Abstract

Current research suggests a possible connection between social capital and its effect on self-reported health. However, this relationship has not been extensively studied in urban and rural regions of Indonesia. This research aimed to evaluate the differences in sociodemographic factors, social capital, and self-rated health between rural and urban populations and to identify sociodemographic and social capital elements related to self-rated health in the urban and rural areas of Sumatra Utara Province, Indonesia. This study makes use of secondary data from the Happiness Level Measurement Survey Report 2021. A two-stage, one-phase sampling method was employed for sample selection. The Chi-square test was utilized to examine the distribution of factors related to respondents in both urban and rural settings. Multivariable Cox regression analysis was conducted to determine associations with self-rated health. Our findings indicate that rural inhabitants exhibit a higher rate of poor self-rated health compared to their urban counterparts (p-value<0.001). Significant differences were observed between urban and rural areas in terms of sex, education, trust, social participation, and feelings of safety. The multivariable Cox regression analysis revealed that being younger, having higher education, possessing greater trust, and engaging in high social participation was associated with a lower risk of poor self-rated health for both urban and rural residents. Consequently, trust and social participation could reduce the occurrence of poor self-rated health.

Abstract

Introduction : Non-communicable diseases (NCDs) such as hypertension, diabetes mellitus, and heart disease have become serious global health problems, with increasing prevalence, especially in developing countries such as Indonesia. NCDs contribute to a large proportion of mortality and morbidity, prompting the need for further research to understand the factors contributing to the incidence of these diseases. Objective : To analyze the relationship between physical activity and NCD incidence in Indonesia based on a collection of journals that have been studied. Methods: This research uses Systematic literature review method with The source of information for the article was done through a search from PubMed, ScienceDirect, and Google Scholar published from 2019 to 2024 obtained 13 articles according to the inclusion criteria. Results: The study showed that people with low levels of physical activity have a higher risk of experiencing NCDs, while sufficient physical activity can significantly reduce the risk of NCDs. Conclusions : There is a significant relationship between physical activity and the incidence of NCDs. The need for a more systematic intervention program including counseling and provision of supporting facilities is expected to reduce the prevalence of NCDs and improve the quality of life of the community

Abstract

The implementation of hypertension guidelines is crucial for controlling hypertension and reducing its complications. This study aims to evaluate the implementation of the 2024 hypertension guidelines at Puskesmas X in terms of the percentage of controlled, uncontrolled, and Missed Visits in hypertensive patients. Data were collected from the hypertension control program records at Puskesmas X, obtained through the community health center's information system, for four months before and four months after the new guidelines were implemented. This study employed a retrospective quantitative design with a comparative approach, using an independent t-test to determine statistical significance. The statistical results showed a significant increase in the percentage of controlled hypertension after the implementation of the guidelines (p < 0.05), while uncontrolled hypertension and Missed Visits did not show significant changes. These findings provide an initial evaluation of the effectiveness of the new guidelines in improving hypertension control and highlight the need for additional interventions to improve patient adherence to follow-up visits.

Abstract

Sodium-glucose cotransporter-2 inhibitors (SGLT2is) have a broad range of clinical indications and are recommended by various guidelines for the management of diabetes, heart failure, and chronic kidney disease. However, prescribing rates for these agents are suboptimal towards various intended indications in many countries, including Indonesia. The aim of this study was to analyze the prescribing practices of SGLT2is among endocrinologists, cardiologists, and nephrologists in Indonesia. A qualitative method was utilized to provide a comprehensive understanding of this phenomenon. Semi-structured online interviews were performed with 18 participants, comprising seven specialized doctors as primary informants; eight key informants representing pharmacies, governmental bodies, and pharmaceutical marketing representatives; and three patients as supplementary informants. Furthermore, closed observations of two specialist doctors were undertaken as an additional data collection method. Informants were recruited using criterion and snowball sampling methods. The Theoretical Domains Framework was used as an interview guide; all interviews were audio visual-recorded, transcribed verbatim, and subsequently analyzed for thematic content with NVivo version 12. Seven themes emerged concerning the prescribing behavior of SGLT2is: socioeconomic status, therapeutic rationality, utilization barriers, utilization optimization, the urge to prescribe, therapeutic expectations and targets, and aspects of the health system and ethical considerations. Most informants possessed favorable perspectives regarding using SGLT2is when prescribed appropriately, yet specific elements necessitate enhancement to refine therapeutic justification, including initiatives to incorporate SGLT2is into the national formulary.

