian ini bertujuan untuk merancang sistem informasi pemeriksaan medis untuk mempercepat layanan pemeriksaan medis di Rumah Sakit Brebes. Penelitian ini adalah penelitian tindakan dengan menggunakan pendekatan kualitatif. Ini membantu untuk melanjutkan analisis masalah dan kebutuhan pengguna. Desain sistem informasi diselesaikan dengan metode air terjun melalui 4 langkah, seperti analisis dan definisi kebutuhan, desain sistem, implementasi dan pengujian unit, serta integrasi dan pengujian sistem. Pengujian sistem menggunakan pengujian Black Box. Bahasa pemrograman yang digunakan dalam penelitian ini adalah versi php yang>> = 5.6 dan kerangka depan Codeigniter. Sementara, antarmuka menggunakan HTML, CSS dengan Framework Bootstrap, Javascript dengan database MySQL. Penelitian ini melibatkan 20 orang yang mengelola sistem informasi pemeriksaan medis di Rumah Sakit Brebes. Hasilnya dijelaskan dalam bentuk sistem informasi pemeriksaan medis, yang mengintegrasikan proses dan hasil pemeriksaan medis, serta laporan pemeriksaan medis keseluruhan pasien. Kualitas informasi tentang pengembangan sistem meningkat dalam semua aspek, yaitu ketersediaan, kenyamanan, kesesuaian, kelengkapan dan ketepatan waktu, dengan skor rata-rata tertimbang sebelum pengembangan 1,91 dan setelah pengembangan 3,16, sehingga terjadi peningkatan 1,25. Pengujian Black Box di entri akhir dan administrasi bagian pemeriksaan kesehatan menunjukkan tindakan pengguna dan respons sistem dilakukan dengan baik dan sesuai dengan tujuan pengguna sistem. Sistem informasi pemeriksaan medis ini dapat mempercepat layanan pemeriksaan medis. Abstract

Maternity class which aimed to decrease maternal mortality rates, has been implemented in Kendal District since 2010. Maternity class in Cepiring (one of the primary health care center in Kendal) has been held in 15 places but the rates of postpartum visit (KF3) still low in 2014. The research objective was to analyze the association between the maternity class attendance and knowledge, attitude and practice of postpartum careamong post partum mothers inCepiring Primary Health Care Center working area. This study was an explanatory study with a cross-sectional approach. The population was 204 postpartum mothers who lived in Cepiring during January-March 2016. Among them 66 subjects were chosen through purposive sampling method. Data were analyzed by Chi Square tests. The results showed that precentage of postpartum mother’s with low konwledge was 54.5%, postpartum mother’s with good attitude was 68.2%. postpartum mother’s with good practice was 57.6%. There were associations between the maternity class attendance and knowledge (p=0.002) and attitude (p=0.013). There was no association between the maternity class attendance and  postpartum care practice (p=0.646). It is concluded that the maternity class attendance was associated to knowledge and attitude in postpartum care. It was suggested that the midwives motivate the postpartum mothers to attend the maternity class and apply their knowledge of post partum care in practice.

Abstract

Dysmenorrhea often occurs on adolescents resulting in disrupted daily activities, especially learning activities at school. This research’s purpose is to analyze the relationship between anemia, physical activity, calcium and magnesium intake with adoslescents dysmenorrhea’s incidences in  Senior High School 4 Pekalongan. This research uses observational analytic with cross sectional design. Population are 442 adolescents of Senior High School 4 Pekalongan. Subjects are 80 people (5  from each class) chosen by respondents’ willingness to check for hemoglobin levels. Purposive Sampling technique is used. Dysmenorrhea’s incidences and pain degree data conducted by interview and numeric rating scale questionnaires. Anemia measurement conducted by Family Dr® strip test. Physical activity data conducted by recall for two consecutive days. Calcium and magnesium intake data conducted by Food Frequency Questionnaires. Data analyzed by Chi Square test. The results showed that 58.8% subjects suffered from anemia. 52,5% had very light physical activity. 77,5% consumed deficient calcium intake and 67,5% sufficient magnesium intake. There is a relationship between anemia (OR = 8.9; 95% CI: 2.6-30.7; p = 0.0001), very light physical activity (OR = 4.3; 95% CI: 1.3-13, 5; p = 0.019), deficient calcium intake (OR = 10.6; 95% CI: 3.1-35.3; p = 0.0001), and deficient magnesium intake (OR = 5.51; 95% CI: 1,1-26,0; p = 0,039) with the dysmenorrhea’s incidences in adolescents. It is suggested for adolescents to consume foods and beverages which contains calcium, magnesium and iron such as milk, nuts, tofu, tempeh, vegetables, especially spinach, broccoli, kale and doing extracurricular activities at school.
