Abstract

Perianesthesia education is essential for improving patient safety. However, a needs assessment at RS TK II Udayana identified gaps in the delivery of structured perianesthesia education. This program aimed to improve the knowledge of patients and families through the implementation of the ATIKA (Integrated Access to Anesthesia Health Information) method based on the Patient and Family Centered Care (PFCC) approach. A one-group pretest–posttest design was conducted involving 20 patients and their family members scheduled to undergo anesthesia. Educational interventions were delivered using PowerPoint, leaflets, and the ATIKA Digital Booklet. Participants knowledge was assessed using a structured questionnaire and analyzed with the Wilcoxon signed-rank test. The proportion of participants with good knowledge increased from 25% (n = 5) to 75% (n = 15), while poor knowledge decreased from 50% (n = 10) to 10% (n = 2). Statistical analysis showed a significant improvement in knowledge following the intervention (Z = −3.832; p < 0.001). The program also produced sustainable educational resources, including presentations, leaflets, and the ATIKA Digital Booklet. These findings indicate that the ATIKA method effectively enhances patient and family knowledge and supports the development of a structured, sustainable, patient- and family-centered perianesthesia education system.

Abstract

Background: Hospital employees operate in complex work environments where organizational rewards, workload, and work stress may influence job performance and ultimately affect healthcare quality and patient safety. Although these factors have been widely investigated, the evidence remains fragmented and context-specific. This systematic review synthesizes current evidence on the relationships between rewards, workload, work stress, and hospital employee performance. Methods: A systematic review was conducted following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. Literature searches were performed in PubMed, Scopus, and Google Scholar using predefined keywords related to rewards, workload, work stress, employee performance, and hospitals. Quantitative studies involving hospital employees that examined at least one of the three organizational factors and reported employee performance outcomes were included. Twenty-six studies met the eligibility criteria and were narratively synthesized. Result: All included studies employed cross-sectional designs and were conducted in Indonesia, with nurses representing the predominant study population. The evidence consistently indicated that higher workload and work stress were associated with poorer employee performance. Most studies also reported that rewards, particularly financial compensation and incentive systems, were positively associated with employee performance through enhanced motivation and job satisfaction. However, one study found no significant association. Variability in reward-related findings appeared to reflect differences in organizational context and reward system implementation. Conclusion: Rewards, workload, and work stress are key organizational factors associated with hospital employee performance in Indonesia. Optimizing workload distribution, implementing fair and transparent reward systems, and strengthening workplace stress management may contribute to improved workforce performance and service quality. Given that all included studies were conducted in Indonesia and used cross-sectional designs, further longitudinal and multinational research is needed to strengthen the evidence base and improve the generalizability of these findings.

Abstract

The National Health Insurance (Jaminan Kesehatan Nasional/JKN) program administered by BPJS Kesehatan has covered 278.1 million participants, representing 98.45% of Indonesia’s population. The increasing number of participants has consequently led to longer waiting times at primary healthcare facilities (Fasilitas Kesehatan Tingkat Pertama/FKTP). To address this issue, BPJS Kesehatan introduced the online queue feature through the Mobile JKN (MJKN) application in 2018. However, the utilization rate of the online queue system in Blitar Regency (10.65%) and Nganjuk Regency (12.28%) remains below the national target of 18%. This study aimed to analyze the influence of FKTP facility characteristics, including ownership status, facility type, and service type, on the adoption of the MJKN online queue system in Blitar and Nganjuk Regencies. This study employed a quantitative approach with a cross-sectional design. The population consisted of 133 FKTP, of which 103 facilities were selected using proportional random sampling. Secondary data were obtained from the Health Facilities Information System (HFIS) and the BPJS Kesehatan Business Intelligence Dashboard. Data were analyzed using binary logistic regression. The results showed that the type of FKTP significantly influenced the utilization of the MJKN online queue system (p=0.044; OR=14.786). In contrast, FKTP ownership (p=0.396; OR=3.702) and service type (p=0.550; OR=1.966) did not significantly affect online queue utilization. In conclusion, the type of FKTP plays a significant role in determining the achievement of MJKN online queue utilization, whereas ownership and service type do not. Therefore, FKTP and BPJS Kesehatan should formulate targeted strategies and prioritize optimization efforts based on FKTP characteristics to improve the adoption of the MJKN online queue system. Further studies are also recommended to examine individual-level determinants among MJKN application users.

