Abstract

Initially palliative care is emphasized at the end of life, but the right thing is to begin identifying palliative care needs when the disease is diagnosed. Palliative care is the treatment needed to improve the quality of life of patients and their families that suffering from life threatening and progressive disease such as cancer, non-cancer diseases, and HIV/AIDS (human immunodeficiency virus/acquired immunodeficiency syndrome). Palliative care decision must be in place from the beginning of treatment, so the patient’s preferences are met. The goals of this care are to reduce physical, social, psychological, and spiritual suffers. Further treatment plan is based on discussion between patient, family, and health care worker. The final result is identification and decision that can be written in advanced directives.

Abstract

Introduction. There are only few studies about correlation between non traditional risk factor and diastolic dysfunction in RA patients. This study aims to determinate the correlation between non traditional risk factors, including disease duration, disease activity and disability score with the diastolic dysfunction of women with RA. Methods. A cross-sectional, consecutive sampling study was conducted to 52 RA women without any previous cardiovascular disease history. All participants underwent an echocardiography to assess diastolic dysfunction and other findings associated. Duration of disease was assesed by direct interview, while the disease activity by calculating DAS28 and disability score by HAQ-DI. Results. Diastolic dysfunction was found in 30.8 % of study participants (13.5 % for each low and moderate grade, while severe was 3.8% ). Median duration of disease was 26.5 months (range 2-240), mean DAS28-CRP score was 2.69±1.11 while median DAS28-ESR score was 3.65 (range 1.13-7.5), and median HAQ-DI score was 0.29 (range 0-2.38). LV hypertrophy was found in 34.61% participants. Mean EF was 66.7±5.76%. Valve abnormality was found in 34.6% study participants. Correlation between duration of disease, DAS28-CRP, DAS28-ESR and HAQ-DI score with E/A in sequence was r= -0.065 (p=0.89), r=0.393 (p=0.38), r=0.357 (p=0.43), r=0.630 (p=0.12); while with E/E’ in sequence was r=0.136 (p=0.77), r= - 0.536 (p=0.21), r= - 0.393 (p=0.38), r=0.374 (p=0.41). Conclusions. Duration of the disease, disease activity score, and disability score in rheumatoid arthritis women had no correlation with diastolic dysfunction.

Abstract

Introduction. Endothelial progenitor cell (EPC) is considered possible to reduce asymmetric dimethylarginine (ADMA) levels, which is a marker of improved endothelial function. However, clinically, there is still a debate about the role of EPC. This study was conducted to determine the correlation of elevated levels of erythropoietin (Epo) and the number of EPC CD34 + and CD133 + with improvement of endothelial function in renal failure patients 3 months after kidney transplantation. Methods. A cross-sectional study was conducted in prior and 3 months after kidney transplantation of renal failure patients who underwent a kidney transplant in Cipto Mangunkusumo hospital. The study included 21 subjects who enrolled from July 2013 to February 2014. Blood samples prior and 3 months after kidney transplantation were collected to evaluate the level of Epo, numbers of EPC CD34+ and CD133+ and level of assymetric dimethylarginine (ADMA). Statistical analysis was performed using Pearson or Spearman correlation test. Results. Results of this study showed that prior to kidney transplantation, level of Epo was increased but not statistically significant (p>0.05). The EPC numbers of CD34+ and CD133+ were significantly increased (p0.05). There was no association between the elevated level of Epo and the numbers of EPC CD34+ and CD133+ with the improvement of endothelial function three months after kidney transplantation. Conclusions. In three months after kidney transplantation, there is an elevated level of Epo, the numbers of EPC CD34+ and CD133+ and decrease level of ADMA. However, there was no association between the elevated level of Epo and the numbers of EPC CD34+ and CD133+ with the improvement of endothelial function in patients 3 months after kidney transplantation.

