Abstract

Background Sepsis and septic shock are global health problems with high morbidity and mortality rates. The ability to identify sepsis and septic shock with tools available in health facilities in Indonesia is an important step in reducing the morbidity and mortality of this disease. One of the tests that can be performed is a complete blood count such as the number of leukocytes, platelets, hemoglobin levels, and neutrophil-lymphocyte ratio (NLR). The costs incurred for complete blood counts are low and generally available in all health facilities. Methods This study is an analytical observational study with a cross sectional research design using medical record data of sepsis and septic shock patients in the PICU of RSUP Dr. M. Djamil Padang in 2020 - 2022. Univariate analysis is presented in the form of a frequency distribution table and bivariate analysis using the Chi Square test. Results The results showed that the most common diagnosis was septic shock (74.5%), male (57.5%), age under one year (38.3%), and death outcome (59.6%). Bivariate analysis found that there was a difference in platelet count between sepsis and septic shock (p=0.004) and there was a difference in NLR between sepsis and septic shock (p=0.024). Conclusions The conclusion of this study is that there are differences in platelet count and NLR between sepsis and septic shock patients in the PICU of RSUP Dr. M. Djamil Padang.

Abstract

The COVID-19 pandemic has affected the clinical learning environment of medical students, who are required to have knowledge, attitudes, and take actions to control COVID-19 infection. The pandemic can be one of the stressors for students undergoing clinical clerkships. Incorrect knowledge, attitudes, and actions can result in infection transmission and cause an increase in stress. The aim of this study was to determine the relationship between knowledge, attitudes, and preventive actions of COVID-19 with the level of stress among medical students undergoing clinical clerkships at the Faculty of Medicine, Andalas University. This study was an analytical research with a cross-sectional approach. The research sample consisted of 246 individuals obtained using cluster random sampling techniques. The research data were in the form of primary data obtained through filling out questionnaires regarding knowledge, attitudes, and preventive actions of COVID-19, and DASS-42. Data analysis used the Spearman rank test with statistical significance determined if p-value < 0.05. Bivariate analysis showed a significant relationship between knowledge, attitudes, and actions with the level of stress with the value of knowledge (p = 0.008, r = -0.168), attitudes (p = 0.045, r = -0.128), and actions (p = 0.011, r = -0.162). The conclusion of this study is that there is a significant relationship, a negative relationship pattern, and a very weak strength of the relationship between knowledge, attitudes, and preventive actions of COVID-19 with the level of stress among medical students undergoing clinical clerkships at the Faculty of Medicine, Andalas University

Abstract

This research explored whether vitamin D status is related to Insulin Resistance (IR) and pancreatic β-cell performance among non-diabetic obese employees at Dr. M. Djamil Padang Hospital, West Sumatra, Indonesia. Insulin resistance and β-cell activity were assessed using the Homeostatic Model Assessment—specifically Homeostasis Model Assessment of Insulin Resistance (HOMA-IR) as an indicator of insulin resistance and Homeostasis Model Assessment of Beta-Cell Function (HOMA-B) to evaluate β-cell function. The study applied a cross-sectional design conducted from January to July 2020 and involved 81 obese hospital staff without diabetes. Measurements included Fasting Blood Glucose (FBG), Fasting Insulin (FI), and Serum 25-Hydroxyvitamin D [25(OH)D], all analyzed with automated instruments. HOMA-IR and HOMA-B indices were derived from FBG and FI using standard formulas. Statistical analysis used the Spearman correlation test with a significance threshold of p<0.05, followed by multivariate linear regression. The participants were predominantly female (58%), aged 18–58 years, with a mean Body Mass Index (BMI) of 31.46±3.99 kg/m². Average vitamin D levels were 14.84±5.48 ng/mL, while median values for HOMA-IR and HOMA-B were 2.30 and 193.09%, respectively. The results indicated that vitamin D concentrations did not show a meaningful correlation with either HOMA-IR (r=0.071; p=0.530) or HOMA-B (r=−0.106; p=0.347). BMI demonstrated the strongest association with HOMA-IR (β=0.165), and HOMA-IR was the variable most strongly related to HOMA-B (β=21.83). Overall, this study concludes that baseline vitamin D levels are not significantly linked to insulin resistance or pancreatic β-cell function, as reflected by HOMA-IR and HOMA-B measurements, among non-diabetic obese staff at Dr. M. Djamil Padang Hospital.

Abstract

Nutritional status is an important indicator that determines the quality of health and development of school-age children. Adequate nutrition supports children's physical growth and cognitive abilities, and contributes to academic achievement. This community service activity aims to analyze the nutritional status of Bustanul Ulum Elementary School students by measuring height, weight, and Body Mass Index (BMI). The method used is quantitative descriptive with a sample of 150 students from grades 1 to 6. Measurements were carried out using standard and calibrated measuring instruments, while BMI was calculated using the formula body weight (kg) divided by the square of height (m²). The results showed that 75% of students had normal nutritional status, 10% were underweight and stunted, and 15% were overweight and obese. Factors that influence nutritional status include an unbalanced diet, lack of physical activity, and family socio-economic background. Children from families who have access to healthy food and good nutrition education tend to have better nutritional status. Based on these results, a comprehensive intervention program is needed in the form of nutrition education for students and parents, provision of healthy food at school, and promotion of regular physical activity. Collaboration between schools, health workers, and parents is essential to create an environment that supports optimal and sustainable child growth and development.