Abstract

Hypertension remains a major global health concern, causing numerous deaths worldwide. Approximately 1.13 billion people suffer from hypertension, with an estimated 10.44 million deaths annually due to hypertension and its complications. This study aims to explore the knowledge, attitudes, and actions taken by hypertensive patients in Bukittinggi City. The research was conducted in the Mandiangin Health Center Working Area, involving 17 hypertension patients, one officer responsible for the Non-Communicable Diseases (NCD) program, and the head of the community health center. This qualitative study used a phenomenological approach, identifying three main themes: knowledge, attitude, and action. The sub-themes include "awareness of hypertension diagnosis," "emotional response to diagnosis," "hypertension management efforts," and "support from health workers." Findings indicate that most patients were unaware of their hypertension diagnosis until they experienced symptoms or were screened during routine checkups. The dominant hypertension management strategies involved seeking treatment at the community health center and maintaining dietary modifications. This study highlights the experiences of hypertensive patients in improving knowledge, attitudes, and behavioral strategies for better hypertension control.

Abstract

Obesitas pada remaja telah menjadi masalah kesehatan masyarakat yang signifikan di Indonesia. Berbagai faktor seperti pola makan, aktivitas fisik, pengetahuan gizi, dan gaya hidup berkontribusi terhadap peningkatan prevalensi obesitas. Penelitian ini bertujuan untuk mengidentifikasi dan menganalisis faktor-faktor yang mempengaruhi obesitas pada remaja di Indonesia berdasarkan literatur yang ada. Penelitin menngunakan metode literaur review dengan pencarian pada basis data akademik seperti PubMed, IEEE Xplore, ScienceDirect , dan Google Scholar didapatkan didapatkan 62 artikel yang sesuai dengan kriteria inklusi antara tahun 2016 hingga 2024 sesuai dengan kriteria inklusi. Penelitian dilakukan pada bulan Oktober 2024. Hasil menunjukkan bahwa faktor-faktor seperti konsumsi makanan cepat saji, rendahnya aktivitas fisik, kebiasaan sedentari, dan kurangnya pengetahuan gizi memiliki hubungan signifikan dengan kejadian obesitas pada remaja. Penelitian ini menekankan pentingnya intervensi berbasis sekolah dan masyarakat untuk meningkatkan kesadaran dan pengetahuan mengenai pola makan sehat dan pentingnya aktivitas fisik. Saran bagi para pemangku kebijakan adalah merancang program pencegahan obesitas yang komprehensif bagi remaja di Indonesia. Keywords: Obesitas, Remaja, Indonesia, Faktor Risiko, Tinjauan Literatur

Abstract

Food waste adalah limbah makanan yang dihasilkan pada tahap distribusi dan konsumsi yang  berdampak signifikan terhadap lingkungan, ekonomi, dan sosial. Di Indonesia, masalah ini sangat mengkhawatirkan, dengan 23-48 juta ton food waste dihasilkan per tahun. Salah satu studi di China menyatakan bahwa kantin Univeristas merupakan salah satu tempat penghasil food waste terbesar. Universitas memiliki peran penting dalam mengelola lingkungan yang berkelanjutan, termasuk pengelolaan food waste yang baik. Penelitian ini bertujuan untuk mengetahui faktor yang memengaruhi perilaku pengelolaan food waste oleh tenant kantin dengan menggunakan Theory of Planned Behavior (TPB). Metode yang digunakan dalam penelitian ini adalah metode kualitatif dengan desain studi kasus. Informan utama berjumalah 10 orang yang merupakan tenant kantin di universitas yang telah berjualan minimal 6 bulan. Informan penelitian dipilih secara purposive dengan mempertimbangkan prinsip kecukupan dan kesesuaian. Cara pengumpulan data melalui Focus Group Discussion (FGD). Hasil penelitian menunjukkan bahwa sikap, norma subjektif, dan perceived behavioral control memengaruhi perilaku tenant dalam mengelola food waste. Sikap tenant terhadap food waste bervariasi, sementara norma subjektif mengindikasikan adanya dukungan komunitas dalam pengelolaan makanan yang tidak terjual. Secara umum tenant juga mampu merencanakan dan mengevaluasi menu makanan yang dijual. Studi ini menyarankan pentingnya edukasi tenant mengenai isu food waste dan kebijakan universitas yang terintegrasi untuk mendorong pengelolaan food waste yang lebih baik

