Abstract

Tuberculosis (TB) is an infectious disease caused by the bacterium Mycobacterium tuberculosis (M.tb) and is still a major health problem in the world. Immunity against tuberculosis is very complex because it involves almost all components of the immune system. One of the cells that are responsible for cell-mediated immunity are lymphocytes, especially T helper lymphocytes are divided into Th1 (proinflammatory cytokines IFN-γ) and Th2 (anti-inflammatory cytokine IL-4). EC610 fusion antigens (ESAT-6-CFP-10) is a peptide containing a specific antigen M.tb. Order to determine the differences between the secretion of IFN-γ and IL-4 after stimulation EC610 fusion antigens (ESAT-6-CFP-10) in patients with active pulmonary TB and latent TB.Research with a quasi-experimental design in laboratory in vitro in patients with active pulmonary tuberculosis group of 21 samples and 28 samples of latent TB. Venous blood sampling in vitro anticoagulants heparin and PBMCs isolated using a Ficoll-Paque ™, then cultured with antigen fusion EC610 for 24-72 hours with a CO2 incubator at a temperature of 370 C. The levels of IFN-γ and IL-4 by ELISA method. Statistical analysis with alternative test non-parametric Mann Whitney. IFN-γ secretion in patients with active pulmonary TB group (6700 pg / ml) was higher than the latent (6000 pg / ml) after stimulation by antigen fusion EC610, but not significant (p = 0.769). While the secretion of IL-4 levels (180 and 60 pg / ml) there was a significant difference between the groups (p = 0.000).

Abstract

The Secretion of IFN-γ and IL-2 After ESAT-6-CFP-10 Fusion Antigen Stimulation in Active and Latent TB Patients. This study held to discover how immune responses work and to know the pathogenesis of active TB and latent TB patients. This study used PBMC to stimulate T Cells with ESAT-6 and CFP-10 antigen fusion, and measure the level of IFN-γ and IL-2 with ELISA antibody sandwich (U-Cytech). 16 ml of blood were drawn to 5 tubes. ESAT-6 CFP-20 inducted one tube with QuantiFERON for IFN-γ assay. The other four tubes were PBMC isolated using Ficoll-Paque, and pre-incubated with stimulation of ESAT-6 CFP-10 fusion antigen for 24-72 hours at 370 C and measured using T-Spot and ELISA reader. We got from this study that there are no significant differences in IFN-γ levels for both groups with active TB and latent TB. Measurement of IL-2 levels showed significant differences between the two group.

Abstract

The incidence of Metabolic Syndrome is 20-25% in the world and 13.13% in Indonesia. Metabolic syndrome is not a diagnosis but a collection of various symptoms that can develop into diseases or chronic conditions such as hypertension, hypercholesterolemia and diabetes mellitus (DM), which results in cardioserebrovascular disease. Metabolic Syndrome is more common in women than men. This community service is carried out at the request of the Taklim Assembly ladies Nurul Hikmah in the neighborhood of RW.15 Unsri lecturer’s residents, Bukit Lama, Ilir Barat 1 Palembang, mostly aged over 40 years. In this activity it was found that the majority of participants' awareness of the dangers of Metabolic Syndrome was still low. This is indicated by the number of participants who had excess body weight and waist circumference (44.29%), increased blood pressure (45.71%) and increased cholesterol levels (77.14%). The participants felt very helped and hoped that these community service activities or similar could be routinely carried out in the future.

Abstract

Di Indonesia angka prevalensi diabetes mengalami peningkatan cukup signifikan selama lima tahun terakhir. Di tahun 2013, angka prevalensi diabetes pada orang dewasa mencapai 6,9 % dan ditahun 2018 angka terus melonjak menjadi 8,5%. Alpukat adalah salah satu buah yang memiliki efek antidiabetik yang dapat membantu tubuh dalam mengkompensasi adanya gula darah dalam tubuh. Penelitian ini bertujuan untuk menilai efektivitas antara ekstrak etanol biji alpukat dengan acarbose terhadap penurunan kadar glukosa darah 2 jam post prandial. Metode yang digunakan merupakan penelitian eksperimental secara in vivo dalam bentuk pre and post test only. Penelitian ini menggunakan 25 ekor tikus dan dibagi secara acak menjadi 5 kelompok dengan jumlah tikus lima ekor perkelompok yaitu kontrol positif (Acarbose), Kontrol negatif (NaCMC), ekstrak etanol biji alpukat 250 mg/kgBB, ekstrak etanol biji alpukat 500 mg/kgBB dan ekstrak etanol biji alpukat 1000 mg/kgBB. Seluruh kelompok diinduksi aloksan 160 mg/kgBB sehingga memiliki glukosa darah 2 jam post prandial (>180 mg/dL) (pretest). Setelah 14 hari diberi perlakuan dilakukan pengukuran glukosa darah 2 jam post prandial (posttest). Hasil penelitian menunjukan bahwa ekstrak etanol biji alpukat efektif dalam menurunkan kadar glukosa darah 2 jam post prandial hampir menyerupai kontrol positif (Acarbose)

