Abstract

Introduction. Holistic approach in psychosomatic focus on spirituality factor and spiritual support is expected to improve services and psychological condition of the patients. Inflammatory response during hemodialisis procedure hence increased with the evidence of increasing level of serum interleukin-6 (IL-6). Further research is still needed to see the spiritual factors that can decrease the inflammatory factors. Methods. Cross sectional study on 51 chronic hemodialisis patients at RSUP. H. Adam Malik and RSU dr. Pirngadi Medan between July-August 2014. Serum IL-6 was measured using quantitative enzyme-linked immune sorbent assay (ELISA) methods. Blood samples and spiritual aspect assessment by handing out FACIT Sp-12 questionnaire to patients were taken in the morning, 30 minutes before hemodialisis. Results. Subscale meaning 10.67 (SB 2.66), peace 9.63 (SB 2.19) and faith 11.47 (SB 2.91). Median serum IL-6 level is 5,63 (1,48- 28,88) pg/mL. Median FACIT Sp-12 is 30,00 (18-48). Correlation test between serum IL-6 level and spiritual aspect have shown statistically weak negative correlation (correlation coefficient -0,330, p=0.018). Conclusions: spirituality level in chronic hemodialisis patients are higher. Weak negative correlation between serum IL-6 level and spiritual level on chronic hemodialisis patients was found in this study.

Abstract

Introduction. Hyperglycemia during hospitalization is a risk factor that can be managed in order to reduce mortality. Inspite of hyperglycemia, glucose variability also brings negative outcome to cells. Studies about glucose variability effect to mortality had been studied using many variables of glucose variability. Methods. Retrospective cohort study is done to 280 critical ill patient in ICU and HCU in Cipto Mangunkusumo Hospital who admitted to critical care between January 2012-August 2013. MAG change and glucose standard deviation are divided into 4 quartiles. Relationship between MAG change and glucose standard deviation are analyzed using Chi Square test. To control the confounders (MSOFA score, Charlson comorbidities index, hypoglycemia, and hyperglycemia), logistic regression is done Results. Median of MAG change is 3.3 mg/dL/hour and median of glucose standard deviation is 37.63 mg/dL. Mortality proportion is higher in upper quartile of MAG change and glucose standard deviation compared to lower quartile. OR of upper quartile MAG change to ICU mortality is OR 4.26 (95% CI 1.98-9.15) and OR of upper quartile glucose standard deviation to ICU mortality is OR 2.78 (95% CI 1.35-5.71). These results are adjusted to MSOFA score, hypoglycemia, and hyperglycemia. In logistic regression test, fully adjusted OR are 3.34 (95% CI 1.08-10.31) and 0.90 (95% CI 0.28-2.88) for MAG change and glucose standard deviation, respectively. Conclusions. Mortality proportion of upper quartile of MAG change (>8.1 mg/dL/hour) is higher than lower quartile (59 mg/dL) is higher than lower quartile(<22.7 mg/dL), but the difference is not statistically significant.

Abstract

Introduction. Breast cancer is the most prominent cancer and causing death in women. Treatment of breast cancer requires immunohistochemistry (IHC) to determine prognostic and predictive and imaging tests to determine staging. However, currently in Indonesia only 17 hospital can conduct IHC test. RSUD Kota Bogor like other Type B Hospital have no IHC test facilities. To fullfil that test, its important to refer patient to referal hospital. Because of limitation there are patients who did not do IHC test and continued get anthracycline chemotherapy. The objective of this research is to find out immunohistochemistry profile of breast cancer patients who received anthracycline-based chemotherapy in RSUD Kota Bogor. Methods. A descriptive retrospective study by collecting the breast cancer patients medical records was done since January 2014 to December 2017 in RSUD Kota Bogor. Results. Total of 174 subjects were include in this study which consist of 84 subjects (48%) with IHC test and 90 subjects (52%) without IHC test. The immunohistochemistry profile of breast cancer patients who received anthracycline-based chemotherapy since January 2014 to December 2017 in RSUD Kota Bogor were luminal type (64%), HER2-enriched (18%), and TNBC (18%). Conclusion. Luminal type is the most molecular subtypes in breast cancer patients who received anthracycline-based chemotherapy in RSUD Kota Bogor. However, because of limitation, since January 2014 to December 2017 only 48% of breast cancer patient that get anthracyclin chemotherapy who got IHC test in RSUD Kota Bogor.

