Abstract

Introduction: Experiential marketing has become a growing trend in promotional activities by emphasizing personalization and active customer involvement. Preliminary observations indicate a decline in outpatient visits among patients with general insurance and non-JKN coverage in 2023, which may be influenced by a decrease in their intention to use outpatient services. This condition highlights the need for hospitals to implement experiential marketing strategies to enhance patient interest and experience. Objective: This study aims to empirically examine the influence of patient experience and service perception on the intention to use outpatient services, with Strategic Experiential Modules (SEMs) as an intervening variable. Methods: This quantitative research employs a causal design involving outpatient service users as the population. A non-probability sampling technique was used, yielding 249 respondents. Data were collected using a questionnaire and analyzed using Structural Equation Modeling (SEM) with AMOS software. Results: The findings reveal that patient experience and service perception simultaneously have a significant positive influence on the intention to use outpatient services through SEMs. Service perception directly shows a significant positive effect on both SEMs and intention to use outpatient services, while SEMs also significantly enhance usage intention. Service perception emerges as the dominant variable in increasing SEMs and usage intention, whereas patient experience has the weakest effect. Conclusion: Patient experience and service perception collectively influence the intention to use outpatient services with SEMs as an intervening variable. Strengthening Customer Relationship Management is essential by improving human resources and optimizing service asset.

Abstract

Introduction: Neurosurgical services in Class B hospitals present a high level of complexity due to limitations in technology, facilities, and human resources. This study aims to explore the subjective experiences of neurosurgical medical teams, comprising neurosurgeons, nurses, anesthetists, medical equipment technicians, and operating room managers, in interpreting their roles, confronting challenges, and developing adaptive strategies to support excellent healthcare services at a Class B hospital. Methods: Employing a qualitative phenomenological approach, data were collected through in-depth interviews and analyzed thematically following Colaizzi’s method. Results: The findings indicate that service excellence in neurosurgical care within a resource-limited hospital is maintained through the dynamic interaction between professional meaning-making, persistent multidimensional challenges, and adaptive resilience strategies. While individual and team-level adaptations effectively sustain daily performance, limited systemic support poses challenges to long-term sustainability. Conclusion: Overall, the study highlights that the success of excellent neurosurgical services in Class B hospitals depends more on personal resilience and team cohesion than on systemic support alone. Recommendation: It recommends systemic improvements in management, human resource policies, integrated communication systems, and sustainable psychosocial support to ensure the continuity of high-quality services amid structural limitations

Abstract

Caring behavior is the core of high-quality nursing services. In practice, it is influenced by various psychological factors and work-related stress. Self-compassion and mindfulness are believed to help nurses maintain emotional balance in delivering empathetic and humane nursing care. This study aims to analyze the effect of self-compassion and mindfulness on nurses’ caring behavior, with work stress as a mediating variable. A quantitative approach with a cross-sectional design was used. Descriptive analysis employed the Three Box Method, while inferential analysis used SEM-PLS. The study population consisted of all nurses at Cilincing District General Hospital (N = 113), using total sampling. The results of the study indicate that the proposed model is considered fit (p = 0.133) and appropriate for testing the influence of self-compassion and mindfulness on caring behavior, with work stress as a mediating variable. Self-compassion has a significant positive effect on caring behavior (p = 0.000), while mindfulness does not have a direct effect (p = 0.891), but shows a significant indirect effect through the reduction of work stress (p = 0.044). Both self-compassion (p = 0.029) and mindfulness (p = 0.000) negatively affect work stress. Interestingly, work stress positively influences caring behavior (p = 0.030). To enhance the influence of self-compassion and mindfulness on caring behavior, hospitals should develop psychological intervention programs based on self-compassion and mindfulness to help nurses manage stress and strengthen caring practices. A systemic approach that considers organizational factors is also necessary to support the consistent implementation of caring behavior.

