Abstract

Introduction. The emergence of the Omicron variant during the pandemic had an impact on the effectiveness of the COVID-19 vaccine. The spread of Omicron increased rapidly even in populations that had been vaccinated with booster doses, causing a high incidence of infection. This study aimed to determine the effectiveness and factors related to the effectiveness of the second mRNA booster vaccine as well as antibody levels after the second mRNA booster vaccine in healthcare workers at Cipto Mangunkusumo National General Hospital. Methods. This study used a cohort design. Total sampling of 271 healthcare workers were obtained from previous research and continued with consecutive sampling. Selected subjects were interviewed, and blood samples were taken to measure IgG sRBD antibody levels. The effectiveness of the vaccine was calculated, and bivariate and multivariate analysis was carried out to assess the factors that influenced the effectiveness of the second mRNA booster vaccine. Results. A total of 9 subjects (5.6%) were infected with COVID-19 in the second booster vaccine group and 11 subjects (9.7%) in the first vaccine group. The effectiveness of the vaccine against COVID-19 infection was 42% (CI 95% -37 – 75%), p = 0,215. Age, gender, hypertension, type 2 DM, obesity, smoking, and COVID-19 infection before booster have no relationship to the effectiveness of the second booster mRNA vaccine. In 48 subjects who had their antibody levels checked during the first and second boosters, there was an increase from 4,743 AU/ml (IQR 6,053.3) to 8,532 AU/ml (IQR 8,040.3). Conclusions. The effectiveness of the second mRNA booster vaccine against COVID-19 infection in healthcare workers was 42%. There were no factors related to the effectiveness of the second mRNA booster vaccine. The sRBD antibody levels after the second booster vaccine were higher than the first booster vaccine.

Abstract

The coexistence of depression and type 2 diabetes mellitus (T2DM) can significantly worsen disease prognosis and lower quality of life. Emerging evidence suggests that vitamin D deficiency contributes to the progression of T2DM and is closely associated with the development of depression. The aim of this study was to investigate the effects of cholecalciferol on depression in patients with T2DM, exploring its mechanisms by analyzing its impact on C-peptide, serotonin, and neurotrophin-3 levels. A double-blind, randomized, placebo-controlled clinical trial was conducted at Cipto Mangunkusumo General Hospital, Jakarta, Indonesia, from April 2021 to September 2022. Patients with T2DM and depressive symptoms were randomly assigned to two groups: received 4000 IU of cholecalciferol daily and received a placebo for 12 weeks. Depression was assessed using the Beck Depression Inventory-II (BDI-II) before and 12 weeks after the intervention. The levels of C-peptide, serotonin, and neurotrophin-3 were measured at the end of the fourth week of intervention using the enzyme-linked immunosorbent assay (ELISA) method. Between-group comparisons were made using independent Student t-tests and Mann-Whitney U tests. Paired Student t-tests or Wilcoxon tests were applied for within-group comparisons between pre- and post-intervention. A total of 70 T2DM patients with depression were included in this study, comprising 38 patients in the cholecalciferol group and 32 in the placebo group. C-peptide levels increased significantly in the cholecalciferol group compared to the placebo group (p=0.006). No significant differences were observed in serotonin and NT-3 levels between the cholecalciferol group compared to the placebo group. The cholecalciferol group had a significantly greater reduction in BDI-II scores compared to the placebo group (p<0.001). This trial highlights that taking cholecalciferol might help ease mild to moderate depression symptoms in patients with T2DM by enhancing c-peptide levels, though its effects on serotonin and neurotrophin-3 are still unclear.

