Abstract

Posisi merangkak dapat membantu meringankan rasa sakit, posisi ini juga dapat membantu memutar janin dari posisi oksiput posterior untuk berputar menjadi posisi oksiput anterior. Tujuan penelitian untuk mengetahui gambaran lama persalinan kala I fase aktif pada primigravida dengan posisi merangkak di wilayah Kerja Puskesmas Tamansari. Metode penelitian yang digunakan adalah quasi eksperimen. Sampel penelitian adalah seluruh ibu hamil primigravida di wilayah Kerja Puskesmas Tamansari, dengan taksiran persalinan bulan Oktober sampai November 2017 berjumlah 18 orang. Hasil penelitian menunjukkan rata rata lama persalinan kala 1 fase aktif pada primigravida dengan posisi merangkak adalah 251 menit (4.11) jam, sebanyak 10 orang (55.56%) melalui kala I fase aktif dalam waktu >251 menit, dan sebanyak 8 orang (44.44%) melalui kala I fase aktif selama <251 menit. Saran untuk tenaga kesehatan atau penolong persalinan dapat menerapkan teknik posisi merangkak saat memberikan asuhan kebidanan pada ibu bersalin kala I fase aktif sehingga akan mempercepat proses persalinan.

Abstract

Low back pain is defined as located pain and discomfort below the costal margin and above the superior gluteal line, with related pain in the lower limb, and repeated incorrect posture of work activities that demand effort with excess of flexion, rotation, vibration on the chest and carrying weight, being chronic if its persists for three months or more (Huldani, 2012; Utami and Dewi, 2015). This research aimed to find out the efficacy of accupressure on Dachangshu, Shenshu, and Yaoyangquan using pretest-posttest group design by comparing the mean decrese in pain threshold VAS, RMDQ and Test Straight Leg between control using Dingchuan and Waihuaijian and therapy group using Shenshu, Dachangshu, and Yaoyangquan twice a day with paired T test formula. The mean decreased in pain threshold VAS, RMDQ and Test Straight Leg obtained form therapy group toward 16 elderly above 45 years for 4 weeks showed that there is a significant result (p=0,000). Thus, it conclud that accupressure therapy using Dachangshu, Shenshu and Yaoyangquan can be used to treat low back pain.

Abstract

Background: Preeclampsia has been a major problem for obstetric care in Indonesia due to risks of preterm birth and lower Apgar score. Objective: This study is to examine relationships of preeclampsia, preterm births, and Apgar Score.Methods: This used an analytic study with retrospective case-control design in Dr. Soetomo Academic Hospital. A case group was taken by total sampling from medical records of all patients who had preterm delivery in 2017, and a control group of term deliveries was taken by random sampling. Then the data were analysed by using Chi-square test and Fisher’s Exact Test.Results: There were 80 samples for each group. 63.75% of patients by age of 20-35 years were who delivered preterm birth, 40.00% had normal BMI, 40.00% were nulliparous, 92.50% did not have history of preterm labour, 50.00% had preeclampsia with severe features, and 46.25% had spontaneous vaginal delivery. Around 47.14% neonates from preeclamptic women were born at 32-<37 weeks gestation, 50.00% were born with low birth weight, 52.86% had the first minute Apgar score <7, and 72.86% had the fifth minutes Apgar score =7. The statistical analysis showed a significant relationship between the preeclampsia and the preterm birth (p<0.007; OR=2.54, 95% CI 1.34-4.83). The preeclampsia was also related to lower Apgar score at 1st minute (p<0.042; OR=2.03, 95% CI 1.07-3.85) and 5th minute (p<0.046; OR=2.42, 95% CI 1.08-5.41). Preterm neonates born from preeclamptic mothers were related to lower Apgar score at 1st minute (p<0.002; OR=5.82, 95% CI 1.99-17.02) and 5th minute (p<0.001; OR 17.31, 95% CI 2.15-139.54).Conclusion: Preeclampsia could make pregnant women at risks of delivering preterm birth and neonates with low Apgar score.