Abstract

Introduction. Depression occurs at high rates in people with chronic diseases, including bronchial asthma, with the prevalence of depression approaches 50% in tertiary care asthma clinic. Tumor necrosis factor alpha (TNF-α) is known to play a critical role in the pathogenic mechanism of a number of chronic inflammatory disease, including bronchial asthma and depression. There has not been any research data on the subject in Indonesia. The objective of this study was to investigate the correlation between depressive level and TNF-α level in uncontrolled bronchial asthma. Methods. This was a cross sectional study conducted in 40 patients with uncontrolled bronchial asthma at the allergy immunology clinic outpatient of Dr Moh Hoesin Hospital Palembang, during June 2014 until August 2014. Uncontrolled bronchial asthma was assessed using the Asthma Control Test (ACT) questionnaire, whereas depressive symptoms were assessed by Beck Depression Inventory (BDI) questionnaire, and diagnosis was confirmed by the criteria of the Diagnostic and Statistical Manual for Psychiatry-IV Text Revision (DSM-IV TR) / International Code Diagnose 10 (ICD-10). Serum levels of TNF-α was measured by the method of quantitative enzyme-linked immunosorbent assay (ELISA). Results. The median value of the level of depression and serum TNF- α in this study were 16 (10 - 45) and 4.09 (1.29 - 19.57) pg/mL. There was no significant correlation between depressive level and TNF-α level ( r = -0.265 , p = 0.098 ). Conclusions. There was no significant correlation between depressive level and TNF-α level in uncontrolled bronchial asthma

Abstract

Pendahuluan. Mencapai kontrol asma masih merupakan tujuan yang sulit dicapai sebagian besar pasien meskipun telah tersedia pedoman manajemen asma internasional dan terapi yang terbukti efektif. Tingkat kontrol asma yang rendah diantaranya disebabkan kepatuhan terhadap pengobatan yang rendah. Hingga saat ini faktor-faktor yang berhubungan dengan kepatuhan berobat pasien asma tidak terkontrol masih sedikit diteliti. Penelitian ini bertujuan mengetahui profil kepatuhan berobat pada pasien asma tidak terkontrol dan hubungan antara faktor terkait pasien, faktor terkait penyakit, faktor terkait pengobatan, faktor terkait sosial ekonomi dan faktor terkait sistem pelayanan kesehatan terhadap kepatuhan berobat pasien asma tidak terkontrol. Metode. Penelitian potong lintang terhadap pasien asma tidak terkontrol yang datang ke poliklinik alergi dan imunologi RSCM dari bulan Februari 2012 hingga Mei 2012. Semua pasien asma yang memenuhi kriteria dilakukan anamnesis, pemeriksaan fisis lengkap, pengisian kuesioner, tes kontrol asma, tes Beck Depression Inventory dan Asthma General Knowledge Questionnaire. Analisis awal hubungan kepatuhan dengan faktor-faktor yang diteliti menggunakan uji Chi-square. Kemudian dilanjutkan dengan regresi logistik untuk setiap variabel yang bermakna. Hasil. Dari 125 pasien asma tidak terkontrol didapatkan kepatuhan rendah sebesar 56 %. Dilakukan analisis bivariat terhadap faktor-faktor yang berhubungan dengan kepatuhan yaitu usia, pendidikan, pengetahuan asma, penghasilan, depresi, sediaan obat yang digunakan, keyakinan pasien terhadap dokter, asuransi kesehatan dan hubungan keluarga dan didapatkan pengetahuan bermakna dengan p=0,001 (IK 1,939-24,789). Kemudian dilakukan regresi logistik didapatkan pengetahuan paling berpengaruh terhadap kepatuhan (p=0,003) dengan OR 6,933 (IK 1,939-24,789) Simpulan. Kepatuhan berobat pada pasien asma tidak terkontrol masih rendah dengan pengetahuan merupakan faktor yang berhubungan dengan kepatuhan.