Abstract

The incidence of psychosomatic disorders is increasing in Indonesia, and therefore screening instruments that are culturally appropriate for the Indonesian population are needed. The aim of this study was to assess the validity and reliability of the Shatri Sinulingga psychosomatic test (SSPT) questionnaire as a screening instrument for psychosomatic disorders in Indonesia. An analytic descriptive cross-sectional study divided into two stages (questionnaire formulation and distribution through the Psikosom.id application) was conducted in 2023. The validity test was carried out using the product moment technique, and Cronbach's alpha assessment was carried out to evaluate the reliability of the questionnaire. A total of 372 participants were included, with a mean age of 39.24 years old. The SSPT questionnaire scores had strong correlations with the scores of hospital anxiety and depression scale (HADS) (r=0.668; p<0.001) and the Symptom Checklist-90 (SCL-90) (r=0.674; p<0.001); and moderately correlated with the score of fatigue severity scale (FSS) (r=0.505; p<0.001) and Pittsburgh Sleep Quality Index (PSQI) (r=0.492; p<0.001). The correlation coefficient (r) value of each SSPT question ranged from 0.389–0.726, and all r higher than r table with an overall Cronbach’s alpha of 0.744. The SSPT questionnaire's strong predictive performance: had a sensitivity of 75.6% and specificity of 71.4% with an area under the curve (AUC) of 81% (95%CI: 76.6–85.4%; p<0.001) to HADS; sensitivity of 80% and specificity of 58.6% with an AUC of 75.1% (95%CI: 70–80.2%; p<0.001) to FSS; sensitivity of 64% and specificity of 67.5% with an AUC of 71.2% (95%CI:  65.4–77%; p<0.001) to PSQI; and had sensitivity of 78% and specificity of 67.4% with an AUC of 80.9% (95%CI:  76.6–85.3%; p<0.001) to SCL-90. The study highlights that the SSPT questionnaire is valid and reliable to be used as a screening instrument for psychosomatic disorders in Indonesia.

Abstract

Burnout syndrome (BOS) is an occupational phenomenon highly prevalent among healthcare workers, particularly physicians and nurses. Despite its prevalence, no single therapy universally addresses all cases of BOS. The aim of this study was to develop a novel approach to managing BOS through traditional music therapy, evaluated from psychosomatic, neurological, immunological, and endocrine perspectives. The study involved 80 participants who were randomly assigned to either the intervention or control group. The intervention group received traditional music therapy for 10–15 minutes, three times a week, over four weeks. Key outcomes were assessed at weeks 2 and 4. Measurements included the Maslach Burnout Inventory (MBI), heart rate variability (HRV), saliva cortisol, saliva β-endorphin, saliva immunoglobulin A (IgA), and serum FOXP3. Instruments included the MBI-HSS questionnaire and HRV measuring devices. Over four weeks, significant improvements were observed in the MBI scores (p=0.001), HRV (p=0.001), and FOXP3 delta (p=0.035) in the intervention group compared to the control group. However, no significant differences were found for cortisol, β-endorphin, or IgA. These findings suggest that traditional music therapy positively impacts the psychological, neurological, and immunological aspects of BOS and potentially influences immunological and endocrine responses. Future research should explore the effects of longer intervention durations, test varying doses, and examine the combination of music therapy with other non-pharmacological treatments to enhance its therapeutic potential.

Abstract

Introduction: Heart rate variability (HRV) is the fluctuation in the interval between two heartbeats. Decreased HRV indicates less flexibility of the autonomic nervous system (ANS) to physiological changes and is associated with poorer disease progression and mortality in several diseases. We reviewed conditions where HRV can be utilized as a predictive tool for palliative patients. Methods: We performed a comprehensive search in PubMed, ScienceDirect, and Google Scholar to find articles related to HRV and prognosis for cancer-related disorders, cardiovascular disease, and sepsis. 41 relevant articles were selected and reviewed. Results: ANS changes due to chronic cancer pain and metastastic bone cancer can be detected by HRV analysis and increased vagal activity is related with longer survival on advanced cancer patients. Lower HF is a risk factor for shorter life expectancy of metastatic or recurrent breast cancer. SDNN and RMSSD reduction correlates with poorer survival in NSCLC and metastatic brain cancer. Increased risk of stroke was observed in patients with lower RMSSD and SDNN. Reductions of both scores and LF were associated with two times increased risk of myocardial infarction and higher mortality one year after stroke. A decrease in HRV is related to mortality but is not specific to cardiovascular disease. SDNN and HF are HRV parameters that are most consistently correlated with sepsis and can predict mortality within 30 days of hospitalization. Conclusion: HRV analysis helps in assessing the prognosis of diseases. Typically, reduced HRV is linked to worse outcomes and increased mortality rates in conditions such as cancer, cardiovascular disease, and sepsis.

