Abstract

Background: Both depression and diabetes mellitus have been related to a higher risk of developing coronary heart disease. Inflammation may be important in the pathogenesis of atherothrombosis. There are many markers of inflammation, one of them is high sensitivity-C reactive protein. Objective: To determine the difference of high sensitivity-C reactive protein in the diabetic patients with depression and in the diabetic patients without depression. Methods: This study used cross-sectional design and examined 64 diabetic patients, aged 30-60 years old. The patients were divided into two groups, Le. with and without depression group.We measured depression using the DSM IV. The diagnosis of diabetes mellitus used PERKENI criteria 2006. We measured the serum concentration of CRP with a high sensitivity assay. Other measured factors included gender, marital status, occupation, blood glucose, HbA 1 c, body mass index, smoking and duration of diabetes. The association between hyperglycemia and hs-CRP were also studied. Results: There were 32 diabetic patients with depression and 32 diabetic patients without depression. Body mass index were corelated with depression in diabetic patients (p = 0.008) but the other factors such as gender, age, marital status, occupation, blood glucose, HbA 1 c, duration of diabetes were not corellated with depression in diabetic patients (p > 0.05). The mean of hs-CRP consentration was higher in diabetic patients with depression (5183.44:t3974.66 vs 2103,17:t1502.94 mg II, p=0.001) than in diabetic patients without depression. There was no association between blood glucose control (HbA 1 c) and hs-CRP concentration. Conclusion: High sensitivity-C reactive protein concentration was higher in diabetic patiens with depression compared with diabetic patients without depression.Key words: hs-CRP - with and without depression diabetes mellitus type II

Abstract

Objectives : This study was aimed at detecting the occurrence of hip dislocation in children with cerebral palsy (CP) by seeking the association among the degree of hip adductor spasticity, level of Gross MotorFunction Classification System (GMFCS) and Migration Percentage (MP).Methods : It was a cross sectional study with inclusion criterias were children with cerebral palsy, both male and female aged 2-10 years old, and parent’s approval by signing informed consent. Exclusion criterias werecomorbidities of other neuromotor impairments, such as spina bifida; other neuromuscular or musculoskeletal diseases, etc; uncooperative; and presenting severe diseases at the day of assessment. Measurement methodsincluded the Modified Tardieu Scale (MTS) R2, R1 and R2-R1 components to measure hip adductor spasticity; Gross Motor Function Classification System (GMFCS) protocol to assess gross motor ability; andAnterior-Posterior (AP) pelvic plain radiograph to calculate the Migration Percentage (MP) value.Results : At the initial phase, 31 children were included, however only 57 legs were analyzed for hip adductor spasticity and MP. There were no correlation between the degree of hip adductor spasticity and MPboth the R2 with MP (r = -0.060; p = 0.658), and R1 with MP (r = -0.136; p = 0.314). Moreover, there was insignificant difference between level of GMFCS and MP (p = 0.831).Conclusion : This study indicates nill correlation between the degree of hip adductor spasticity and the MP, and insignificant difference between the level of GMFCS and MP to detect the occurence of hip dislocationin children with cerebral palsy.Keywords : Modified Tardieu Scale, R2, R1, level of GMFCS, AP pelvic plain radiograph, MP value.

Abstract

Background: Sexual dysfunction and anxiety frequently happens on patients after acute myocardial infarction (AMI) and can affect patients’ quality of life. The purpose of this study was to examine the assosiation of sexual function post-AMI patients with anxiety. Methods: It was a cross-sectional study. Respondents are patients in Integrated Cardiac Clinic of Cipto Mangunkusumo Hospital that meet inclusion and exclusion criteria. They signed informed consent. Sexual function was assessed using International Index of Erectyle Function (IIEF) and anxiety was assessed using Hamilton Anxiety Rating Scale (HAM-A). Results: Post-AMI patients had erectile dysfunction (82.5%), orgasm dysfunction (72.5%) and libido dysfunction (93.8%). Respondents expressed sexual intercourse dissatisfaction (97.5%) and overall dissatisfaction (90%). The proportion of post-AMI anxiety was 52.5%. There was no assosiation between sexual function post-AMI with anxiety. Conclusion: Anxiety and sexual dysfunction post-AMI is a considerable problem. Factors that affect anxiety and sexual dysfunction post-AMI needs to be explored further so that an integrated management guidelines could be proposed.

