Abstract

Background Allergic disease in infancy mostly related to cow'smilk allergy. The prevalence of cow's milk hypersenstivity in chil-dren has increased steadily in the past years; therefore diagnosticaccuracy is important and every symptomatic child with positivespecific lgE should be followed by elimination and provocation.Objective To diagnose cow's milk hypersensitivity on children un-der three years old using elimination and provocation methods.Methods This was a clinical diagnosis study, in which childrenwith allergy symptoms were examined for cow's milk sensitivityusing prick test or lgE Radioallergosorbent test (RAST). Thosewith positive results underwent an elimination for minimal of 2weeks and were challenged aterwards. The study was a qualitativediagnostic test with the gold standard of recurrence of symptomsafter challenge test.Results There were forty subjects included in the challenge pro-cess with mean age of 17 months old. Boys and girls were equallydistributed. There were 6 7% subjects with positive results onchallenge with positive prick test and 64% positive challenge withhistory of positive lgE RAST. Sixty-five percents of subjects withpositive results on challenge had more than 2 weeks eliminationperiod. There was no statistical significance found in childrenwith positive results on challenge, using neither prick test orlgE RAST.Conclusions Cow's milk protein allergy can be diagnosed on chil-dren less than 3 years old by applying elimination and challengeprocedure. lgE sensitivity alone can not determine hypersensitivity

Abstract

Introduction Periventricular leucomalacia (PVL) is a major causeof neurodevelopment delay in premature infants, so early detec-tion of the preterm infant at high risk for the subsequent develop-ment of this lesion is critical.Objectives To determine the prevalence of PVL in premature in-fants hospitalized in neonatal ward, Cipto Mangunkusumo Hospi-tal using cranial ultrasound scans and define its characteristicsMethods Premature infants hospitalized in the neonatal ward fromJanuary to July 2003 were included in this study. Clinical featureswere retrieved from medical charts. Ultrasound scan was performedonce, after the age of 7 days and interpretations were read sepa-rately by two consultants of the radiology division.Results Fifty-one infants were included, 21 with <32-week gesta-tion, 30 with >32-week gestation; birth weight range were 1000 to2600 gram. Nineteen infants had cystic lesion and/or dilatation ofthe ventricle from the cranial ultrasound scan. The prevalence ofPVL in gestational age (GA) of <32 weeks was 6/21 and that in GAof >32 weeks was 13/30. Risk factors found in infants with PVLwere maternal infection, respiratory distress, sepsis and circula-tory failure.Conclusion The prevalence of PVL in preterm infants in CiptoMangunkusumo neonatal ward was higher than that was reportedin developed countries. Maternal infection, respiratory distress,sepsis, and circulatory failure which were commonly found in theseinfants were factors to be considered as risks for PVL

Abstract

Background Human immunodeficiency virus (HIV) is expandingrapidly and was reported double in several places in Indonesia Toour knowledge, reports regarding HIV-infected infants are stillscarce.Objectives To investigate the incidence of HIV-infected infantsborn to HIV- mothers who had received prophylaxis therapy at birth.Methods A prospective hospital-based cohort study was held fromJanuary 2003 until December 2004 in Cipto Mangunkusumo Hos-pital, Jakarta. The inclusion criteria were mothers with positive HIVand their infants had been given anti retroviral (ARV) therapy. Thebabies were followed up monthly and the status of infection wasdetermined by PCR at the age of 4 weeks and 6 months. Outcomewas measured based on PCR assays or clinical signs of HIV in-fection.Results The mothers’ age ranged from 19 to 27 years. All of themwere carrying their first child and only 41% mothers took ARV pro-phylaxis. Almost all mothers underwent caesarean section and theinfants had formula feeding. HIV infection was diagnosed in 7 in-fants and 2 of them had RNA assays more than 5,000 copies/ml.Six infants were negatives whereas 3 infants were diagnosed asindeterminate HIV infection and needed further examination. Oneneeded no further investigation as the mother was seronegative.Conclusions Preventing HIV transmission from mother to infantcan be done by giving ARV during prenatal, intrapartum, and post-natal period to the newborn. In our hospital, transmission was con-firmed in 6 of 17 infants. Unison protocol must be used and popu-lation of HIV-pregnant mother must be registered in order to knowhow high the transmission rate among Indonesian HIV people

Abstract

Background Systemic lupus erythematosus (SLE) is a complex disease with various manifestations. Interleukin-6 (IL-6) is a pleiotropic cytokine with a wide range of biological activities which plays an important role in immune regulation and inflammation. Serum level of IL-6 may be used as a parameter of disease activity, especially in pediatric SLE patients with mild disease activity or total remission with conflicting clinical manifestations and Systemic Lupus Erythematosus Disease Activity Index (SLEDAI) scores. Objective To identify the characteristics of serum IL-6 concentration in pediatric SLE with mild disease activities and total remission. Methods This case-control study was performed at the allergy-immunology outpatient clinic, Department of Child Health Dr. Cipto Mangunkusumo Hospital, Jakarta and Dr. Sardjito Hospital, Yogyakarta. Serum IL-6 concentration and disease activity were assessed in all pediatric SLE patients aged 1-18 years old. Disease activity was assessed with SLEDAI scores and serum level of IL-6 was measured by enzyme-linked immunosorbent assay. Results Among 60 subjects included in this study, 30 subjects with mild activities were in the case group and 30 subjects with total remissions were in the control group. There was no difference in serum IL-6 concentration between the case and control group (OR 0.483; 95%CI 0.041to 5.628; P=0.500). In this study, 2 subjects with urinary tract infection had high serum IL-6 concentrations. Conclusion There is no difference in serum IL-6 concentration between pediatric SLE patients with mild disease activities compared to total remissions.

Abstract

Background Henoch-Schönlein purpura (HSP) is the most common systemic vasculitis in children and is usually self-limited with a favorable prognosis. However, recurrence in children is associated with a poorer prognosis, i.e., a higher risk of progressing to chronic kidney disease (CKD) as a long-term complication. In Indonesia, the recurrence rate of HSP and its predictors in children have not been well established . Objective To estimate the incidence of recurrent HSP and determine its predictors in children at Dr. Cipto Mangunkusumo National General Hospital (RSCM). Methods A retrospective cohort review of medical records followed children aged <18 years at RSCM for 6 months after HSP diagnosis based on the the European League Against Rheumatism (EULAR)/ Paediatric Rheumatology European Society (PRESS)/Paediatric Rheumatology International Trials Organization (PRINTO) criteria. Multivariate, Cox logistic regression, and Kaplan-Meier analyses were performed. Results This study included 116 children aged 2–17 years with HSP. Twenty-six (22.4%) of the subjects experienced recurrence, with an incidence of 3.56 per 100,000 person-years. The only statistically significant predictor for recurrence was  the presence of infection after the first episode of HSP (HR 11.301; 95%CI 4.327 to 29.519; P<0.001). The cumulative survival of subjects with infection for recurrence over 6 months  was  51%, with mean 5.3 months survival duration (95%CI 4.76 to 5.99; P< 0.0001). Chronic kidney disease, a long-term complication of HSP, was noted in 22 (19%) participants. Conclusion Recurrence of HSP was  observed in 22.4% of our subjects within 6 months follow up. However, subjects with a history of infection after their first episode of HSP resolution should be notified about the possibility of recurrence. Chronic kidney disease occurred in 22 participants (19%), possibly becoming a long-term complication of HSP.