Abstract

This study examines the impact of personality and work engagement on Organizational Citizenship Behavior (OCB), with work motivation serving as a mediating variable, among inpatient nurses at South Jakarta Type B Hospital. OCB is crucial for enhancing organizational performance and service quality, especially in healthcare settings, where nurses' extra-role behaviors significantly contribute to patient care. The research utilized a quantitative approach with a survey design, involving 143 nurses from infection and critical inpatient wards. Data were collected through structured questionnaires and analyzed using Structural Equation Modeling–Partial Least Squares (SEM-PLS). The results indicate that personality positively influences both work motivation and OCB. Work engagement also positively impacts work motivation and OCB. Additionally, work motivation significantly affects OCB and partially mediates the relationship between personality, work engagement, and OCB. These findings suggest that fostering nurses' work engagement and motivation, supported by positive personality traits, can enhance OCB, leading to better organizational performance and improved healthcare service quality.

Abstract

Hospital is one of the health service facilities that tries to overcome the health service balance between investment costs and the availability of applicable unit costs. Calculation of unit costs has an important role, including being used for budget planning, cost control, subsidies and as information in decision making. In the era of National Health Insurance (JKN) in Indonesia, Hospitals that cooperate with the Social Security Administering Agency (BPJS) Health are paid using the Indonesian Case Base Groups (INACBGs) package system. The acquisition of claim payments at Dr. Mintohardjo Naval Hospital according to the INACBGs rate compared to Hospital costs has a negative difference value for Bronchopneumonia cases. The purpose of this study was to analyze the difference in unit costs of Bronchopneumonia services based on Clinical Pathway towards the determination of INACBGs Rates and Hospital Rates at Dr. Mintohardjo Naval Hospital. This type of research is quantitative, comparative analytical observation using a cross-sectional research design according to the Hospital perspective. The subjects of the study were Medical Records and BPJS Kesehatan claim files for Bronchopneumonia services for the period April to September 2023, totaling 61 files. The study was conducted from January 2024 to April 2024. Data analysis used the paired t-test statistical method in the statistical test application (SPSS Version 26). The results of the study on calculating unit costs using the Activity Based Costing (ABC) method are as follows: 1) There is a difference between the calculation of unit costs with INACBGs rates of negative 6.2% and unit costs with Hospital rates of positive 11.2%, 2) There is a difference in the unit cost of Bronchopneumonia services based on Clinical Pathway with the Activity Based Costing method against INA-CBGs Rates, 3) There is a difference in the unit cost of Bronchopneumonia services based on Clinical Pathway with the Activity Based Costing method against Hospital Rates. The suggestions in this study are: 1) For the financial management of RSAL Dr. Mintohardjo to improve competence in calculating unit cost rates for services based on clinical pathways with the Activity Based Costing method, one of which is through training, 2) Updating and conducting clinical pathway audits periodically, 3) Re-evaluating hospital rates to increase the margin of each service so that the margin obtained is in accordance with what has been set, 4) Digitizing patient usage data that was previously still written manually to avoid errors in entering details of service costs.

Abstract

Abstract

The Emergency Department (ED) plays a pivotal role as the frontline of hospital services, requiring rapid and accurate responses to ensure patient safety and optimal care outcomes. This study aims to analyze the influence of nurse competence and inter-team coordination on the effectiveness of patient transfers from ED to inpatient units, with nurse work motivation as a mediating variable. A quantitative approach was employed with hypothesis testing using path analysis (Structural Equation Modeling - SEM). The study was conducted at Graha Juanda Hospital, Bekasi, involving a sample of 55 nurses selected through total sampling from both the ED and inpatient departments. Primary data were collected through structured questionnaires, and data analysis was performed using SPSS 26. The results demonstrate that nurse competence and team coordination significantly affect service effectiveness, both directly and indirectly through work motivation. Specifically, high levels of nurse competence enhance the ability to assess, monitor, and manage patients efficiently, while effective team coordination fosters a collaborative environment that streamlines patient care transitions. However, work motivation plays a crucial role in mediating these relationships. The findings also indicate that low work motivation—linked to factors such as limited professional development opportunities, high workloads, and lack of recognition—contributes to delays in transferring patients to inpatient care, with the average Length of Stay (LOS) exceeding six hours. The study concludes that improving nurse competence, strengthening inter-team coordination, and enhancing nurse motivation are critical strategies to improve the quality and timeliness of emergency services. By offering continuous professional development, balancing workloads, and creating an environment where nurses feel valued, hospitals can significantly reduce delays in patient transfers. This integrated internal performance approach is vital for reducing LOS in the ED, ensuring timely care, and enhancing patient satisfaction. Furthermore, fostering a positive work environment that prioritizes both individual nurse growth and team collaboration is essential for sustaining high-quality emergency services and seamless, patient-centered care transitions.