Abstract

Daun rambai Baccaurea dulkis Muell. Arg. merupakan salah satu tumbuhan khas kalimatan tengah dan kalimatan selatan yang daunnya secara empiris dapat digunakan masyarakat sebagai obat luka serta penghilang bekas luka pada kulit. Tujuan dari penelitian ini untuk menilai efektivitas ekstrak etanol daun rambai (Baccaurea dulkis Muell. Arg) Penelitian ini bertujuan untuk menilai efek topikal ekstrak daun rambai Baccaurea dulkis Muell. Arg. Arg terhadap kadar TGF-β1 pada fase inflamasi akut luka sayat tikus (Rattus norvegicus) Jantan galur Wister. Penelitian ini merupakan suatu penelitian eksperimental laboratorium in vivo, dengan metode pre and post test only pada tikus Wister. Penelitian ini dilaksanakan dari bulan Juli sampai dengan oktober tahun 2020. Pelaksanaan penelitian dilakukan di Laboratorium Biomolekuler dan Animal House Fakultas Kedokteran Universitas Sriwijaya Palembang. Subjek penelitan berupa tikus putih wister dengan jenis kelamin jantan yang dibagi ke dalam 5 (lima) kelompok perlakuan secara random, yaitu kelompok kontrol negatif (aquades), kelompok kontrol positif (povidone iodine), kelompok ekstrak daun rambai konsentrasi 1%, kelompok ekstrak daun rambai 2% dan kelompok kelompok konsentrasi 4%. Data yang diperoleh kemudian dianalisis secara statistic menggunakan SPSS versi 19 dengan Uji uji parametrik, uji T berpasangan (paired t-test) uji T tidak berpasangan (Independen t-test) atau Mann Whitney. Hasil penelitian menunjukan waktu penyembuhan luka sayat tikus putih jantan pada hari ke 3 konsentrasi ekstrak etanol daun Baccaurea dulkis Muell.Arg. 2% dan 4% tidak berbeda bermakna dengan efektivitas Povidone Iodine dan pemberian daun rambai pada tikus luka sayat dengan Povidone Iodine dilihat dari kadar rerata TGF-β 1 pada penyembuhan luka. Tidak terdapat perbedaan yang bermakna kadar rerata TGF-β1 pada tikus luka sayat dengan penggunaan daun rambai dengan Povidone Iodine pada penyembuhan luka

Abstract

Background: Chronic kidney disease (CKD) is a pathophysiological process with various etiologies resulting in a progressive decline in kidney function and generally ends in kidney failure. During hemodialysis, inflammation and activation of TNF-α and other proinflammatory cytokines occur. This inflammatory condition causes a decrease in the effectiveness of HD in reducing the mortality of patients with CKD. Garlic has the potential as an anti-inflammatory agent due to its high antioxidant content in garlic. This study aimed to determine the effectiveness of garlic extract (Allium sativum) on decreasing serum levels of tumor necrosis factor-α (TNF-α) in chronic hemodialysis patients at Dr. Mohammad Hoesin General Hospital, Palembang, Indonesia. Methods: This study is an experimental study with a randomized, controlled, double-blind trial crossover design. A total of 40 research subjects participated in this study which was grouped into treatment (garlic extract) and placebo groups. TNF-α levels were measured using the ELISA method. Data analysis was carried out using SPSS univariate and bivariate. Results: The results of the study showed that the administration of garlic extract was able to reduce TNF-α levels by 12.5 pg/mL, p<0.05. In contrast, in the placebo group, there was an increase in TNF-α levels of 8.3 pg/mL, p<0.05. Conclusion: Garlic extract (Allium sativum) is effective in reducing tumor necrosis factor-α (TNF-α) serum levels in chronic hemodialysis patients at Dr. Mohammad Hoesin General Hospital, Palembang, Indonesia.

