Abstract

Background: Sarcopenia has a notable impact on the prognosis of hepatocellular carcinoma (HCC). The prevalence of sarcopenia in patients with HCC is relatively high, with studies reporting rates between 39% and 41.7%. A substantial proportion of individuals diagnosed with HCC also exhibit muscle wasting and reduced muscle function. This study aims to determine the prevalence of sarcopenia in Indonesian patients with HCC (based on the Japanese Socuety of Hepatology (JSH) JSH criteria) and to examine its correlation with poor performance status.Methods: We performed a cross-sectional study of 85 HCC patients between January and October 2021). The skeletal muscle index at L3 (L3 SMI) was measured on CT scans (applying JSH cut-offs for sarcopenia). Clinical data, including Eastern Cooperative Oncology Group (ECOG) performance status, were collected. Bivariate analyses and logistic regression were performed to identify factors associated with sarcopenia.Results: A total of 85 HCC patients (median age: 52 years) were included in the analysis. Sarcopenia was observed in 49.4% of the cohort. Bivariate analysis revealed a statistically significant association between sarcopenia and an ECOG performance status of 2+ (p = 0.003), as well as a MELD-Na score of ≥15 (p = 0.023). In multivariate analysis, poor ECOG-PS remained independently associated with sarcopenia (adjusted OR 4.17; 95% CI 1.50–11.56, p = 0.006).Conclusion: Sarcopenia is highly prevalent among Indonesian HCC patients, and it is strongly associated with poor ECOG performance status.

Abstract

Background: Endoscopic retrograde cholangiopancreatography (ERCP) alone may not be sufficient for treating difficult common bile duct stones. To address this challenge, several other modalities exist, including extracorporeal shock wave lithotripsy (ESWL) and biliary stent placement. This study aims to investigate the effectiveness of ESWL following biliary stent placement, in comparison to biliary stent placement alone , in the treatment of difficult common bile duct stones.Methods: A retrospective cohort study was conducted using medical record data from the Gastrointestinal Endoscopy Center Registry of Dr. Cipto Mangunkusumo General National Hospital. This study evaluated 126 subjects with difficult bile stones, who were divided into two groups: one group received biliary stent placement alone, while the other underwent extracorporeal shock wave lithotripsy (ESWL) following biliary stent placement. The effectiveness of each treatment was assessed based on the rate of complete clearance of bile stones.Results: Of the total participants, 72 underwent ESWL following biliary stent placement, while 54 received biliary stent placement alone. The rate of complete stone clearance was 69.1% in the ESWL group and 64.8% in the stent-only group. However, the addition of ESWL as an adjuvant therapy did not result in a statistically significant improvement (p = 0.703; 95% CI: 0.816–1.351). Normal body mass index was a factor that influences the effectiveness of ESWL after biliary stent placement (p = 0.002).Conclusion: The ESWL procedure as adjuvant therapy following biliary stent placement has the same level of effectiveness as biliary stent placement alone.

Abstract

Background: Inflammatory bowel disease (IBD) is an autoimmune disease that is influenced by food, an important factor in accelerating its clinical disease activity because of intestinal inflammation trough formation of antigen-antibody complex. Food-specific IgG examination can identify the types of person foods consumes that are maybe responsible for disease activity. It is useful in treating IBD without risking malnourishment as it is tailored to the individual immune profile.Method: This is a cross-sectional study involving 113 patients diagnosed with IBD by colonoscopy. Examination of serum IgG specific for 220 types of foods was performed using ELISA and immuno-array techniques. Disease clinical activity was assessed using the Mayo index and Crohn's disease activity index.Results: The highest proportion of dietary IgG in Crohn's disease was peas (100%), barley (97.9%), eggs (95.9%), milk (81.6%), and corn (75.5%); while in ulcerative colitis it was barley (98.4%), peas (96.8%), egg whites (92.2%), corn (82.8%), and prunes (78.1%). In ulcerative colitis, there was a weak negative correlation between cashew nuts IgG (r = -0.347; p = 0.041) and chickpeas IgG (r = -0.473; p = 0.017) with clinical disease activity; while in Crohn's disease, a weak positive correlation with disease activity was seen in barley (r = 0.261; p = 0.042).Conclusion: There was a weak negative correlation between cashew and chickpea-specific IgG antibodies with clinical activity of ulcerative colitis, and a weak positive correlation between barley-specific IgG antibodies and Crohn's disease clinical activity.