Abstract

ABSTRACTBackground: Spontaneous bacterial peritonitis (SBP) is a common serious complication of liver cirrhosis mainly caused by bacterial translocation (BT) into ascites fluid. The most essential issue that affects BT is small intestinal bacterial overgrowth (SIBO), which usually caused by gastrointestinal dysmotility. This study was aimed to evaluate the prokinetic effects of domperidone on gastrointestinal motility and small intestinal bacterial overgrowth and the correlation between restoration of motility and the incidence of SIBO in patients with liver cirrhosis.Method: A cross-over double blind clinical trial was conducted on patients who were treated at the ward and outpatient clinic from Division of Hepatology in Cipto Mangunkusumo Hospital, Jakarta and Soedarso Hospital, Pontianak between September 2010 and March 2011. All patients suffered from liver cirrhosis with ascites, gastrointestinal dysmotility and SIBO were included in the study. Out of 34 eligible patients, 16 patients received placebo and 18 patients received prokinetics (domperidone). Wilcoxon test was performed to analyze the comparison of SIBO before and after treatment in the placebo group; while paired T-test was employed for the prokinetics (domperidone) group. To evaluate improved balance of SIBO in the placebo and domperidone group, a Chi-square test was performed.Results: In the placebo group, 61.8% patients experienced SIBO; while in the prokinetics group, SIBO occurred only in 2.9% patients. Restored gastrointestinal motility took place in the prokinetics group with reduced median value for orocaecal transit time from 120 minutes into 90 minutes (p = 0.0001). In contrast, it went worse in the placebo group, i.e. from 90 minutes into 110 minutes (p = 0.002). There was a significant correlation between restored gastrointestinal motility and SIBO (p = 0.0001). Similarly, so does the effect of administering prokinetic agent on restored gastrointestinal motility (p = 0.0001) and SIBO (p = 0.0001).Conclusion: The administration of prokinetics has been proven effective to restore gastrointestinal motility that may lead to reduced incidence of SIBO in patients with liver cirrhosis. Keywords: prokinetics, breath hydrogen test, dysmotilitas, bacterial overgrowth

Abstract

Background: Percentage of patients who had liver stiffness measurement failure using transient elastography varied between 2-10%; mainly caused by obesity. XL probe is expected to increase the success rate of liver stiffness measurement in patients with obesity. The objective of this study is to evaluate the success of liver stiffness measurement using M and XL probes and its influencing factors.Method: Patients who fulfilled the inclusion criteria were included in this study. Examination results were then analysed using statistical analysis unpaired t-test or Mann-Whitney and McNemar statistical tests.Results: From 92 NAFLD patients with obesity who were studied, the proportion of success in measuring liver stiffness using M probe was 57.6%, while that of XL probe was 88.0%. This difference was statistically significant (p 0.001). BMI, SCD, and thoracic circumference were associated with success in measuring liver stiffness using M probe, with p value of 0.007, 0.001, and 0.001 respectively. The results of Mann-Whitney statistical test revealed median value of BMI and SCD of patients who had liver stiffness measurement failure using M probe were 32.7 kg/m2 and 2.6 cm respectively. T-test results showed that the mean value of thoracic circumference of patients who had liver stiffness measurement failure using M probe was 97.8 cm.Conclusion: Proportion of success in measuring liver stiffness in NAFLD patients with obesity using XL probe was better compared to the M probe. BMI, SCD, and thoracic circumference were associated with the success of measuring liver stiffness using M probe. The same variables were not associated with XL probe.