Abstract

Chronic Kidney Disease (CKD) is a condition of kidney damage with a decrease in glomerulus filtration rate of 60 ml/min/1.73 m3 for a duration of more than three months. CKD is closely related to the patient’s quality of life because the patient must carry out various lifestyle restrictions to slow the progression of kidney damage. Lifestyle restrictions often cause problems in the psychosocial field that increase the severity of symptoms and decrease kidney function faster. This evidence-based case review was compiled to determine the effectiveness of hypnotherapy in improving the quality of life of CKD patients. Literature searches were conducted in four databases, namely PubMed, Cochrane, EMBASE, and Scopus. There are three literatures that fit the eligibility criteria. The critical review was conducted using guidelines created by the Centre for Evidence-Based Medicine, University of Oxford. Hypnotherapy consistently shows improved quality of life in the form of decreased symptoms of anxiety, depression, drowsiness, fatigue, and improved social functioning and relationships with family. Hypnotherapy can be considered a complementary therapy in the management of CKD patients with minimal side effects by considering the patient’s accessibility and preferences.

Abstract

BACKGROUND The rising incidence of thyroid cancer in Indonesia has become a public health concern. This study was aimed to evaluate the validity of the thyroid cancer-quality of life-Indonesian version (ThyCa-QoL-ID). METHODS This cross-sectional study involved translating the original ThyCa-QoL questionnaire from English to Indonesian (T1), which was then back-translated into English (T2). The T2 questionnaire was compared with the original ThyCa-QoL questionnaire to refine T1, resulting in T3. The T3 questionnaire underwent cognitive and transcultural validation through individual assessment and focus group discussions with 10 doctors and 10 nurses at Cipto Mangunkusumo Hospital and St. Carolus Hospital, and 5 patients with thyroid cancer from St. Carolus Hospital. This process ensured cultural, language, and literacy adaptation processes for patients across different age groups. RESULTS The ThyCa-QoL questionnaire demonstrated validity, with all questions showing correlation calculation (r) above the r table. Test-retest reliability was measured by calculating the intraclass correlation coefficient for each question on the ThyCa-QoL questionnaire. The reliability test can be seen in the value of Cronbach’s alpha (0.915), exceeding the standard Cronbach’s alpha value standard (0.6). CONCLUSIONS The ThyCa-QoL-ID was confirmed as a valid and reliable tool for assessing the QoL of patients with thyroid cancer in Indonesia.

Abstract

Background: Inflammatory Bowel Disease (IBD), which is chronic and often recurrent, causes psychological changes in patients. This puts IBD patients at a higher risk of developing various mental disorders, especially anxiety and depression. IBD patients who had symptoms of anxiety or depression tended to show a lower quality of life than patients without those symptoms.Methods: This research uses a cross-sectional design. Data collection was carried out using primary data obtained through interviews with IBD patients who were undergoing outpatient treatment from October to December 2023. Anxiety and depression were measured using the Hospital Anxiety Depression Scale (HADS) and quality of life for IBD patients was measured using the Inflammatory Bowel Disease Questionnaire 9 (IBDQ-9). These questionnaires have been translated into Indonesian and validated. The research sample is an accessible population that has passed the inclusion and exclusion criteria using unpaired numerical analytical sample calculations. The sample used in this research was 172 subjects.Results: In the multivariate analysis, after controlling for confounding variables, the fully adjusted prevalence ratio (PR) for anxiety symptoms was 1.182 (CI: 1.061–1.317, p = 0.002). Similarly, the fully adjusted PR for depression symptoms was 1.221 (CI: 1.075–1.386, p = 0.002).Conclusion: Anxiety and depression are more strongly associated with a decline in the quality of life of IBD patients based on the IBDQ-9 after controlling for confounding variables.Keywords: Anxiety, depression, IBD, quality of life

