Abstract

Background: During the COVID-19 pandemic, health service providers, especially hospitals, are expected to be able to optimize all their resources in providing services to patients. Assessment of hospital readiness in dealing with the COVID-19 pandemic is needed to ensure that hospitals continue to maintain and improve the quality of health services and patient safety. Purpose: The purpose of this study was to analyze the effect of hospital classification, type of service, referral status and accreditation status on hospital readiness in dealing with the COVID-19 pandemic in East Java. Methods: The design of this study is a quantitative observational analytic type with a cross sectional approach. The population of this study is 385 hospitals in East Java. The sampling technique of probability sampling with a stratified random sampling approach obtained a sample of 129 hospitals. The data analysis used is Dummy Multiple Linear Regression. Results: The results showed that the classification of the hospital with a p value = 0.001 and the accreditation status with a p = 0.000 had an effect on the readiness of the hospital to face a pandemic. Meanwhile, the type of service with a p value = 0.397 and referral status with a p value = 0.077 did not affect the readiness of the hospital to face a pandemic. Conclusion: Hospital classification and accreditation status influence hospital readiness to face the COVID-19 pandemic in East Java. Therefore, regular monitoring and evaluation of readiness to face a pandemic is important for all hospitals to carry out through independent assessments. Making efforts to improve each component, especially in class C and D hospitals, as well as collaborating with the Health Service is very necessary to fulfill regulations and implement improved services for both COVID-19 and non-COVID-19 patients.

Abstract

The dynamism of the application information technology in the health sector, especially in hospitals, which is currently being developed is the electronic medical record (EMR). Global competition in the era of industrial revolution 4.0 makes all hospitals have to be ready for the changes that will occur in the management of medical record management. This study aims to analyze the influence of organizational culture with individual readiness as a mediator on championing behavior in the implementation of electronic medical records (EMR) at Muhammadiyah Hospital. Respondents amounted to 133 people consisting of professional health workers who will later be involved implementation of EMR. The research instrument was in the form of a questionnaire via a google form link. Analysis using SEM (structural equation model) with Lisrel. The results showed that organizational culture significantly affected individual readiness (?=0.67, t-value=7.40). Organizational culture has a significant effect on championing behavior (?=0.42, t-value=4.10). Individual readiness significantly affects championing behavior (?=0.48 , t-value = 4.56). Individual readiness is also a significant mediator of the relationship between organizational culture and championing behavior (complementary mediation type). A strong organizational culture will support the readiness of the organization's planned changes. A strong organizational culture also influences behavior to fight for the success of the change plan. Through individual readiness, organizational culture influences behavior to fight for the successful implementation of EMR. So, organizations need to prepare individuals well for the success of the change plan. Keywords : Championing Behavior, EMR, Organizational Culture, Readiness for Change

Abstract

Background: The 2019 Global Youth Survey shows data that 19.2% or 50 million young man and women 13-15 years old smoke worldwide. BPS data through the 2021 Susenas shows that teenage smokers in Nganjuk Regency are 20.12%. The high prevalence of teenage smokers indicates the need for intervention to reduce the number of adolescent smokers as quickly as possible. Purpose: This study aims to determine the relationship between knowledge of the dangers of smoking and attitudes toward smoking with the desire to stop smoking in adolescents in the Nganjuk Regency. Methods: An observational design with a cross-sectional approach was used in this study. The sample in this study was 300 respondents who were taken using cluster sampling from 10 junior high schools in Nganjuk Regency. There are three questionnaires that use in this study. Data analysis used the Pearson correlation test and Linear Regression. Results: The percentage of teenage smokers in Nganjuk Regency is quite high at 33% with a dominance of male youth who start smoking by 11% at the junior high school level. P-value of 0.00 (> 0.05), which means that there is a relationship between attitudes about cigarettes and knowledge of cigarette dangers on intention to quit smoking. Beta value (0.311 > 0.193) said that attitudes about cigarettes have a 31.1% effect in making someone have the intention to quit smoking in a negative relationship. Conclusion: Attitudes about cigarettes variable have a stronger influence on intention to quit smoking than knowledge of cigarette. This shows that to increase the success of the smoking cessation program for adolescents in Nganjuk District, interventions are needed to increase negative attitudes and knowledge of the dangers of smoking.