Abstract

Background: Severity of hepatocellular carcinoma (HCC) that is assessed using Barcelona Clinic Liver Cancer (BCLC) classification is a main prognostic factor of hepatocellular carcinoma. Assessment of the serum level of Vascular Endothelial Growth Factor (VEGF) is considered to reflect the severity of HCC. However, there is still no fundamental basis of the association of severity of HCC with the serum VEGF level. The objective of this study is to know the relationship between serum VEGF levels with the severity of HCC by assessing the average difference of serum VEGF level in various severity of HCC.Method: This study was a cross sectional study to identify the association between serum VEGF levels with the severity of HCC based on BCLC classification. This study was performed in Cipto Mangunkusumo Hospital between January and May 2015. Statistical tests used to evaluate the association between serum VEGF level with BCLC classification was one-way ANOVA analysis, and continued with post hoc Tukey Schaffe analysis.Results: A total of 61 subjects with HCC were included into this study. In this study, we did not find subjects with stage 0 BCLC. The average of serum VEGF level in stage A BCLC was 288.26 ± 156.6 pg/mL; stage B BCLC: 434 ± 164.8 pg/mL; stage C BCLC: 785.57 ± 194.25 pg/mL; stage D BCLC: 1537.97 ± 660.62 pg/mL. One-way ANOVA analysis showed significant difference (P 0.001) between serum VEGF level and HCC severity based on BCLC classification. Post hoc Tukey Schaffe analysis showed the presence of significant difference between stage A and C BCLC (p 0.05) and stage A and D BCLC (p 0.001), stage B and D BCLC (p 0.001), and stage C and D BCLC (p 0.001). There was no significant difference between stage A and B BCLC, and between stage B and C BCLC.Conclusion: It was found that serum VEGF level increased in accordance to the HCC severity based on BCLC classification, particularly for stage B BCLC and above.

Abstract

Background: Chronic functional constipation is a common problem that affects between 15-25% of the population and cause symptoms and disorders that creates discomfort, morbidity, and high costs for health care. Recently, the consumption of probiotics in treating chronic constipation in adults have been investigated. However, there are still limited and controversial evidences available from controlled trials. The aim of this study was to evaluate the effects of Lactobacillus reuteri (L. reuteri) in improving the Agachan constipation score, the number of L. reuteri in the feces and the fecal pH in the patients with chronic functional constipation.Method: A double-blind, placebo randomized controlled trial (RCT) was conducted in 40 adults (12 males/28 females with mean age 45.95 ± 16 years) affected by chronic functional constipation according to Rome III criteria. Patients were randomly assigned to receive a supplementation of L. reuteri or placebo for 4 weeks.Results: At week 4, the decrease in Agachan constipation score was from 17.00 to 8.00 with p 0.001, the increase number of L. reuteri was from 6.80 x 107 to 2.12 x 108 with p 0.001 and the decrease of pH feces was from 5.44 (SD 0.70) to 4.78 (SD 0.56) with p 0.001 in the L. reuteri group, otherwise in the placebo group there were no significant results in Agachan constipation score, the number of L. reuteri and fecal pH assessed.Conclusion: L. reuteri is more effective than the placebo group in improving the Agachan constipation score, increasing the number of L. reuteri in the feces and decreasing the fecal pH in adult with chronic functional constipation.

Abstract

Background: Liver cirrhosis is on of health problems in Indonesia. Decreasing number of liver cells in cirrhosis cause some complications, one of which is malnutrition. In Indonesia, coconut milk is one of nutrients sources which quite popular. Coconut milk is thought to have hepatoprotective effect and be able to be eliminated by cirrhosis patients. A study about effect of coconut milk supplementation to nutritional status parameters in liver cirrhosis patients is really needed.Method: This study is a randomized controlled trial with parallel design. This study is conducted in Cipto Mangunkusumo Hospital Jakarta from February to March 2014. Patients are given late night snack (LNS) in form of agar that contained 25 grams of sugar and additional 50 cc coconut milk in group I (experimental group) and additional drink that contained 25 grams of sugar in group II (control group). Variables that measured to look changes in nutritional status between the two groups are triceps skinfold thickness (TSF), mid arm muscle circumference (MAMC), body mass index (BMI), body fat mass (BFM), prealbumin levels and serum albumin levels.Results: A total of 35 patients with liver cirrhosis in this study to the completion. There are three parameters of nutritional status that increasing better in the group receiving a LNS from combination of carbohydrates and coconut milk, compared to group receiving carbohydrates alone. Those three parameters are mean (SD) MAMC with amount of 9.41 (6.43) mm in group I and 4.68 (5.76) mm in group II, p = 0.028. Median (range) BFM changes 0.67 (-2.47-3.80) kg in group I and 0.21 (-4.96-1.99) kg in group II, p=0,373. Mean (SD) serum albumin level change 0.17 (0.31) g/dL in group I and 0.07 (0.41) g/dL in group II, p = 0,426. There is an increase in the TSF measurement in all subjects after one month of LNS supplementation, but not found any significant differences of TSF changes between the two groups. While the measurement of BMI and serum prealbumin did not show any significant changes after treatment in all subjects research.Conclusion: One month LNS 200 kkal supplementation with combination of carbohydrates and coconut milk have a better effect on the improvement of nutritional status in patients with liver cirrhosis, compared to carbohydrates alone. This shown by the better parameters of MAMC, BFM and increased serum albumin in group I, compared to group II.