Abstract

Pendahuluan: Prevalensi sirosis cukup tinggi di Indonesia yang mayoritas populasinya adalah muslim. Pada saat menjalani puasa Ramadhan yang merupakan kewajiban umat muslim terjadi berbagai proses metabolik seperti penurunan glikokogenesis, peningkatan glukoneogenesis dan ketogenesis, dapat mempengaruhi keadaan klinis, nutrisi dan bokimiawi pasien sirosis hati yang juga mengalami proses hiperkatabolik. Penelitian tentang pengaruh puasa Ramadhan pada pasien sirosis hati di Indonesia belum pernah dilakukan. Berdasarkan kenyataan diatas perlu diteliti bagaimana perubahan terhadap status nutrisi, status fungsi hati, pembentukan badan keton dan keseimbangan nitrogen pada pasien sirosis hati yang menjalankan puasa Ramadhan. Metode: Penelitian ini merupakan studi pre dan post, yang menilai perubahan pada pasien sirosis hati yang berpuasa Ramadhan: saat Ramadhan dan pasca Ramadhan. Pada subjek penelitian dilakukan pengambilan data-data dasar, anamnesis dasar. Pada minggu ke 4 saat puasa Ramadhan dilakukan pengukuran antropometrik, yaitu timbang berat badan, indeks massa tubuh (IMT), ketebalan triceps skinfold (TSF), dan mid-upper arm circumference (MUAC). Pada subjek juga dilakukan pengambilan darah vena untuk pemeriksaan laboratorium dan pengukuran ekskresi nitrogen urin yang ditampung dalam 24 jam. Pada minggu ke empat pasca Ramadhan subyek dievaluasi kembali dengan prosedur yang sama seperti saat Ramadhan. Hasil: Rerata (SD) indeks massa tubuh saat puasa Ramadhan adalah 25,112 (4,05) kg/m2 dan pasca Ramadhan 25,25 (4,01) kg/m2 (p = 0,438). Didapatkan rerata (SD) midarm muscle circumference (MAMC) saat Ramadhan 25,77 (3,077) cm dan rerata (SD) pasca Ramadhan 25,96 (3,42) cm (p=0,228). Tidak ada perbedaan bermakna secara statistik antara kadar 3-β-hidroksi butiran darah saat puasa Ramadhan dan 4 minggu pasca Ramadhan. Rerata (SD) saat puasa Ramadhan 2,44 (2,93) gram/24 jam, sedangkan rerata (SD) keseimbangan nitrogen pasca Ramadhan 0,51 (3,16) gram/24 jam (p=0,037). Simpulan: Tidak ada perubahan status fungsi hati yang dinilai dengan Skor Child Pugh saat puasa Ramadhan dan pasca Ramadhan. Indeks massa tubuh dan ketebalan TSF membaik pasca Ramadhan. Tidak ada perbedaan kadar 3-β-hidroksi butirat darah saat puasa Ramadhan dan pasca Ramadhan. Terdapat keseimbangan nitrogen yang lebih positif saat puasa Ramadhan dibandingkan pasca Ramadhan.

Abstract

Depression is a common condition in heart failure (HF) and is considered a risk factor for cardiovascular disease. Depression disorder in patients with heart disease paradigmatically defines a psychosomatic-somatopsychic challenge to any health delivery. However, despite the devastating effects of depression, it is often underdiagnosed and receives little attention in heart failure patients. This review provides an extensive overview of HF regarding epidemiology, disease pathophysiology, diagnosis, and management from the latest literature. Based on the literature review, the prevalence of depression in heart failure in Indonesian patients was 5.3-42%. Psychological stress experienced by people suffering from depression can cause dysregulation in the sympathetic nervous system and hypothalamic–pituitary–adrenal (HPA) axis. This mechanism, in turn, has some deleterious downstream effects, including the development of hypertension, left ventricular hypertrophy, coronary vasoconstriction, endothelial dysfunction, platelet activation, and the production of pro-inflammatory cytokines. The Diagnostic and Statistical Manual of Mental Disorders-5 (DSM-5), published by the American Psychiatric Association, is used to diagnose depression in patients with HF. Meanwhile, the Patient Health Questionnaire (PHQ)-2 and PHQ-9 are commonly used as screening tools for depression in patients with HF. The Biopsychosocial-spiritual model as a psychosomatic approach have might reduce or prevent depression and thus improve quality of life and other outcomes. Evidence reveals that both psychotherapeutic treatment (e.g., cognitivebehavioral therapy) and pharmacologic treatment (e.g., use of the selective serotonin reuptake inhibitor sertraline) are safe and effective in managing depression in patients with cardiovascular disease. In conclusion, the review of articles in this paper shows that there is a high prevalence of depression in heart failure, but it is often not recognized by doctors, there is a relationship between psychosomatic, and heart failure, non-pharmacological interventions such as psychotherapy and pharmacological therapy have benefits. Future research is needed to create evidence-based evaluations and treatment algorithms tailored to the specific needs of the target population.