Abstract

Despite the numerous benefits of palliative care for cancer patients, there have been few studies on palliative care services for terminal cancer patients, particularly near the end of life. This study aimed to evaluate whether there were differences in length of stay and cost of care associated with how early or late a patient received palliative care intervention. Another objective was to compare the length of stay and cost of care of those who received palliative care intervention and those who did not. This study used a cohort retrospective design at Hospital A, Jakarta, Indonesia, from January to December 2019. The diagnosis of terminal cancer was based on medical records. Data on length of stay and costs of care were based on medical records and finance billing. The hospitalized terminal cancer patients (392) were recruited by consecutive sampling. The length of stay and costs of care for patients with advanced cancer who received palliative care consultations were longer and higher than for patients who did not receive them. However, if palliative care consultation is provided early, the increase in length of stay and costs are less.

Abstract

Susceptibility to infection and increasing antibiotic resistance put burn patients at risk of developing infections caused by multidrug-resistant organisms (MDRO). This condition can progress to sepsis, increasing morbidity and mortality. This retrospective cohort study employed the medical record data of patients treated at Public Hospital X in Jakarta, Indonesia, from January 2020 to June 2022. Of 160 subjects, most were aged <60 years (82.5%) and had comorbidities (16.88%). The most common cause of burns was fire (86.25%). The use of medical devices was 90.63%, with a 14-day median length of stay. The most common gram-negative MDRO pathogens were K. pneumoniae (29.91%), Enterobacter sp (22.32%), and Acinetobacter (20.54%); 45% of patients infected with MDRO died. The bivariate analysis found an increased risk of death due to MDRO infection in burn patients (RR 1.103; 95%CI 1.004-1.211, p-value = 0.046). After adjusting for role variables (age, comorbidities, total body surface area, use of medical devices, length of stay) and from multivariate analysis, the confounding variables for MDRO infection and mortality were length of stay and age. MDRO infection increases the mortality rate in burn patients. Mortality in burn patients due to MDRO infection is greater than non-MDRO.

Abstract

Sindrom kelelahan kronis yang juga dikenal sebagai Myalgic Encephalomyelitis (ME), adalah kondisi kronis kompleks yang ditandai oleh kelelahan parah, disertai nyeri muskuloskeletal, gangguan kognitif, dan sistem autonom. Patofisiologi CFS melibatkan inflamasi kronis, disfungsi mitokondria, dan ketidakseimbangan sistem kekebalan, yang ditunjukkan dengan tingginya kadar sitokin pro-inflamasi. Aktivitas sitokin ini berperan dalam munculnya gejala utama seperti kelelahan, nyeri, dan penurunan kognitif. Diagnosis CFS merupakan proses yang kompleks karena didasarkan pada metode eksklusi dan belum adanya biomarker definitif sehingga menyebabkan banyak pasien tidak terdiagnosis. Heart Rate Variability (HRV) telah diajukan sebagai metode non-invasif untuk menilai disfungsi autonom pada CFS, di mana HRV rendah menunjukkan peningkatan risiko kelelahan dan disfungsi parasimpatis. Penggunaan HRV dan sitokin sebagai biomarker potensial menawarkan pendekatan yang lebih terukur dalam mendiagnosis CFS serta memprediksi prognosis. Namun, heterogenitas dalam hasil penelitian terkait peran sitokin menekankan pentingnya konsistensi metodologis di masa mendatang. Studi lanjutan tentang HRV dan inflamasi pada CFS diharapkan dapat berkontribusi pada strategi terapi yang lebih spesifik.

Abstract

BACKGROUND Decreasing the quality of life (QoL) of patients with inflammatory bowel disease (IBD) will increase morbidity and mortality. A valid and reliable instrument is needed to assess the QoL of patients with IBD. This study aimed to analyze the validity and reliability of the Indonesian version of the 9-item Inflammatory Bowel Disease Questionnaire (IBDQ-9). METHODS This cross-sectional study was conducted using the Indonesian version of the IBDQ-9 in adult patients with IBD at the Gastroenterology Outpatient Clinic, Cipto Mangunkusumo Hospital, Jakarta, in November 2022. Patients aged 18 to 59 years who had experienced IBD for at least 2 weeks and provided informed consents were included. The total score of the IBDQ-9 Indonesian version was compared with the 36-item Short-Form Health Survey (SF-36) using the Spearman's correlation test. Reliability tests were examined using Cronbach’s alpha and the intraclass correlation coefficient (ICC). RESULTS A total of 124 participants were included in this study. The Spearman's test showed a high and significant result for the IBDQ-9 Indonesian version and the SF-36 (r = 0.769 and p<0.001). Cronbach’s alpha and ICC coefficient were equal to 0.883. CONCLUSIONS This study provided evidence of the good validity and reliability of the IBDQ-9 Indonesian version for assessing the QoL of patients with IBD in Indonesia.