Abstract

Introduction. Acute kidney injury (AKI) is associated with higher mortality rate in COVID-19 patients due to inflammation and immune dysregulation. This study aimed to correlate serum levels of IL-6, IL-10, and TNF-α with serum creatinine changes and their roles to predict incidence of acute kidney injury (AKI) in patients with moderate and severe COVID-19. Methods. This prospective cohort study was conducted among patients with moderate to severe COVID-19 in Pertamina Central Hospital Jakarta, Indonesia during November 2020 to January 2021. All serum levels of cytokines (IL-6, IL-10, and TNF-α) and creatinine were collected on the first day and seventh day of hospitalization. They might be collected earlier if the patients died or discharged early. AKI was defined as deterioration in serum creatinine or urine output based on Kidney Disease Improving Global Outcomes (KDIGO) guideline. The correlation between cytokine and creatinine serum level changes were analyzed using Spearman test. Receiver operator characteristic curve was calculated to explore predictive roles of cytokines on AKI incidence. Results. A total of 43 patients were included in the study, with a mean age of 59.3 years (SD 12.59), and the majority were male (74%). The incidence of AKI was 7%. Serum creatinine changes were correlated with serum levels changes IL-10 (r= -0.343; p=0.024), but not for IL-6 (r=-0.198; p=0.202) and TNF-α (r=-0.129, p=0.409). Meanwhile, serum TNF-α level on the first day was able to predict AKI incidence on the seventh day of hospitalization (AUC 85%; p=0.045; 95% CI=0.737 to 0.963). Conclusions. TNF-α on the first day is potential to be predictor on AKI incidence on the seventh day in moderate and severe COVID-19 patients.

Abstract

Introduction. The elderly population is increasing globally, including in Indonesia, where 10.7% of the population in 2020 were elderly. Insomnia is a common health issue among older adults and has been identified as a predictor of depression. This study aims to identify factors associated with depressive symptoms in elderly outpatients with insomnia. Methods. This cross-sectional study was conducted in 2024 on patients aged ≥60 years at the outpatient unit of Cipto Mangunkusumo Hospital (RSCM), Jakarta, Indonesia, using consecutive sampling to select participants. Elderly patients were screened using the Pittsburgh sleep quality index (PSQI), and those with a score >5 (indicating insomnia) were included as study subjects. Data were collected through interviews, covering sociodemographic characteristics, level of loneliness, duration of insomnia, depressive symptoms [Geriatric Depression Scale–15 (GDS-15) items], functional status [Barthel Activities of Daily Living (B-ADL)], cognitive status [Mini-Mental State Examination (MMSE)], nutritional status [Mini Nutritional Assessment (MNA)], and chronic diseases [Cumulative Illness Rating Scale for Geriatrics (CIRSG) score]. Results. The study included 209 elderly subjects with insomnia, with a mean age of 72.88 (SD 6.98) years, and a depression prevalence of 6.7%. Bivariate analysis showed significant associations between dependency (PR 5.24; 95% CI 1.50 – 18.29), malnutrition (PR 11.54; 95% CI 4.77 – 27.92), and chronic disease with a CIRSG score ≥9 (PR 4.15; 95% CI 1.18 – 14.50) and depressive symptoms in elderly patients with insomnia. No significant associations were found between sociodemographic factors, loneliness, duration of insomnia, or cognitive status and depression. Multivariate logistic regression analysis revealed a statistically significant association between malnutrition and depressive symptoms (p < 0.001). Conclusions. There is a significant association between malnutrition and depressive symptoms in elderly outpatients with insomnia. Further cohort studies are recommended to explore the causal relationship between malnutrition and depression in elderly patients with insomnia.