Abstract

Background: A quarter of child population experiences sleep problems in their first three years of life. Inadequacy and problems of sleep for children may be caused by various causes that impact their mental health, emotional states, physical states, and immune systems. This also may culminate to behavioural problems. Objective: The aim of this study is to identify the prevalence of sleep problems in 0–36 months old Indonesian children. Methods: A cross-sectional study was conducted in Tulungagung, East Java, Indonesia. Children aged 0–36 months old were enrolled by using a quota sampling. Brief Infant Sleep Questionnaire (BISQ) was used in this study to assess the sleep problems. All obtained data were presented as a distribution and percentage of each variable referring to the BISQ indicators. Results: A total of 493 children were enrolled in this study. This study found that there were 153 children (31%) who had experienced sleep problems, 79 children (16%) who had nocturnal sleep duration less than 9 hours, 62 children (12,8%) who had nocturnal waking more than 3 times, and 20 children (4%) who had duration of wakefulness during the night more than 1 hour respectively.Conclusion: Although majority of parents thought that there were no sleeping problems with their children, the prevalence of sleep problems in 0–36 months old Indonesian children was quite high (31%), suggesting low parental awareness towards sleep problems of their children.

Abstract

Background: Disorders of the menstrual cycle are common in women aged 18-26 years. Indo­nesian Basic Health Research year 2013 data shows that as many as 13.7% of women of repro­ductive age experience problems with irregular menstrual cycles. Disturbances in the mens­trual cycle can cause infertility and decrease the quality of life of students. The purpose of this study was to determine the relationship between body mass index (BMI), nutritional intake patterns, and living arrangements with the menstrual cycle of female students. Subjects and Method: A cross-sectional study was conducted at the Faculty of Medicine, Airlangga University, from July to September 2022. A total of 176 female students from the Faculty of Medicine, Airlangga University, class of 2018-2021 were selected using a purposive sampling technique. The dependent variable is the menstrual cycle. The independent variables are body mass index (BMI), patterns of nutritional intake, and living arrangements. The instrument used in this research is a questionnaire. Research data were analyzed using binary logistic regression. Results: Overweight BMI (OR= 49.32; 95%CI= 9.57 to 254.35; p<0.001) and patterns of excess nutrition (OR= 28.69 95%CI= 4.97 to 165.94; p<0.001) have a relationship with female students' menstrual cycles Faculty of Medicine, Airlangga University Conclusion: Overweight BMI and nutritional intake patterns together have a relationship to the menstrual cycle of female students at the Faculty of Medicine, Airlangga University. Abstract

Background: The increase of calorie intake can increase the risk of obesity and diabetes mellitus. One of the efforts to reduce the risk is doing physical exercise. Types of physical exercise that can affect the system of female reproduction are not clearly defined. Purpose: To analyse the effect of moderate intensity continuous physical exercise on FSH levels in female rats exposed to a high-calorie diet. Methods: Laboratory experimental was research with Randomized Post test only Control Group Design. Sampel size was 27 rats (150-200, adults) in the estrus phase, then divided into 3 groups (negative control group, positive control group (high-calorie diet), and the Moderate Intensity Continuous Training group (MICT with high-calorie diet)). Results: The results showed that there was an significant effect of continuous physical exercise in moderate intensity on FSH level in female rats exposed to other group. The FSH level in the MICT group had the lowes level (16.78 ±4.83 mIU/ml). Conclusion: There is an effect of MICT on FSH level decreases in female rats exposed to a high diet calories. This can be caused, that high-calorie diet can inhibit GnRH secretion and continuous moderate-intensity physical exercise changes the rat energy metabolism

Abstract

The health problems of the elderly in ​​the Puskesmas Industri, Gresik work area align with the Indonesian health phenomenon where the number of elderly and degenerative diseases is high. Health cadres training in the work area of ​​the Puskesmas Industri is needed to optimize the role of cadres in educating and implementing a healthy lifestyle in older adults. The Community Partnership Program, focusing on health as part of goal 3 of The Sustainable Development Goals (SDGs), was carried out on September 1, 2022, and was attended by 57 cadres from 6 villages. Health information was provided as a provision for cadres to provide education to the public about the healthy lifestyle of the elderly. The results showed that there was an increase of 17,26 points (34,76%) after training. The lowest post-test results were on the topic of BMI and its interpretation, as well as physical activity in the elderly, so further action is needed. During FGD, the Elderly Health Book from the Ministry of Health of the Republic of Indonesia was introduced. The results of the FGD showed that the introduction of the Elderly Health Book should be continued so that health cadres can consistently use it. Health cadres have a significant role in realizing the elderly health program from the Indonesian Ministry of Health. This health cadre training has proven to be very necessary because it can increase understanding as a provision for cadres in providing education to the public regarding the healthy lifestyle of the elderly.