Abstract

Background: Inflammatory bowel disease diagnosis was still based on invasive examination, such as endoscopy and histopathology. Fecal calprotectin was a non-invasive  intestinal inflammation marker, but several study give a different result in its diagnostic value and correlation to inflammatory bowel disease. This research was aimed to prove that fecal calprotectin examination has a high diagnostic value in diagnosing inflammatory bowel disease, and also correlate to its clinical stages.Method: This is a cross sectional study to do a diagnostic test in several hospital in Jakarta, from September 2014 to February 2015. Receiver operating characteristic (ROC) curve was made to get fecal calprotectin diagnostic level and Krusskal Wallis test was performed to identify fecal calprotectin difference among each inflammatory bowel disease clinical stages.Results: A total of 71 patients with inflammatory bowel disease was invoved in this research, based on colonoscopic examination result. Among them, 57 patients was confirmed to have intestinal inflammation based on histopathology result. Fecal calprotectin level was found to be higher in patients with inflammatory bowel disease than patients without intestinal inflammation (553,8 µg/g vs. 76,95 µg/g, p 0,001).  A cut off point of 179,3 µg/g was gathered, with 96% sensitivity (95% CI: 0,88-0,99), 93% specificity (95% CI: 0,69-0,99), and 99,5% area under curve (AUC) 99,5% (95% CI: 0,98-1,00). A significant difference was found between fecal calprotectin in each inflammatory bowel disease clinical stages (p 0,001).Conclusion: Fecal calprotectin has a high diagnostic value for inflammatory bowel disease (IBD) and strongly correlate to its disease clinical stages.

Abstract

Background: Rheumatoid arthritis (RA) is an independent risk factor of cardiovascular disease (CVD), causing 30-50% mortality in RA patients. Metabolic syndrome is a combination of metabolic disorders that can be manifested as central obesity, impaired blood pressure, or abnormalities of carbohydrate and lipid metabolism, which is closely associated with CVD in RA patients. The prevalence and factors associated with the incidence of metabolic syndrome in RA patients vary in previous studies. This study aimed to determine the prevalence and factors associated with the incidence of metabolic syndrome in RA patients at Cipto Mangunkusumo National Central General Hospital, Jakarta. Methods: A total of 145 RA patients at the Rheumatology Clinic of Cipto Mangunkusumo National Central General Hospital from October to December 2021 were included as research subjects by consecutive sampling method. History taking about the disease and demographic data, physical and anthropometric examination, and laboratory tests were done on the patients. The prevalence of the metabolic syndrome was determined using harmonization criteria. Statistical analysis was performed to determine the association of the variables of age, smoking habit, disease activity, duration of disease, glucocorticoid treatment, and methotrexate treatment with metabolic syndrome in RA patients. Results: The prevalence of metabolic syndrome was 19.3% in RA patients with a mean age of 49.8 ± 11.6 years. The variables of gender, age, deformity, physical activity, extraarticular manifestation, smoking habit, disease activity, disease duration, glucocorticoid treatment, and methotrexate treatment were not statistically significant to the incidence of metabolic syndrome in RA patients. Conclusion: The prevalence of metabolic syndrome in RA patients in this study was 19.3%, and no variable was associated with the prevalence of metabolic syndrome.