Abstract

The success of diabetes prevention is influenced by several factors, including access to information, family support, diet, activity patterns, nutritional status, and self-efficacy. These factors play a significant role in the self-care practices of individuals with diabetes. Self-care refers to actions taken by patients to improve their ability to meet their daily needs independently, enabling them to prevent and manage their illness and adhere to treatment and recommendations provided by healthcare services. This study aims to identify the self-care practices of individuals with diabetes. The research involved 40 respondents diagnosed with diabetes. The average self-care score among diabetic patients in the working area of Banjar Baru Selatan Community Health Center was 68.53, with the lowest score being 39 and the highest score 92. Establishing self-care behaviors as part of a cultural norm requires consistent repetition and routine, to the extent that these behaviors may eventually become secondary needs. One approach to cultivating such habits in individuals with diabetes is to focus on addressing their weaknesses through methods that align with their routines and learning patterns. This strategy can help foster motivation and enthusiasm in managing their condition.

Abstract

All adolescents are entitled to access reproductive health services that are appropriate to their developmental stage, both through formal and non-formal education. Preliminary findings indicate that students generally lack adequate knowledge of reproductive health, with limited information provided both at school and within the family. Teachers report that students tend to feel embarrassed or consider discussions on marriage and reproductive health to be taboo. This study aims to analyze the implementation of adolescent reproductive health education programs at the senior high school level in Ogan Ilir Regency, Indonesia. This qualitative study employed in-depth interviews with teachers, community health center personnel, and family planning counselors, along with two key informants, four student focus group discussions (FGDs) and document analysis. The findings reveal that all schools have made efforts to implement adolescent reproductive health education through teacher-led instruction, the Youth Care Health Services (Pelayanan Kesehatan Peduli Remaja/PKPR) program, and the Youth Information and Counseling Center (Pusat Informasi dan Konseling Remaja/PIK-R) initiative. However, personnel involved in these programs require further training to enhance their knowledge and competencies. While implementation guidelines are available, several adjustments are often made during practice. Key challenges identified include limited budget allocations, insufficient facilities and resources, and restricted accessibility for some adolescents. Although reproductive health education through teacher instruction, PKPR, and PIK-R programs provides significant benefits for adolescents, further efforts are needed to optimize their implementation and outreach.

Abstract

Stoma bags are essential for colorectal cancer (CRC) patients with stomas. However, access and affordability in Indonesia remain challenging as the National Health Insurance (NHI) does not fully cover these devices. This study aimed to describe the availability and distribution of stoma bags for patients with CRC in national referral and private hospitals, focusing on outpatient access. A qualitative case study was conducted through observations and in-depth interviews with four enterostomal therapy nurses and six patients across three national referral hospitals and one private hospital in the Jakarta Metropolitan Area, Indonesia. Notably, the availability of stoma bags was limited in both hospital types, especially for outpatients. Most patients faced financial barriers, as they had to purchase stoma bags independently at high prices because of limited hospital supply and lack of NHI coverage. Moreover, hospital policies restricted the number of bags distributed, worsening access issues. These challenges increase stoma-related complication risk and negatively affect patients’ quality of life. Integrating stoma bags into NHI coverage, regulating prices, and improving distribution are recommended to reduce healthcare disparities and improve outcomes for CRC patients, aligning with Sustainable Development Goal 3.