Abstract

Introduction: Postoperative pain is the most undesirable consequence of the surgery. If it is not managed properly, it can lead to a long healing. However, assessment and treatment of postoperative pain in surgical wards still have not received attention. Differences in patient's pain level after surgery and after being transferred to the surgical ward is very important in monitoring the effectiveness postoperative pain management. Objective: This study was conducted to determine the overview of pain level experienced by patients following orthopedic surgery and to know the individual factors that can affect the patient's pain level. Method and Material: This research was observational analytic with 43 orthopedic postoperative patients as a sample. The Patients' pain level was measured by the Numeric Rating Scale (NRS) at one hour after surgery and 24 hours after surgery. Result and Discussion:The Pain level one hour after surgery varied between pain level 0 as much as 53% to pain level 8 as much as 4.7%. The results of measurements of pain 24 hours after surgery only 23.3% of the patients who did not complain of pain, and there was a patient who experienced pain level 10. The results of the statistical calculation, the difference between the level of pain one hour and 24 hours post-surgery obtained value of p=0.037 (p<0.05). Conclusion: There was a significant difference between the pain level at one hour and 24 hours post-surgery. It might be due to the process of peripheral and central sensitization in patients with delayed pain management. It also may be influenced by individual factors as well as medical personnel.

Abstract

Introduction: McGrath® MAC videolaryngoscope is a single-handed device designed to facilitate intubation in patients both in patients with normal airway conditions (without any complications) or airway conditions with complications such as cervical spine and/or anatomic abnormalities. Objective: This study aims to compare McGrath® MAC videolaryngoscope and direct laryngoscope using Macintosh blades as learning material or study simulators for medical personnel (including anesthesiologist and paramedics) and novice operator (medical students). Method: this study is a systematic review using the PRISMA method which was carried out systematically. Data was collected through Pubmed, direct science, EBSCOHost, and Proquest using the keywords ‘airway management ', ‘laryngoscopy', and 'manikin'. Journal included based on published publication time between 2008 and 2020, a study using SimMan Laerdal Airway manikin, a journal discussing intubation using McGrath® MAC videolaryngoscope and direct laryngoscope with Macintosh blades here, where it is normal airway (without complications) and difficult airway. Results: 1556 journals were collected through 4 journal search sites and then carried out a screening process for the publication year approved in 2008 to 2020. Four studies use adult manikin SimMan Laerdal Airway including 247 participants were included in this systematic review. Conclusion: Based on journals that have been reviewed, McGrath® MAC videolaryngoscope provides better and superior results compared to Macintosh in terms of the success rate and visualization of glottis. Also, the intubation time using McGrath® MAC videolaryngoscope is shorten compared to Macintosh both on the normal airway (without complication) and difficult airway. The participants (medical personnel and novice operators) in all studies that reviewed prefer to use McGrath® Mac videolaryngoscope instead of using direct laryngoscope with Macintosh blade for Endotracheal Intubation mainly used for learning or study simulators.

Abstract

Highlight: The degenerative process plays a dual role, both protecting from and causing disc herniation. Men have a quicker degeneration rate at a younger age, while women have a quicker rate at an older age. Occupational factors such as physical workload, repetition, bending, and sitting periods all contribute to disc herniation.   ABSTRACT Introduction: Low back pain (LBP) refers to a sensation of pain or discomfort occurring in the region between the lowest costal and gluteal crease, with or without leg pain. Damage to the annulus fibrosus causes disc herniation, allowing the nucleus pulposus to herniate into the spinal canal. In less than 5% of cases, LBP is caused by disc herniation. Objective: This study examined the profiles of individuals with herniated LBP discs and related factors at Dr. Soetomo General Academic Hospital Surabaya. Methods: The study was conducted at Dr. Soetomo General Academic Hospital Surabaya using secondary data from medical records, which was then analyzed using descriptive statistics. Results: Fifty-eight LBP patients with herniated discs were enrolled in this study. The patient sample ranged from 23 to 74 years old, with a mean of 47,98±14,92. The majority of patients (29.3%) were 31–40 years old. Male and female prevalence rates were the same. Employees have the highest percentage of any occupation group, with 56.9%. Conclusion: Some of the findings clarify previous studies. Age, gender, and occupation were all linked to the occurrence of either LBP or disc herniation. However, our data cannot indicate whether or not these factors contribute to the occurrence of LBP and disc herniation. Therefore, more research is required to understand these factors and their relation to disc herniation and LBP.