Abstract

BACKGROUND The prevalence of type 2 diabetes mellitus (T2DM) is increasing and commonly accompanied by comorbidities, such as depression. Vitamin D levels have been associated with T2DM and depression although the mechanism is uncertain. This study aimed to compare vitamin D levels between patients with T2DM with and without depression in the Indonesian community, where such research is rare. METHODS This cross-sectional study was conducted at Cipto Mangunkusumo Hospital. The participants who met the inclusion criteria on an outpatient basis were screened for depression using the Beck Depression Inventory-II (BDI-II) questionnaire and then divided into 2 groups: patients with T2DM with (BDI-II ≥14) and without (BDI-II <14) depression. Both groups were examined for vitamin D levels using the ELISA method, and an analysis of the mean difference between both groups was performed. RESULTS Of 60 patients, 23 (38%) experienced depression. The median vitamin D levels were 21.8 ng/ml (IQR 14.9–26.6) in the depression group and 26.5 ng/ml (IQR 23.96–34.08) in the non-depression group (p = 0.001). After performing multivariate analysis with confounding variables, the adjusted OR of variables (sex, sun exposure score, and body mass index) was 1.123 (95% CI: 1.003–1.259; p = 0.045). CONCLUSIONS Vitamin D levels were significantly lower in patients with T2DM with depression. Future studies should be carried out to determine the benefits of vitamin D supplementation in patients with T2DM with depression and their pathophysiology.

Abstract

Cisplatin has been used extensively as a cancer treatment. Nephrotoxicity, which is assessed by blood urea levels, blood creatinine, and estimated glomerular filtration rate (eGFR), is caused by cisplatin metabolites that build up in the kidneys. Because of these indicators' numerous flaws, optimal biological markers are required. One of the key mediators of inflammatory processes, such as kidney fibrosis and inflammation, is periostin. In cancer patients undergoing high-dose cisplatin therapy, the purpose of this study is to ascertain how urine periostin changes and how it relates to kidney function. This cross-sectional study was carried out at the National Center General Hospital of Cipto Mangunkusumo's medical hematology-oncology outer clinic and medical hematology-oncology ward on the eighth floor starting in November 2019 and ending when the minimum sample was obtained through consecutive sampling. Data was analyzed by IBM SPSS Statistics for Windows version 23.0 based on the research objective. Of the 37 responders, 70.3% were men, 29.7% were between the ages of 41 and 50, 78.4% were married, 59.5% had completed high school, 37.8% were employed, 59.5% had NPC, and 64.9% had a Karnofsky score of 80. Between before and one week following chemotherapy II, the respondents' blood creatinine and urea levels rose. The eGFR value has also decreased. Periostin levels, on the other hand, tended to rise one week following treatment III after declining during chemotherapy I and II (p value>0.05). Urine periostin levels and other kidney function indicators did not significantly correlate (p>0.05), according to the correlation test, and several domains had negative directions. The correlation coefficient values were modest (r = 0.017-0.254). There is a changing of urine periostin level of malignant patients receiving high dose cisplatin therapy which increase after the third chemotherapy. No significant correlation was found between periostin levels and kidney function in malignant patients with high-dose cisplatin therapy.

Abstract

Background: Patients with metastatic colorectal cancer (CRC) have a poor prognosis, with a 5-year survival rate of only 14%. Early detection and early intervention may improve outcome. Both circulating tumour cells (CTC) and serum carcinoembryonic antigen (CEA) have been suggested as diagnostic biomarkers for metastatic CRC. This study explored the performance of CTC and CEA as a tools for the detection of metastatic colorectal cancer in Indonesia.Methods: This study was conducted from December 2024 to April 2025 on metastatic colorectal cancer patients from three hospitals in Jakarta. CTC was analysed using the gradient density method and flow cytometry. CEA was analysed using chemiluminescent microparticle immunoassay.Results: This study recruited 160 patients with colorectal cancer patients of whom, 45% were known to have metastatic colorectal cancer. The median age was 57 (47—66) years and analysis was done at one time point only. The area under the curve (AUC) for CTC, CEA, and the combination of both CTC and CEA in diagnosing metastatic colorectal cancer was 0.579, 0.811, and 0.703, respectively. CTC showed 56.94% sensitivity and 50.00% specificity in detecting metastatic colorectal cancer. Meanwhile, CEA showed higher sensitivity (72.22%) and specificity (72.72%). Combination of both CTC and CEA increased sensitivity to 91.67%, but with a lower specificity of 37.50%. The optimal cut-off for CTC and CEA were 34.5 cells/3 ml and 18.31 ng/ml, respectively.Conclusion: CEA showed a better performance than CTC in diagnosing metastatic colorectal cancer. Combination of CTC and CEA showed promising potential as a valuable surveillance tool for detecting metastasis in colorectal cancers, but not as a primary diagnostic tool.