Abstract

Community Health Centers serve as critical primary healthcare providers, yet optimizing patient utilization in remote regions remains challenging. This study analyzes the influence of patient attitudes, health worker support, and social support on the interest in utilizing health services at the Larat Community Health Center. An observational, quantitative, cross-sectional design was employed to evaluate these psychosocial determinants among 389 respondents selected through purposive sampling from a total population of 14,354. Multiple linear regression analysis assessed the simultaneous and partial effects of the independent variables on interest in service utilization. Descriptive statistics revealed that 48.8% of respondents had adequate attitudes, 56.3% perceived sufficient health worker support, and 55.0% reported adequate social support, while 51.9% demonstrated a medium level of interest in utilization. The regression model demonstrated a highly significant simultaneous influence (p-value = 0.000), with these three variables collectively explaining 67.7% of the variance in patient interest. Healthcare utilization interest emerges as a complex behavioral outcome driven by the dynamic interplay among individual mindsets, clinical staff support, and community networks, rather than by isolated individual choices. Policymakers must implement integrated, holistic interventions that address all three psychosocial pillars to sustainably enhance primary care engagement, improve health literacy, and reverse declining visit rates in marginalized archipelagic communities.

Abstract

Keluarga memiliki peran yang begitu penting bagi lansia yang menderita hipertensi khususnya dalam pemberian diet. Peran tersebut antara lain membimbing dan memecahkan masalah sehingga pemberian diet pada lansia dengan hipertensi dapat dikelola dengan baik oleh keluarga. Adapun penelitian ini bertujuan untuk mengetahui faktor-faktor yang berhubungan perilaku keluarga dalam pemberian diet hipertensi pada lansia. Faktor-faktornya adalah pengetahuan, sikap, umur, jenis kelamin, pendidikan, ketersediaan fasilitas kesehatan, dan tenaga kesehatan. Penelitian ini bersifat deskriptif analisis kuantitatif dengan menggunakan cross sectional. Jumlah sampel dalam penelitian ini sebanyak 30 responden. Pengambilan sampel menggunakan total sampling. Instrumen yang digunakan dalam penelitian ini adalah dengan wawancara menggunakan kuesioner. Data dianalisis dengan menggunakan analisis univariat dan analisis bivariat. Hasil analisis bivariat menunjukkan bahwa (a) ada hubungan antara pengetahuan dengan perilaku keluarga dalam pemberian diet hipertensi pada lansia ; (b) ada hubungan antara sikap dengan perilaku keluarga dalam pemberian diet hipertensi pada lansia ; (c) ada hubungan antara umur dengan perilaku keluarga dalam pemberian diet hipertensi pada lansia; (d) tidak ada hubungan antara jenis kelamin dengan perilaku keluarga dalam pemberian diet hipertensi pada lansia; (e) tidak ada hubungan antara pendidikan dengan perilaku keluarga dalam pemberian diet hipertensi pada lansia; (f) tidak ada hubungan antara ketersediaan fasilitas kesehatan dengan perilaku keluarga dalam pemberian diet hipertensi pada lansia; (g) tidak ada hubungan antara tenaga kesehatan dengan perilaku keluarga dalam pemberian diet hipertensi pada lansia. Keluarga perlu menyadari tentang pentingnya perannya dalam menjaga atau memantau nutrisi yang diberikan pada lansia dengan hipertensi. Keluarga dan penderita hipertensi harus bekerja sama agar penderita hipertensi patuh menjalani diet rendah garam agar tekanan darah penderita hipertensi dapat terkontrol dengan baik. Pada penelitian ini bahwa tingkat Pengaruh Pendidikan Kesehatan terhadap Pengetahuan Lansia di Posyandu Lansia Kelurahan Manisrenggo sebelum kita melakukan penyuluhan pada lansia dari 23 responden memiliki kategori cukup atau 43% dari 23 responden yaitu 10 responden. Namun, setelah kita melakukan penyuluhan tentang pengaruh pendidikan kesehatan terhadap pengetahuan lansia mengalami kenaikan dari 43% kategori cukup menjadi 52% memiliki kategori baik yaitu 12 orang.