Abstract

Introduction. Study reported that malondialdehyde (MDA) levels as an in vivo marker of oxidative stress were increased in patients undergoing hemodialysis. Antioxidants allicin compounds contained in garlic (Allium sativum) are known to have antioxidant activity by inducing antioxidant enzymes such as superoxide dismutase (SOD), catalase (CAT) and inhibiting prooxidant enzymes (nitric oxide synthase, xanthine, NADPH oxidase, and cyclooxygenase). This study aimed to investigate the effectiveness of Allium sativum to reduce level of MDA serum and improve quality of life in patients with chronic hemodialysis. Methods. This study was a crossover randomized double-blind placebo-controlled trial. The subjects were 40 chronic kidney disease (CKD) stage 5 patients with chronic hemodialysis (HD). Subjects were randomly divided into 2 groups, namely the group that received 1,000 mg of garlic extract per day (2 x 500 mg) for 6 weeks and the group that received a placebo, which was then carried out during a washing period for 2 weeks and then the two groups were crossover. Each group measured the levels of serum MDA before and after treatment, also fill in kidney disease quality of life (KDQOL) SF-36 questionnaire. Results. Out of 40 subjects, most of them were male (57.5%) with median of age of 48.6 (range 26-59) years. It was found that serum MDA levels before and after garlic extract administration were 2.423 (SD 0.236) nmol/l and 1.649 (SD 0.26) nmol/l (p<0,001) with a mean change of -0.782 (SD 0.735) nmol/l. Statistical analysis showed a significance difference of MDA serum between the two groups. There was significant improvement in quality of life before and after garlic extract administration from 57.81 (SD 16.95) to 67.62 (SD 13.61) with a mean change of 9.81 (SD 3.34). But there was no significant difference between quality of life and MDA serum. Conclusion. There is effectiveness of garlic extract to decrease serum MDA and improve the quality of life in patients with CKD stage 5 using chronic HD at Mohammad Hoesin Hospital, Palembang.

Abstract

Introduction. Geriatric patients have multipathological characteristics, low physiologic reserves, atypical clinical signs and symptoms, decreased functional status, and nutritional disturbances. This situation is a risk factor for polypharmacy. Polypharmacy is closely related to drug side effects, drug interactions, and patient compliance. Polypharmacy is thought to have a relationship with the length of stay in geriatric patients. The purpose of this study was to determine the correlation between polypharmacy and length of stay in geriatric patients at Moehammad Hoesin Hospital Palembang. Methods. This was a cross-sectional study with a correlation test design conducted at Dr. Mohammad Hoesin Hospital (RSMH) Palembang from April 2021 to July 2021. The sample was geriatric patients aged > 60 years. We collected data on the number and types of drugs given and the length of hospitalization. All data processing and analysis in this study were conducted by using SPSS version 22 for Windows. Results. Of 56 geriatric patients, there was 28 female (50%) and 28 male (50%). The median value of the number of drugs was 7 (3-32). Patients without polypharmacy were 7 (12.5%), and patients with polypharmacy were 49 (87%). The median length of hospitalization for geriatric patients at RSMH was 11.5 (range 1-47) days. There was a correlation between polypharmacy and length of stay (r=0.277; p value=0.039). Conclusion. There is a weak correlation between polypharmacy and length of stay (r=0.277; p=0.039).

Abstract

Introduction: Panretinal laser photocoagulation (PRP) is a standard treatment for severe nonproliferative and proliferative diabetic retinopathy. Twenty-milisecond duration PRP show same effectiveness with 100-ms standard PRP in inhibit neovascularization progression. This shorter pulse tend to minimize retinal neuronal defect and visual field defect. This study aim to analyze the difference of visual field defect in diabetic retinopathy (DR) patients treated with 20-ms PRP compared with 100-ms PRP in Moh. Hoesin Hospital Palembang. Methods: A clinical trial with single blinding on severe-very severe NPDR and early PDR eyes treated with PRP between June and August 2016. Forty eyes (25 patients) were randomized into two groups. Twenty eyes were treated with 20-ms PRP, and other 20 eyes treated with 100-ms PRP. Visual field defect was evaluated using Humphrey Field Analyzer 30-2 SITA Standard at baseline and 2 weeks follow-up. Result: Unpaired t-test showed significant difference in mean deviation (MD) after laser on NPDR eyes (p=0.042, p<0.05), meanwhile there was no significant difference in early PDR eyes (p=0.17, p>0.05). In NPDR eyes, more MD improvement was found in 20-ms PRP group (0.79±0.93 dB) than in 100-ms group (-0.04±0.61 dB). In early PDR eyes, MD improvement was bigger (1.0±0.88 dB) in 20-ms PRP group than in 100-ms group (0.10±1.47 dB). There was no significant difference in pattern standard deviation (PSD) on both group at any DR grade (p=0.208; p=0.201; p>0.05). Conclusion: After 2 weeks, 20-ms PRP caused more improvement and lesser visual field defect (p=0.042, p<0.05) on NPDR eyes. There was no significant difference in PSD on both groups.