Abstract

Introduction. Improved survival of breast cancer patients are not necessarily followed by improvement of quality of life. According to the European Organization for Research and Treatment of Cancer (EORTC), cancer patients’ quality of life are very important to be evaluated. In Indonesia, breast cancer patients cannot be assessed their quality of life accurately because there is no valid and reliable questionnaire. This study aims to prove that the European Organization for Research and Treatment of Cancer Quality of Life Questionnaire-Breast 23 (EORTC QLQ-BR23) is a valid and reliable measuring instrument to be used in Indonesia. Methods. This study was a cross-sectional study. The EORTC QLQ-BR23 questionnaire was first translated into Indonesian language before being tested on 10 respondents. The translation results were then used in 100 breast cancer patients. Test-retest was assessed with intraclass correlation coefficient (ICC), while the internal consistency was assessed by Cronbach’s alpha. Construct validity was assessed by multi-trait scaling analysis and the criteria validity assessed by identifying the correlation between domains of European Organization for Research and Treatment of Cancer Quality of Life QuestionnaireCancer 30 (EORTC QLQC30) and EORTC QLQ-BR23 with Short Form 36 (SF36). Results. From September to October 2015, a total of 100 breast cancer patients on treatment were enrolled in this study. ICC value (1 hour interval) of all EORTC QLQ-BR23 domains were very good (ICC> 0.8). ICC values (30 day interval) decrease in all EORTC QLQ-BR23 domains. Almost all of the EORTC QLQ-BR23 domains have a cronbach’s alpha value > 0.7 except for the arm symptom domain (Cronbach’s alpha: 0.643). Multi-trait scaling analysis showed a fairly high correlation between the score of the questions with a score of his own domain, while the relationship of the questions to different domains always have a lower correlation than the domain itself. In criteria validity test, obtained 19 correlation with r > 0.3 between domains of EORTC QLQ-C30 and EORTC QLQ-BR23 with SF36. Conclusion. EORTC QLQ-BR23 is a valid and reliable instrument for assessing quality of life of breast cancer patients in Indonesia

Abstract

Hepatocelullar carcinoma (HCC), the most common primary liver malignancy is the fifth most common cancer and the third most frequent cause of cancer related death globally. Pain comprises one the most common symptoms of HCC and may have severely negative effects on patient’s quality life. Furthermore, patients with HCC often presented with a cirrochis,so to control pain in this patient poses a clinical challenge. This case report discuss about a young 26-years-old male patient who presented with a four-month history of right upper quadrant abdominal pain and weight loss. HbsAg positive and serum alpha-fetoprotein level was high. Computed tomography (CT) and liver biopsy confirmed the presence of primary hepatocellular carcinoma.,with lung metastatic from the radiologic x-ray. Patient came with a loss of consciousness due to hypoglicemia. During hospitalization, patient complaint of abdominal pain and was given morphine intermediate release (MIR) to control the pain. By administering measurable and well-controlled opioids, the patient’s suffering can be reduced, thus improving the patient’s quality of life

Abstract

Introduction. Health-related quality of life among HIV patients can be used as one of several indicators of successful therapy. WHOQOL-HIV BREF questionnaire is one of the instruments used to assess the patients’ quality of life that has been used in many countries, but never been tested for its validity and reliability in Indonesia. This study aims to determine the validity and reliability of the questionnaire WHOQOL-HIV BREF in bahasa Indonesia as a tool for measuring the quality of life of HIV patients. Methods. A cross-sectional study was conducted in HIV Integrated Service Unit Cipto Mangunkusumo General Hospital (RSCM) Jakarta in November 2016 with consecutive sampling method. The study was conducted in two phases: first phase for the language and cultural adaptation process and second phase to test the validity and reliability of the questionnaire. Results. Fifty six respondents filled the questionnaire, 69.6% of them were male. Using a multi-trait scaling analysis, correlation coefficient value had a high correlation with the total score domain. This meant that it had a good validation. Correlation between questionnaire domain WHOQOL-HIV BREF and SF-36 questionnaire domain obtained 6 significant domains that shown strong correlation coefficient (r = 0.60 to 0.79; p <0.005). Reliability of the questionnaire was assessed by intraclass correlation coefficient of each domain, which was range from 0.401 to 0.484 with Cronbach Alpha 0.513 to 0.798 . Conclusions. The questionnaire WHOQOL-HIV BREF in the Indonesian language is valid and reliable.