Abstract

Background: Hospital nurses must continuously adapt to changes in healthcare technology, patient-safety standards, and service-quality expectations. A learning organization is therefore essential, yet the psychological mechanism that links leadership commitment and open organizational culture with organizational learning among hospital nurses remains insufficiently explained. Objective: This study analyzed the associations of perceived leadership commitment and open organizational culture with learning organization, with happiness at work as a mediating variable among nurses at An-Nisa Hospital, Tangerang. Method: An explanatory cross-sectional study was conducted among 225 staff nurses selected using purposive sampling. Data were collected using a validated 45-item questionnaire with a four-point Likert scale. Instrument validity, reliability, descriptive statistics, and structural model testing were analyzed using SPSS 25 and SmartPLS 3. Direct and indirect effects were evaluated using Structural Equation Modeling-Partial Least Squares. Result: Perceived leadership commitment and open organizational culture explained 54.8% of the variance in happiness at work. The final model explained 69.3% of the variance in learning organization and demonstrated acceptable model fit (SRMR = 0.055). Happiness at work had the strongest association with learning organization (? = 0.574, p < 0.001), followed by open organizational culture (? = 0.251, p = 0.004) and perceived leadership commitment (? = 0.104, p = 0.048). Happiness at work partially mediated the association of perceived leadership commitment (? = 0.127, p < 0.001) and open organizational culture (? = 0.351, p < 0.001) with learning organization. Conclusion: Nurses’ happiness at work is a central psychological pathway through which leadership commitment and open organizational culture are translated into organizational learning. Recommendation: Hospital managers should strengthen fair leadership practices, transparent communication, shared organizational values, and nurse well-being programs to support clinical learning. Further longitudinal and multi-site studies are recommended to confirm temporal relationships and improve generalizability.

Abstract

Background: Electronic medical record (EMR) adoption is essential for improving documentation accuracy, information accessibility, service efficiency, and patient safety. In outpatient care, EMR adoption is particularly important because healthcare professionals work in a high-volume and time-sensitive service environment. Objective: This study aimed to examine the effects of system quality and service quality on EMR adoption, with user satisfaction as a mediating variable, among healthcare professionals in a regional hospital outpatient department. Method: A quantitative cross-sectional analytical study was conducted among 97 Care Provider Professionals in the outpatient department of Dr. Dradjat Prawiranegara Regional Hospital, Serang. Respondents were selected using purposive sampling. Data were collected using a structured questionnaire and analyzed using Structural Equation Modeling-Partial Least Squares (SEM-PLS). Result: User satisfaction had a positive and significant effect on EMR adoption (? = 0.600; t = 4.075; p < 0.001). System quality significantly influenced user satisfaction (? = 0.202; t = 2.805; p = 0.005) and EMR adoption (? = 0.506; t = 5.573; p < 0.001). Service quality significantly influenced user satisfaction (? = 0.770; t = 12.540; p < 0.001), but its direct effect on EMR adoption was not significant (? = -0.164; t = 0.966; p = 0.334). The model explained 83.3% of the variance in user satisfaction and 78.4% of the variance in EMR adoption. Conclusion: EMR adoption in outpatient care is primarily influenced by system quality and user satisfaction. Service quality contributes to EMR adoption indirectly by improving user satisfaction rather than through a direct pathway. Recommendation: Hospitals should strengthen EMR system reliability, responsive technical support, user-centered training, and routine user satisfaction monitoring to sustain EMR adoption.