Abstract

Introduction. Cardiovascular disease is one of the health problems in Indonesian pilgrims and has become the main cause of death for pilgrims in the past 3 years. Most of them are considered high risk for cardiovascular disease. Some of those risk factors are predicted to have strong association with the pilgrims mortality rate. This study is meant to seek for risk factor associated with the pilgrims mortality rate. Methods. Observational study method of the Indonesian pilgrims in 2017 was conducted to assess the characteristics of Indonesian pilgrims who follow the hajj in 2017 as well as the characteristics of pilgrims who died from cardiovascular disease. Result. The proportion of mortality from cardiovascular disease was 49.2% of all deaths in the Indonesian hajj pilgrims in 2017. Generally, pilgrims are in the category of high risk. Pilgrims who died from cardiovascular disease are mostly aged over 60 years (76.7%) with cardiovascular risk factors including hypertension (53.4%) and obesity (37.3%), and pilgrims with the final wave departure time (53%). Conclusios. The proportion of deaths from cardiovascular disease in the Indonesian pilgrims in 2017 is 49.2%. The characteristics of pilgrims who died with the cause of cardiovascular disease are generally age more than 60 years, pilgrims with hypertension, obesity, and final departure time.

Abstract

Introduction. Chemotherapy is one of therapy choices for the advanced Non-Small Cell Lung Cancer (NSCLC). The success in therapy is measured with the 1-year survival, 2-year survival and the Progression Free Survival (PFS). The success is influenced by many factors: resistant to the citostatic, dosage, administer intensity, chemotherapy regiment, type histology, stage, performance status, comorbidity and social economic. In Indonesia, funding and chemotherapy regiment become the challenge for the success of therapy. Methods. The study used the Retrospective Cohort study with survival analysis. The Patients included in this study were the advanced NSC Lung Cancer (At least Stadium IIIa) who came to RSKD and RSCM during Jan 2006 – December 2010 for their first chemotherapy until finished the cycle (6 times) and had monitored for 2 years. Data was analyzed using cox regression analysis SPSS 16.0, and featured on the Kaplan Meier Curve. Results. Fifty five patients used EC and the other 55 patients used DC. There’s difference on survival where 1 year survival EC is 30,9% and DC is 47,3%, with p 0.030. Two year survival CE is 0% and for DC is 5.5%, with p 0.003. Also with the Median time survival between EC for 27 weeks and DC for 38 weeks with p < 0.016. Compared to DC, EC chemotherapy can increase the death risk by HR 1,684 (CI 95% 1,010-2,810), twenty four weeks PFS with EC is 54.5%, DC is 32.7% with p= 0.022. Conclusions. The survival with cisplatin-docexatel is better compared to cisplatin-etoposide, this applies to PFS as well.

Abstract

Introduction. Diabetic peripheral neuropathy (DPN) in the lower extremity caused by complications of diabetes in the microvascular which can damage nerve in the lower extremity. Then, it is necessary to be aware of the process of aterosclerosis elsewhere, both in the macro and microvascular arteries in the lower extremity. This study was conducted to assess the association of the degree of diabetic peripheral neuropathy assessed by Toronto clinical scoring system (TCSS) with the process of atherosclerosis in the blood vessels of the lower extremity, both macrovascular with ankle brachial index (ABI) and toe brachial index (TBI), as well as microvascular with transcutaneus partial oxygen pressure (TcPO2 ) in patients with type 2 diabetes mellitus. Methods. Cross-sectional study was carried out in patients with type 2 DM with DPN with TCSS values> 5 in the Integrated Cardiac Polyclinic, Endocrine and Metabolic Polyclinic, and Internal Medicine Polyclinics at Cipto Mangunkusumo Hospital. Data were obtained from interviews, medical records, as well as ABI, TBI and, TcPO2 examinations. Bivariate analysis of each variable was performed using Spearman test. Results. Total of 36 subjects who met the selection criteria were included in the study, the average age was 62 years (SD 9.2) with 20 (55.6%) of whom were women and the median duration of diabetes was 12 years. Based on bivariate analysis with the Spearman test, there was a statistically significant negative correlation with moderate correlation coefficient between the degree of diabetic peripheral neuropathy assessed by TCSS with ABI (r = -0.475, p = 0.003) and TBI (r = -0.421, p = 0.010). The TcPO2 examination also found a statistically significant negative correlation with moderate correlation coefficient (r = -0.399, p = 0.016). Conclusion. There is a statistically significant negative correlation between the degree of diabetic peripheral neuropaty with ABI, TBI, and TcPO2 examinations.