Abstract

Background: Stroke is a disease that causes chronic disability. It is hoped that the combination of Motor Relearning Program (MRP) and Positive Affirmation (PA) will be able to improve the functional abilities and quality of life (QoL) of post-stroke patients considering that there is no intervention that combines these two methods. Purpose: The purpose of this study was to analyze the effect of the combination of MRP and PA on the functional abilities and QoL of post-stroke patients at Ngudi Waluyo Wlingi Hospital. Methods: The research design used is quantitative research using the true experimental pre and post with controlled design. The sampling technique used was Simple Random Sampling with a sample of 7 people given a combination of MRP and PA (Group A), 7 people given MRP treatment (Group B), 6 people given PA treatment (Group C), and 7 people in the control group given conventional physiotherapy treatment (CPT) (Group D). The research was carried out for 4 weeks with 3 sessions. Functional ability was measured using Barthel Index and QoL was measured using Stroke Impact Scale. Paired sample t, ANOVA, Tukey post hoc, and Games-Howell post hoc tests was used to analyzed the data. Results: With the exception of the PA group, which had a p-value of 0.000 > 0.05 for functional ability, the results of the paired t-test showed that the p-value was 0.000 for both functional ability and QoL for all treatment groups. This suggests that functional abilities were unaffected by PA. ANOVA test results revealed a significance value of 0.000, below the 0.05 alpha threshold. This indicates that the pre-post test results for each treatment group differ significantly from those of the control group. Group C (PA) and Group A (a mix of MRP and PA) had the biggest mean difference, according to the Tukey test, with a value of 38.80. Additionally, the Games-Howell test's QoL scores showed. Conclusion: PA treatment has no effect on improving the patient's functional ability, but has an effect on patient’s QoL, so it is recommended to combine it with MRP treatment. The combination treatment of MRP and PA is recommended to be given simultaneously to improve the functional ability and QoL of post-stroke patients for maximum results.

Abstract

The lack of interest of independent medical practice healthcare facilities and independent dentist practice healthcare facilities in registering health facilities and reporting national quality indicators (national quality indicators) as a condition for accreditation will affect the quality of health facilities so that self-efficacy and motivation are really needed to increase this interest. The aim of this research is to analyze the influence of self efficacy and motivation on national quality indicators reporting interest through health facility registration interest as an intervening variable (study on independent medical practice healthcare facilities and independent dentist practice healthcare facilities in the kediri regency area). The research design used was analytical observational with a "cross sectional" approach. The sampling technique used was proportionate random sampling with a sample of 158 health facilities. Data were collected from 158 healthcare facilities in Kediri Regency using a structured questionnaire with a Likert scale (1–5). The questionnaire was distributed via WhatsApp and Google Forms, ensuring efficient and accessible data collection. The data analysis technique uses test path analysis. The research results show that in hypotheses 1,2,3,4,5 the significance value is <0.05 so that there is an influence of self-efficacy and motivation on interest in registering health service facilities, there is an influence of self-efficacy and motivation on interest in reporting national quality indicators , there is an influence of interest in registering health service facilities on interest in reporting national quality indicators , whereas in hypotheses 6 and 7 the significance result is >0.05 so it cannot prove that there is an influence of self-efficacy and motivation on interest in reporting national quality indicators through interest in registering service facilities. Health. The importance of awareness of health facilities, especially for independent medical practice healthcare facilities and independent dentist practice healthcare facilities, to carry out a series of accreditation steps to improve the quality of health facilities needs to receive more attention from the health service and independent medical practice healthcare facilities and independent dentist practice healthcare facilities as health service providers.

Abstract

The implementation of Electronic Medical Records (EMR) in Community health centers is a strategic step in improving the efficiency and quality of health services. This study aims to analyze the implementation of RME in the West Kotawaringin Regency Health Center with a qualitative descriptive method. This study aims to analyze the application of EMR in the West Kotawaringin Regency Health Center with a qualitative descriptive approach. The research was carried out on three health centers that have implemented EMR, namely the Arsel, Mendawai, and Kumpai Batu Atas Health Centers, in the April-June 2024 period. Data was collected through in-depth interviews and documentation, including aspects of patient registration, data distribution, filling in clinical information, data processing, financing claims, data storage, quality assurance, and EMR content transfer. The 15 respondents consisted of medical personnel and community health centers administration. Data analysis involves three key steps: data reduction, data presentation, and conclusion and verification. The results showed that although the majority of respondents understood the benefits of RME, they faced various obstacles, such as limited technology infrastructure, lack of training for healthcare workers, and system integration problems. The use of EMR has accelerated the service process, but there is still a reliance on manual methods in several stages. In addition, data reporting is often constrained by technical issues and coordination between units. The study findings indicate that while most respondents recognize the benefits of electronic medical records , several challenges hinder optimal implementation. Key obstacles include limited technological infrastructure, insufficient training for healthcare workers, and difficulties in system integration.