Abstract

Introduction. Patients with pulmonary tuberculosis (TB) had an increased risk for depression up to 1.53 times from a large cohort study. Studies have found an increase in the neutrophil - lymphocyte ratio (NLR) in various mood disorders including depression, also the value of the platelet-lymphocyte ratio (PLR) associated with major depression accompanied by psychotic symptoms. This study aimed to describe the peripheral blood characteristics, NLR, and PLR in pulmonary TB patients with depression. Methods. A cross-sectional study of 106 non-multidrug-resistant pulmonary TB patients was done at outpatient department of Cipto Mangunkusumo General Hospital, Jakarta from August to October 2018. The diagnosis of depression was made by interview according to Diagnostic and Statistical Manual of Mental Disorder-V (DSM-V) criteria, and severity of depression is determined using Beck Depression Inventory-II (BDI-II). The laboratory tests were done to obtain complete peripheral blood results, NLR, and TLR. Parametric test was used for numeric variables with even distribution, and nonparametric test for variables with uneven distribution. Results. From 106 patients with non-multidrug-resistant pulmonary TB, the proportion of depression was 32%. White blood cell count (p=0.024), and absolute lymphocte count (p=0.004) is lower in depressed TB patients compared to nondepressed TB patients. There were no significant relationship between NLR and TLR and the severity of depression in depressed pulmonary TB patients (p>0.05). Conclusion. White blood cell count and absolute lymphocyte count are lower in depressed pulmonary TB patients. Meanwhile, RNL and PLR are higher in depressed pulmonary TB patients eventhough not statistically significant.

Abstract

Introduction. In patient with stable coronary artery disease (CAD), severity of stenosis is closely related to prognosis. It is known that several clinical parameters and recently-developed strain echocardiography can predict severity of stenosis. Assessment of clinical parameters, altogether with strain echocardiography is expected to make better prediction. This study aim to determine whether clinical factors, i.e. age, sex, diabetes, typical angina, and history of myocardial infarction, and strain echocardiography parameter, i.e. global longitudinal strain (GLS), can predict severity of coronary artery stenosis measured with Gensini score,and to further develop a prediction model based on significant parameters. Methods. This is a cross-sectional study taken at Dr. Cipto Mangunkusumo National Central General Hospital during period March – May 2019. Patient with stable CAD scheduled to undergo coronary angiography is recruited consecutively. Bivariate analysis using chi-square is performed to each predictor. Significant predictors are further analysed using backward stepwise logistic regression. A prediction model is then developed based on significant predictors by multivariate analysis. Results. The study group include 93 subjects. Significant predictors on bivariate analysis include diabetes melitus (OR 2.79; 95% CI:1.08-7.23), history of myocardial infartion (OR 4.04; 95% CI:1.51-10.80), typical angina (OR 5.01; 95% CI:1.91-13.14), and GLS ≥-18.8 (OR 30.51; 95% CI:10.38-89.72). Significant predictors on multivariate analysis are typical angina (OR 4.48; 95% CI:1.39-14.47) and GLS ≥18.8 (OR 17.30; 95% CI:5.38-55.66). Predicton model is not developed because there are only two significant predictors. Conclusions. Typical angina and GLS are predictors of stenosis severity in patient with stable CAD. Age, sex, diabetes, and history of myocardial infarction are not significant predictors. A prediction model can not developed because there are only 2 significant predictors.

Abstract

Introduction. Nosocomial pneumonia is a lung infection that occurs after the patient is hospitalized for more than 48 hours, without any signs of pulmonary infection at the time of treatment. When compared with young individuals, elderly individuals are more likely to have community-sourced and nosocomial infections with worse outcomes. Comprehensive Geriatric Assessment (CGA) domains are expected to explain the factors that contribute to nosocomial pneumonia in elderly patients. This study aimed to determine the proportion of elderly treated at Dr. Cipto Mangunkusumo National Central General Hospital who experienced nosocomial pneumonia and whether the CGA domains influence the occurrence of nosocomial pneumonia. Methods. A retrospective cohort study was conducted by analyzing the medical records of patients aged 60 years or older who were hospitalized in the medical ward of Geriatric Internal Medicine at Dr. Cipto Mangunkusomo National Central General Hospital in January – September 2019. We also collected secondary data from the geriatric division’s research. The sample consisted of inpatients aged ≥60 years who were hospitalized for more than 48 hours. Those who died within the first 48 hours of hospitalization and subjects with incomplete CGA domain data were excluded from the study. The criteria for nosocomial pneumonia used in this study followed the CDC’s pneumonia criteria for geriatric patients. Data processing was conducted using the application of Statistical Product and Service Solutions (SPSS) 16. Results. Of 228 subjects, the proportion of nosocomial pneumonia in elderly patients hospitalized was 31,14%. The mean age was 69 years with the subject’s age range between 60-89 years. Nutritional status (OR 2.226; CI95% 1.027-4.827) and functional status (OR 3.578; 95%CI 1.398-9.161) are factors that influence the incidence of nosocomial pneumonia in elderly patients who are hospitalized at Dr. Cipto Mangunkusumo National Central General Hospital. Conclusions. The proportion of elderly patients with nosocomial pneumonia was 31.14%. Nutritional status and functional status are factors that influence the incidence of nosocomial pneumonia in elderly patients who are hospitalized at Dr. Cipto Mangunkusumo National Central General Hospital.
